1.Application of laparoscopic and endoscopic cooperative surgery in treatment of gastric stromal tumor

Gastrointestinal stromal tumor (GIST) is the most common mesenchymal tumor in the GI tract. Surgical resection of the local disease is the gold standard therapy. In recent years, in virtue of the development of minimal invasive surgery such as laparoscopic surgery and endoscopic surgery, laparoscopic and endoscopic cooperative surgery (LECS) has been applicable for treatment of gastric GIST. This article reviewed the recent advances in LECS in treatment of gastric GIST.
2.Current status of multidisciplinary management for adenocarcinoma of esophagogastric junction

The incidence of adenocarcinoma of esophagogastric junction is increasing worldwide during recent decades.The therapeutic strategies have been transformed from surgery alone to multimodal treatments involing also perioperative chemoradiotherapy.Although there are still many problems on preoperative staging,surgical approach,excisional extent and perioperative chemoradiotherapy regimen,multidisciplinary team collaboration can provide an optimal diagnosis and treatment to achieve the principles of standardization and individualization in malignant tumor,which will prolong the survival and improve the quality of life for every patient.
3.Damages of blood-retinal barrier ultrastructure after infrasound exposure

Objective To investigate the possible damaging effect of infrasound on ultrastructure and permeability of rats′ blood retinal barrier (BRB). Methods Twenty mature male rats, averagely divided into 5 groups according to the exposure duration, were exposed to infrasound at a 16 Hz frequency and 130 dB sound pressure level in a pressure chamber for 2 hours per day. After exposed for 0, 1 day, 7, 14, and 21 days respectively, ultrastructural changes of rats′ BRB were observed through injection of lanthanum (La) nitrate solution, which was used as a tracer to demonstrate the breakdown of the BRB. Results With prolonging the duration of infrasound exposure, BRB structure lesion, chondriosome tumefaction, endoplasmic reticulum expandedness, membrane disc damage, retinal pigment epithelial cells distortion and putrescence, karyotheca expandedness, and La leakage on each level of retina aggravated gradually. Conclusion Infrasound may cause the breakdown of BRB, and the lesions aggravated with prolonged infrasound exposure time.
4.High RNA accumulation in Candida tropicalis is affected by specific growth rate and different medium composition

Batch and continuous fermentation were adopted to investigate the effect of specific growth rate and amino acid components on RNA accumulation in Candida tropicalis ATCC 20408 in fermentation medium ( FM), yeast peptone dextrose medium (YPD), molasses fermentation medium ( MFM) and FM without corn steep liquor. The data showed that obvious differences in intracellular RNA accumulation were observed at different cell growth phases in bath fermentation prosess, and RNA level reached 11. 8% (g-RNA /g-DCW) during exponential phase, and only 6.9% during stationary phases. It was also found that intracellular RNA accumulation increased with the increase of specific growth rate in continue fermentation prosess, and the highest RNA level reached 15. 6% with the glucose conversion rate of 42. 8% at the dilution rate of 0. 5 h-1. Furthermore, the data showed that RNA lever was notably increased in batch fermentation process when amino acids or peptone was added into the fermentation medium containing no corn steep liquor. Taken together, it was reported for the first time that specific growth rate and amino acid components plays a leading role on the intracellular RNA accumulation in C. tropica lis, and specific growth rate is more important.
5.Adherent-invasive Escherichia coli isolated from the patient with inflammatory bowel disease in China

Objective To study adhesion and invasion of adherent-invasive Escherichia coli strains and its ability of survival and proliferation in macrophage cells.Methods Bacteria from the mucosa tissues of ileum,colon and rectum of inflammatory bowel disease (IBD)patients were isolated,cultured and identified.The toxic genes were confirmed by polymerase chain reaction.The adhesion and invasion ability of bacteria and its survival and proliferation ability in macrophage cells were observed after co-culture with cells.Results Five adherent-invasive Escherichia coli strains were isolated from the intestinal mucosa specimens of two patients with Crohn′s disease and one patient with ulcerative colitis.The adhesion rates of five bacteria stains (2A,2B,15A,15B,19B)to Caco-2 cells were (1 .4 ± 1 .3 )%,(1 .5 ± 0.6 )%, (0.6±0.1)%,(1 .4±0.4)% and (1 .1 ±0.8)%,respectively.The adhesion rates to Int-407 cells were (1 .0±0.8)%,(1 .5 ±0.8)%,(1 .0 ±0.8 )%,(1 .0 ±0.8 )% and (0.3 ±0.0 )%,respectively.The invasion rates to Hep-2 cells were (10.1 ±7.0)%,(0.7 ±0.4)%,(0.4 ±0.3 )%,(2.2 ±1 .0)% and (2.1 ±1 .8)%,respectively.The invasion rates to Int-407 cells were (0.7 ±0.5 )%,(0.5 ±0.3 )%, (2.8±1 .2 )%,(0.7 ±0.5 )% and (0.5 ±0.4)%,respectively.All five adherent-invasive Escherichia coli strains could survive and proliferate in macrophage cells.Conclusions Five adherent-invasive Escherichia coli strains are isolated for the first time in Chinese patients with IBD,and all of them could survive and proliferate in macrophage cells.
6.Electrical pain thresholds of face and finger in patients with chronic masticatory muscle pain.

OBJECTIVETo compare electrical sensory threshold (EST) and electrical pain threshold (EPT) of bilateral masseters, temporal muscles and finger region of chronic masticatory muscle pain (MMP) patients and the controls, and to analyze the mechanism of pain threshold change.
METHODSTwelve patients with MMP and 12 matched controls participated in this study. EST and EPT of bilateral masseters, bilateral temporal muscles, and left middle finger were recorded and compared within group and between groups.
RESULTSCompared to the controls, the EPTs of patients significantly decreased [left masseter (5.9 +/- 1.1) mA,right masseter (5.7 +/- 1.0) mA, left temporal muscle (5.3 +/- 1.4) mA, right temporal muscle (5.5 +/- 1.7) mA,finger (5.2 +/- 1.0) mA] (P < 0.001). There was no significant difference in EST between patients and the controls. No relationship was found among VAS, ESP, and EPT.
CONCLUSIONSDecreased pain thresholds of masseters, temporal muscles and fingers in MMP indicated that peripheral sensitization was induced in MMP patients. Decreased pain threshold in patients meant that MMP might influence the central modulation of nervous system.

7.Clinical research of heart rate variability and deceleration capacity in children with idiopathic ventricular premature ;contraction

Objective To explore the relationship between heart rate variability (HRV) and deceleration capacity (DC) in children with idiopathic ventricular premature contraction of different origins. Methods The clinical data from 155 children with idiopathic ventricular premature contraction were retrospectively analyzed. According to the age, the children were divided into young children group (3 years old), preschool age group (3-6 years old) and school age group (6-16 years old). Each group was divided into the right ventricular type group and the left ventricular type group respectively according to the origin site. The differences of HRV and DC were compared among groups. Results Among three different age groups, the DC and time domain of HRV were signiifcantly different (P all?0 . 05 ). In young children group, RMSSD, HF, LF/HF, DC, and PNN 50 were statistically different between right ventricular type group and left ventricular type group (P all?0 . 05 ). In preschool age group, RMSSD, LF, HF, LF/HF, and DC were statistically different between right ventricular type group and left ventricular type group (P all?0 . 05 ). In school age group, the RMSSD, HF, LF/HF, and DC were statistically different between right ventricular type group and left ventricular type group (P all?0 . 05 ). Conclusions Children with idiopathic ventricular premature contraction have impaired regulations of cardiac autonomic system which mainly manifests as reduced tension of vagus nerve. Ventricular premature contraction originated from ventricle preponderance (young children and preschool children are right preponderance while school children are left preponderance) increases the risk of malignant arrhythmia.
8.Establishment and application of TaqMan real-time RT-PCR for the detection of hepatitis A virus.

OBJECTIVETo establish a specific TaqMan-based Real-time PCR assay for the detection of hepatitis A virus in serum samples.
METHODSAccording to the references, primers-probe sets which were located in 5'-NCR, the most conservative part of HAV genome were designed and therefore we established a TaqMan real-time RT-PCR assay with great performance of specificity, sensitivity and reproducibility. And then it was used in the detection of HAV RNA in serum from HAV patients.
RESULTSThe HAV Real-time RT-PCR assay established in this study were able to detect HAV RNA and its detection limit ranged from 0.1CCID50/reaction to 0.01CCID50/reaction. When the detection of a same sample was repeated for three times, coefficients of varistion (CV) of intra- and inter-assay were calculated and they were all less than 2.0% and 2.6% respectively. Our data suggested that there were 5.18 x 10(2) - 4.93 x 10(7) RNA copies in 1 ml of the serum from acute HAV patients.
CONCLUSIONThe TaqMan-based Real-time PCR assay established in this study was specific and precise for the rapid detection of HAV RNA. It was applied successfully in the pathogen detection of clinical samples.

9.Detection of Helicobacter species related genes coding for 16S rRNA in the liver tissue of patients with chronic liver disease.

OBJECTIVETo investigate the infection of Helicobacter pylori (Hp) in the liver tissue of patients with chronic liver disease and the association between Hp and chronic liver disease.
METHODSLiver tissue samples were obtained by liver biopsy and surgical resection from 30 healthy subjects, 30 patients with chronic hepatitis, 30 with cirrhosis and 30 with liver cancer. All the samples were confirmed by pathological examination. The gene fragment coding for 16SrRNA were amplified by PCR with sequence analysis.
RESULTSThe PCR product of the 16SrRNA gene was 109 bp in length. Hp 16SrRNA was detected in 18 out of 30 liver biopsy samples from patients with primary cancer (60.0%), in 14 samples from patients with liver cirrhosis (47.0%), and in none of the samples from normal subjects or patients with chronic hepatitis. Sequencing analysis of Hp 16SrRNA gene in the liver tissue showed a 98.8% homology with the gene fragment encoding Hp 16SrRNA.
CONCLUSIONHp is identified in the liver tissue of patients with chronic liver disease, suggesting the possible correlation between Hp infection and hepatocellular carcinoma.

10.Clinicopathological features and management of gastrointestinal stromal tumors complicated with synchronous other alimentary malignant tumor.

OBJECTIVETo explore the clinicopathologic features, treatment and prognosis of gastrointestinal stromal tumor (GIST) complicated with synchronous other alimentary malignant tumors.
METHODSClinical data of 525 patients with GIST undergoing surgical treatment from August 2004 to November 2012 in Shanghai Renji Hospital were reviewed retrospectively, among whom 46 patients presented synchronous other alimentary malignancy.
RESULTSGIST and other alimentary malignancy coexisting cases were less likely to be screened out preoperatively (2.2%, 1/46) and associated with elder age (P=0.001), more likely arise from stomach (P=0.000), smaller tumor maximum diameter (P=0.000), and lower mitotic count (P=0.000). According to NIH postoperative risk classification, there were 36 at very low risk, 9 at low risk, and 1 at high risk. Although the risk of GIST recurrence was lower for GIST and other alimentary malignancy coexisting cases, their 5-year survival rate was lower than that of patients with GIST alone (36.1%VS. 82.2%, P=0.000).
CONCLUSIONSGIST patients complicated with synchronous alimentary malignant tumor are usually low or very low risk and has minimal impact on the prognosis. Survival depends primarily on the synchronous alimentary malignant tumors. Therefore, it is reasonable to lay emphasis on the treatment of the alimentary malignant tumor, and perform synchronous resection of GIST if possible.
