1.Effect of benazepril on expression of insulin receptor and its substrate -1 protein in renal tissue cell membrane in diabetic rats

AIM: To study effect of benazepril (an ACE inhibitor) on expression of insulin receptor (IR) and its substrate-1(IRS-1) protein in renal tissue cell membrane in diabetic rats. METHODS: The rats were randomly divided into following groups: control (n=6), streptozotocin induced diabetic (n=7) and diabetic treated with benazepril (n=7). Body weight, kidney weight and kidney weight/body weight were observed after 4 weeks of treatment. ACE activities in plasma, renal tissue were measured by the fluorimetric assay. The expressions of IR and IRS-1 protein were determined by Western blot analysis in renal tissue cell membrane. RESULTS: After 4 weeks of treatment, benazepril significantly ameliorated kidney hypertrophy in diabetic rats. ACE activities in plasma, renal tissue were redunced by approximately 92.00% and 88.77%, respectively. Western blot analysis showed that the expressions of IR and IRS-1 protein were increased by 2.1 and 1.5 folds in renal tissue cell membrane in diabetic rats. However, benazepril reduced expression of IR and IRS-1 protein by 45.74% and 47.66%, respectively. CONCLUSIONS: Increased expression of IR and IRS-1 protein might be related to abnormally active glucose metabolism in diabetic rat kidney. Down-regulation of expression of IR and IRS-1 protein might be one of important machnisms of Benazepril nephroprotection on diabetic rats.
2.Effects of ACE inhibitor on the expression of GluT4 mRNA and protein in diabetic rat heart

AIM To study effects of ACE inhibitor on the ex pression of GluT4 mRNA and protein in diabetic rat heart. METHODS The following groups of rats were randomly designed: control rats, diabetic ra ts and diabetic rats treated with benazepril (an ACE inhibitor). After 4 weeks o f treatment, ACE activities were determined by fluorimetric assay in plasma and heart; Myocardial GluT4 mRNA expression were measured by Northern blot analysis; GluT4 protein expression were measured by Western blot analysis. RESULT S After 4 weeks of treatment, there was a significant increase in myocard ial ACE activities despite a trend toward reduce in plasma activitics, ACE activ ities were inhibited by 92 1%, 89 0% in plasma and heart, respectively; Northe rn blot analysis revealed that the expression of GluT4 mRNA was reduced in diabe tic rat heart by 40%, and treatment with benazepril did not prevent the diabetes -induced reduction of GluT4 mRNA; However, Western blot analysis revealed that benazepril did prevent the diabetes-induced reduction of GluT4 protein in cell membrane of diabetic rat heart. CONCLUSION Benazepril could significantly suppress ACE activities in diabetic rat heart, bu t it did not influence the expression of the myocardial GluT4 mRNA. However, it did prevent the diabetes-induced reduction of GluT4 protein in cell membrane of rat heart.
3.Evaluation of community health service institutions responsiveness in Henan Province:Key in-formant survey

Objective:To understand the responsiveness of community health service institutions in Henan Province. Methods:This research collected information through questionnaire investigations. The object of the investigations is key informants familiar with community health service institutions. Various dimensions of responsiveness within Henan province community health service institutions are determined through a multiple-input pecking order graph. The fuzzy comprehen-sive evaluation method is used to evaluate the level of responsiveness. Responsiveness inequality index is used to measure the distribution of reactions. Results:A total of 431 valid key informants were surveyed. The various dimensions of the questionnaire of Cronbach alpha coefficient was greater than 0. 623;the total scale of the Cronbach alpha coefficient was 0. 850;the correlation coefficient of each dimension of test-retest reliability were greater than 0. 907. The levels of responsive-ness and distribution index were 8. 33 and 0. 2744, respectively;each dimension by order of importance:timely attention>dignity>infrastructure>communication>confidentiality >social support >selective >autonomy. Approximately 50% of key informants considered that vulnerable groups in community health service institutions were sometimes or often treated unfairly. Conclusion:Key informants considered the responsiveness level of overall higher community health service institu-tions in Henan Province was high, and the distribution of responsiveness was relative equilibrium. However, there is a large shortage of infrastructure, privacy protection, doctor-patient communication, etc.
4.Influence of CYP2C19*2 gene polymorphism on MACE after PCI

Objective:To explore the impact of cytochrome P450 oxidase (CYP2C19) *2 genotype on major adverse cardiovascular events (MACE) after percutaneous coronary intervention (PCI) and impact of MACE on blood stasis indexes .Methods :From Jan 2010 to Jan 2011 ,a total of 355 patients with blood stasis syndrome undergoing PCI were enrolled .All patients took aspirin and clopidogrel conventionally before PCI ,then maintained for one year at least after PCI .According to MACE or not ,patients were divided into MACE group (n=64) and no MACE group (n=291) .Turbidimetry method was used to measure platelet aggregation rate before and one week after PCI ,and polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP ) was used to test CYP2C19 * 2 gene polymorphism .Results:(1) Compared with no MACE group ,there were significant rise in maximum platelet aggregation rate [MPA ,(36.38 ± 14.57)% vs .(44.87 ± 11.63)% ] and residue platelet aggregation rate [RPA , (25.76 ± 12.69)% vs .(34.45 ± 15.58)% ] , P< 0.01 both ;in percentage of CYP2C19*2 gene mutation (GA+AA) (36.43% vs .75.0% ,χ2 =27.832 ,P<0.01) in MACE group .(2) Multi-factor Logistic regression analysis in-dicated that >65 years ,CYP2C19*2 gene mutation (GA+AA) ,renal dysfunction (creatinine>1.5mg/dl) and re-duced left ventricular ejection fraction (LVEF< 50% ) were independent risk factors for MACE after PCI (OR=2.164~5.947 , P<0.05 or <0.01);(3) Binary linear correlation analysis indicated that CYP 2C19*2 gene muta-tion was positively correlated with RPA after PCI ( r=0.437 , P<0.001) .Conclusion:CYP2C19*2 gene mutation affects antiplatelet activity of clopidogrel ,so it′s helps to recognize MACE risk in patients with coronary heart dis-ease and blood stasis after PCI .
5.The minimally invasive surgery for treating recurrent lumbar disc protrusion

Objective To study the clinical effect of the minimally invasive surgery for treating recurrent lumbar disc protrusion . Methods Thirty operative cases of recurrent lumbar disc protrusion treated with the minimally invasive surgery from July 2009 to July 2013 were retrospectively analyzed for statistically describing the age ,body mass index ,recurrent segment ,operating time ,intr-aoperative bleeding volume and postoperative painless walking time .The recurrence segments were 1 case of L34 ,10 cases of L45 and 19 cases of L5 S1 ;the visual analogue scale (VAS) was adopted to evaluate the operative effect .Results The operating time was 65 min on average (50-100)min;mean intraoperative bleeding volume was 145 mL(100-180)mL ;average length of hospital stay was 13 d (9-16)d;average hospitalization costs were 7 300 Yuan;average postoperative painless walking time was 11 d (7-15)d;post-operative non-manual labor work time was 27 .3 d .The VAS score was reduced from (7 .3 ± 1 .3) before operation to (3 .1 ± 0 .9) after operation and (2 .2 ± 0 .6) at the last follow-up ,which was statistically significant(P<0 .05) .Followed up for 6-33 months , average 16 .5 months .Superior efficacy in 16 cases ,gord in 7 cases ,average in 5 cases ,bad in 2 cases ,the rate of good efficacy was 76% .Conclusion The minimally invasive surgery is one of effective methods to treat postoperative recurrence of lumbar disc pro-trusion .
6.An initial study on serum CA125 in diagnosis to lung cancer

35u/ml as positive limit, the sensitivity of CA125 for diagnosis of lung cancer is 53.6%. There was no significant difference between stage Ⅰ,Ⅱ lung cancer and benign, healthy groups, but the level of serum CA125 of stageⅢ?Ⅳ was higher than control groups. Thirty eight of forty lung cancer patients with higher CA125 level dropped to normal after therapy. Conclusions:Serum CA125 has high diagnosis value in lung cancer. It can be a vital index in lung cancer staging and prognosis.

Aim To observe the expression of migration inhibitory factor (MIF) in the enteric neurons,and to explore the nervous regulation on MIF activity in experimental colitis.Methods Colitis was induced in sensitized rat and mouse by 2,4-dinitrochlorobenzene(DNCB)enema.MIF activity was measured both in the mesentery lymphocyte(by MTT)and in the enteric neurons(by immunofluorescence double staining).6-OHDA was intraperitonealy (ip) administered to mouse before DNCB treatment.Norepinephrine(NE) was added to lymphocyte culture in vitro during MIF preparation.Results The expression of MIF protein in enteric neuron was increased in DNCB-induced colitis in rat.ip 6-OHDA in colitis mouse(38~150 mg?kg-1) resulted in a further increase of MIF activity than ip vehicle in colitis mouse (P
8.Influence factor analysis of 656 cases with AMI undergoing emergency PCI

Objective:To analyze influence factors in patients with acute myocardial infarction (AMI) undergoing e‐mergency percutaneous coronary intervention (PCI ) .Methods:Clinical data of 656 patients ,who received emer‐gency PCI because of AMI in our hospital from Jan 2005 to Jan 2015 ,were retrospectively analyzed .Short -term prognosis ,including incidence rates of heart failure (HF) ,cardiogenic shock (CS) ,arrhythmias and mortality , were compared and analyzed among these patients from onset related factors ,coronary disease and vascular reope‐ning time etc .Results:Mean onset age of men was significantly younger than that of women (P<0.05) .Incidence rate of AMI in winter and spring was significantly higher than those in summer and autumn (χ2 = 6.244 , P=0.012) .According to complicated with hypertension ,DM ,dyslipidemia and smoking or not ,patients were divided into corresponding disease group and normal group ;only incidence rates of CS (37.78% vs .29.62% ) and arrhyth‐mia (47.78% vs .38.24% ) in DM group were significantly higher than those of non -DM group ,mortality rate of dyslipidemia group was significantly lower than that of normal blood lipid group (0.73% vs .3.69% ) , P<0.05 or< 0.01 ,there were no significant difference in incidence rates of other events between normal group and disease group (P>0.05 all) .Incidence rate of arrhythmia in RCA group was significantly higher than those of LAD group , LCX group and multi -vessel group (56.36% vs .31.55% ,37.50% ,34.38% ) ,mortality of LM group was signifi‐cantly higher than those of LAD group ,RCA group and LCX group (25.00% vs .1.79% ,0.91% ,0% ) , P<0.05 or <0.01. Incidence rate of HF in 10~12h group was significantly higher than those of 0~3h ,4~6h and 7~9h group (79.46% vs .61.70% ,66.81% ,64.78% ) ,incidence rate of arrhythmia was significantly lower than those of 0~3h ,4~6h and 7~9h group (32.14% vs .55.32% ,43.81% ,44.65% ) ,and incidence rate of CS was significant‐ly higher than that of 0~3h group (35.27% vs .21.28% ) , P<0.05 or <0.01. Conclusion:Onset age ,season and DM ,coronary disease extent ,vascular reopening time are risk factors influencing short‐term prognosis of AMI .
9.Albumin bound paclitaxel for the treatment of retreatment advanced non-small-cell lung cancer

Objective To observe the efficacy and safety of albumin bound paclitaxel in the treatment of retreatment advanced non-small-cell lung cancer (NSCLC).Methods Retreatment NSCLC patients failed from first line regimen or beyond were treated with albumin bound paclitaxel weekly by intravenous dose of 130 mg/m2 on day 1 and day 8,with a 21-day cycle.Efficacy was evaluated every two cycles and side effects were observed during each cycle.Results None of 69 patients achieved complete remission (CR),15 patients (21.7 %) achieved partial remission (PR),and 23 patients (33.4 %) achieved stable disease (SD).Objective response rate (ORR) was 21.7 %,disease control rate (DCR) was 55.1%,and progress free survival (PFS) time was 3.8 months.Efficacy was not correlated with gender,age,histology and lines of previous treatment (all P > 0.05).Main adverse reactions included neutropenia,alopccia and neurotoxicity,which were all tolerable.Conclusion Weekly albumin bound paclitaxel is effective and well tolerated in the treatment of retreatment advanced NSCLC,which can be considered as the second line or beyond regimen.
10.An outbreak and treatment of Norwegian scabies occurred in a hospital

Objective To investigate the causes and treatment measures of an outbreak of Norwegian scabies oc-curred in a hospital.Methods In May 2013,an outbreak of Norwegian scabies among health care workers(HCWs) occurred because of the misdiagnosis of a patient with Norwegian scabies,epidemiological investigation was carried out by healthcare-associated infection(HAI)control department,medical intervention and disinfection and isolation measures were performed.Results A total of 27 HCWs and patients’relatives developed Norwegian scabies.After active medical treatment,patients’condition improved;all appliances used by patients were cleaned and disinfected after being wrapped and sealed with plastic bags for one week.Epidemic trend of infection was under control and no new case was found.Conclusion With highly contagious,Norwegian scabies can be spread in local area,it is neces-sary to improve HCWs’diagnostic ability to this disease and take effective measures to prevent the epidemic once HAI occur.
