Objective To investigate the correlation between plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) and haemodynamics evaluated by right-heart catheterization in patients with chronic thromboembolic pulmonary hypertension (CTEPH).Methods Fifty-three patients with CTEPH were retrospectively reviewed.All patients underwent right-heart catheterization to detect haemodynamics.The level of plasma NT-proBNP was measured by electrochemical luminescence.The indexes were analyzed.Results Pulmonary artery systolic pressure was (84.9 ± 21.6) mmHg (1 mmHg =0.133 kPa).Pulmonary artery diastolic pressure was (31.1 ± 9.7) mmHg.Pulmonary artery pressure was (49.3 ± 13.4) mmHg.Pulmonary vascular resistance was (1 047.4 ± 86.1) dyn ·s/cm5.Pulmonary capillary wedge pressure was (8.16 ± 3.02) mmHg.Right cardiac output was (3.36 ± 1.42) ml.Right cardiac work was (1.97 ± 0.95) kg·m.The level of plasma NT-proBNP in CTEPH was (2 301.5 ± 1 787.3) ng/L.The level of plasma NT-proBNP in CTEPH had positive correlation with pulmonary vascular resistance (r =0.429,P =0.02),and had negative correlation with right cardiac output (r =-0.583,P＜ 0.01) and right cardiac work (r =-0.521,P ＜ 0.01).The level of plasma NT-proBNP in CTEPH had no correlation with pulmonary artery systolic pressure,pulmonary artery diastolic pressure,pulmonary artery pressure,pulmonary capillary wedge pressure (P ＞ 0.05).Conclusion The level of plasma NT-proBNP can be used as a better predictor for evaluating pulmonary vascular resistance and right cardiac function in CTEPH.

The expression cDNA library of A. tabescens was constructed by SMART technique, which useλTriplEx2 as a vector. The titer and the percentage of the constructed library were about 1.0 × 106pfu/mland 98.3% respectively, and the titer and the capacity of the amplified library were about 3.1 × 108pfu/mland 4.2 × 1010. The library was used to provide expressed sequence tags (ESTs). 147 Expressed SequenceTaqs (ESTs) were gained from 176 clones, which were selected randomly and sequenced at the 5'end. Thesequences were submitted to the EMBL database. Blasting the sequences in the GenBank, 43 of them werefound that they have significant similarity with data in GenBank. EST AJ620046 was has significantsimilarity with the arabinosidase of Bacteroides thetaiotaomicron. Using SMART-RACE a full-length cDNA ofAJ620046 was successfully obtained. In order to initially characterize the biochemical properties ofAJ620046, the ORF of AJ620046 named AF was cloned and expressed in Pichia Pastoris yeast.Recombinant pHIL-S1-AF constructed by inserting AF into pHIL-S1 was transformed into Pichia PastorisGS115. Preliminary experiments indicated that AJ620046 was expressed as a 32 kDa protein in recombinantyeast.

Objective To investigate the effects of Tat-NBD(NEMO-binding domain,NBD)peptide on LPS-stimulated AR42J acinus cells inflammatory response.Method Lipopolysaccharide(LPS)was added to culture media at doses of 10 mg/kg for 24 hours to stimulate the AR42J cells.For pretreatment.cells were incubated with different peptides for 2 hours before LPS stimulation.The expression of TNF-α mRNA WaS conducted using a semi-quantitative RT-PCR method.TNF-α protein in culture medium were detected by enzyme linked immunosorbent assay(ELISA).The expression and translocation of the NF-kB-p65 protein of AR42J was determined by Strept Actividin-Biotin Complex(SABC)method.Results LPS(10 mg/L)resulted in an increase of TNF-αmRNA and TNF-αprotein,whereas significant inhibitory effects were observed when cells were incubated with Tat-NBD(WT)just on dose of 0.1 me/L(P＜0.05).The Tat-NBD(WT)peptide decreased inflammatory cytokine expression by a dose-dependent manner and its peak role was on dose of 100 mg/L.Consisting with TNF-α expression decrease,NF-kB-p65 expression signitieantly decreased in Tat-NBD(WT)pretreatment group,especially on the largest dose.NO significant changes in the control peptide group.Conclusions Tat-NBD(WT)peptide can inhibit the inflammation of acinus simulated by LPS.

OBJECTIVETo study the relationship between Chinese medical syndrome types and metabolomics of non-small cell lung cancer (NSCLC) patients.

METHODSTotally 120 NSCLC patients were assigned to asthenia syndrome group and sthenia syndrome group, 60 in each group. Meanwhile, 60 cases of benign pulmonary nodules in physical examinations were recruited as the control group. Tumor tissues or benign pulmonary nodules tissues were obtained by thoracoscope. Changes of their metabolites were observed using gas chromatography-mass spectrometry (GC-MS). Their differences were studied using principal component analysis (PCA) and partial least-squares discriminant analysis (PLS-DA). ROC curve analysis was performed in different metabolic compounds of sthenia and asthenia syndromes groups. The area under the curve (AUC) was calculated to evaluate the sensitivity of diagnosing syndrome types.

RESULTSCompared with the control group, difference existed in 16 compounds. Of them , contents of citric acid, pyruvic acid, alanine, choline phosphate, glycerol phosphate choline, linoleic acid, oleic acid, lactic acid, inositol were more in the two tumors group than in the control group. Difference existed in 10 compounds between the sthenia syndrome group and the asthenia syndrome group. Of them, citric acid, pyruvic acid, alanine, choline phosphate, glycerol phosphate choline, lactic acid, and inositol were more in the asthenia syndrome group than in the sthenia syndrome group. Contents of valine, glucose, and glutamine were more in the sthenia syndrome group than in the asthenia syndrome group. ROC curve analyses of different compounds indicated that AUC of lactic acid and glucose was more than 0.8 (P < 0.01); AUC of inositol, choline phosphate, and glycerol phosphate choline was more than 0.7 (P < 0.01); AUC of valine, citric acid, glutamine, alanine, and pyruvic acid was more than 0.6 (P < 0.05).

CONCLUSIONSThere existed certain correlation between CM syndrome types and metabolomics of lung cancer. Lactic acid, glucose, inositol, choline phosphate, glycerol phosphate choline, valine, citric acid, glutamine, alanine, pyruvic acid were sensitive diagnostic compounds, and the first four kinds were most sensitive compounds.

AIM: To survey the pathogenesis, clinical characteristics and surgical treatment for the correction of A-pattern exotropia.METHODS: The case history and surgical effect of 62 A-pattern exotropia in 1825 strabismus were studied. The superior oblique muscles weakening or the horizontal muscles procedures were performed in all patients according to the dysfunction of the superior oblique muscles, horizontal deviation, with or without binocular vision. Follow-up after surgery was 6-18months with mean duration of 9 months.RESULTS: A-pattern exotropia accounts for 3.4% of all strabismus. A-pattern exotropia accounts for 21.2% of A-V syndrome. After surgery, 52 cases got alignment, 56 cases gained A-sign correction and 54 cases recovered binocular vision.CONCLUSION: A-pattern exotropia is a common type in strabismus, its etiology is mainly oblique muscle dysfunction.Different surgical procedures should be selected according to overaction degrees of oblique muscle and binocular vision function, which can acquire the satisfied results.

OBJECTIVE: To study the pharmacokinetic parameter and the relative bioavailability of ferulic acid in ordinary Guipi pills and ultramicro Guipi pills in rabbit.METHODS: HPLC-MS/MS was used to identify the concentration of ferulic acid in the blood sample.RESULTS: Plasma concentration of ferulic acid in ordinary pills was different from that in ultramicro pills.The main pharmacokinetic parameters of ferulic acid in ordinary pills vs.that in ultramicro pills were as follows:t1/2a:(17.59?2.57) min vs.(23.51?4.49) min;t1/2?:(300.91?38.74) min vs.(167.17?45.66) min;tmax: (9.61?1.69) min vs.(12.33?2.27) min;Cmax:(58.92?8.42) ng?mL-1 vs.(112.35?31.29) ng?mL-1;AUC: (5 851.67?895.49) ng?min-1?mL-1 vs.(8 586.13?1 497.62) ng?min-1?mL-1.There were significant difference in Cmax and AUC between ordinary pills and ultramicro pills.CONCLUSION: The application of super fine crushing technique in the preparation of Guipi pills could improve the bioavailability of Guipi pills.

Objective:To compare the effect of ordinary Guipiwan and Ultra-micro Guipiwan on the rat models of both qi and blood defi ciency.Methods:Rat models of both qi and blood defi ciency,induced by re1easing blood and intraperitoneal injections of cyclophosphamide,were divided into model group,high,middle,and low dosage ultra-micro Guipiwan solution groups,ordinary Guipiwan solution group and blank control group.Drugs were given once a day intragastrically,successively for 14 days.At the end of the experiment,the countings of red cells,white blood cells,platelet,hemoglobin and the levels of granulocyte-macrophage colony stimulating factor in serum and were measured.Results:Ultra-micro Guipiwan increased the levels of blood cells,platelet,hemoglobin and the levels of granulocyte-macrophage colony stimulating factor in serum of the rats with both qi and blood defi ciency,the effect was better than Guipiwan(P

AIM:To study the effect of remifentanil on monophasic action potential and transmural dispersion of repolarization (TDR) in the 3-layer myocardium of isolated rabbit hearts .METHODS:Adult rabbits (n=18, 2.0 ~2.5 kg) were used to isolate the hearts for preparing Langendorff perfusion model .The hearts were randomly divided into 3 groups after perfusion with K-H solution for 15 min: the perfusion in control group ( C group ) continued for 60 min; the hearts in remifentanil group ( R group ) were perfused with 12 μg/L remifentanil K-H solution for 60 min; the hearts in remifentanil+aminophylline group ( RA group ) were given 60-min perfusion of 12 μg/L K-H remifentanil +30 mg/L aminophylline .The HR and 3 layers of myocardial monophasic action potential ( MAP) in the left ventricular anterior wall were recorded at time points after balanced infusion for 15 min ( T0 ) , and continued perfusion for 15 min ( T1 ) , 30 min ( T2 ) and 60 min ( T3 ) .The monophasic action potential duration of repolarization at 90%( MAPD90 ) and the transmural dispersion of repolarization (TDR) were calculated.The early afterdepolarization, delay afterdepolarization and arrhythmia were also observed.RESULTS:In R group, slower HR and prolonger MAPD90 and TDR at T1 ~T3 were observed as com-pared with those at T0(P<0.05).R group showed slower HR and longer MAPD 90 and TDR than C group and RA group (P<0.05).CONCLUSION:Remifentanil slows the HR, extends the MAPD90 and increases the TDR, thus being prone to induce reentry.Aminophylline makes HR faster and MAPD90 shorter, thereby reducing the TDR.

Objective To evaluate the effect of aminophylline on monophasic action potential (MAP) during remifentanil-induced negative chronotropic effect in the isolated rabbit hearts.Methods Eighteen healthy adult rabbits,weighing 2.0-2.5 kg,wereused in the study.Their hearts were excised and retrogradely perfused in a Langendorff apparatus.After 15 min of stabilization with K-H solution,the isolated hearts were randomly divided into 3 groups (n =6 each) using a random number table:control group (group C),remifentanil group (R group),and remifentanil + aminophylline group (RA group).Group C was perfused with 37 ℃ K-H solution for 60 min.Group R was perfused with K-H solution containing remifentanil 12 ng/ml for 60 min.Group RA was perfused with K-H solution containing remifentanil 12 ng/ml and aminophylline 30 μg/ml for 60 min.At 15 min of stabilization and 15,30 and 60 min of perfusion,HR and MAP in the myardium of left ventricle were recorded:MAP duration at 90％ and 50％ repolarization (MAPD90,MAPD50) was calculated.The early after depolarization,delay after depolarization and arrhythmia were recorded.Results Compared with group C,HR was significantly decreased at 15,30 and 60 min of perfusion,and MAPD50 and MAPD90 were prolonged in goup R,and HR was increased in group RA.HR was significantly higher,and MAPD50 and MAPD90 were shorter in RA group than in group R.No early after depolarization,delay after depolarization or arrhythmia developed in each group.Conclusion Aminophylline antagonizes remifentanil-induced negative chronotropic effect through shortening monophasic action potential duration in the myocardium of left ventricle of the isolated rabbit hearts.
