Objective To perform the comparative analysis on the use situation of 3 kinds of dry chemical analyzers according to the industrial standards YY/T0655-2008.Methods With the Roche Cobas P800 automatic biochemical analyzers as the reference instrument,the accuracy,intra-assay precision,linearity,etc.in 3 kinds of dry chemical analyzers were evaluated.Results 3 kinds of dry chemical analyzers met the requirements of the industrial standards YY/T0655-2008,but the precision of imported instrument was significantly better than that of the domestic instrument.Conclusion In order to ensure the comparability and consistency of the detection results among instruments,the instruments should be regularly calibrated and performed the comparative experi-ments,the premise of calibration and comparison should be using 1/4 of the precision level in the CLIA′88 indicators as the indica-tor,not satisfying with the precision requirements in YY/T0655-2008,only in this way can the waste of blood and donors going a-way be avoided.

Objective To study the clinical characteristics of severe pneumonia in children , to explore the significance of neutrophil elastase detection. Methods Patients were divided to severe pneumonia group and non-severe pneumonia group Fever, cough, lung wet rale duration and image changes were statistically analyzed. The percentage of abnormal lymphocyte in peripheral blood was counted by Wright's dyeing method. The neutrophil elastase expression in peripheral blood was determinated by enzyme-linked immunosorbent analysis. Results In severe pneumonia group,atelectasis occurred in 5 cases,8 cases complicated with heart failure, 8 cases complicated with respiratory failure and 4 cases were complicated with toxic encephalopathy. In severe pneumonia group , fever time was 13.5±5.1 days, cough time was 15.1±3.2 days, the time of lung wet rale duration was 12.2±2.3 days, which were significantly longer than that of non-severe pneumonia group (t = 2.346,2.457,2.346,P < 0.05);In severe pneumonia group , pathogens included streptococcus pneumoniae , respiratory syncytial virus and adenovirus , etc.In severe pneumonia group,the percentage of abnormal lymphocyte in peripheral blood smear increased with virus infection (8.1±1.2)%. In 12 cases of severe pneumonia,the expressions of neutrophil elastase were 127.6± 12.5 ng/ml, more than that of non-severe pneumonia group (75.4 ± 6.4 ng/ml,t=3.047, P<0.05). Conclusion Severe pneumonia is a serious diseases impacting children health. Detection of neutrophil elastase shows a markedly clinical value to judge the severity of pneumonia.

Objective To investigate the potential factors affecting the survival rate of extremely low birth weight infant (ELBWI) and to make out a practical strategy. AlM:To investigate the features on prolonged central serous chorioretinopathy ( CSCR ) by optical coherent tomography ( OCT ) and to provide the basis of deciding the pathogenetic condition and prognosis. METHODS: Eighty - five patients who had been diagnosed with CSCR were grouped by suffering time as below: 32 patients suffered longer than 6mo as the prolonged and 53 patients with CSCR cured within that time. The imaging features of OCT were compared between the above groups.RESULTS:The incidence rate of neuroepithelial serous detachment extent above 500μm associated with pigmentary epithelial detachment in suffering eye and pigmentary epithelial damage in contralateral eye was significantly different between two groups. However, the incidence rate of neuroepithelial serous detachment extent above 4 000μm was not significant difference.CONCLUSlON:OCT could display clearly the change of every layer of retina with simplicity and visibility, which supplies us a new horizon to diagnose and trace CSCR. We could decide the pathogenetic condition and prognosis in accordance with the features of OCT, to provide references for the diagnosis and treatment of CSCR.

Objective To identify the potential correlation of serum 25-hydroxyvitamin D3 [25 (OH)D3]and C-reactive protein (CRP) with coronary artery disease.Methods We measured the serum 25 (OH)D3 and CRP levels in 416 suspected coronary heart disease patients who underwent coronary angiography.Gensini scores were used to assess the severity of coronary stenosis.Based on the results of coronary angiography,the 416 patients were divided into normal group (n =246),stable angina group (n =110),and acute coronary syndrome (ACS) group (n =60).Results The serum levels of 25 (OH) D3 in the normal group and stable angina group were significantly higher than that in the ACS gropu [(33.15 ± 20.95) nmol/L vs.(15.55 ±5.7) nmol/L,(28.93 ± 16.45) nmol/Lvs.(15.55 ± 5.7) nmol/L,bothP＜0.001].TheCRPlevelin the normal group was significantly lower than those in the stable angina group and the ACS group [(4.33 ± 0.12) rng/Lvs.(5.68 ± 0.25) mg/L,(4.33 ± 0.12) mg/Lvs.(5.73 ± 0.31) mg/L,bothP＜0.001].The serum 25 (OH)D3 was not correlated with CRP or Gensini score in the normal group.In the ACS group,the serum 25 (OH) D3 was negatively correlated with CRP level and Gensini score (r =-0.026,P =0.045 ; r =-0.256,P =0.048),while CRP level was positively correlated with Gensini score (r =0.459,P ＜0.001).Inthe stable angina group,the serum 25 (OH)D3 was also negatively correlated with CRP level and Gensini score (r =-0.211,P =0.027; r =-0.208,P =0.029),and the latter two were positively correlated (r =0.574,P ＜ 0.001).Conclusions Low serum 25 (OH) D3 levels are associated with coronary artery stenosis.Combining with CRP,it may be involved in the pathogenesis of coronary heart disease through inflammatory mechanism.

Objective To evaluate the feasibility,safety and accuracy of endoscopic ultrasonography guided fine-needle aspiration(EUS-FNA) in patients with mediastinal benign and malignant lesions.Methods A linear array scanning endosonography (Pentax-EG3630U) and a 22-guage (Wilson-CooK) needle were used to perform EUS-FNA in 22 patients with mediastinal lesions.Results EUS-FNA revealed 15 patients with carcinoma and 6 cases with benign tumors,which were confirmed by mediastinoscopy.One patient was not diagnosed because of inadequate specimen.None of the patients had complications caused by any procedures.Conclusion EUS-FNA is safe,reliable and accurate in diagnosis of mediastinal benign and malignant lesions.

Objective To evaluate the effect of hyperthermic intraperitoneal perfusion on blood coagulation in patients undergoing laparoscope-assisted radical surgery for gastric cancer.Methods Forty patients of both sexes, aged 40-60 yr, weighing 50-80 kg, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ , undergoing elective radical surgery for gastric cancer, were equally randomized into control group (group C) and hyperthermic intraperitoneal perfusion group (group HIP).The patients were treated with hyperthermic intraperitoneal perfusion for 1 h after the end of the radical surgery in group R.Before induction of anesthesia (T0) , immediately before hyperthermic intraperitoneal perfusion (T1), and at 1 h of hyperthermic intraperitoneal perfusion (T2) , venous blood samples were collected, blood coagulation was measured using thromboelastography, and the reaction time, coagulation time, α angle and maximal amplitude were recorded.Results Compared with group C, no significant change was found in blood coagulation parameters at T0 and T1 (P＞0.05), the reaction time and coagulation time were significantly increased, and α angle and maximal amplitude were decreased at T2 in group HIP (P＜0.05).Conclusion Hyperthermic intraperitoneal perfusion can improve blood coagulation in the patients undergoing laparoscope-assisted radical surgery for gastric cancer.
