1.Areola incision combined with intraoperative B-ultrasound guided treatment in multiple breast fibroadenoma surgery for 152 cases

Objective To explore the surgery methods for multiple fibroadenoma of breast,provide a ref erence for the surgical treatment of breast multiple fibroadenoma.Methods The clinical data of 152 cases of breast multiple fibmadenoma admitted from January 2008 to October 2012 in Department of Breast Surgery,Obstetrics and Gynecology Hospital of Fudan University were analyzed retrospectively.All the cases were applied intraoperative B-ultrasound guided,taken the areola incision through breast subcutaneous layer approach for resection of multiple breast fibroadenoma.The surgical incision design,surgical procedures and results of operations were analyzed.Results All 152 cases fibroadenoma were resected which were guided by B-ultrasound with areola incision.One month and three months after operation,the assessment of physician-patient for the scars were different.There was no significance in the Pearson Correlations which were 0.894 (P =0.106) and 0.905 (P =0.065),respectively.But twelve months later,it satisfied with the appearance of scar,either the patients or the doctors (P < 0.05).The Pearson Correlation was 0.946 (P <0.001).Conclusions B-ultrasound guided areola incision through the breast subcutaneous layer approach could removal multiple breast fibroadenoma at one time,patients were satisfacted with the good cosmetic results,we believe this operation method has short operation time and good clinical value.
2.Pathology and treatment of idiopathic membranous nephropathy in children

Objective To study features of clinical manifestation,pathology and treatment effects of idiopathic membranous nephropathy(IMN).Methods A retrospective study was performed in patients with membranous nephropathy through renal biopsy diagnosis in Beijing Children's Hospital Affiliated to Capital Medical University from November 2009 to February 2013.Patients were diagnosed according to renal biopsy apart from hepatitis B virus-associated glomerulonephritis,Henoch-Schonlein purpura nephritis,lupus nephritis,tumor and drug associated nephritis.The study summed the clinical and pathological features of IMN and evaluated the effect of treatment according the follow up.Results (1) There were 13 patients,with the ratio of male and female being 1.0 ∶ 1.6,among them 1 patient was younger than 6 years old,3 cases were between 6 and 12 years old,and 9 cases were elder than 12 years old.Nine patients were nephrotic syndrome,and 3 patients had renal insufficiency.(2)Ten patients had infection,among them 5 cases were upper respiratory infection with agent unfound,4 patients were mcoplasma infection,1 patient was influenza B virus infection,1 patient had positive result of Hepatitis B surface antibody(HBsAb) and Hepatitis B core antibody (HBcAb),and 2 patients had positive result of purified protein derivative (PPD).(3)Pathological feature of IMN was atypical membranous nephropathy.Atypical membranous nephropathy was characterized by an irregular,thickened glomerular basement membrane(GBM),mesangial cells,and a mesangial matrix with mild to moderate hyperplasia.Immune complexes were stained in the subepithelial region,GBM,and mesangial region under light microscopy.Electron microscopy showed various sizes,density,and shapes of electron-dense deposits in multiple regions.The cases of immunofluorescence were 13 for IgG,10 for C3,9 for IgM,9 for C1q,5 for IgA,2 for C4,and 0 for fibrinogen.Seven patients were strongly positive(3 + and above) for IgG and one patient was strongly positive for C3.(4)Seven patients had complete remission,three patients had partial remission,and three patients had non-remission.Conclusions (1) In this study,children with IMN were mainly girls,and the age distribution was beyond 12 years.Nephrotic syndrome was the most common clinical manifestation.(2)Atypical membranous nephropathy was the most common pathological feature in children with IMN,which should be distinguished from hepatitis B virus-associated glomerulonephritis by testing HBV antigen in renal biopsy.(3) Prednisone was effective for IMN children,but it was suggested to give enough and long course of application.Effect of prednisone combined with cyclophosphamide was still undefined.

4.Clinical Observation on 30 Cases of Cerebrovascular Dementia Treated with Shen Tong Capsule

Fifty cases of mild - moderate cerebrovascular dementia selected by referring to the diagnositic criterion DSM- I -R for cerebrovascular dementia (VaD) established by America Society of Psychoneurology issued by WHO were randomly divided into Shen Tong Capsule treatment group (30 cases) and hydergine treatment group (20 cases). Modified Mental State Examination (MMSE) and Blessed Behavior Scale (BBS) were used to calculate changes of marks before and after treatment for assessment of therapeutic effects. Results show that Shen Tong Capsule has therapeutic effect on key symptoms and clinical behaviour of mild - moderate disturbance of intelligence, with the total effective rate of 86. 67%. Differences of accumulated marks in both MMSE and BBS before and after treatment in the treatment group are superior to those of the control group respectively.

OBJECTIVETo investigate the effects of different acupoint combination on prevention and treatment of stress gastric ulcer and to search for a better acupoint combination.
METHODSThe theoretical basis and experimental research on treatment of stomach diseases with the combination of he-(sea) points and front-mu points as well as back-shu points and front-mu points are analyzed in this article.
RESULTSThe combination of he-sea points and front-mu points is suitable to be applied on acute diseases, febrile diseases and diseases of fu organs. While the combination of back-shu points and front-mu points is more applicable on chronic gastric diseases.
CONCLUSIONIt is proved that the combination of he-sea point (Zusanli ST 36) and front-mu point (Zhongwan CV 12) has reliable effect on prevention and treatment of stress gastric ulcer.

7.A Control Study of N-acetylcysteine Therapy for Chronic Obstructive Pulmonary Disease

Objective NAC therapy was used to observe the effect of TNF-?,hs-CRP,PaO2,PaCO2and lung function in patients with COPD,so as to evaluate the therapeutic effect. Methods 65 patients with COPD were recruited and the patients were randomly divided into 2 treatment groups:control group (n=31) and experimental group (n=34). Patients in the two groups received the conventional treatment of COPD in unchanged dosage for the duration of the study except stopping using antibotic drug after infection was resolved. NAC tablets were taken orally at a dose of 600mg once a day in experimental group. After 8 weeks treatment,we appraised lung function and the plasma level of TNF-?,hs-CRP,PaO2 and PaCO2. Results Compared with normal controls,the plasma levels of TNF-? and hs-CRP increased significantly. Objective To summarize the 18F-FDG PET/CT findings of LCH in children,and explore its value in the diagnosis of LCH.Methods PET/CT imaging and clinical data of 13 patients (6 males,7 females;average age (3.0±2.3) years) with LCH confirmed by histology before treatment from August 2011 to December 2015 in Xin Hua Hospital were retrospectively analyzed.Results All 13 patients have different degrees of bone destruction with increased metabolism,the common lesion sites were craniofacial bone,spine,limb long bones,ribs/chest/shoulder blade and pelvic bone.Lymph node lesions which manifested lymph node enlargement with increased metabolism were found in 10 cases,and the SUVmax was 4.0±1.3.Diffuse FDG uptake in spleen was found in 10 cases.There were 4 cases with liver lesions,3 with lung lesions,1 with high metabolic nodules in muscle,1 with orbital lesions and 1 with intraspinal high metabolic nodules.Conclusion 18F-FDG PET/CT could display the distribution and activity of LCH,and plays an important role in the diagnosis and systemic evaluation of LCH.
9.Effect and influencing factors on 131I treatment for cervical lymph node metastasis from papillary thyroid cancer

Objective To assess the effect and influencing factors on 131I treatment for cervical lymph node metastasis (LNM) after operation in patients with PTC.Methods PTC patients (n =117;45 males,72 females;average age (45.17± 15.50) years) with postoperative cervical LNM from January 2010 to December 2014 were analyzed retrospectively.LNM was diagnosed by surgical pathology,imaging results and clinical follow-up.Single factor analysis was performed in age,gender,operation mode,TNM stage,131I treatment time and other factors.The results for 131I treatment effect included CR,PR,NC.Two-sample t test and x2 test were used.Results Among the 117 PTC patients with postoperative LNM,53 (45.3%) cases had non-131 I-avid metastasis.Fifty of the 64 (54.7%) patients with 131I-avid metastasis were treated with 131I.Nineteen patients (38.0%) achieved CR,22 (44.0%) achieved PR,and 9 (18.0%) showed invalid results.Fourteen of the 64 patients underwent another cervical lymph node dissection.Nine patients achieved CR,5 patients achieved PR,and 4 PR patients were then treated with 131I and finally achieved CR.Single factor analysis showed that the influencing factors of non-131I-avid lymph node included patients' age (t =3.459),serum Tg level (x2 =6.698) and metastasis with 1s F-FDG uptake (x2 =26.928;all P<0.05).The influencing factors of 131I treatment effect included lymph node dissection procedure (x2 =6.487),unilateral or bilateral lesion (x2=5.187) and LNM size (x2=8.099;all P<0.05).Conclusions 131I treatment is ineffective for nearly 50% of patients with non-131I-avid LNM.The influencing factors of 131I treatment effect include the lymph node dissection procedure,unilateral or bilateral lesions and LNM size.
10.Establishment of Quality Standard for Kechuanning Granules

Objective:To establish the quality standard for Kechuanning granules. Methods: A TLC method was used for the qualitative study on the main ingredients, Radix stemonae,ephedra and bitter almond. An HPLC method was used to determine the con-tents of ephedrine hydrochloride,pseudoephedrine hydrochloride and amygdalin. The separation of targeted compounds was performed on a Tech Mate C18-ST column (250 mm × 4. 6 mm,5 μm) . The mobile phase consisted of methanol(A) and 0. 1% phosphoric acid (B) with gradient elution at a flow rate of 1. 0 ml·min-1. The detection wavelength was 207nm and the column temperature was 35℃. Results:The spots in TLC were quite clear without any interference from negative control. The linear relationship of ephedrine hydrochloride,pseudoephedrine hydrochloride and amygdalin was 1. 826-182. 600μg·ml-1( r=0. 9997),1. 848-184. 815μg·ml-1 (r=0. 9997) and 2. 981-298. 069 μg·ml-1(r=0. 9996),respectively. The average recovery was 97. 00%,97. 40% and 98. 30%(RSD=3.7%,2.5%,1.6%)(n=9),respectively. Conclusion: The method is sensitive,accurate,simple and reproducible,which can be used for the quality control of Kechuanning granules.
