Medullary thyroid carcinoma (MTC) is a rare endocrine tumor characterized by high malignancy, early metastasis and easy recurrence. Sporadic medullary thyroid carcinoma (sMTC) is the main pathological type of MTC, accounting for about 75%. Compared with hMTC which has more clear germline mutations in the RET gene, the pathogenesis of sMTC is still unclear. We mainly retrospectively reviewed the characteristics of gene variants and the associations of their gene variants with tumor risk, including RET gene variants, somatic RAS gene variants, CDKN gene variants and other gene variants, aiming to provide novel directions for the evaluation of patients in terms of etiology.

Objective To study the relationship of surgical procedures with clinical effect in herniat-ed lumbar disc in order to improve operative methods and obtain a better outcome. Methods Retrospec-tive analysis was carried out in7235patients with herniated lumbar disc, who had been operated by removal of nucleus pulposus using small incision and fenestration in our hospital since1983,313cases of whom re-ceived second operation because of postoperative complications. There were187males and126females aging from27to62years(mean,45.8years),the incidence of morbidity was4.32%.During the same period,552patients,who had less approved operative result primarily treated in other hospitals, were admitted to our hospital. There were317males and235females aging from31to64years(mean,46.0years).Results The postoperative complications could be divided into short term(within1month after operation)and mid-dle to long term(more than1month after operation)groups. The total446cases with short term compli-cation were133cases primarily treated in our hospital and313cases in other hospital, including of re-herni a-tion or incomplete nuclear extirpation in77treated with second operation, discitis in106treated with con-servative therapy for 67and second operative management for 39,canal hematoma compression in76treated surgi cally,multi-level herniation and leakage in21and mislocalization in59re-operated,injury of nerve root or cauda equina in85treated with nerve exploration, release and anastomosis, residual materials in canal in17with removal of foreign body, venous thrombus in2,and arachnoiditis in3.The total424cases with middle to long term complications were185cases primarily treated in our hospital and239cases in other hospitals, including of nerve root adhesion in159,recurred disc herniation in122,segmental instability in81,and ia-trogenic spinal stenosis in62.Conclusion Many factors may influence the outcome of herniated lum bar disc which can be abolished by sufficient preparation, careful operation and proper managememt.

Body fat of 12 male adults were measured by water displacement me-thod(density method) at every morning for 5 successive days. The standard deviation of single observation was 0.29kg calculated by mean residual lung volume method. It was significantly lower than the value (0.5kg) calculated by the ordinary method (p

International Eye Science 2014;(6):1095-1099
AIM:To explore the diagnosis and treatment methods of radioaction-induced optic neuritis ( RION) through the clinical dates of 17 patients.
METHODS: It was a retrospective case series study. From August 2008 to October 2013, 17 cases (24 eyes) of Rion clinical dates from Chinese PLA General Hospital were studied. The diagnosis methods including visual acuity, pupil, fundus, visual field, fundus fluorescein angiography (FFA), visual electrophysiological testing, and head MRI. To analysis the clinical date of patients with diagnosis of RION by statistical description.
RESULTS: The deterioration degree of vision: 13 eyes were classified as Ⅳ, 9 eyes as Ⅲ, 2 eyes as II. Ten eyes RAPD ( + ) , visual electrophysiology is extinguished. The retina of 5 eyes showed flame hemorrhages and cotton wool spots exudation. Optic nerve head edema in one eye. T1 - weighted MRI enhanced in 19 eyes which showed optic nerve of the intracranial and intratubal segments abnormal changed, optic chiasm and pituitary stalk signal abnormalities and enhancement of the optic nerve. Tortuous optic nerves and rough edges were observed in 5 eyes. Treatment effect: 4 eyes of visual acuity improved, 1 eye from blindness to light perception,1 eye from 0. 08 to 0. 2, 1 eye from 0. 4 to 0. 6,1 eye from 0. 04 to 0. 15, the rest of the cases did not see any improvement.
CONCLUSION: The unique clinical manifestation of RION can provide objective basis for clinical diagnosis in time, but there have not been proven any effective treatments.

A RP-HPLC method was established for simultaneous determination of phellodendrine hydrochloride (PH1), magnoflorine hydrochloride (MH), jatrorrhizine hydrochloride (JH), palmatine hydrochloride (PH2) and berberine hydrochloride (BH) in Phellodendri Chinensis Cortex by using ionic liquids as mobile phase additives. The separation was performed on a Kromasil C18 (4.6 mm x 250 mm, 5 μm) coupled with ultraviolet (UV) detection. The effect of extraction solvent, detection wavelength, length of alkyl chain on different imidazolium ionic liquids and concentration of ionic liquids on the separation and determination of alkaloids were investigated. Ionic liquid, [BMIm] BF4, can obviously improve the resolution and peak shape. This ILs-HPLC method is simple, rapid, and reliable, which can be used for determination of alkaloids in Phellodenddri Chinensis Cortex.

Objective To investigate the morbidity and characteristic of nosocomial infections in end stage renal disease(ESRD) patients undergoing blood purification. Methods Medical records of ESRD patients undergoing blood purification from 2004 to 2005 were enrolled in this retrospective study of hospitalized cases. The clinical data of nosocomial infections in hemodialysis (HD)and peritoneal dialysis(PD)patients were analyzed separately. Results Nosocomial infection was identified in 76 of the 400 enrolled patients. In HD patients, pulmonary infection was the most common nosocomial infection(53.3%),most due to gram-negative microorganisms, followed by bloodstream(16.7%)and urinary tract infection(15%),most of both were due to gram-positive bacteria.Pulmonary infection was usually complicated.Bloodstream infection was associated with the duration of placement of central vein catheters.Asymptomatic bacteriuria accounted for most of the urinary tract infection. In PD patients, most infections were pulmonary infection(65.5%),mainly caused by fungal pathogens, followed by peritonitis(20.7%), mainly due to gram-positive bacteria.Certain proportion of infections in both groups was caused by multiple microorganisms or identified in multiple sites. Conclusions The prevalence of nosocomial infection is high in hospitalized patients undergoing blood purification. The infection is complicated in terms of pathogen and clinical picture.Pulmonary infection is the most common in- fection. The prevalence of fungal infection is increasing.Effective prevention and therapeutic measures should be applied more vigorously in ESRD patients.
