1.Autologous peripheral hematopoietic stem cell transplantation in the treatment of adult dermatomyositis: a case report

Objective To investigate the efficacy of autologous peripheral hematopoietic stem cell transplantation in the treatment of adult dermatomyositis. Methods A 21-year-old patient with dermatomyositis received autologous peripheral hematopoietic stem cell transplantation and was followed up for 6 years. Autologous peripheral hematopoietic stem cells were mobilized by recombinant human granulocyte colony stimulating factor (rhG-CSF) before the transplantation, and the conditioning regimens consisted of cyclophosphamide,methylprednisolone and cyclosporin. Rabbit anti-human T lymphocyte immunoglobulin began to be applied on day 3 after retransfer of stem cells. The improvement in symptoms, physical signs and biochemical indicators was observed, and hematopoietic restructuration and immunity resurrection were evaluated after the transplantation. Results After the transplantation, skin eruption greatly improved and gradually subsided. The muscle force of extremities restored from level Ⅳ before transplantation to level Ⅴ. The level of creatine kinase declined sharply after transplantation, but gradually returned to previous level. Leucocyte count began to decrease on the day of retransfer, and returned to the normal level on day 8. Immune function remained normal before and after the transplantation. Conclusion Autologous peripheral hematopoietic stem cell transplantation is an alternative treatment for severe and refractory dermatomyositis.
2.Analysis on the Utilization of Traditional Chinese Medicine Injection in Our Hospital from 2006 to 2008

OBJECTIVE: To analyze the utilization of traditional Chinese medicine (TCM) injections in our hospital in order to provide reference for clinical rational use of TCM injection. METHODS: The utilization of TCM injection in our hospital from 2006 to 2008 was statistically analyzed in terms of consumption sum, DDDs and its order. RESULTS: The consumption sum and DDDs of TCM injection in our hospital increased year by year, especially stasis-removing agents and antitumor drug. Over the 3 years the proportion of consumption sum of TCM injections in the total consumption sum were 57.3%, 59.4% and 55.9%, respectively. There was poor synchrony between consumption sum order and DDDs order. CONCLUSION: TCM injections are used rationally in our hospital. In addition, safety reevaluation of TCM injection should be improved.

OBJECTIVETo investigate the effects and underlying mechanisms of interferon-alpha (IFN-alpha) on the isolated Langendorff perfused rat hearts and the isolated papillary muscles.
METHODSThe left ventricular developed pressure (LVDP), maximal rise/fall rate of left ventricular pressure (+/-dP/dt(max)), left ventricular end-diastolic pressure (LVEDP), heart rate (HR) and coronary flow (CF) were recorded in isolated Langendorff perfused rat hearts. The average contractile force was measured in the isolated papillary muscles of rat right ventricle.
RESULTIFN-alpha (10 - 10,000 U/ml) induced a concentration-dependent decrease of LVDP and +/-dP/dt(max), and increase of LVEDP and CF in the isolated perfused rat heart (P < 0.05), and decrease of the average contractile force of the papillary muscle (P <0.05). Pretreatment with L-NAME (10(-4) mol/L), an inhibitor of nitric oxide synthase, attenuated the effect of IFN-alpha in the isolated rat hearts and the isolated papillary muscles (P <0.05). Isoproterenol (ISO, 10(-9) - 10(-6)mol/L) increased the contractile force of the rat papillary muscles in a concentration-dependent manner. Perfusion for 10 min with IFN-alpha at 1,000 U/ml attenuated the enhancing effect of ISO. Pretreatment with L-NAME reduced the effects of IFN-alpha on the isolated papillary muscles.
CONCLUSIONIFN-alpha may induce a negative inotropic effect in normal and beta-adrenergic activated cardiac muscles and this effect at least partly be mediated by nitric oxide.

5.The prevalence and influencing factors of common chronic diseases among residents in Aheqi County, Xinjiang

Abstract: Objective To understand the prevalence and influencing factors of major chronic diseases among Kirgiz residents in Aheqi County, Xinjiang, and to provide theoretical basis for the prevention and control of chronic diseases in this area. Methods The data of residents in Aheqi County, Kizilsu Kirgiz Autonomous Prefecture, Xinjiang in 2020 for health checkup were selected, and the permanent residents of Kirgiz nationality aged ≥18 were selected as the survey objects, and the prevalence of major chronic diseases and their influencing factors were analyzed by Logistic regression model. Results A total of 21 935 patients were enrolled, and 10 602 patients suffered from chronic diseases with a prevalence rate of 48.33%, including 4 929 cases of hypertension with a prevalence rate of 22.47%, 4 238 cases of obesity with a prevalence rate of 19.32%, 3 610 cases of dyslipidemia with a prevalence rate of 16.46%, 1 632 cases of anemia with a prevalence of 7.44%, and 1 236 cases of type 2 diabetes with a prevalence of 5.63%. The prevalence of hypertension, obesity, dyslipidemia and type 2 diabetes increased with age, while the prevalence of anemia decreased with age (P<0.01). The prevalence of hypertension dyslipidemia and type 2 diabetes was higher in males than in females, and the prevalence of obesity and anemia was higher in females than in males, with statistical significance (P<0.01). Logistic regression analysis showed that 30-<40 years old (OR=1.836, 95%CI: 1.565-2.034), 40-<50 years old (OR=2.916, 95%CI: 2.613-3.255), 50-<60 years old (OR=5.244, 95%CI: 4.651-5.913), 60 years old and above (OR=10.866, 95%CI: 9.533-12.385), government personnel (OR=1.789, 95%CI: 1.405~2.279), professional and technical personnel (OR=1.774, 95%CI:1.372-2.295), the office staff (OR=1.923, 95%CI: 1.418-2.607) occasional alcohol consumption (OR=1.157, 95%CI: 1.055-1.270) and occasional exercise (OR=1.498, 95%CI: 1.238-1.812) were risk factors for chronic disease. Conclusion The prevalence of chronic diseases among residents in Aheqi County of Xinjiang is at a low level. Local health institutions have strengthened health education and publicity on multiple chronic diseases and the prevention and control of anemia among young and middle-aged women.
6.The role of mitochondrial K+ channels in the cardioprotection of puerarin against hypoxia/reoxygenation injury in rats.

OBJECTIVETo determine whether the cardioprotection of puerarin (Pue) against hypoxia/reoxygenation injury is mediated by mitochondrial ATP-sensitive potassium channel (mitoK(ATP)) and/or mitochondria calcium-activated potassium channel(mitoK(Ca)).
METHODSCardiomyocytes were isolated from male Sprague-Dawley rats and hypoxia/reoxygenation injury was induced by myocyte pelleting model. Cell viability was assessed by trypan blue exclusion and mitochondrial membrane potential was measured by loading with TMRE. The opening of mitochondrial permeability transition pore was determined spectrophotometrically.
RESULTSPretreatment with Pue at 0.24 mmol/L for 5 min increased the cell viability against hypoxia/reoxygenation injury, while mitochondrial ATP-sensitive potassium channel inhibitor 5-hydroxydecanoate (5-HD, 100 micromol/L, 20 min) or mitochondrial calcium-activated potassium channel blocker paxilline (Pax, 1 micromol/L, 5 min) attenuated the effect of puerarin. The pretreatment with Pue at 0.24 mmol/L for 5 min attenuated collapse of delta-psim induced by hypoxia/reoxygenation injury, 5-HD and Pax abrogated the effect of Pue. In mitochondria isolated from hearts pretreated with Pue, a significant inhibition of Ca(2+)-induced swelling was observed, and this inhibition was attenuated by 5-HD and Pax.
CONCLUSIONThese findings indicate that Pue protects cardiomyocytes against hypoxia/reoxygenation injury via activating mitoK(ATP) channel and mitoK(Ca) channel, and inhibiting mitochondrial permeability transition pore opening.

To compare the pharmacokinetics of syringin, eleutheroside E and isofraxidin after intravenous administration of each monomer and Ciwujia injection. Twenty-four Sprague-Dawley rats were randomly divided into four groups and intravenously administrated with syringin, eleutheroside E, isofraxidin, and Ciwujia injection, respectively. The concentrations of the three components in rat plasma were determined by LC-MS/MS. DAS 2.0 software was applied to calculate the pharmacokinetic parameters while the SPSS 17.0 software was used for statistical analysis. Significant difference (P < 0.05) was found between each monomer and the injection on the main pharmacokinetic parameters such as AUC, CL and t1,/2. Compared with the injection, the group treated with the syringin has obvious decrease in AUC, and increase in CL while the group treated with eleutheroside E has obvious increase in AUC, and decrease in CL The t1/2 of isofraxidin was prolonged in Ciwujia injection. Pharmacokinetic characters of the ingredients in the injection varied greatly from the monomer. Other constituents in the injection may have an impact on the pharmacokinetic profiles of these three components.

8.Study on in vivo pharmacokinetics of cucurbitacin injection in rats.

To establish a method for the determination of cucurbitacin in plasma samples, in order to study the in vivo pharmacokinetic characteristics of cucurbitacin in rats. Rats were intravenously injected with cucurbitacin. With diphenhydramine as the internal standard (IS), the plasma concentrations of cucurbitacin in rat plasma at different time points were determined by liquid chromatography tandem mass spectrometry (LC-MS/MS). With electrospray ionization source, the positive ion detection in the multiple reaction monitoring mode was conducted to determine the ion-pairs for target compound and IS were m/z 503.2/113.1 and m/z 256.0/167.2, respectively. Agilent ZOBAX SB-C18 column (2.1 mm x 50 mm, 1.8 microm) was adopted and eluted with methanol and 0.1% formic acid (55:45), and the flow rate was 0.2 mL x min(-1). DAS 2.0 software was applied to fit the blood concentration and calculate corresponding pharmacokinetic parameters. The rats were intravenously injected with cucurbitacin at the concentration of 3.0 mg x kg(-1). The target blood quality concentration show good linear relations within the range of 10.5-3 150 microg x L(-1) (R2 = 0.996), the lower limit of the standard curve was 10.5 microg x L(-1), and the signal to noise ratio S/N = 12. Intra- and inter-day precisions RSD was less than 6.9% and 14%, respectively; The accuracy RE ranged between 0.20% and 3.7%; The extraction recoveries ranged between 92.7% and 97.1%. Regarding the pharmacokinetic parameters of tail intravenous injection of cucurbitacin, AUC (0-t) was (811.615 +/- 111.578) microg x h x L(-1), (t1/2) was (1.285 +/- 1.390) h, CL was (3.627 +/- 0.487) L x h x kg(-1), and V(d) was (6.721 +/- 7.429) L x kg(-1). In this study, researchers established a simple, accurate, sensitive and highly specific method for determining the blood concentration of cucurbitacin, and reported the in vivo pharmacokinetic characteristics of cucurbitacin in rats for the first time.

9.Comparative study between MRI and echocardiography in noncompaction of ventricular myocardium

Objective To investigate the MRI and echocardiography manifestations of noncompaction of ventricular myocardium(NVM)and assess the role of MRI in the diagnosis of NVM by comparing it with echocardiography.Methods Fourteen cases of NVM diagnosed by echocardiography were examined with MRI,including scanning of black-blood sequences,double inversion recovery fast spin echo (DIRFSE)and triple inversion recovery fast spin echo(TIRFSE),and white blood sequence:fast imaging employ steady state acquisition(FIESTA).Scanning plane includes short axis view,four-chamber view and long axis view.Results Both MRI and echocardiography displayed involvement of left ventricles in thirteen cases and involvement of double ventricles in one case.Apexes of heart and the intermedius are commonly affected.MRI showed 54 segments and echocardiography showed 53 segments affected,and there is no significant difference between the capability of MRI and echocardiography(P=1.000).The affected myocardium consisted of two layers:subendoeardial noncompacted myocardium and epicardial compacted myocardium,and the ratio measurement of N/C by MRI was 3.37?0.89 and it was 3.19?0.82 by echocardiography.Noncompacted myocardium was characterized by prominent and excessive myocardial trabeculations and deep intratrabecular recesses,in which the blood flow was communicated with the ventricle.One case was complicated with ventricular aneurysm,and coronary arteriography was performed with unremarkable findings.One case underwent heart transplantation because of progressive heart failure, Gross findings demonstrated prominent muscular trabeculations with deep intratrabecular recesses,which coincided well with MRI findings.Conclusion The MRI manifestation of NVM is characteristic,and MRI with multiple series and planes is helpful in the diagnose of NVM.Compared with echoeardiography,MRI could display the pathological cardiac muscle more clearly,because of its high soft-tissue resolution and spatial resolution.

The mRNA levels of adiponectin in adipose tissue and adiponectin receptor R1 in skeletal muscle of type 2 diabetic rats were examined by semi-quatitative RT-PCR.The expression of adiponectin receptor R1 was not altered in the skeletal muscle of type 2 diabetic rats as compared with normal rats.The serum adiponectin level was decreased in diabetic rats due to the decline of adiponectin mRNA level in adipose tissue and rosiglitazone improved the adiponectin deficiency.
