1.Effects and mechanisms of NECA inhibit endoplasmic reticulum stress to against myocardial ischemia reperfusion injury in rats

Objective Adenosine receptor agonist NECA has a certain myocardial protection, but the specific mechanism is not clear.This paper aimed to study the effect and mechanism of NECA inhibiting endoplasmic reticulum stress to against myocardial ischemia reperfusion injury in rats.Methods 56 Wistar rats of SPF grade were selected and divided into Sham group, I/R group, NECA group and TUDCA group through random number table method.We established the isolated rat heart ischemia reperfusion model by using the Langendorff device.Sham group: heart threaded but not ligated, Kerb-Henseleit buffer continuous infusion 170min;I/R group: heart stability 20min, ischemia 30min, reperfusion 2h;NECA group and TUDCA group: heart stability 20min, ischemia 30min, reperfusion 2h, perfusion solutions containing 0.1μmol/L NECA and 30μmol/L TUDCA were respectively given at 5min before reperfusion, and ended at 30min after reperfusion.Transmission electron microscope was used to evaluate alterations of the myocardial ultrastructures.Western blot analysis was used to detected the expression levels of endoplasmic reticulum stress IREl-XBPl signaling pathway marker protein IRE1α, XBP1s.Immunohistochemical staining was used to detect the expression of IRE1α.Results The results of transmission electron microscopy showed that most of the myofilament ruptured, sarcomere contracture deformation, visible mitochondrial vacuoles degeneration in I/R group, and injury in NECA group and TUDCA group were less than the I/R group, appeared as the filaments arranged more neat, sarcoma only had mild contracture.Immunohistochemical results showed that IRE1αpositive staining was not found in the sham group, and the area of positive staining of IRE1α in I/R group was significantly increased, while the NECA group and TUDCA group were significantly decreased.Compared to the Sham group, the expression level of IRE1α and XBP1s was significantly increased in I/R group(P<0.05);but compared with the expression level of IREα and XBP1s in I/R group(1.72±0.27, 0.97±0.19), the NECA group(1.14±0.16, 0.6±0.13) and the TUDCA(1.07±0.27, 0.58±0.15) group were significantly decreased(P<0.05).Conclusion NECA can reduce endoplasmic reticulum stress through inhibiting IREl-XBPl pathway to protect the myocardium.
2.Effect of lysophosphatldic aeid on blood-brain barrier permeability and its mechanism

Objective To explore the effect of lysophosphatidic acid(LPA)on blood-brain barrier(BBB) permeability and its possible mechanism.Methods LPA or LPA+suramin(L+S)were stereotaxically injected into the right eaudate nucleus in SD rats in vivo.Evans blue(EB)was used to quantitatively measure the permeability of BBB at different time points.The expression of matrix metalloproteinase-9 was detected by immunohistochemistry technique.The pathological ultrastruetural changes of BBB were assessed by transmission electron microscopy.Results The BBB permeability began to increase after LPA administered into ipsilateral eaudate nucleus,and reached the peak at 24h.Then the permeability of BBB gradually lowered after 48h.In comparison with the same time points of control group,there were quite significant differences(P<0.01).After L+S was injected,the change of BBB permeability had differences in comparison with those of LPA group in the same time points,(P<0.05).MMP-9 positive cells were mainly vascular endothelial cells.The numbers of MMP-9 positive blood vessels grew at 6h in LPA group,and the expression of it reached maximum at 24h,then the number of it decreased at 48h,showing significant statistical differences in comparison with the L+S group(P<0.01),It was observed microscopically that ultrastrueture of BBB of the LPA group was changed sharply,such as basement membrane roughed and fragmented,astroeyte end-feet swolled markedly and perivaseular space enlarged obviously.But there were no remarkable changes in BBB in L+S group.Conclusion LPA can induce increase of BBB permeability and its possible mechanism is the strong expression of MMP-9 protein produeted by endothelial cells through the mediation of LPA receptor,leading to degradation of basement membrane.

Genomic DNA was extracted from the blood samples of 3 patients from 1 pedigree with congenital nephrogenie diabetes insipidus (NDI) and their 12 family members.The whole coding region of the arginine vasopressin receptor 2 (AVPR2) gene was amplified by PCR and then directly sequenced,A mutation of AVPR2 gene [g1236T→C (L292P)]was found in 3 patients.The patients' mothers were found to have both mutant and normal alleles.
5.The effect of overweight or obesity on the negative conversion time of SARS-CoV-2 nucleic acid

Abstract: Objective To investigate the relationship between the body mass index (BMI) levels and the negative conversion time of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) nucleic acid in adult coronavirus disease 2019 (COVID-19) patients and the asymptomatic persons. Methods Asymptomatic infected patients and confirmed COVID-19 patients admitted to Chengdu Public Health Clinic Center from February 2021 to November 2021 were dynamically included. The epidemiological and clinical characteristics of the objects were collected, and the SARS-CoV-2 nucleic acid testing of the objects during their hospitalization was continuously monitored, and the negative nucleic acid conversion time was recorded. The t test or Wilcoxon rank sum test, χ2 test or Fisher's exact probability method examine were used to distribute characteristics of each group of variables and the connection between different variables, respectively. Then the variables showed differences in distribution (P<0.05) between different BMI groups were included in the multivariate Cox proportional risk regression model. Results A total of 253 subjects ranged from 18 to 63 years old, with M(P25, P75) age of 37.0 (30.0, 47.0) years old, were included in this study. The male to female ratio was 4.16 to 1. The BMI was (23.97±3.33) kg/m2. 50.59% (128/253) of the objects were overweight or obese, and 78.13% (100/128) were overweight. The negative time of SARS-CoV-2 nucleic acid conversion of all subjects ranged from 1 to 71 days, with M(P25, P75) of 7.0 (2.0, 18.0) days (P<0.001). The negative time of SARS-CoV-2 nucleic acid conversion of the normal weight or the thin, and the overweight or obese were 5.00 (2.00, 19.00) and 8.00 (2.00, 17.75) days respectively. The results of multivariate Cox's proportional hazards regression model showed that the BMI levels may not be associated with the negative conversion time of SARS-CoV-2 nucleic acid (HR=1.090, 95%CI: 0.843-1.410, P=0.510). Conclusions Adult asymptomatic persons and confirmed COVID-19 patients are mainly middle-aged and young males, and overweight or obesity is relatively common. Overweight or obesity cannot be considered as an independent factor influencing the negative conversion time of SARS-CoV-2 nucleic acid.
6.Clinical diagnosis and treatment of superior tibial fracture complicating with crotch injury of distal popliteal artery.

OBJECTIVETo investigate the clinical effects of treatment for superior tibial fracture complicating with crotch injury of distal popliteal artery by an alternative micro-surgical operation.
METHODSDuring Feb. 2002 to Oct. 2007, there were 19 patients with superior tibial fracture complicating with crotch injury of distal popliteal artery included 15 males and 4 females aged from 21 to 48 years (means 35 years). There were 6 cases complicating with fracture of tibial plateau, 3 cases complicating with nerve injury. The tibial fracture were fixed with external fixator and the anterior tibial artery and vein and posterior tibial artery and vein were treated by transplantable great saphenous vein (Y-shape) combined with windowing for interosseous membrane by the posterior and anterolateral incision. Evaluations of clinical effect were performed according to Rasmussen functional score system.
RESULTSAll patients survived after operation. All fracture achieved bony union, the union time was from 3 to 14 months (means 5.5 months). All patients were followed-up for from 8 to 23 months (means 13 months). The mean Rasmussen functional score was (27.0 + 2.9). The results were excellent in 11 cases, good in 7, and fair in 1.
CONCLUSIONThe surgical exploration should be done as soon as possible when diagnosis of injuries of large arteries is definite or highly suspected. Simultaneous reconstruction of both posterior tibial artery and anterior tibial artery associated with vein can reduce the rate of disability and recover function of limb.

7.Relationship between neurogenic urination and psychological status in school children.

OBJECTIVETo study whether anxiety and depression are associated with the development of neurogenic urination in children.
METHODSA total of 136 9 to 12-year-old children with neurogenic urination (case group) and 136 age-matched healthy children (control group) were enrolled. The Screen for Children Anxiety Related Emotion Disorders (SCARED) and Depression Self-rating Scale for Children (DSRSC) were used to evaluate the psychological status. The incidences of anxiety and depression as well as the SCARED and DSRSC scores were compared between two groups. Logistic regression analysis model was used to evaluate the relationship between psychological status and the development of neurogenic urination.
RESULTSThe case group was found to have a higher incidence of anxiety and depression compared with the control group (P<0.01). The SCARED score in the case group (28.1 ± 8.6) increased significantly compared with 14.4 ± 4.9 in the control group (P<0.01). The DSRSC score in the case group was also significantly higher than in the control group (13.5 ± 4.8 vs 9.1 ± 3.2; P<0.01). The logistic regression analysis showed that the children with anxiety (SCARED-score>23) had a 1.224-fold increased risk for the development of neurogenic urination compared with the children with the SCARED-score≤23 and that the children with depression (DSRSC-score≥15) had a 1.148-fold increased risk for the development of this disorder.
CONCLUSIONSAnxiety and depression participate in the development of neurogenic urination in school children.

8.Analysis of the curative effect of 162 cases of thoracic and lumbar tuberculosis.

OBJECTIVETo explore the differences between conservative and surgical treatment for thoracic and lumbar spine tuberculosis on indications and clinical results.
METHODSFrom May 2000 to June 2008, 162 cases of thoracic spinal tuberculosis patients were studied retrospectively, including 89 males and 73 females, aged from 4 to 71 years (means 43.6 years). Among them, 38 cases of onset time was short, X-ray showed a narrow cone gap and MRI showed vertebral body signal changes, the ones who had better general condition were applicated of anti-TB drugs [6HREZ (S)/6-9HRE] and traditional Chinese medicine dialectical; 51 cases with thick swollen and vertebral body marginal damage but not affect the stability of spine, were treated with spinal tuberculosis debridement surgery; 73 cases with abscess, vertebral destruction of center, spinal instability or spinal cord function associated with damage to persons, were treated with surgical debridement and interbody bone grafting and fixation. Clinical observation were evaluated according to the standard cure for bone tuberculosis.
RESULTSThirty-eight cases by conservative treatment, had been cured in 1 to 2 years. Focus of infection cleared in 51 cases, 3 cases occurrenced the sinus next to incision, 4 cases of kyphosis angle (Cobb) lost 5 degrees-10 degrees compared with that before treatment (means 4.5 degrees). Seventy-three cases by internal fixation obtained neurological deficiency symptoms recovery; 2 occurenced the sinus next to the incision, 3 cases appeared subcutaneous emphysema of iliac area and local pain, but need no special treatment. A total of 136 patients were followed-up from 12 to 60 months with an average of 32.8 months, and had been all clinically cured.
CONCLUSIONPatients with early detection only on the imaging showing vertebral lesions, without obvious sequestrum, abscesses, can be selected for conservative anti-tuberculosis treatment. Patients combined with abscess, vertebral destruction on light degree and not affected the stability of the spine, can be removed by simple surgery to obtain better efficacy. Patients with abscesses, sequestrum, spinal vertebral instability leading to heavy damage associated with spinal cord or nerve function impairment,will need surgical removal of lesions of tuberculosis, give graft and spinal fixation at the same time.

9.Efficacy comparison of different forms of dog-days acupoint application for prevention and treatment of chronic bronchitis

Objective: To analyze the clinical effect of traditionaldog-days acupoint application and Magic Acupuncture Patch (Manji) in the prevention and treatment of chronic bronchitis (CB) in remission stage in the past five years, and explore the principle of action and effective stimulation, to provide the evidence for treating CB by acupoint application. Methods: A retrospective study was conducted on 405 patients with CB who met the inclusion criteria. All patients were treated with dog-days acupoint application or Magic Acupuncture Patch between 2013 and 2017. The clinical data of 405 patients were statistically analyzed to compare the prevention and treatment effects of dog-days acupoint application and Magic Acupuncture Patch, and different degrees of stimulation of dog-days acupoint application. Results: Among the dog-days acupoint application groups, the total effective rate was 63.6% in the light stimulation group, 93.1% in the moderate stimulation group, and 94.8% in the strong stimulation group. The differences in the total effective rate between the light stimulation group and the moderate stimulation group, as well as the strong stimulation group, were statistically significant (both P<0.05). There was no significant difference in the total effective rate between the moderate stimulation group and the strong stimulation group (P>0.05). The total effective rate was 83.9% in the dog-days acupoint application group, versus 45.4% in the Magic Acupuncture Patch group, and there was a statistically significant difference between the two groups (P<0.05). Conclusion: The efficacy of dog-days acupoint application in the prevention and treatment of CB is better than that of Magic Acupuncture Patch; the degree of stimulation is the basis for the effect of dog-days acupoint application on prevention and treatment of CB, and the moderate and strong stimulations are more appropriate.
10.Effect of integrated nursing intervention on the pulmonary function of patients having thoracic tuberculosis transthoracic approach surgery

Objective To explore the effect of nursing intervention on the pulmonary function of patients having thoracic tuberculosis transthoracic approach surgery.Methods 80 cases of patients who were to have thoracic tuberculosis transthoracic approach surgery were randomly divided into the intervention group and the control group,each with 40 cases.The control group received conventional nursing before and after operation,while the intervention group received nursing intervention in addition.Their pulmonary function,arterial blood gas analysis,comparison of vital capacity (VC),maximal ventilatory volume (MVV),forced expiratory volume in one second (FEV1),a second volume than the forced vital capacity (FEV1/FVC),arterial partial pressure of oxygen (PaO2),arterial partial pressure of carbon dioxide (PaCO2) and arterial oxygen saturation (SaO2) were compared on the 1st day in hospital,1st and 7th day after operation.Results The indicators of pulmonary function on the 1st day in hospital had no significant differences between two groups (P > 0.05).The VC (t =6.326,5.276,respectively),MVV (t =5.958,4.742,respectively),FEV1 (t =5.565,4.356,respectively),FEV1/FVC (t =5.066,4.866,respectively),PaO2 (t =5.213,4.113,respectively) and SaO2 (t =8.937,6.837,respectively) on the 1st and 7th day after operation were all higher in the intervention group than in the control group,while the PaCO2 was higher in the control group than in the intervention group [(48.7 ± 3.1) % vs (40.7 ± 2.5) %,(44.3 ± 3.2) % vs (38.2 ± 2.4) %],and the differences were statistically significant (t =3.157,2.557,respectively ; P < 0.05).Conclusions Integrated nursing intervention can effectively improve the pulmonary function of patients having thoracic tuberculosis transthoracic approach surgery,and prevent pulmonary complications.
