1.Keap1-tat peptide attenuates oxidative stress damage in hippocampal CA1 region and learning and memory deficits following global cerebral ischemia

Objective:To design Keap1-tat peptide and explore its neuroprotective role on hipocampal CA1 neuron,as well as the effect on spacial learning and memory function following global cerebral ische-mia.Methods:Adult male Sprague Dawley (SD)rats were subjected to global cerebral ischemia (GCI) by four-vessel occlusion for 1 5 min and randomly divided into five groups:sham,sham+Keap1-tat,is-chemia/reperfusion (I/R),Keap1-tat peptide-and vehicle-administrated groups.For Keap1-tat or vehi-cle groups,the rats were treated with Keap1-tat (30,50,1 00 μg in 5 μL 0.9%saline)or the same vo-lume vehicle by intracerebroventricular injection (icv)30 min prior to ischemia.Cresyl violet staining was used to observe the surviving neurons and 4-hydroxy-2-noneal (4-HNE ) and 8-hydroxy-2′-deox-yguanosine (8-OHdG)immunostaining were used to detect the change of markers response to oxidative stress in hippocampal CA1 region.The spatial learning and memory function of the rats was evaluated using Morris water maze.Results:Compared with sham group,the number of surviving neurons in ische-mia-reperfusion and vehicle groups significantly decreased in the hippocampal CA1 region (P<0.05 ), while administration of Keap1-tat significantly decreased the damage following GCI (P<0.05),and the dose of 50 μg existed the most effective neuroprotective role.Furthermore,immunostaining intensity of 4-HNE and 8-OHdG,markers of oxidative stress damage attenuated by Keap1-tat peptide as compared with vehicle group in CA1 region.Of significant interest,the time of finding underwater platform in Keap1-tat group animals was significantly short,and after removing the platform,the probe time of Keap1-tat group animals in the original quadrant where the platform was significantly increased compared with that of vehi-cle and I/R group animals (P<0.05).Conclusion:Keap1-tat peptide can effectively attenuate neuro-nal damage in hippocampal CA1 region and improve learning and memory function,which might bedue to the attenuation of oxidative stress caused by GCI.
2.Low dose of genistein attenuates neuronal injury and improves learning and memory functions of rats following global cerebral ischemia

Objective To explore the neuroprotective role of Genistein (GEN) on hippocampal CA1 neurons and the possible mechanism following global cerebral ischemia ( GCI) in rats.Methods Seventy five rats were subjected to global cerebral ischemia ( GCI ) by four-vessel occlusion and randomly divided into five groups , sham, ischemia/reperfusion (I/R), GEN, ICI 182,780 and vehicle groups.Fluoro-Jade B and neuron-specific nuclear-binding protein ( NeuN) staining was used to observe CA 1 neuronal survival .TUNEL was used to detect apoptotic neurons .Spatial learning and memory function of the rats were evaluated by Morris water maze .Results The best dose of neuroprotective role of GEN was 1.0mg/kg body weight.Compared with sham, TUNEL-positive neurons in the hippocampal CA1 region increased significantly in I/R and vehicle groups (P<0.01), while post-treatment with GEN (1.0mg/kg) at 5min after ischemia by tail vein injection decreased markedly (P<0.01).Treatment of 1.0mg/kg GEN markedly attenuated spatial learning and memory deficits of the rats after ischemic insult compared to I /R group.Furthermore, ICI 182,780 significantly abolished the neuroprotective role of GEN (P <0.01).Conclusion The low-dose (1.0mg/kg) GEN significantly attenuates neuronal damage and cognitive deficits following GCI in rats , and the mechanism may be involved in estrogen receptor activity.

Objective To investigate expression of Human papillomavirus (HPV) 11 type E7 protein antigen in prokaryotic cells and its potential use for the serodiagnosis of condyloma acuminatum (CA).Methods The full-length gene encoding for HPV11 E7 protein was amplified by PCR,and cloned into vector pET32a(+) to form recombinant pET32a(+)/HPVll E7 plasmid.The fusion His-E7 protein was expressed and analyzed by using SDS-PAGE and Western blotting.Using ELISA assay,HPV11 E7 fusion protein were also used to screen human serum IgG antibody from 93 patients with CA,43 patients with cervix cancer and 58 healthy control subjects.Results Highly expressed fusion His-E7 protein was obtained,and purified protein served as a special diagnostic antigen to screen human serum antibody for CA serodiagnosis.It showed that CA group,cervix cancer group and healthy control human serum IgG antibody average value were 1.545?0.131,0.586?0.155 and 0.674?0.150 respectively,positive rate were 76.3% (71/93),11.6% (5/43) and 5.2% (3/58).There was significantly difference between the CA group to compare cervix cancer group and healthy control (P
4.Long-term results of prophylactic cranial irradiation for limited-stage small-cell lung cancer in complete remission.

BACKGROUNDBrain metastasis is one of the most important causes of treatment failure in patients with small cell lung cancer (SCLC). This study was conducted to evaluate the effects of prophylactic cranial irradiation (PCI) on survival and brain metastases for patients with limited stage small cell lung cancer in complete remission.
METHODSFifty one patients with limited stage SCLC in complete remission after chemoradiotherapy were randomly divided into PCI group (n = 26) and control group (n = 25). Patients in the PCI group received PCI at a dose of 25.2 to 30.6 Gy in 1.8 to 2.0 Gy per fraction. The Kaplan-Meier method and Log rank test were used to analyse and compare survival rates, and chi(2) test was used to compare the incidences of cranial metastases in two groups.
RESULTSThere was no significant difference in clinical characteristics of patients such as age, sex, effect of treatment before PCI between the two groups. The incidence of brain metastases was 3.8% in the PCI group in contrast to 32.0% in the control group (chi(2) = 5.15, P = 0.02). The 1, 3, 5-year survival rates were 84.6%, 42.3%, 34.6% respectively in the PCI group and 72.0%, 32.0%, 24.0% respectively in the control group, with no difference between the two groups (chi(2) = 2.25, P = 0.13). No serious sequelae were observed in patients receiving PCI.
CONCLUSIONFor patients with limited stage SCLC responding completely to chemotherapy plus radiotherapy, PCI can decrease the incidence of brain metastases and improve survival rate.

5.A study on oral health behavior and other related factors between children with high dmft and no caries.

OBJECTIVETo explore the risk factors of children with high dmft.
METHODSIn suburban of Guangzhou, oral health of 401 3 - 4-year-old children were examined and structured questionnaire were completed by their parents. 120 children with highest number of dmft (dmft > or = 5) and 118 caries-free children were chosen for case-control analysis.
RESULTSThe results of logistic regression analysis showed that the factors associated with high dmft were developmental defect of enamel, visible plaque index, frequency of toothbrushing, frequency of sugar consumption, and income.
CONCLUSIONSAdvocating brushing teeth at least twice daily, controlling the frequency of sugar consuming, reducing the developmental defect of enamel and paying more attention to the oral health of lower income population may effectively reduce dental caries of the children.

6.Phenolics from traditional Chinese medicine Sargentodoxa cuneata.

AIMTo study the chemical constituents of Sargentodoxa cuneata.
METHODSTo isolate compounds with chromatography technology and to elucidate their structures by spectral analysis.
RESULTSTen phenolics were isolated from Sargentodxa cuneata and their structures were determined as 1-O-(vanillic acid )-6-O-( 3", 5"-dimethoxy-galloyl)-beta-D-glycoside (I), (-)-epicatechin (II), phydroxyphenylethanol ferulate (III), chlorogenic acid (IV), methyl chlorogenate (V), apocynin (VI), vanillic acid (VII), protocatechuic acid (VIII), 3,4-dihydroxy-phenylethanol (IX), tyrosol (X).
CONCLUSIONCompound I is new compound, compound III-VI and VIII-X were isolated from Sargentodoxa cuneata for the first time.

7.Establishment and practice of the teaching mode of multidisciplinary collaboration on nursing training among elderly patients in ICU

Objective To investigate the on-job training mode suitable for ICU nursing staff for elderly patients,in order to improve the comprehensive treatment and nursing capabihties of ICU nursing staffs in critically ill elder patients with a variety of diseases.Methods Nurses from coronary care unit (CCU),respiratory intensive care unit (RICU),and surgery intensive care unit(SICU) were recruited to participated in the training.Every three months were considered as a training cycle,each ICU took responsibility for one-month training program for the participants in turns.Satisfaction rate and examination scores concerning the first aid theory and first aid skills of nurses were evaluate and compared before and after training.Results After receiving the training program,satisfaction rates for the first aid theory and the first aid skills of nurses in patients and their families were 96.6% and 95.2%,and higher than that were 83.7% and 86.3% before the training program; 95.5% and 93.7% among doctors after the training were higher than 80.9% and 84.6% before the training program; 98.7% and 97.6% among nursing staffs after the training were higher than 80.9% and 84.6% before the training program; and these differences were statistically significant between before and after the training (x2 =8.900,4.395,5.224,4.940,12.890,7.680,respectively; P <0.05).The examination score of the first aid theory before the training was (59.2 ± 4.2) and lower than that was (89.7 ± 3.7) after the training,the examination score of the first aid skills of nurses before the training was (63.8 ± 4.6) and lower than that was (90.3 ± 5.3) after the training,these differences were statistically significant between before and after the training (t =-59.12,-41.06,respectively; P < 0.01).Conclusions The establishment of the teaching mode of multidisciplinary collaboration on nursing training among elderly patients in ICU,which can develop the comprehensive treatment and nursing capabilities of ICU nursing staffs and improve the satisfaction rate for nursing job and the rate of successfully rescuing critically ill patients,can be used to achieve the sharing of human resources,medical instruments and information resources,and is feasible in nursing practice in ICU and more worthy of clinical promotion and application.
8.A study on the relationship between interleukin-10 promoter polymorphism and autoimmune liver disease.

OBJECTIVETo investigate whether three biallelic polymorphisms at the position -592, -819 and -1082 in the promoter region of the IL-10 gene were associated with the incidence of autoimmune liver disease.
METHODSThe IL-10 -592 and IL-10-1082 polymorphisms were genotyped by polymerase chain reaction-restriction fragment length polymorphisms analysis (PCR-RFLP), while polymerase chain reaction- sequence specific primer (PCR-SSP) assay was used to detect IL-10 -819 polymorphisms.
RESULTSAmong 54 Chinese patients with AIH or 77 Chinese patients with PBC versus healthy controls, the frequency of AA, GA genotypes at IL-10 gene promoter -1082 position was 87.0% or 83.1% versus 90.0%, 13.0% or 16.9% versus 10.0%, respectively (P > 0.05), the GG genotype in Chinese populations is absent; the frequency of CC, CT, TT genotypes at IL-10 gene promoter -819 position was 11.11% or 9.1% versus 8.1%, 44.4% or 53.3% versus 45.0%, 44.4% or 37.7% versus 46.9%, respectively (P > 0.05); the frequency of CC, CA, AA genotypes at IL-10 gene promoter -592 position was 4.9% or 14.3% versus 10.0%, 51.2% or 53.3% versus 51.9%, 43.9% or 32.5% versus 38.1%, respectively (P > 0.05). No alleles differed significantly in each groups.
CONCLUSIONThere were no association between IL-10 gene polymorphisms and autoimmune liver disease

9.Surgical treatment for local recurrence of rectal carcinoma after operation.

OBJECTIVETo evaluate the value of reoperation for local recurrence of rectal carcinoma.
METHODSThe data of 62 cases with post-operative local recurrence of rectal carcinoma were analyzed retrospectively.
RESULTSAll the 62 patients received reoperation. Thirty two of those patients were treated with radical resection (16 patients combined multiple organ resection), 6 palliative resection, 11 colostomy, and 13 laparatomy only. The 1-, 3- and 5-year survival rates in the patients accepted radical resection were 90.6%, 59.4% and 18.8% respectively. But in patients undergone palliative resection and combined therapy, survival time was 6-24 months with median survival time of 16 months. The patients, accepted laparatomy and intra-abdominal chemotherapy, all died within 2-14 months postoperatively. For patients with postoperative recurrence time >5 years, <2 years and 2-5 years, the reoperation resection rates were 100%(11/11), 62.9%(22/35), and 31.3%(5/16) respectively, and there were significant differences among 3 groups (P<0.01). The rate of reoperation resection of pure local recurrence was 80.0%(32/40). The rate of reoperation resection of local recurrence, associated with near organ invasion, was 27.3%(6/22). The difference was significant(P<0.01). The reoperation resection rate of first operation with Dixon or Miles was 61.9%(26/42) and 30.0%(6/20), and the difference was significant as well(P<0.05).
CONCLUSIONSThe recurrence of rectal carcinoma still needs positive operation in order to prolong the survival time and improve the quality of life of the patient. First operative procedure, post-operative recurrence time and recurrence type are important factors of reoperative resection.

10.A systematic review and meta-analysis on efficacy and safety of cardiac resynchronization therapy alone or in combination with implantable cardioversion defibrillation in patients with mild to severe heart failure

Objective To evaluate the efficacy and safety of cardiac resynchronization therapy (CRT) alone or in combination with implantable cardioversion defibrillation (ICD) in patients with mild to severe heart failure.Method Electronic searches of MEDLINE,EMBASE,CENTREN and affiliated clinical trial registration data center,US Food and Drug Administration reports,CBMdisc,VIP,and CNKI databases from establishment to Dec 2010,using the search terms "CRT,heart failure"," biventricular pacer,heart failure","biventricular pacing,heart failure",and "biventricular pacemaker,heart failure",were performed to identify randomized controlled trials (RCTs).Meta-analysis was performed by using RevMan 5.0 software after the strict evaluation of the methodological quality of the included RCTs.Results A total of 23 trials including 8521 patients were included.In patients with New York Heart Association (NYHA) class Ⅰ/Ⅱ,CRT improved left ventricular ejection fraction (LVEF) [weighted mean difference (WMD) =0.05,95 % CI 0.01-0.08],reduced heart failure hospitalizations [risk ratio (RR) =0.70,95%CI 0.61-0.81] and all-cause mortality (RR =0.78,95% CI 0.65-0.93) with increasing complications (RR =1.74,95% CI 1.42-2.13).In patients with NYHA classⅢ/Ⅳ,CRT improved LVEF (WMD =0.03,95% CI 0.01-0.05),reduced both heart failure hospitalizations (RR =0.64,95% CI 0.55-0.73) and all-cause mortality (RR =0.80,95% CI 0.70-0.91) without increasing complications (RR =1.01,95% CI 0.91-1.12).Compared with ICD alone,CRT in combination with ICD significantly improved LVEF (WMD =0.03,95% CI 0.00-0.06),reduced heart failure hospitalizations (RR =0.73,95% CI 0.64-0.82) and all-cause mortality (RR =0.82,95% CI 0.72-0.95) without increasing complications (RR =1.36,95% CI 0.91-2.03) in patients with NYHA class Ⅰ-Ⅳ symptoms.Conelusions CRT offered additional benefits on top of standard medication for heart failure patients with ventricular dyssynchrony in terms of improving LV function,and reducing heart failure hospitalization and all-cause mortality,regardless of NYHA class.CRT offers also additional benefit in heart failure patients implanted with ICD.However,CRT is associated with more adverse events in patients with NYHA class Ⅰ/Ⅱ.
