1.Clinical Study on the Staging of Colorectal Carcinoma using Fat-dense Contrast Medium and Enhanced Multislice Spiral CT

Objective To assess the value of enhanced multislice spiral CT (MSCT) scan using fat-dense enema for the staging of colorectal carcinoma.Methods 33 cases with colorectal carcinoma confirmed by colonoscopy were examined by enhanced MSCT scan ,from the dome of the diaphragm to symphysis pubica,after bowel cleaning,administration of smooth muscle relaxant, and rectal fat-dense contrast agent insuffation .Carcinomas were staged with TNM by axial,multiplannar reconstruction(MPR) and CT virtual endoscopy (CTVE) images. The staging results on CT were compared with that of postoperative pathological examination in all cases.Results All the 33 colorectal carcinoma were demonstrated clearly by MSCT ,the total accurate rate in staging with the TNM classification was 78.78%.The sensitivity and positive accurate values for T staging were 100%and 87.88%(29/33),respectively. The sensitivity and positive accurate values for in detecting lymph node involvement were 86.36%(19/22)and 68.18%(15/22), respectively. Five cases in M stage were all diagnosed correctly. Conclusion Using fat-dense contrast medium and enhanced multislice spiral CT is of very important value in staging of colorectal carcinoma.
2.Analysis of the Application of Antibacterials in 36 Secondary Hospitals from Xi’an Area during Second Quarter in 2015

OBJECTIVE:To investigate the effectiveness of antibacterial clinical application special rectification activity,and to provide reference for rational use of antibacterials. METHODS:Referring to rational antibacterials application index stated in Na-tional Antibacterial Clinical Application Special Rectification Activity in 2013,the utilization of antibacterials in 36 secondary hospi-tals from Xi’an area during 2nd quarter in 2015 were analyzed retrospectively in terms of drug use index,types,consumption sum,DDDs and DDC,etc. RESULTS:Among 36 secondary hospitals from Xi’an area,there were only 3 hospitals up to the anti-bacterials use indicator standard(8.33%);antibacterial use of 91.66% inpatients was up to the standard;55.56%of antibacterials use density was up to the standard;50.00% of outpatient antibacterials drug prescriptions was up to the standard;77.78% of emer-gency antibacterials drug prescriptions was up to the standard;13.89% of antibacterials was used in typeⅠincision surgery for pro-phylactic use. Restricted use antibacterial accounted for 48.53% in consumption sum and Non-restricted use antibacterial accounted for 75.31% in DDDs. The consumption sum(12 771 265.34 yuan)and DDDs(291 542.31)of cephalosporin ranked first,mainly were second generation cephalosporin(accounting for 35.22% of consumption sum and 42.98% of DDDs). Top 10 drugs in the list of consumption sum were all injection;there was no special class of antibacterials but some antibacterias with high price and poor efficacy in top 10 ones. Top 10 drugs in the list of DDDs were non-restricted use and restricted use antibacterials with low price and good efficacy,among which there were 6 oral dosage forms. CONCLUSIONS:The application of antibacterials is still can not reach national standard in 36 secondary hospitals from Xi’an area. It is necessary to be further strengthen the management of inpa-tient antibacterial use density,the proportion of antibacterials used in outpatient and emergency patients,prophylactic application of antibacterials in type Ⅰ incision,the application of antibacterials with high price and poor therapeutic efficacy should be monitored closely.

5.Correlation of macrophage-related cytokines and silent information regulator 1 and forkhead box protein O3 levels in peripheral blood mononuclear cells in patients with active pulmonary tuberculosis

Abstract: Objective To explore the correlation between the levels of silent information regulator 1 (SIRT1) and forkhead box protein O3 (FOXO3) in peripheral blood mononuclear cells of patients with active pulmonary tuberculosis (APTB) and macrophage-related cytokines-inducible nitric oxide synthase (iNOS) and arginase-1 (Arg-1). Methods A total of 64 APTB patients who were treated in Yubei Hospital, the First Affiliated Hospital of Chongqing Medical University from January 2020 to December 2021 were gathered as the APTB group, 59 people with latent tuberculosis infection (LTBI) were gathered as the LTBI group, and 62 healthy people were gathered as the control group. Quantitative real-time PCR (qPCR) method was performed to measure the levels of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells. The enzyme-linked immunosorbent assay (ELISA) was performed to measure serum iNOS and Arg-1 levels; ROC curve was used to analyze the value of SIRT1 mRNA and FOXO3 mRNA levels in the differential diagnosis of LTBI and APTB; Pearson correlation was performed to analyze the correlation of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells of APTB patients with serum iNOS and Arg-1 levels. Results The levels of SIRT1 mRNA, FOXO3 mRNA and serum iNOS in peripheral blood mononuclear cells decreased in control group, LTBI group and APTB group, and the level of serum Arg-1 increased in turn (P<0.05). The AUCs of SIRT1 mRNA and FOXO3 mRNA in differential diagnosis of LTBI and APTB were 0.876 and 0.887, respectively, the sensitivity was 71.2% and 76.3%, and the specificity was 96.9% and 90.6% respectively. The levels of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells of APTB patients were positively correlated (r=0.500, P<0.05), and they were positively correlated with serum iNOS and negatively correlated with serum Arg-1 (P<0.05). The SIRT1 mRNA, FOXO3 mRNA and serum iNOS in peripheral blood mononuclear cells of APTB patients after 6 months of treatment were higher than those before treatment, and serum Arg-1 was lower than before treatment (P<0.05). Conclusions The levels of SIRT1 mRNA and FOXO3 mRNA in peripheral blood mononuclear cells of APTB patients are low, and they are positively correlated with macrophage-related cytokine iNOS and negatively correlated with Arg-1.
6.Change of peripheral blood appetite regulation factor of anorexia children and infect of child anorexia granule.

Study the infect of child anorexia granule on serum ghrelin and leptin of anorexia children and its clinical efficacy. Selected 81 cases of anorexia children aged 1-6 years old into treatment group (42 cases) and control group (39 cases), in addition, 30 case healthy children as healthy control group. The control group children were treated with domperidone suspension 0.3 mg x kg(-1) x d(-1), tid, orally 30 minutes before meals. Treatment group were treated with child anorexia granule, 1-3 years 1 package, bid; 4-6 years 1 package, tid; po, 4 weeks as a course of treatment. Study the change of serum ghrelin and leptin before and after therapy. The study demonstrates that before treatment, the serum ghrelin level of disease group was lower than healthy group (P < 0.01), and the serum leptin level was higher than healthy group (P < 0.01). After treatment, the serum ghrelin level both increase, and the serum leptin decline. And the change of treatment group was significantly different with control group (P < 0.01). And the clinical effective rate are 95.23% and 74.35% (P < 0.01). After 6 months of follow-up visit, the children weight significantly increase in treatment group (P < 0.01). Results indicate that child anorexia granule can facilitate secretion of ghrelin, and inhibit secretion of leptin, so as to work up an appetite. And the molecular mechanism is its infect on serum ghrelin, leptin.

Type 1 diabetes (T1D) results from dysfunction of pancreatic islets β cells.Recent studies supported that endoplasmic reticulum (ER) stress takes an important role in pancreatic β cell excessive loss,resulting in T1D.Here,we aimed to review the relationship between ER stress and T1D.Additionally,we also reviewed the potential mechanisms underlying ER stress mediated T1D.Studies have shown that severe ER stress is directly involved in the pancreatic β cells destruction and pathogenesis of T1D.ER stress plays a key part in pancreatic β cells and T1D,which will help in developing new effective therapeutics for T1D.

10.Evaluation of thiopurine methyltransferase genotyping and enzyme activity detection in treatment of patients with inflammatory bowel disease

Objective To assess the predictive value of thiopurine methyltransferase genotyping and enzyme activity in relation to side effects in patients with inflammatory bowel disease (IBD) who were treated with azathioprine (AZA). Methods One hundred and eleven IBD patients (26 with ulcerative colitis and 86 with Cronh's disease) with indication of AZA administration between April 2004 and Dec. 2009 were enrolled. All patients received 2 mg/kg of AZA daily. Polymerase chain reaction and high performance liquid chromatography were used to genotype the TPMT * 2, * 3A, * 3B, * 3C and to detect TPMT activity, respectively. The association of TPMT genotype and activity with side effects was analyzed in patients treated with AZA for 24 weeks or more, or in those discontinued AZA because of adverse effects. Results Adverse effects were reported in 38(33. 9%) patients, the most frequent being myelosuppression (20. 5%). The frequency of TPMT * 3C heterozygous mutation was 0. 9% (1/112). The TPMT activity was (12. 9±4. 8) U/ml RBC with unimodal distribution. One patient with TPMT * 3C heterozygous mutation developed myelosuppression at the 4th week after AZA treatment. The TPMP genotype myelosuppression patients. Conclusions TPMT genotype mutation and low enzyme activity can be used to predict myelosuppression with high specifically and low sensitivity. In patients treated with AZA, co-administration of 5-ASA results in a high frequency of myelosuppression with no effect on TPMT activity.
