2.Clinical observation of Artisan iris - clip intraocular lens implantation at stage Ⅰ

To observe the clinical efficacy of Artisan iris-clip intraocular lens ( lOL) implantation on cataract with a wide range of lens dislocation.
●METHODS: Twenty-four hospital patients (28 eyes) of cataract with lens subluxation by trauma (20 patients, 20 eyes) and Marfan's syndrome (4 patients, 8 eyes) during May 2007 to December 2011 were selected. All of them underwent phacoemulsification or an anterior vitrectomy and Artisan iris - clip lOL implantation at stage Ⅰ. Postoperative visual acuity, corneal endothelial cell count, intraocular lens position and postoperative complications were observed. Follow-up time was 6mo.
●RESULTS: Compared with the preoperative, postoperative best corrected visual acuity improved to varying degrees, visual acuity of 0. 1-0. 5 was in 20 eyes, 0. 5-1. 0 in 8 eyes. After followed up 1-6mo, no artificial lens shift, iris inflammation, iris atrophy and corneal decompensation and other complications were observed.
●CONCLUSlON: Cataract surgery with a wide range of lens dislocation is difficult, but as long as surgeons who have a wealth of clinical experience and superb skills, still can carry out phacoemulsification surgery or anterior vitrectomy and Artisan iris - clip lOL implantation and obtain a better outcome.
4.Chemical constituents of Artemisia frigida

Objective To study the chemical constituents of the CHCl3 and EtOAc extracts from Artemisia frigida.Methods The chemical constituents in A.frigida.were isolated with silica gel and LH-20 chromatography and their structures were identified by means of spectra,in same cases by direct comparison with authentic samples.Results Thirteen compounds were obtained and identified as quercetin(Ⅰ),luteolin(Ⅱ),5,7,3'-triterhydroxy-4'-methoxy flavone(Ⅲ),5,7,3'-triterhydroxy-6,4'-dimethoxy flavone(Ⅳ),5,3'-dihydroxy-6,7,4'-tritermethoxy flavone(Ⅴ),5,3'-dihydroxy-3,6,7,4'-tetramethoxy flavone(Ⅵ),methyl phenol(Ⅶ),7-hydroxy coumarin(Ⅷ),7-methoxy coumarin(Ⅸ),caffeic acid(Ⅹ),?-sitosterol(Ⅺ),6,7-dihydroxy coumarin(ⅩⅡ),and 7-hydroxy-5,6-dimethoxy coumarin(ⅩⅢ).Conclusion All these compounds are isolated from this plant for the first time.
5.Early-pathologic Changes of Gastric and Duodenal Mucosa in Children Infected by Different Types of Helicobacter Pylori

Objective To investigate the early-pathologic changes in children′s antrum infected with different types of Hp and study the Hp isolate′s pathogenic.Methods The serum types of CagA and VacA from Hp were determined by Westen-Blot in 70 patients with Hp positive and 36 patients with Hp negative.The standard of gastritis pathologic classification was accordance with that of international made in Sydney. The pathogenic of Hp affecting was evaluated by the degree of inflammation, severity of active gastritis,lymph follicles and atrophy.Results The detection rate of type Ⅰwith high virulence in Hp isolates was 68.1%,mid-type isolates was 27.7% and type Ⅱ with low virulence isolates was 4.2%.To observe the pathologic distinction in 49 patients with type Ⅰisolate,20 patients with mid-type isolate and 3 patients with type Ⅱ isolate,the type Ⅰ and mid-type isolates had significant difference in inflammation and their activity in either antrum or duodenal ampulla.Three patients with type Ⅱ isolate have not active gastritis.Type Ⅰand mid-type isolates had significant difference in lymph follicles,and the lymph follicles caused by type Ⅰwere significant higher than those caused by mid-type.But there were no significant differences in intestinal metaplasia and atrophy.Conclusions TypeⅠisolate with high virulence is the main detection isolate of children infected by Hp in our district.There is inflammation occurrence in antrum specimens in childhood who infected with Hp.
6.Effectiveness of ultrasound-guided botulinum toxin injection combined with orthosis training in the treatment of lower limb spasticity

Objective To observe the effect of ultrasound-guided injection of botulinum toxin type A (BTX-A) combined with orthosis training for the treatment of lower limb spasticity for children with cerebral palsy.Methods Fifty-four patients with spastic cerebral palsy were randomly divided into an observation group and a control group using a random number table.Patients in the control group were given conventional rehabilitation training,while those in the observation group were additionally given ultrasound-guided BTX-A injection,followed by daily knee-ankle-foot orthosis rehabilitation training 24 h after the injection.Before and 6 weeks after treatment the lower limb function of the 2 groups was evaluated using the modified Ashworth scale (MAS) and the gross motor function measure (GMFM).Adductor angles,popliteal angles and motor ranges of the ankle joint were also measured and compared.Results After 6 weeks the average MAS scores were lower than before treatment in both observation[(1.26±0.63) vs (3.07±0.68)] and control group [(2.56±0.71) vs (2.89±0.64).And in both groups the average GMFM scores,adductor angles and popliteal angles,as well as range of motion of the ankle were all much better than before treatment.The observation group,however,improved significantly more than the control group(P < 0.05).Conclusion Ultrasound-guided injection of BTX-A can effectively alleviate spasticity of lower extremity for children with cerebral palsy with accurate positioning,obvious curative effect and less adverse reactions.Knee-ankle-foot orthosis rehabilitation training after the injection can further alleviate spasticity and improve the motor function of the limbs.
7.Treatment of HBeAg positive chronic hepatitis B by xiaoyao powder combined with interferon-alpha: a clinical observation.

OBJECTIVETo study the efficacy of Xiaoyao Powder (XYP) combined with interferon alpha (IFN-alpha) in treating HBeAg positive chronic hepatitis B (CHB) patients and the effect on their quality of life (QOL).
METHODSTotally 193 patients with HBeAg-positive CHB confirmed by liver biopsy were randomly assigned to 2 groups, Group A (94 cases) and Group B (99 cases). IFN-alpha1b was subcutaneously injected to patients in Group A at the dose of 50 microg, thrice per week. Those in Group B additionally took XYP. The therapeutic course for all was 24 weeks. Clinical efficacy was observed by assessing ALT restoration rate, HBeAg negative rate, HBeAg conversion rate, HBV DNA negative rate, complete response rate, partial response rate, and symptoms integral. The evaluation of QOL was performed by using chronic liver disease questionnaire (CLDQ) score. Adverse reaction occurrence rate was observed in the two groups.
RESULTSBetter effects were obtained in Group A on ALT restoration rate, HBeAg negative rate, HBV DNA negative rate, complete response rate, partial response rate, TCM symptoms integral, the total effective rate of TCM sysmptoms, CLDQ score, and adverse reaction rates, showing statistical difference when compared with Group B (P < 0.05, P < 0.01).
CONCLUSIONXYP could elevate the efficacy of TCM symptoms of HBeAg-positive CHB patients and anti-viral effect, improve their QOL, and reduce adverse reaction of IFN-alpha.

8.The application of multi-slice spiral CT portography in pancreatic portal hypertension disease

Objective To study the application of multi-slice spiral CT(MSCT) portography in diagnonsis of pancreatic portal hypertension. Methods Forty-seven patients with lesion in body or tail of pancrease,47 normal subjects and 126 patients with portal hypertension underwent MSCT portography with LightSpeed 16 CT scanner. The inner diameter of portal system and the main collateral veins were measured in maximun intensity projection (MIP) image. The volume of liver and spleen were also measured in volume rendering (VR) image. The liver parenchyma and main portal vein enhancement in portal vein phase were also taken. The endoscopy examination was made in 57 patiens with portal hypertension. Results In 47 patiens with lesion in body or tail of pancreas , stenosis or occlusion of spleen vein were found in 38 patients(pancreatic portal hypertension in 27 patients, chronic and acute pancreatitis in 11 patients). In 38 patients with pancreatic portal hypertension, esophageal varices was found in 5 patients(13.2%), gastric fundus varix in 25 patients (65.8%), gastric body variees in 22 patients (57.9%), short-gastric vein/post-gastric vein(SGV/PGV) in 26 patients (68.4%), coronal gastric vein in 26 patients (68.4%),dilated gasto-omenta vein in 24 patients(63.2%), mesenterica varicesin 1 patient, splenic vein occlusion in 14 patients (36.8%), splenic vein stenosis in 23 patinets(63.2%). Conclusions The patients with pancreatic portal hypertension were demonstrate characteristic changes in MSCT portography. The MSCT portography is helpful in etiological diagnosis of pancreatic portal hypertension by supplying images in vessel morphology.
9.Correlation between the changes of neural cell apoptosis and caspase-3 gene expression after the brain traumatic injury in rats

Objective To observe the correlation between the changes of neural cell apoptosis arid caspase-3 gene expression in brain tissues following acute severe traumatic injury to brain(TIB).Method A total of 120 adult Spraque-Dawley rats were divided into a control group(n=8),TIB group(n=56)and TIB with administration of caspase-3 inhibitor group(n=56).TIB models of rats were made with Feeney's method.The z-DEVDfmk(5 μg),caspase-3 inhibitor,was administered by intracerebral infusion,and the rats were sacrificed 1,6,24,48 hours and 3,7,14 days postinjury(n=8 for each interval).The specimens of the injured cerebral cortex,suhcerticai white matter,hippocampus,dentate gyrus and contrahteral corresponding brain tissues were taken for detecting apoptesis of neural cells by the terminal deoxynucleotidyl transferase mediated DUTP nick end labeling (TUNEL)methods and flow cytomeay.Caspase-3 mRNA and protein expression were detected by using RT-PCR,immunohistochemistry and western blot analysis.The caspase-3 activity was detected by using caspase-3 fluorescent assay kit.Student t-test and Spearman correlation analysis were used to analyze the data with SPSS version 10.1 software package.Results Apoptesis indexes(AI)and the apoptesis percentage(AP)of neural cells in the injured brain regions increased quickly after injury,and reached its peak 24 to 48 hours later,then decreased slowly,but it remained at higher level above that of normal till 14 days later(P<0.01).The levels of caspase-3 mRNA,eastme-3 protein and caspase-3 activity were increased significantly post injury,and reached its peak at 24 to 48 hours,then it gradually decreased.Compared with control group,the levels ofoptical density of caspase-3 proteins in the injured hippocampus and subcortical white matter at 24 and 48 hours post injury increased 1484% and 1690%,caspase-3 mRNA expressiom increased 1043%and 1180%,and the degreas of caspase-3 activity increased 148% and 183%,respectively.The expression of caspase-3 proenzyme and its P17 subarrit increased.After trealment with caspase-3 inhibitor z-DEVD-fmk,the levels of caspase-3 mRNA,protein expression and caspase-3 activity were significantly decreased.and AI and AP were significantly decreased as well.The correlation between caspase-3 mRNA and level of neural apoptesis was positive(r=0.821,P<0.01),and it was likewise between caspase-3 protein and level of neural apoptosis(r=0.638.P<0.01).Interestingly enough,a positive correlation was found between caspase-3 mRNA and easpase-3 proteins(r=0.945,P<0.01).Conclusions The activation of caspase-3 leads to apoptosis of neural cells after acute TIB.The expression of caspase-3 are consistent with apoptosis of neural cells following TIB.The regulation of caspase-3 induced by TIB occurs at a ceriain critical link before transduction.Caspase-3 inhibitor can efficiently inhibit apoptosis of neural cells following TIB.
10.Construction of Saccharomyces cerevisiae haploid mutant deficient in lanosterol synthase gene.

Lanosterol synthase is encoded by the erg7 gene and catalyzes the cyclization of 2, 3-oxidosqualene, which is a rate-limiting step of the inherent mevalonate (MVA)/ergosterol metabolic pathway in Saccharomyces cerevisiae. The intermediate 2, 3-oxidosqualene is also the precursor of triterpenoids. Therefore, the cyclization of 2, 3-oxidosqualene is the key branch point of ergosterol and triterpenoids biosynthesis. Down-regulation of 2, 3-oxidosqualene metabolic flux to ergosterol in S. cerevisiae may redirect the metabolic flux toward the triterpenoid synthetic pathway reconstructed by the synthetic biology approach. To construct erg7 knockout cassette harboring the loxP-Marker-loxP element, long primers were designed, which were homologous to the sequences of both erg7 ORF and plasmid pUG66. The cassette was transformed into diploid wild strain INVSc1 by LiAc/SS Carrier DNA/PEG method and then erg7 gene haploid deficient mutant was obtained by homologous recombination. The results of semiquantitative PCR and real-time quantitative PCR revealed that erg7 expression level in erg7 gene haploid deficient mutant is one time lower than that in wild strain. The results of TLC and HPLC showed that the ergosterol content in deficient mutant decreased to 42% of that in wild strain.
