1.Establishment and evaluation of indirect competitive enzyme-linked immunosorbent assay for detecting the concentration of trypsiongen activation peptides

Objective To establish an ELISA method for detection of the concentration of TAP in human urine samples.Methods TAP-BSA was used as coating antigen. The TAP was a competitor to TAP-BSA. They reacted to the limited amount of monoclonal antibody against TAP.Results The optimal concentration of the coating antigen was 250ng/ml.The dilutions of monolclonal antibody against TAP and sheep anti-mouse IgG were 25?g/ml and 1∶4 000 respectively. The optimal range was from 0.69 to 1 000ng/ml.The assay provided a sensitivity of 0.69ng/ml.The coefficients of variation of intra-assay and inter-assay were 9.10% and 10.33% respectively. The average recovery rate of TAP was 97.70%.Conclusion The indirect competitive enzyme-linked immunosorbent assay is established.
2.Application of confocal laser scanning microscopy in the diagnosis of superficial cutaneous fungal infections

Objective To estimate the performance of confocal laser scanning microscopy (CLSM) in the diagnosis of superficial cutaneous fungal infections. Methods This study recruited 59 patients with clinically suspected superficial cutaneous fungal infections.Three typical lesions were selected in each patient for CLSM and microscopic examination.Results CLSM revealed hyphae in stratum corneum in 56% (14/25) of tinea manus or pedis and 79.17% (19/24) of tinea cruris lesions,7 out of 8 tinea manus or pedis and 94.12%( 16/17 ) of tinea cruris early lesions (< 3 weeks ),and 41% (7/17) of tinea manus or pedis and 3 out of 7 tinea croris old lesions (> 3 weeks).All the CLSM-positive specimens were positive for microscopic examination,and among the CLSM-negative specimens,fungal elements were observed by microscopic examination in 8 out of 11 tinea manus or pedis specimens,4 out of 5 tinea cruris specimens,1 tinea manus or pedis and 1 tinea cruris specimen of early lesions,and 7 out of 10 tinea manus or pedis and 3 out of 4 tinea cruris specimens of old lesions.No hypha was found by CLSM in any of the 10 tinea versicolor specimens,while microscopic examination revealed fungal elements in 8 of them.Neither CLSM nor microscopy revealed fungal elements in lesions from 5 patients with tinea manus or pedis and 5 patients with tinea cruris after treatment with topical bifonazole cream for 2 weeks.Conclusions CLSM shows a good consistency with light microscopy in the examination of early lesions of tinea marnus and pedis as well as tinea croris,and may serve as a valuable tool for clinical diagnosis.
3.The influence of early motor cardiac rehabilitation on the life quality of acute myocardial infarction patients

Objective To observe the influence of early motor cardiac rehabilitation on the life quality of acute myocardial infarction patients. Methods 30 acute myocardial infarction patients underwent percutaneous transluminal coronary intervention (PCI) were randomly divided into rehabilitative group and control group which consist of 15 patients. The rehabilitative group received the individual sports programme and health education in addition to conventional treatment and nursing. The control group was just treated by traditional way. Barthel index was used to evaluate the self-care ability and life quality. Results The rehabilitative group had significantly higher self-care ability compared with control group after 30 days' treatment[(85.48 ±6.77)points vs (69.35 ±6.46)points] (P<0.05).The rehabilitative group could care by themselves and carry on the light physical activity. In addition, heart rehabilitatione frequency was the favorable factor for improving the life quality of acute myocardial infarction patients.Conclusion Early motor cardiac rehabilitation could effectively improve the life quality of acute myocardial infarction patients which had a wide clinical application.
4.Clinical analysis of 182 cases of traumatic and hemorrhagic shock

Objective To discuss the importance of saving the life of patients and the severity assessment in early emergency for the trauma and hemorrhagic shock patients in the first-aid of pre-hospital and emergency department.Methods Retrospective anal-ysis of the 182 patients data.Results With early and aggressive treatment,177 cases of survival,5 cases of death,and the survival rate 97.3%.Conclusion The disease of traumatic shock patients is complex,high mortality.the early detection,taking timely and effective rescue measures are the key to increase the survival rate.

6.Effect of phosphatidylinositol 3-kinase on inducible nitric oxide synthase in bronchiole epithelial cells of asthmatic rat

AIM: To investigate the effect of phosphatidylinositol 3-kinase (PI3K) inhibitor wortmannin on expression of inducible nitric oxide synthase (iNOS) in bronchiole epithelial cells of asthma rat.METHODS: 24 male Sprague-Dawley rats were randomly divided into 3 group, namely control group, asthma group and PI3K inhibitor wortmannin+asthma group. The numbers of total cells and eosinophil cells in bronchoalveolar lavage fluid (BALF) were counted. The expression of inducible nitric oxide synthase was detected by immunohistochemistry method, and iNOS mRNA was detected by reverse transcription-polymerase chain reaction (RT-PCR). The activity of PI3K, iNOS and content of NO in lung homogenates were tested by spectrophotometry.RESULTS: The numbers of eosinophils and the ratios of eosinophils to total cells in BALF in asthma group were significantly higher than those in control group, while those in wortmannin+asthma group were decreased compared to asthma group. The activity of PI3K, iNOS and content of NO in lung homogenats in asthma group were significantly higher than those in control group, while those in wortmannin+asthma group were decreased compared to asthma group. The expression of iNOS protein in bronchiole epithelial cells and mRNA in lung homogenates in asthma group were markedly increased compared to control group, while those in wortmannin+asthma group were decreased compared to asthma group.CONCLUSION: PI3K regulates the expression of iNOS in airway of asthma rat and affects inflammatory reaction in airway.
7.Clinical features of Gorham-Stout syndrome and literature review

Objective To describe the clinical presentations,radiographic findings and histological pathology of bones,diagnosis,treatment options and prognosis for patients with Gorham-Stout syndrome (GSS).Methods Clinical data of 5 GSS patients seen from January 1980 to January 2008 were reviewed.Results(1)There were 2 males and 3 females,aged 15 years to 37 years(mean age was 30.2 years).(2)All of thern had osteolysis,but the site and extent of involved bones were not the same.Three cases had large amount of bloody pieural effusion and two of them had also chylous effusion.All of the 5 cases had no evidence of malignancies.Four cases accepted bone biopsy.Among them,2 cases having local puncture and open biopsy showed typical bone pathologic manifestations.(3)Various forms of treatment including bisphosphonates,calcium supplementation,active vitD3 treatment,local radiation therapy and surgical ligation of thoracic duct were tried.(4)Follow up and clinical outcomes:the two cases,who had only bone osteolysis remained stable.Of the other three cases who had bone osteolysis associated with pleural effusion,one patient needed interrupted effusion drainage with stable bone impairment and the other two cases were out of contact.Conclusions GSS is a rare disorder charactcrized by progressive osteoIysis.The clinical presentations of this disease are variable and depend on the sites of involvement.There were no standard therapy available.Prognosis depends on the site of involvement,extent of the disease and presence of complications.Those who have plueral effusion had poor prognosis.
8.A biomechanical study of upper thoracic vertebras translaminar screws fixation

Objective To compare the biomechanical pull-out strength (POS) of three different fixations in upper thoracic vertebras using translaminar screws (TLS), translaminar facet screws (TLFS), and transpedicle screws (TPS), respectively. Methods Nine fresh human cadaveric cervicothoracic junction spines specimens which including T1-T3 vertebras were harvested. The vertebras specimens were scanned using dual-energy radiograph absorptiometry for bone mineral density. Both of screw insertion techniques at each vertebrae was randomized. All the screw insertions were based on direct observation and the CT scan on the pedicles. The peak of insertional torque (IT) was recorded and axial pull-out testing was performed to simulate intraoperative failure of fixation. Results The mean peak IT of the TFLS, TPS and TLS were (0.43±0.01), (0.40±0.01), (0.35±).01) N·m, respectively. There was no statistically significant difference between the TFLS and TPS, and between the TPS and TLS was same. But the TFLS generated statistically greater peak 1T in comparison with the TLS(t=-13.86, P<0.05). The mean POS of TLFS was (771±106) N,which had no statistically significant difference in comparison with the TPS(733±65) N. And the TLS (663±86) N was same. But the TFLS generated statistically greater POS in comparison with the TLS (t=9.907, P<0.05). The peak IT showed a strong positive correlation with POS in three screw techniques. Bone mineral density correlation with POS in all methods of fixation. Conclusion It was not a significant difference to compare POS of TLS and TLFS to that of TPS respectively. TLS and TLFS appear to be a biomechanically sound alternative in the upper thoracic spine, and appear to be a safe and effective technique for instrumenting the upper thoracic spine.
9.Study on in vivo pharmacokinetics of cucurbitacin injection in rats.

To establish a method for the determination of cucurbitacin in plasma samples, in order to study the in vivo pharmacokinetic characteristics of cucurbitacin in rats. Rats were intravenously injected with cucurbitacin. With diphenhydramine as the internal standard (IS), the plasma concentrations of cucurbitacin in rat plasma at different time points were determined by liquid chromatography tandem mass spectrometry (LC-MS/MS). With electrospray ionization source, the positive ion detection in the multiple reaction monitoring mode was conducted to determine the ion-pairs for target compound and IS were m/z 503.2/113.1 and m/z 256.0/167.2, respectively. Agilent ZOBAX SB-C18 column (2.1 mm x 50 mm, 1.8 microm) was adopted and eluted with methanol and 0.1% formic acid (55:45), and the flow rate was 0.2 mL x min(-1). DAS 2.0 software was applied to fit the blood concentration and calculate corresponding pharmacokinetic parameters. The rats were intravenously injected with cucurbitacin at the concentration of 3.0 mg x kg(-1). The target blood quality concentration show good linear relations within the range of 10.5-3 150 microg x L(-1) (R2 = 0.996), the lower limit of the standard curve was 10.5 microg x L(-1), and the signal to noise ratio S/N = 12. Intra- and inter-day precisions RSD was less than 6.9% and 14%, respectively; The accuracy RE ranged between 0.20% and 3.7%; The extraction recoveries ranged between 92.7% and 97.1%. Regarding the pharmacokinetic parameters of tail intravenous injection of cucurbitacin, AUC (0-t) was (811.615 +/- 111.578) microg x h x L(-1), (t1/2) was (1.285 +/- 1.390) h, CL was (3.627 +/- 0.487) L x h x kg(-1), and V(d) was (6.721 +/- 7.429) L x kg(-1). In this study, researchers established a simple, accurate, sensitive and highly specific method for determining the blood concentration of cucurbitacin, and reported the in vivo pharmacokinetic characteristics of cucurbitacin in rats for the first time.

10.Real-time myocardial contrast echocardiography with multi-indexes for the detection of coronary artery stenosis: comparison with gated single photon emission computed tomography

Objective To evaluate value of multi-indexes of real-time myocardial contrast echocardiography (RT-MCE) for the detection of coronary artery disease(CAD). Methods A total of 35 patients scheduled for coronary angiography underwent RT-MCE, and were undergone gated single photon emission computed tomography(gated-SPECT) after RT-MCE shortly. Coronary angiography was performed within one week of RT-MCE in all patients. All patients were divided CAD and no-CAD group. The observing indexes included: (1)The images of RT-MCE were analyzed quantitatively from microbubble replenishment curves for myocardial perfusion by using the Qlab software; A,β and A×β were compared between two groups. The sensitivity and specificity of RT-MCE for detection of CAD were performance by ROC curves Gensini score were calculated in 22 patients. Then the correlation analysis was used for comparing the value of A,β and A×β with Gensini score. (2) The sensitivity and specificity of gated-SPECT and RT-MCE for assessment of CAD were calculated by 4 score method. (3) RT-MCE for the detection of coronary artery stenosis with multi-indexes. Results (1) A,β and A×β were significant difference between two groups. The cutoff of A,β and A×β was 4. 58,0. 64 and 2. 73,then the sensitivity and specificity of RT-MCE for detection of CAD were 86. 0% , 80. 2% , 88. 9% and 84.1 % , 64. 6% , 79. 9 % , respectively. The correlation index was - 0. 79, - 0. 51 and - 0. 76 comparing the results of A, β and A ×β with Gensini. (2) The sensitivity and specificity of gated-SPECT for assessment of CAD were 84. 8 % and 82. 7% ,respectively. (3) The sensitivity of multi-indexes RT-MCE increased, the sensitivity was 89. l%,90. 4% and 96.3% when combinated A and β,A and A×β,and β and A×β, respectively. Conclusions RT-MCE with multi-indexes has a valuable application for assessment of CAD. The severity of CAD could be evaluated by RT-MCE.
