Objective: To explore the changes of cerebrovascular reserve capacity in patients with symptomatic unilateral high-grade carotid stenosis before and after angioplasty and stenting (CAS). Methods: The cerebrovascular reserve capability was evaluated with transcranial Doppler (TCD) ultrasonography for hypercapnia induced by breath holding, which affected the blood velocity in middle cerebral artery. The breath-holding index (BHI) in 15 symptomatic patients with unilateral severe carotid stenosis and 15 asymptomatic patients with carotid stenosis and 10 normal controls were compared, and the changes of the BHI in 15 symptomatic patients with carotid stenosis before and after CAS were compared and analyzed. Results: The BHI of the stenotic side in symptomatic patients with unilateral carotid stenosis (0.5 ± 0.2) was significantly lower than that in asymptomatic patients with carotid stenosis (0.9 ± 0.3, P < 0.001) and normal controls (1.1 ± 0.3, P < 0.001); and there were no significant differences between the BHI of the unaffected side in symptomatic patients with unilateral carotid stenosis (0.9 ± 0.3) and the unaffected side in asymptomatic patients with carotid stenosis (1.1 ± 0.2) and in normal controls (1.1 ± 0.3, P > 0.05). The BHI of the stenotic side in symptomatic patients with unilateral carotid stenosis improved significantly after CAS (1.1 ± 0.2, P < 0.001) as compared with before operation, however, there was no significant change in the BHI of the unaffected side (P > 0.05). Conclusion: The significantly impaired cerebrovascular reserve capacity on the stenotic side in symptomatic patients with unilateral high-grade carotid stenosis was significantly improved after CAS.

Objective To study the relationship between apoptosis regulation and epidermal proliferation in psoriasis. Methods The expression of apoptosis molecule PDCD5 (programmed cell death 5) on psoriatic lesions was observed by direct immunofluorescence. The positive rates and the average of fluorescence intensity of PDCD5 were analyzed quantitatively with FACS and computer CELL Quest software. The expression of PDCD5 mRNA in psoriatic lesions was detected by RT-PCR. Results The expression of PDCD5 protein was obviously lower in psoriatic epidermal cells than that of the normal skin. Objective To investigate the expression of paired box2(Pax2),proliferation cell nuclear antigen(PCNA) and cell apoptosis in steroid-sensitive and steroid-resistant groups with primary nephrotic syndrome(PNS) and to find out the action of Pax2 expression in PNS with steroid-resistance.Method The expressions of Pax2,PCNA were evaluated by immunohistochemistry and cell apoptosis by fluorescence micoscope.Results Pax2 expression in renal tubule had a positive correlation with PCNA expression in steroid-sensitive group.In steroid-resistant group,Pax2 expression had no correlation with PCNA.Pax2 had a negative correlation with cell apoptosis.Conclusions Pax2 proper expression facilitate PCNA expression and repair tubulointerstitial lesions in steroid-sensitive group.Renal tubular epithelial cell proliferation coordinated with cell apoptosis.Pax2 overexpression in steroid-resistant group lead to the decrease of cell proliferation and cell apoptosis and lead to the severe tubule lesions,which made to glucocorticoid resistance.

Objective To investigate the imaging features and preoperative staging value of ultrasonic endoscopy and gastrointestinal ultrasonic contrast in the gastric carcinoma examination.Methods One hundred and sixty eight patients with gastric carcinoma verified by operation and pathology in our hospital from October 2010 to October 2015 werecollected.Then their imageological data including ultrasonic endoscopy and gastrointestinal ultrasonic contrast examinations were retrospectively analyzed and compared with the postoperative pathological results.Results The accuracy rate of preoperative T staging in ultrasonic endoscopy was significantly higher than that of gastrointestinal ultrasonic contrast,the difference had statistical significance(P＜0.05);the accuracy rate of preoperative N/M staging in gastrointestinal ultrasonic contrast was s1gnificantly higher than that in ultrasonic endoscopy,the difference had statistical significance(P＜0.05).Conclusion Ultrsonic endoscopy and gastrointestinal ultrasonic contrast have different advantages and values in preoperative staging:ultrasonic endoscopy has higher accuracy rate in the diagnosis of primary gastric carcinoma;gastrointestinal ultrasonic contrast is better in diagnosing metastasis of gastric carcinoma,moreover which shows the consistency with postoperative pathological staging.

Objective To study the relationship between radiation gastritis ( RG ) caused by esophageal cancer radical postoperative prophylactic IMRT and the dose of gastric. Methods A total of 49 esophageal cancer patients received postoperative adjuvant radiatherapy after esophagectomy, we analysised the dose?volume histogram of treatment plan and the the occurrence of RG, To analysis those physical indicators may be related to the occurrence of RG. The of ROC curve to analysis the physical index of the occurrence of acute radiation gastritis, Chi square test is compared between groups, Logistic method was used to multivariate analysis. Results The whole group, a total of 19 cases ( 39%) patients with symptoms of acute RG level 2 or higher. The results of ROC curve analysis showed that the physical variables associated with the occurrence of RG include stomach Max,stomach Mean,LSTT5?LSTT40 and V5?V50.According to the ROC curve,the cutoff values of the physical parameters of the patients were grouped. The incidence of acute≥2RG in the group of above boundary value patients significantly higher than below boundary value group, the physical index of the differences between groups were significant (P=0. 000?0. 022),except V5.The results of Logistic multivariate analysis showed that LSTT5 and V40 were independent predictor of≥2 RG (P=0. 026,0. 001). Conclusions According to the results of this study,We should pay attention to the protection of the stomach in develop a treatment plan for the prevention of postoperative patients with esophageal cancer,dose?volume histogram can well predict the occurrence of RG. Stomach LSTT5 and V40 were independent predictor of≥2 RG.The results of this study need to be further studied.

Objective To evaluate global and segmental right ventricular ( RV ) systolic functions in patients with excessive volume or pressure load using real‐time three‐dimensional echocardiography ( RT‐3DE) . Methods Forty‐five patients with RV volume overload ,45 patients with RV pressure overload and 45 healthy subjects were underwent RT‐3DE . RV global and segmental ( inflow ,body ,outflow ) end‐diastolic volume (EDV) ,end‐systolic volume (ESV) ,stroke volume (SV) and ejection fraction (EF) were analyzed with TomTec software . The correlations between EF with the three‐dimensional method and two‐dimensional parameters including right ventricle systolic pressure( RVSP) were discussed . Results Global EDV and ESV increased significantly in both patient groups compared with controls ( all P < 0 .05) ,but there was no difference between two patient groups ( P >0 .05) .Compensated increase of SV was found in sixty percent of patients with volume overload but none with pressure overload ( P < 0 .05) . Global EF decreased significantly in both of patient groups (all P <0 .05) ,which was more significant patients with pressure overload ( P < 0 .05 ) . Different patterns of the regional dysfunction were found among the different RV segments . No correlation was found between RVSP and global or segmental EF in patients with pressure overload . Conclusions RT‐3DE could be used to assess global and segmental RV systolic function in patients with pressure and volume overload .

Background Inherited retinal degeneration,one of the major causes of blindness worldwide,comprises a large number of disorders characterized by a slow and progressive retinal degeneration.Interleukin-1 (IL-1)was recognized to be involved in inherited retinal degeneration.Whether IL-1 receptor antagonist (IL-1ra) can arrest retinal degeneration is worthy of investigation.Objective This study was to investigate the effects of IL-1ra on photoreceptor apoptosis in Royal College of Surgeons (RCS) rats.Methods The use of the animals complied with the Regulations for the Administration of Affairs Concerning Experimental Animals by State Science and Technology Commission.The SPF RCS rats aged 9,15,16,25,30,35,40,50 and 60 postnatal days were collected,with 9 rats for each age group.Retinal sections were used for the TdT-mediated biotin-dUTP nick-end labeling (TUNEL) cell apoptosis assay.1 μl of IL-1ra (1.8 g/L) was intravitreally injected in the right eyes of 9 RCS rats aged 15 postnatal days and PBS was used in the same way in the fellow eyes.The injection procedure was repeated on the 20 th and 25 th day,respectively.The rats were sacrificed on the 30 th day and retinal sections were prepared for the TUNEL assay.The differences in the percentage of the positive cellular nuclei area among different ages of rats were compared by one-way ANOVA,and the differences in retinal layer thickness between IL-1ra injection group and PBS injection group were assessed by paired t test.Results Photoreceptor apoptosis appeared in 20-day-old RCS rats and progressed and peaked in 30 and 35-day-old rats and then reduced,showing a significant difference among rat of various age groups (F=28.020,P＜0.01).Images from TUNEL assay showed a weaker and less TUNEL staining in the IL-1ra injected eyes than the PBS injected eyes in 30-day-old rats.The total area and relative area of TUNEL positive nuclei were (223.052±153.092) μm2 and (2.206±1.531) ％ in the IL-1ra injected group,and those in PBS injected group were (1235.050±359.767) μm2 and (7.269± 1.624) ％,with a significant difference between them (t =4.370,t=3.250,P＜0.01).The cone and rod thickness was (15.324±9.035) μm in the IL-1ra injected group and (49.566±4.605)μm in the PBS injected group,showing a significant difference (t =22.674,P＜0.01).However,no significant difference was seen in the outer nuclear layer thickness between the two groups (t =0.268,P＞0.05).Conclusions IL-1 participates in the pathogenesis and development of inherited retinal degeneration of RCS rats.The use of IL-1ra might be a potential approach in the treatment of inherited retinal degeneration.
