1.Analyzing the Operation Efficiency of Specialized Hospitals in Guangdong from 2009 to 2014

Objective:To analyze the operational efficiency of public specialized hospitals and private specialized hospitals in Guangdong.Methods:Using the Malmquist index model of DEA to analyze operational efficiency of public specialized hospitals and private specialized hospitals in Guangdong from 2010 to 2015.Results:From 2010 to 2015,the overall operation efficiency of specialized hospitals were in low condition;the operational efficiency of public specialized hospitals was better than private specialized hospitals.Conclusion:Medical and health reform policy should attach great importance to the construction of specialized subject hospitals.Private specialized hospital was the cornucopia of hospital development which needed accurate and professional.
2.An observation on efficacy of lung detoxification soup for treatment of patients with ventilator associated pneumonia

Objective To observe the clinical efficacy of lung detoxification soup for treatment of patients with ventilator associated pneumonia(VAP)and its effects on serum procalcitonin(PCT),high sensitivity C reactive protein (hs-CRP)and white blood cell count(WBC). Methods A prospective randomized controlled trial was conducted, sixty cases of VAP with traditional Chinese medicine(TCM)syndrome of accumulation of phlegm and heat in lung from Department of Intensive Care Unit(ICU)in the First Affiliated Hospital of Anhui TCM University were enrolled, and the CASIO fx-360P function calculator was applied to generate random numbers to proceed to simple randomized dividing into two groups(each,30 cases). In the control group,mechanical ventilation,anti-infection,prevention of stress ulcer,early enteral nutrition,early weaning of ventilator and conventional western medicine treatment were given to the patients,while in the treatment group,based on the above conventional treatments in the control group,additionally the lung detoxification soup was given(soup composition:radix astragali 20 g ,baical skullcap root 15 g,coptis 10 g,shellfish 8 g,pinelliae tuber 10 g,white mustard seed 10 g,heartleaf houttuynia herb 15 g, honeysuckle flower 15 g,forsythia 15 g,banlangen 15 g,rhubarb 10 g,salvia miltiorrhiza 30 g)by nasal feeding, once 100 mL,twice a day. The therapeutic course was 7 days in both groups. The changes of TCM syndrome integral, levels of serum PCT,hs-CRP and WBC before and after treatment and the clinical efficacy were compared between the two groups. Results After treatment,the TCM syndrome integral,the levels of WBC,serum PCT,hs-CRP were significantly lower than those before treatment in both groups,and the decrease in treatment group being more marked〔TCM syndrome integral:4.73±1.10 vs. 10.33±1.46,WBC(×109/L):8.71±0.55 vs. 10.77±0.65,PCT (μg/L):1.64±0.40 vs. 5.71±1.25,hs-CRP(mg/L):4.96±1.06 vs. 8.85±1.53,all P<0.05〕. The total effective rate of treatment group was significantly higher than that in the control group〔(93.33%(28/30)vs. 73.33(22/30),P<0.05〕. Conclusion The lung detoxification soup can significantly improve the clinical symptoms,reduce the levels of WBC,serum PCT and hs-CRP in patients of VAP with TCM syndrome of accumulation of phlegm and heat in lung, therefore the soup may play a beneficial role as an adjuvant therapy for such patients.
3.Analysis of distribution characteristics and drug resistance of pathogens isolated from ascites of patients in Children's Hospital Affiliated to Zhengzhou University, 2015-2021

Abstract: Objective To investigate the distribution and drug resistance of pathogenic bacteria separated from ascites of patients in Children’s Hospital Affiliated to Zhengzhou University from 2015 to 2021, and to provide a basis for rational clinical antimicrobial agents. Methods Bacterial culture, bacterial identification and drug sensitivity analysis were performed on 1 058 non-duplicate ascites culture specimens from January 2015 to December 2021. The clinica1 and microbiologica1 data were ana1yzed by WHONET 5.6 and SAS 9.4 Results Of the 1 058 specimens, 586 (55.39%) were positive for pathogenic bacteria, with a total of 781 strains isolated. There was no significant trend of increase or decrease in the positivity rate over different years. Male children (63.99%) were more prevalent than female children. Appendicitis (59.22%) was the most common disease and Escherichia coli was the most common causative bacteria. Among neonates (≤28 d), the bacteria with the highest detection rate were Klebsiella pneumoniae (23.50%) and Enterococcus faecium (23.50%), while among children (>28 d), the highest detection rate was Escherichia coli (35.98%). Gram-negative bacteria accounted for 64.79% of the 781 strains, mainly Escherichia coli (38.28%), Klebsiella pneumoniae (8.58%), and Pseudomonas aeruginosa (5.89%); Gram-positive bacteria accounted for 29.45%, mainly Enterococcus faecium (8.58%), Streptococcus constellatus (2.69%), and Enterococcus avium (2.43%); fungi accounted for 1.66% and anaerobic bacteria accounted for 4.10%. The resistance rates of Escherichia coli to cefoperazone/sulbactam, piperacillin/tazobactam, imipenem and meropenem were 6.02%, 4.35%, 4.35%, and 3.68%, respectively. The resistance rates of Klebsiella pneumoniae to these drugs were 59.70%, 59.70%, 50.75% and 53.73% respectively. Linezolid-resistant strains of Enterococcus faecium were found. Conclusion Appendicitis is the most common abdominal infection in children, and the distribution of ascites pathogens varies with ages and diseases. The pathogenic bacteria are mainly Gram-negative bacteria, and the drug resistance of Klebsiella pneumoniae was more serious. It is particularly important to use antibiotics correctly and rationally to reduce the emergence of drug resistant bacteria.

6.Photo-toxic effects of indocyanine green based on photodynamic therapy on vascular endothelial cells in ;vitro

Objective To investigate the photo-toxic effects of indocyanine green (ICG) based on photodynamic therapy (PDT) on vascular endothelial cells in vitro. Methods Human umbilical vein endothelial cells were incubated with ICG with different concentrations in 96-well plate. According to the dose of ICG , they were divided into 6 groups as 0, 50, 100, 150, 200 and 250 μmol/L group. Each group consists of 8 wells, 4 of them were irradiated with a 810 nm laser of total power 24.0 J/cm2. Cell viability was measured by MTT assay 24 h after light irradiation. Morphological changes in cells were observed by light microscope. Results With the irradiation of 810 nm light , there was a significant decrease in relative cell viability in all groups except 0 μmol/L group (all P < 0). With the increase of concentration of ICG, the cell viability dropped rapidly (P < 0). Apoptotic changes can be seen in all groups except 0 μmol/L group and necrosis was the main change in cells of 200 μmol/L and 250 μmol/L group. Conclusion PDT with appropriate concentration of ICG has a strong inhibition to vascular endothelial cells in vitro and ICG-PDT may be a good new therapy for nevascularization.
7.Applicntion of early enteral nutrition support in critically ill patients

Objective:To investigate the effect of early enteral nutrition(EEN) on nutritional state and metabolism in the critically ill patients.Methods:65 patients who had been in intensive care unit(ICU) were randomly divided into 2 groups:one group(n=35) as EEN group,another group(n=30) as parenteral nutrition(PN) group.The results of nutrition support were analysed.Results: It was found that serum albumin and hemoglobin were elevated(P0.05).Conclusion: Early enteral nutrition support has better nutritional effect in critically ill patients.
8.High-dose methotrexate and L-asparaginase for the treatment of adult patients with high risk Ph-acute lymphoblastic leukemia

Objective To explore the efficacy and safety of high-dose methotrexate (MTX) and L-asparaginase (L-Asp) for the treatment of adult patients with high risk Ph acute lymphoblastic leukemia (ALL).Methods Five adult patients with high risk Ph-ALL were treated with several courses of MTX (3-5 g/m2 by continually intravenous drip for 24 h) and L-Asp (8 000-10 000 U/time, once a day, 10 times for one cycle).Results Five patients were disease-free survival, their survival time was 60-96 months and the median survival time was 73 months.The chemotherapy-related bone marrow depression was mild.No obvious liver and kidney damage, severe allergic reaction and pancreatitis were observed.Conclusion Highdose MTX and L-Asp are effective and well tolerated, and may contribute to long-term survival of adult patients with high risk Ph-ALL.
9.Analysis of the changes of serum throid hormones in post thoracic surgery patients

Objective To investigate the relationship between thyroid hormone levels and the attack of arrhythmia after thoracic surgery.Methods Information of 44 patients after thoracic surgery form September 2012 to February 2015 in General Hospital of Tianjin Medical University was collected, and divided in to the arrhythmia group and non-arrhythmia group.A retrospective analysis was carried on their ECG, thyroid hormone levels, and physical health, and the parameters were then compared between the two groups.The correlation of triiodothyronine (T3) level and Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ) was analyzed.Results The arrhymia patients' serum T3 and free triiodothyronine (FT3) levels were below normal levels, while the serum T3 and FT3 levels of the non-arrhythmia patients were within normal range, and the differences between the two groups were significant (P<0.05).No notable change of other thyroid hormones levels and correlation of T3, FT3 and APACHE Ⅱ score were found.Conclusions There is a significant difference between the arrhythmia and non-arrhythmia groups in serum T3 and FT3 levels.Perioperative thyroid hormone replacement therapy is worthy of consideration and trial in the clinical prevention and treatment of arrhythmia and prognostic improvement of patients.
10.Influence factors on the cryosurvival rate of post-thaw spermatozoa from sperm donors.

OBJECTIVETo investigate the influence of seasons, blood types and semen parameters on the cryosurvival rate of frozen-thawed spermatozoa from sperm donors.
METHODSWe retrospectively analyzed the influence of seasons, blood types, abstinence period, semen volume, sperm morphology, and pre-freeze sperm motility and concentration on the cryosurvival rate of frozen-thawed sperm in 4 088 semen samples from Shaanxi Human Sperm Bank.
RESULTSThe cryosurvival rate of the post-thaw sperm was correlated negatively with the progressive motility of the pre-freeze sperm (r = -0.262, P < 0.01), but positively with pre-freeze sperm concentration (r = 0.247, P < 0.01), and it was significantly higher in the 6-day abstinence group ([70.2 +/- 5.4]%) than in the other abstinence groups (P < 0.01), so was it in the normal sperm morphology rate > 20% group ([71.4 +/- 5.1]%) than in the others (P < 0.01). The survival rate of the post-thaw sperm was not correlated with seasons, blood types, and semen volume (P > 0.05).
CONCLUSIONSperm motility, concentration and morphology and the abstinence period of sperm donors may be valuable predictors of the cryosurvival rate of post-thaw sperm, but no correlation was found between the survival rate and seasons, blood types and semen volume.
