1.Related factors of prevention in diabetic retinopathy

Objective To explore the related risk factors for diabetic retinopathy(DR)in type 2diabetes.Methods The clinical data of 412 type 2 diabetes patients,diagnosed between 2003 and 2010,were analyzed retrospectively.The diagnosis of DR and proliferative diabetic retinopathy(PDR)was confirmed by ophthalmoloseopy and fundus fluorescein angiography.Glycated hemoglobin Alc,glucose,insulin,and C-peptide of fasting plasma,and 1,2 and 3 hours postprandial plasma were measured.According to the above-mentioned data,get the fluctuation of glucose,insulin and C-peptide of 1,2 and 3hour postprandial plasma.Results The morbidity of DR and PDR increased following the longer disease duration.Age,diabetic duration,body mass index(BMI),hypertension grade,HbAlC,fasting plasma insulin and C-peptide,2 and 3 hours postprandial plasma glucose,1 and 2 hours postprandial plasma insulin,1,2 and 3 hour postprandial plasma C-peptide,1,2 and 3 hours postprandial plasma glucose,insulin and C-peptide fluctuation are different statistically among non-DR group,non-PDR group and PDR group(P<0.05).3 hours postprandial plasma glucose and fasting plasma insulin were risk factors of DR (P<0.05).Conclusions Postprandial plasma glucose and fasting plasma insulin were risk factors of DR.Nevertheless,postprandial insulin,fasting and postprandial C-peptide,postprandial plasma glucose,insulin and C-peptide fluctuation were useful for DR diagnosis.

3.The therapeutic effect of embryonic stem cells on acute lung injury induced by bleomycin in mice

AIM:The aim of this study was to observe and compare the therapy effect of different kinds of embryonic stem cells(ESCs) on pulmonary injury of mice exposed to bleomycin.These embryonic stem cells were C57BL/6J-ESC,S8-ESC and human-ESC.METHODS:(1) Fifty C57BL/6J female mice were divided randomly into five groups,which were blank control group,bleomycin model group,bleomycin model injected with C57BL/6J-ESC group,bleomycin model with S8-ESC group and bleomycin model with human ESC group.Every group has ten mice.(2) The mice of control group were administrated 0.9% sodium chloride solution and the mice in other four groups were administrated bleomycin intratracheally.Three different kinds of ESCs were administrated to the mice in three different ESCs-treated groups respectively one hour after bleomycin exposure.The life-span and hydrocyproline concentration were examined.The pathologic changes of the lung and the engraftment of the ESCs in the injured lung were observed.RESULTS:(1) The death rate in three different ESCs-treated groups declined much more obviously than that in the control group on 8 days after bleomycin exposure(bleomycin model group 50%,C57BL/6J-ESC group 37.5%,S8-ESC group 20%,human-ESC group 20%).(2) The extent of pathologic changes of the lung in S8-ESC group was lighter significantly than that in the bleomycin model group,but in C57BL/6J-ESC group and human-ESC group,their pathologic changes were similar to that in bleomycin model group.(3) The hydrocyproline concentration in S8-ESC group was lower distinctly than that in bleomycin model group(P0.05).(4) The positive signals of three kinds of ESCs could be found in the lung at 1,3,12 and 24 hours after the stem cells were administrated,but signals were the strongest 3 hours after stem cells were given.All of the signals disappeared three days later.CONCLUSION:S8-ESCs transplantation can improve the tolerence of mice to bleomycin and ameliorate acute lung injury.

5.Relationship among the birth defect and heavy metal such as arsenic, cadmium, zinc and copper

The incidence of birth defect in China is still high,which is not only affected by genetic factors,but also affected by heavy metals in surrounding that prejudices foetus' normal development.Heavy metal is a kind of inorganic pollutants with high toxicity.Excessive intake of lead,cadmium,arsenic or mercury,and the insufficient intake of zinc,calcium and copper may both lead to at least 1 kind of birth defect.The interactions between heavy metals also affect the outcomes of pregnancy.This paper reviewed different relationships between heavy metals and birth defect recording to relevant achievements such as animal experimentations and epidemiologic study made by researches from at home and abroad in recent years.It is advised to intake adequate zinc and copper and avoid being exposed to harmful metals to make sure the effective reduction of the incidences of birth defects.This paper will also point out the direction of future research about the relationship among the birth defects and heavy metal.
7.ANALYSIS OF 98 CASES OF ADRENAL INCIDENTALOMAS

In order to explore the clinical feature, diagnosis and differential diagnosis of adrenal incidentalomas, we reviewed 98 cases of adrenal incidentalomas for which adrenalectomy was performed in our hospital from 1993 to 2002. It was found that the average age of the patients was 48 years old, 16 cases (16 3%) had no specific symptoms, and others were incidentally diagnosed. Of the 98 cases, 18 (18 4%) were malignant lesions, including 8 metastatic cancers, 8 adrenal carcinomas and 4 lymphangioma, and 80 were benign lesions. Twenty one cases were functioning adrenal adenoma. Ultrasound, CT scan, MRI or MIBG were effective methods to detect adrenal incidentalomas.
8.ANALYSIS OF 282 CASES OF ADRENALECTOMY

In order to investigate the distribution and diagnosis of adrenal tumors, 282 cases of patients who had undergone adrenalectomy in our hospital from March 1993 to December 2001 were reviewed. There were 147 (52 1%) women and 135 (47 9%) men, with a mean age of 43 6?13 0 (3~75) years. Of the 282 patients, 83 patients (29 4%) were operated on because of hyperaldosteronism, 59 (20 9%) pheochromocytoma, 27 (9 5%) hypercortisolism, 105 (37 2%) incidentaloma, 36 (12 8%) malignant tumors and 13 (4 6%) for other reasons. The average diameter of adrenal tumors was 4 8?3 4 (0 3~22 5)cm. The size of tumor mass was smallestin hyperaldosteronism, while the largest was malignant tumor. The conformation rate of the preoperative diagnosis and pathological findings was 65 2%.
9.Application of prism alternate cover test and perimeter strabismometry in strabismic operation

AlM:To discuss the application of prism alternate cover test ( PACT) and perimeter strabismometry in strabismic operation.METHODS: A total of 145 patients with concomitant esotropia or exotropia, aging 2 ~ 50 years old, were included. They were examined by PACT and perimeter strabismometry before operation. The strabismic operations were performed, the orthotropia rate of postoperation was assessed.RESULTS:Using PACT and perimeter strabismometry, there was a significantly difference of deviating angle ( P<0. 05) in mild and severe strabismic group. But there was no significantly difference of deviating angle (P>0. 05) in moderate strabismic group. The functional cure rate of postoperation was respectively 70. 83%, 67. 01%; the clinical cure rate was 20. 84%, 24. 74%; invalid rate was 8. 33%, 8. 25% .CONCLUSlON: The application of PACT and perimeter strabismometry in strabismic operation design, referring to the eye axial length, this can be helpful of improving the cure rate postoperation and decreasing the danger of reopreation.
10.A clinical trial for preventing postoperative air leakage in video-assisted thoracoscopic bullectomy for primary spontaneous pneumothorax

Objective Purpose This prospective randomized study was to explore the efficacy of different methods in preventing postoperative air leakage of video-assisted thoracoscopic (VATS) bullectomy for primary spontaneous pneumothorax.Methods The patients who underwent VATS bullectomy from January 2015 to March 2016 in Beijing Chao Yang Hospital were enrolled,and they were randomly assigned to experimental group and control group.The experimental group applied an absorbable polyglycolic acid(PGA) sleeve combining with an automatic stapler in the bullectomy.The control group resected the bullae using the automatic stapler alone.Covering the staple line with an absorbable polyglycolic acid sheet and pleural abrasion were performed in both groups.The baseline characteristics of the patients,postoperative air leakage,drainage tube removing time,postoperative hospital-stay,and postoperative complication were recorded.Results Finally,123 patients were enrolled in the study,the experimental group and control group were 58 and 65 cases,respectively.There was no operative mortality in either group.Compared with the control group,the postoperative air leakage (0.53 ± 0.99) days,chest tube drainage (2.98 ±1.03) days,postoperative hospital-stay (3.88-± 0.91) days in the experimental group were all significantly shorter.The rate of postoperative complications in the experimental group was(3.4%),of which one patient catched a pulmonary infection,the other one postoperative atelectasis.However,complications were developed in ten patients (15.4%) in the control group,which was significantly higher compared with the experimental group,including pulmonary infection five cases,postoperative atelectasis three cases,pleural effusion two cases.Conclusion The appliance of PGA sleeve in the operation for primary spontaneous pneumothorax could effectively prevent postoperative air leakage,as well as reduce the postoperative drainage tube removing time.Furthermore,the patient could recover more soon from the surgery,and have a shorter hospital-stay.
