AlM:To investigate the features on prolonged central serous chorioretinopathy ( CSCR ) by optical coherent tomography ( OCT ) and to provide the basis of deciding the pathogenetic condition and prognosis. METHODS: Eighty - five patients who had been diagnosed with CSCR were grouped by suffering time as below: 32 patients suffered longer than 6mo as the prolonged and 53 patients with CSCR cured within that time. The imaging features of OCT were compared between the above groups.RESULTS:The incidence rate of neuroepithelial serous detachment extent above 500μm associated with pigmentary epithelial detachment in suffering eye and pigmentary epithelial damage in contralateral eye was significantly different between two groups. However, the incidence rate of neuroepithelial serous detachment extent above 4 000μm was not significant difference.CONCLUSlON:OCT could display clearly the change of every layer of retina with simplicity and visibility, which supplies us a new horizon to diagnose and trace CSCR. We could decide the pathogenetic condition and prognosis in accordance with the features of OCT, to provide references for the diagnosis and treatment of CSCR.

In this paper,the development trend of medical monitor system is analyzed.The portable trend and network function become more and more popular among all kinds of medical monitor devices.The architecture of medical network monitor system is provided: pocket medical monitor devices based on embedded platform is developed to interact with center database on networks;the telecommunication interface based on 802.11b/g and CDMA is designed to rebuild and extend the functions of traditional imaging devices(ultrasonic?CT?MRI) and the implementation details of terminal,monitor center software,distributed database and two kinds of medical information terminal are especially discussed;distributed medical database is designed for hospital center according to DICOM information model and HL7 standard;hand terminal based on WINCE is developed for nurse's routine care and doctor's auxiliary diagnosis.

Objective To investigate treatment of caesarean scar pregnancy (CSP) and associated problem.Methods From January 2011 to January 2013,57 cases with CSP undergoing treatment in Affiliated Hospital of Hebei University were studied retrospectively.Among 57 patients,it was divided into 12 cases treated by curettage (curettage group) ;22 cases treated by uterine artery embolization(embolization group) ;8 cases treated by laparotomy(laparotomy group) and 15 cases treated by transvaginal debridement (transvaginal group).Results (1) Successful rate of curettage group,embolization group,laparotomy group and transvaginal group were 12/12,86％ (19/22),7/8,13/15.(2) The intraoperative blood loss and the hospital stay were 20 (17-28) ml,(5.1 ± 1.9) days in curettage group,10 (5-15) ml,(10.2 ± 3.2) days in embolization group,200 (80-300) ml,(11.3 ± 3.3) days in laparotomy group,50 (45-100) ml,(6.8 ± 1.2) days in transvaginal group.There was statistically different between curettage group and embolization group (P ＜ 0.05).There was statistically different between laparotomy group and transvaginal group (P ＜ 0.05).Conclusions Curettage,uterine artery embolization,abdominal and transvaginal scar debridement surgery can be used in treatment of the CSP.It should be carefully chosen with surgical intervention,if myometrium was implanted greater than 1/2,or near to serosa.It should be carefully chosen with vaginal surgery,if the blood flow signals was rich around the lesion.

Objective To assess the value of combination of susceptibility weighted imaging (SWI)and dynamic susceptibility-weighted contrast-enhanced (DSC) perfusion-weighted magnetic resonance imaging in astrocytoma grading.Methods SWI and DSC scans were performed in 82 patients with pathologically confirmed astrocytoma.The patient group consisted of grade Ⅱ (15),grade Ⅲ (10),and grade Ⅳ (57).The intratumoral susceptibility signal intensity (ITSS) and relative cerebral blood volume (rCBV) max were used to determine the grade of astrocytomas by Kruskal Wallis test,Welch test,Spearman correlation coefficients,Pearson correlation coefficients,and receiver operating characteristic curve (ROC)statistic methods.Results There were no ITSS in 14 cases of low-grade astrocytomas,the degree of ITSS were grade 1 to 3 in anaplastic astrocytomas,the degree of ITSS were grade 3 in all of the glioblastomas,the degree of ITSS were significant difference in all grades (H =71.96,P ＜ 0.01).rCBV max in grade Ⅱ,grade Ⅲ and grade Ⅳ astrocytomas were 1.26 ± 0.42,3.59 ± 2.09 and 8.34 ± 1.16 respectively,rCBV max were significant difference in all grades (F' =681.72,P ＜ 0.01).ITSS showed significant correlation with rCBV max (r =0.72,P ＜0.01) and tumor grades (r =0.89,P ＜0.01),and rCBV and tumor grades showed significant correlation (r =0.78,P ＜ 0.01).Area under the ROC curve application SWI,DSC,SWI and DSC in differentiation of the grade Ⅱ and grade Ⅲ astrocytomas were 0.99,0.93,1.00,differentiate grade Ⅲ from grade Ⅳ were 0.70,0.94,0.94,and differentiate high-grade from low-grade astrocytomas were 1.00,0.99,1.00.Conclusions ITSS is helpful to determine the grade of astrocytomas.The use of SWI in combination with DSC may improve the diagnostic accuracy of astrocytoma grading.

OBJECTIVE:Taking metoprolol succinate sustained-release tablets as an example,to explore the value of speckle tracking imaging in therapeutic efficacy evaluation of CHF drugs. METHODS:68 CHF patients who had never underwent regular treatment were given Metoprolol succinate sustained-release tablets and anti-heart failure agents for 3 months. NYHA classification, resting heart rate(RHR),6 min walk distance(6MWD)and conventional echocardiographic measurements were tested before and 1 and 3 months after treatment. Dynamic image were collected in the apical 2-chamber view,apical 4-chamber view and left ven-tricular long axis plane for speckle tracking imaging offline analysis. The left ventricular global longitudinal strain (LVGLS) were measured. After 3 months of treatment,CHF patients were divided into standard group(group A,RHR 55-60 times/min)and non-standard group (group B,RHR>60 times/min) according to resting heart rate. The improvement of above indicators were com-pared between 2 groups,and that of LVGLS was also compared. RESULTS:After 1 month of treatment,RHR,LVEDd,LVEF and LVGLS of group A were improved significantly,compared with group B,with statistical significance (P<0.05). After 3 months of treatment,there was statistical significance in above indicators between 2 groups(P<0.05). In group A,RHR,6MWD and LVGLS improved significantly after 1 month of treatment,there was statistical significance,compared with before treatment (P<0.05);there was statistical significance in above indicators after 3 months of treatment,compared with before treatment and after 1 month of treatment (P<0.05). In group B,RHR and 6MWD improved significantly after one month of treatment;there was statistical significance,compared with before treatment(P<0.05);there was statistical significance in significant improvement of each indicator between after 3 months of treatment and before treatment (P<0.05);only there were statistical significance in RHR and NYHA grading,compared with after one month of treatment(P<0.05). CONCLUSIONS:Speckle tracking imaging can early reflect the left ventricular function improvement of CHF patients after anti-heart failure treatment,especially in patients with RHR control standard earlier and more significant. Speckle tracking had good clinical application value in the follow-up treatment of heart failure as routine evaluation index.
