1.The clinical significance of dynamic determination of serum procalcitonin levels in judgement of infection and prognosis in patients with severe craniocerebral injury

Objective To explore the clinical significance of dynamic determination of serum procalcitonin(PCT)levels in judgment of infection and prognosis in patients with severe craniocerebral injury.Methods A total of 80 patients with severe craniocerebral injury were selected and they were divided into infection group and non-infection group.Infection group was divided into improved group and non-improved group.Acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)score and Glasgow coma scale(GCS)score was compared between infection group and non-infection group.The level of serum PCT,white blood cell count(WBC),C-reactive protein(CRP)and interleukin(IL)-6 was compared on the first,third,fifth day of admission in intensive care unit among these groups.Results Infection group in 48 patients,and non-infection group in 32 patients.There was no significant difference in age,sex and GCS score between two groups(P >0.05).APACHE Ⅱ score in infection group was higher than that in non-infection group [(21.71 ± 7.13)scores vs.(15.32 ± 6.17)scores],and there was significant difference (P< 0.05).The level of serum PCT on the first day of admission in intensive care unit in infection group was higher than that in non-infection group[(2.15 ± 1.79)ng/L vs.(0.56 ± 0.47)ng/L],and there was significant difference(P< 0.05).There was no significant difference in WBC,CRP and IL-6 between two groups(P>0.05).The level of serum PCT,WBC,CRP,IL-6 on the third day of admission in intensive care unit in infection group was higher than that in non-infection group,and there was significant difference(P< 0.05).The level of serum PCT on the fifth day of admission in intensive care unit in infection group was higher than that in non-infection group [(3.09 ± 1.98)ng/L vs.(1.06 ± 0.63)ng/L],and there was significant difference (P<0.05).WBC,CRP,IL-6 on the fifth day of admission in intensive care unit was(15.13 ±6.32)×109/L,(103.56 ±40.62)mg/L,(230.41 ± 121.83)ng/L in improved group(38 cases),and (18.62 ±8.89)× 109/L,(134.38 ± 34.16)mg/L,(268.73 ± 138.12)ng/L in non-improved group(10 cases),and there were significant differences between two groups(P < 0.05).The level of serum PCT on the first,third,fifth day of admission in intensive care unit in improved group was lower than that in non-improved group,and there was significant difference(P < 0.05).Conclusion The alteration of serum PCT level may be used as one of the effective predictors for infection in patients with severe craniocerebral injury and for their prognosis.

3.Complex network analysis on dynamic change regularity of combining use of Chinese and western medicine in 27,678 cases with ischemic stroke in acute phase.

The acute phase of ischemic stroke patients are often treated with both Chinese patent medicine:and western medicine therapies in clinical practice. This research included 27,678 cases of the acute phase of ischemic stroke came from 14 3A level hospitals. We collected data from patients with ischemic stroke who used both Chinese patent medicine and western medicine and were hopitalized within 14 days from hospital information system (HIS). Constructing complex network of Chinese patent medicine and western medicine were found to show scale-free network. Hierarchical structure of the core algorithm was used to analyze the characteristics of combined core Chinese patent medicine and western medicine in admission condition of "acute", "critically", and "general" of ischemic stroke acute phase patient within one day, 2-3 days, 4-7 days and 8-14 days. We found that the core Chinese patent medicine mainly used for activate blood and resolve stasis medicine, and phlegm eliminating brain refreshing medicine in all kinds of patients, but the phlegm eliminating brain refreshing medicine were used to reduce with time elapsing. The core western medicine mainly used for anti-platelet medicine, improve circulation medicine, neuroprotective medicine, anticoagulants medicine and dehydration medicine. The dehydration medicine as the core western medicine for critically patients within 14 days, but the patients for general admission as core western medicine within 3 days. The neuroprotective medicine was used to decreases after 7 days in hospital. Combination of Chinese patent medicine and western medicine were mainly for neuroprotective medicine + activate blood and resolve stasis medicine, and anti-platelet medicine + activate blood and resolve stasis medicine, and improve circulation medicine + activate blood and resolve stasis medicine. The phlegm eliminating brain refreshing medicine was mainly combined with neuroprotective medicine by urgent and general admission condition patients, and it was more combined with dehydration medicine by critically admission condition patients. This research found that the dynamic characteristics for the combination of Chinese patent medicine and western medicine of acute phase of ischemic stroke patients by big data analytics and complex networks modeling, and provide basis for acute phase of ischemic stroke patients, it provide basis for ischemic stroke treatment strategy making.

4.Value of acoustic radiation force impulse in the differential diagnosis of benign and malignant hepatic neoplasms

To explore the value of acoustic radiation force impulse (ARFI) technique in the differential diagnosis of benign and malignant hepatic neoplasms.Retrospective analysis of touch tissue quantification (VTQ) and general ultrasonic examination of interesting region were performed for hepatocellular carcinoma (HCC),hepatic hemangioma,focal nodular hyperplasia (FNH) and controls.Significant differences existed between HCC and hepatic hemangioma,hepatic FNH and controls (all P <0.01).No significant difference existed between primary and metastatic HCC (P > 0.05).There was no significant difference between hepatic hemangioma,FNH and controls (all P > 0.05).With a cut-off VTQ value of 1.95 m/s,the sensitivity and accuracy rate in the differentiation of benign and malignant hepatic neoplasms was 90% and 83% respectively.The sensitivity and accuracy rate in general ultrasound examination of hepatic neoplasms was 88% and 70% respectively.Significant differences existed in accuracy rate between two methods (P < 0.01).It was confirmed that ARFI could reflect the stiffness of liver neoplasm noninvasively.Compared with general ultrasonography,ARFI is better at differentiating benign and malignant hepatic neoplasms.
5.Comparison of Experimental Autoimmune Encephalomyelitis in Mice Induced with Myelin Oligodendrocyte Glycoprotein at Different Doses

Objective To compare the condition of illness and pathological characteristics of experimental autoimmune encephalomyelitis (EAE)in C57 BL/6 mouse models induced by myelin oligodendrocyte glycoprotein 35-55 (MOG35-55)at different doses,and provide a reliable animal model for further study of multiple sclerosis(MS).Methods Male SPF-grade C57 BL/6 mice were divided randomly into four groups:normal group and three EAE model groups (MOG35-55 high-dose,middle-dose and low-dose model groups).200,100,50μg MOG35-55/mice were mixed with complete Freund's adjuvant(CFA),respectively,to prepare complete antigen in different concentrations.The mice were anesthetized and injected s.c.over flanks with the complete antigen and injected i.P.with pertussis toxin to establish immunization-induced C57BL/6 mouse-model of EAE.The mice of the normal group were injected with normal saline instead.Since the day of immunization,the incidence,body weight and neurological score of the mice were observed.The mice of different neurological scores in different periods were anesthetized and perfused with saline and followed by 4% paraformaldehyde.The brain and spinal cord of the mice were removed and fixed in the same fixative solution.The brains and spinal cords of the mice were examined by histopathology with hematoxylin-eosin(HE) staining.The mice on the 40th day were sacrificed and perfused with 2% paraformaldehyde and 2% glutaraldehyde, 1 mm~3 pieces of cerebral white matter and intumescentia lumbalis of the spinal cord were taken and ultrathin sections were prepared according to conventional techniques for electron microscopy. Results All the MOG_(35-55) in three different doses induced mouse models of EAE. The disease was with an incidence rate of 100% and a chronic monophasic course. The body weight of the mice in the three groups decreased obviously compared with those in the normal group. The maximum value of neurological score was 1.33,2.25 and 2.50 in the mice of high-, middle-and low-dose groups, respectively. The major histopathological changes observed in the brain and spinal cord of the EAE mice were different degrees of inflammatory cell infiltration around small vessels showing sleeve-like changes, dcmyelination and neuronal karyopyknosis in the acute and remission stages. The main site of the brain inflammation was in white matter around encephalocoele, and also in the DG and CA zones of hippocampus. The spinal cord inflammation was most severe in the lumbosacral region. The above mentioned pathological changes in the low-dose group were more prominent than those in the middle-dose and high-dose groups. The major ultrastructural changes were scattered around encephalocoele, interstitial edema, especially around small blood vessels, and swollen mitochondria with damaged cristae, and some karyopyknosis in vascular endothelial cells. Some tight junctions were blurred. Some dispersed lymphocytes and mononuclear cells were seen in the perivascular space. In lumbar intumescentia of the spinal cord, there were some myelin figures in the white matter myelin sheath. Some of them showed demyelization and structurtal fusion. The cytoplasmic organelles of axons were considerably reduced or even disappeared. The vascular basement membrane showed an increased thickness and focal necrosis in some areas. Conclusion The mouse models of immune-induced EAE are successfully established with MOG_(35-55), especially that induced with MOG in a dose of 50 μg. This mouse model is stable, with a high incidence and low mortality rate, and can be applied for EAE research in the future.
6.Development and Practice of CAI Courseware for Microbial Engineering

This article introduced the development and application effect appraisal of Microbial Engineering CAI courseware for bio-engineering specialization. The courseware focuses on knowledge system integrity, content-rich and gives prominence to the key points. Pictures, animation and video, and audio effects are also utilized appropriately to achieving stimulate students interest in learning and then improve teaching and learning performance. The courseware concentrates on core content of the course, such as fermentation parameters detection and automatic control, and fermentation equipments. The courseware was manufactured using the Powerpoint software. Animation was established with Flash 4 software and the scanning pattern was edited using Adobe photoshop. And chapters of the courseware were composed and administrated using Courseware Master Software. A two-year survey showed that 85% of students satisfied with this courseware.
7.Clinicopathologic features,surgical methods and prognosis in peripheral<2 cm(of diameter) non-small cell lung cancer

1.0~(?)1.5 cm(14.81%)(P=0.10).There was no lymph node metastasis in tumors less than 1.0 cm in diameter. The 5-year survival rates for cases with or without lymph node involvement were 92.31% and 60.0%,respectively,the difference was significant(P=0.000).The 5-year survival rates of 12 patients showing ground-glass opacity(GGO)on chest CT was 91.67% without any lymph node involvement.Conclusions:There is mediastinal and hilar lymph node involvement even with tumor diameter less than 2 cm.The results of the present study suggested that routine lymph node dissection is necessary even for cases with tumor diameter less than 2 cm.However,if the tumor is within 1.0 cm in diameter with obvious GGO showing on chest CT,these are good candidates for partial resection without mediastinal lymph node dissection.
8.Compound ocimum oil oral suspension in the treatment of 382 children with acute diarrhea (Piweixure Syndrome)

Objective To investigate the efficacy and safety of compound ocimum oil oral suspension in the treatment of children with acute diarrhea(Piweixure Syndrome).Methods 382 children with acute diarrhea(Piweixure Syndrome) were selected for the multi center and open clinical trial.All patients was treated by compound ocimum oil oral suspension based on age and weight,6 months below 3mL thrice a day,6-< 12months 5mL thrice a day,12-<24 months 10mL thrice a day,24 months upper 20-40 mL thrice a day.The course was three days.The efficacy and safety was observed.Results The total efficiency was 93.98%,there were 274 cases with marked effect and 85 case with effective in analysis on clinical efficacy.There were significant differences between before and after therapy on symptoms and signs as defecation frequency,defecation characters and defecation disesthesia (x2 =467.43,524.51,263.03,all P < 0.05).Conclusion Compound ocimum oil oral ouspension in the treatment of children with acute diarrhea(Piweixure Syndrome) has definite effect and safety.It would be applied to clinical wildly with advantages as shorten the course,decreased diarrhea times and improved clinical symptoms.
9.Protective effect and mechanism of astragalus injection in cerebral ischemia reperfusion injury in rats

Objective To investigate the neuroprotective effect and mechanism of astragalus injection in cerebral ischemia reperfusion injury in rats .Methods A total of 70 healthy adult male Wistar rats were subjected to establish middle cerebral artery occlusion reperfusion models by inserting a monofilament thread from the external -internal carotid artery and treated by injecting 1g/L intraperitoneally astragalus injection (3ml/kg).The neurobehavioral function of rats was evaluated by Longa ’ s test and the cerebral infarction volume was calculated by tetrazolium chloride staining .The shape and ultrastructure of neurons in parietal cortex were observed by HE stain TEM .The early apoptotic ratio of neurons was detected by flow cytometry .The expressions of c-Jun N-terminal kinase 3 ( JNK3) mRNA and protein were determined by RT-PCR and Western blotting , respectively.Results After treatment with astragalus injection , the expressions of JNK3 mRNA and protein reduced significantly , the number of neuronal apoptosis in parietal cortexminus , the cerebral infarction volume shrink, the neuronal shape and ultrastructure and animal neurobehavioral function were improved significantly than those in model group rats .Conclusion The results suggest that astragalus injection may inhibit neuronal apoptosis , reduce the infarction volume and improve the animal neurobehavioral function by down -regulated the expression of JNK 3 gene following cerebral ischemia in rats .
10.Expression of Low Molecular Weight Proteins in Serum of Ischemic Stroke Patients

Objective To explore the differential expression of low molecular weight proteins in serum of ischemic stroke patients.Methods Serum samples obtained from ischemic stroke patients (n=33) and normal controls (n=39) were analyzed by Matrix Assisted Laser Desorption/ionization Time-Of-Flight Mass Spectrometry (MALDI-TOF MS). The severity of stroke was assessed with the National Institutes of Health Stroke Scale (NIHSS) and functional outcome was assessed with modified Rankin Scale (mRS). Results 55 peaks were significantly different between ischemic stroke patients and the controls (P<0.05). There were 7 peaks between 1000~2000 Da, 4 increased peaks and 3 induced peaks in serum of ischemic stroke patients. One induced peak at 1864(m/z) had highest peak intensity in controls and induced significantly in ischemic stroke patients. While induce degree of this peak didn't correlate with the NIHSS, mRS, stroke risk factors and laboratory data. Conclusion The application of this potential biomarker is not restricted to certain subgroups of ischemic stroke patients,so it may serve as one reverse direction biomarker and provide support for early diagnosis and treatment of ischemic stroke.
