1.The alteration of intestinal flora in patients with reconstruction of the upper gastrointestinal tract

Objective: To investigate changes in species and amounts of intestinal flora after reconstruction of upper digestive tract, and the relationship with the infective complications.Methods: 52 patients with advanced malignancy being performed reconstruction of upper gastrointestinal tract was prospectively observed .The species of bacteria and counts of the colony forming units(CFU) were measured from juice taking from the upper jejunum through nasointestinal tube during operation, the second day and the fifth day after operation. Meanwhile, the plasma endotoxin concentration was determined with the modified chromogenic limulus assay.Results: There weren't significant changes of intestinal flora in species and amounts between groups (P>0.05.) No any signs of bacterial translocation were seen. There was no significant difference for the level of plasma endotoxin between groups.Conclusion: There is no disorder of intestinal flora and bacterial translocation during the perioperative period after upper digestive tract reconstruction successfully.
2.Clinical and MRI features of posterior reversible encephalopathy syndrome in patients with systemic lupus erythematosus

Objective To investigate the clinical and MRI features and pathogenic mechanism of posterior reversible encephalopathy syndrome (PRES) in patients with systemic lupus erythematosus (SLE).Methods Six cases of PRES in SLE proved by integrated clinical diagnosis were collected from January 2008 to December 2013.The clinical and MRI features of these patients were studied retrospectively and the related literatures were reviewed.Results The initial episode of nervous system was involved in 4 cases.The clinical presentations were headache (2 cases),seizures (4 cases),acute confusion state (2 cases),altered mentation (1 case) and vision change (1 case).All cases had lupus nephritis and hypertension,in which 4 cases had renal failure;6 cases were treated with immunosuppressive agents and 4 cases with cyclophosphamide for lupus nephritis when they developed PRES.The vasogenic edema lesions were distributed in the parietal or occipital lobe (5 cases),the frontal lobe (4 cases),temporal lobe (4 cases),basal ganglia (3 cases),splenium (1 case) and cerebellar hemispheres (2 cases).Three major patterns of PRES included dominant parietal-occipital (2 cases),the holohemispheric watershed (3 cases),and superior frontal sulcal (1 case).Conclusions The clinical and MRI findings of PRES in patients with SLE are typical.Awareness of this entity as early as possible can help to improve curative effect and prognosis.
3.Evaluation of the preventive effect of DL0805-2 against monocrotaline induced rat pulmonary arterial hypertension

In the treatment of hypertensive crisis, the novel Rho kinase inhibitor DL0805-2 can rapidly lower systematic blood pressure, reduce pulmonary artery pressure, and has a significant protective effect on lung injury. This experiment intends to evaluate the efficacy of DL0805-2 against pulmonary arterial hypertension (PAH) and preliminarily reveals its underlying mechanism. Animal welfare and experimental procedures are in accordance with the provision of the Animal Ethics Committee of the Institute of Materia Medica, Chinese Academy of Medical Sciences. Sprague Dawley (SD) rats were randomly divided into DL0805-2 low, medium, and high dose groups (1, 3, and 10 mg·kg-1), bosentan positive control group, model group, and blank control group. The drug was administered daily on the 7th day after model establishment by monocrotaline injection. On the 25th day of the experiment, relevant indicators were examined to observe the therapeutic effect of DL0805-2 on pulmonary hypertension. DL0805-2 significantly relieved the abnormal changes in the physiological parameters related to PAH induced by monocrotaline, including reducing right ventricular systolic pressure, alleviating cardiac damage caused by pressure overload, and reducing the levels of endothelin-1 and inflammatory factors in lung tissues. DL0805-2 also attenuated pulmonary arteries remodeling. It was preliminarily discovered that DL0805-2 exerts preventive and therapeutic effect on PAH through Rho-kinase pathway. Our results suggested that DL0805-2 had good therapeutic effects on monocrotaline-induced PAH rat model. It intervened early in the disease process, effectively prevented the development of the disease, and reduced the mortality of the diseased animals. The mechanism is related to Rho-kinase pathway.
4.The neuropsychological and cortical metabolic changes in two patients with Benson's syndrome

Objective Standard neuropsychological assessment plus structural and functional imaging were used in accurate diagnosis of Benson's syndrome (posterior cortical atrophy).Method Serial neuropsychological screening and integrative assessments of visual spatial function, 3D structural MRIimaging and functional FDG-PET imaging were used in two cases of Benson' s syndrome.Results The clinical signs were agnosia, optic ataxia, apraxia, alexia, agraphia and prosopagnosia.MRI imaging revealed bilateral parietal and occipital lobe atrophy.FDG-PET imaging revealed low metabolism in the posterior cortex.The agraphia was constructive: the words were correct but written in the wrong location.Conclusion Standard neuropsychological assessments can recognize the disease nature.When combined with the structural and functional imaging, a correct diagnosis of Benson's syndrome can be made.
5.Application of PDCA Cycle Management Method in Improving Colostomy Bags Replacement Skills of Colostomy Patients

Objective To study the clinical efficacy of the use of PDCA cycle management method in colostomy bags replacement skills of colostomy patients in our gastrointestinal surgery department. Methods 81 colostomy patients from January to May 2017 were selected as control group with general colostomy nursing management method and 69 colostomy patients from June to October 2017 were selected as experimental group with PDCA cycle management method. Patients' colostomy bags replacement qualified rate and nurses' ARC procedure qualified rate are compared between control group and experimental group. Results The incidence of problems from experimental group (5. 7%) was significantly lower than that from control group (17. 5%) with statistically significant difference (x~ 2 = 66. 74, P<0. 01); Patients' colostomy bags replacement qualified rate from experimental group (84. 1%) was significantly higher than that from control group (60%) with statistically significant difference (x ~2 = 11. 05, P<0. 01); Nurses' ARC procedure qualified rate & excellent rate in October (100%& 75%) were significantly higher than those in June (75% & 10%). Conclusion PDCA cycle management method can significantly improve colostomy bags replacement skills of colostomy patients, improve nurses' professional skills, improve nursing quality in our gastrointestinal surgery department.
6.Experimental comparison study of the tissue characteristics in transjugular intrahepalic portosystemic shunt and vascular stent

Objective To investigate the tissue characteristics within vascular stent and transjugular intrahepatic portosystemic shunt(TIPS)on swine and to provide more information for the understanding and prevention of vascular stent and TIPS restenosis.Methods Animal models for TIPS were built in 6 swine and vascular stents were implanted in iliac veins simultaneouly.14-28 days after the operation.the 6 swine were killed to remove the TIPS and vascular stent and the pathological examinations were performed on the tissues within the shunt and stent.The similarities and difierences of the tissues within the shunt and stent were analvzed with Kruskal Wallis test. Results Restenosis of TIPS occurred in 4 models and complete occlusion were seen in 2,while all vascular stents were patent and coated with a thin layer of intimal tissueElectron micmscopic results showed that the tissues in restenotic TIPS were loose and with more extra matrix and fibers.and less smooth muscle,fibroblastic and myofibroblastic cells with different and irregular shape and rich secretory granules.The tissues in patent,TIPS contained more extra fibers,smooth muscle and fibmblastic cells with normal organdie.The intimal tissues in vascular stent contained more fibers and fibroblasts cells.less smooth muscle cells.On immunohistoehemical staining,the tissues in restenotlc and Datent TIPS as well as the intimal tissues in vascular stent had strong positive expression for anti-SMCactin-α.the expression were gradually weakened for PCNA,the intimal tissues in vascular stent had a strong positive expression for vimentin,while the expression of the tissues in restenotic and patent TIPS were weakened gradually.For myoglobulin,the tissues in restenotic TIPS had weakly positive expression,the expression in patent TIPS and vascular stent were almost negative. Western blot results for TGF-β showed that the absorbance ratios of the intima tissues in vascular stent,normal vascular tissues,normal liver tissues.the tissues in restenotic and patent TIPS were 0.23,0,0,0.57 and 0.30 respectively,and they were significantiy different (H=27.8,P<0.01).Conclusions The tissues in restenotic TIPS mainly contain proliferated SMCs which have positive expression for anti-SMC-actin-α,strong proliferation and movement but unstability.The tissues in patent TIPS and intimal tissues in Vascular stent mainly contain fibroblastie cells which have positive expression for vimentin and stability.
7.Current situation of awareness and use of banned pesticides among vegetable greenhouse farmers in a town of Ledu County, China.

OBJECTIVETo investigate the awareness and use of banned pesticides among vegetable greenhouse farmers in a town of Ledu County, Qinghai Province, China and to provide a basis for the management and control of banned pesticides.
METHODSLocal residents who lived in a town of Ledu County for more than 5 years were selected as subjects in August 2012. A questionnaire survey was conducted in 267 vegetable greenhouse farmers who were selected from five villages of the town by cluster random sampling.
RESULTSOf the 267 subjects, 249 (93.26%) completed and returned the questionnaires. Eighty-seven (34.9%) of the 249 subjects were aware of banned pesticides, and the banned pesticide awareness rate varied significantly among the subjects with different education levels (χ(2) = 11.061, P = 0.011). Only education level entered the regression equation in non-conditional logistic regression analysis. Only five (2.0%) of the 249 subjects knew the banned pesticides as well as the details. All the 249 subjects used banned pesticides to varying degrees in the past 5 years. Only 9.0% (17/189) of the vegetable greenhouse farmers knew the time pesticides were initially banned in China.
CONCLUSIONThe banned pesticide awareness rate is relatively low in the town of Ledu County, and this rate is influenced by education level. It is needed to spread the knowledge and hazards of banned pesticides.

8.Effect of Chinese herb Tripterygium wilfordii Hook F monomer triptolide on apoptosis of PC12 cells induced by Abeta1-42.

Recent studies indicate that beta-amyloid (Abeta) is the key factor to cause neuronal degeneration in Alzheimer's disease (AD). In the present study, we set up an Abeta induced PC12 cell damage modle and studied the protective effect and related mechanisms of T(10), monomer extracted from Chinese herb Tripterygium wilfordii Hook F. PC12 cells were treated with different concentrations of Abeta (5x10(-4), 5x10(-3), 5x10(-2), 5x10(-1), 5, 50 micromol/L) for 48 h, cell viability was detected by MTT conversion. The apoptotic rate of PC12 cells was quantitatively determined using FACS assay. After PC12 cells were treated with 1x10(-11) mol/L T(10) for 48 h and then co-treated with 50 micromol/LAbetafor 48 h, the apoptotic rate and the change in intracellular Ca(2+) concentration of PC12 cells were analyzed by FACS assay and confocal, respectively. It was found that 5 micromol/L Abeta decreased the cell viability to 66.3% and 50 micromol/L Abeta decreased it to 55.1%, significantly different from that of the control group. After treatment with 50 micromol/L Abeta for 48 h, the apoptotic rate of PC12 cells increased obviously. The apoptotic rate was 5.37% in the control group, while after treatment with 0.5, 5 and 50 micromol/L Abeta for 48 h, the apoptotic rate of PC12 cells went up to 10.19%, 8.02% and 16.63%, respectively. At the same time, the concentration of intracellular Ca(2+) increased greatly after treatment with 50 micromol/L Abeta for 48 h. At the concentration of 1x10(-11) mol/L T(10) remarkably inhibited the apoptosis induced by 50 micromol/L Abeta. In the naive group, the apoptotic rate was 4.83%. The apoptotic rate went up to 17.24% after treatment with 50 micromol/L Abeta for 48 h. After co-treatment with 1x10(-11) mol/L T(10) and 50 micromol/L Abeta, the apoptotic rate decreased to 8.91%, significantly different from that of the control group. At the same time, at the concentration of 1x10(-11 )mol/L T(10) remarkably inhibited the increase of intracellular Ca(2+) concentration induced by Abeta. The results indicate that T(10) has obvious protective effect on PC12 cells, which may be related to the inhibition of the cell apoptosis and increment of intracellular Ca(2+) concentration induced by Abeta.

9.The effect of DHEA on AKT signal pathway on transplanted Morris hepatomas in rats.

OBJECTIVETo investigate the inhibitory effect of dehydroepaimdrosterone (DHEA) on the growth of transplanted Morris hepatomas (7288CTC) in vivo in rats.
METHODS21 Buffalo rats were randomly devided into 4 groups, including one blank control group (n = 5), one group for tumor-bearing control (n = 6), and 2 experimental groups with DHEA (n = 6) or DHEA-s (n = 4). DHEA or DHEA-s was fed to the rats for 4 weeks immediately after Morris hepatomas (7288CTC) was implanted in both flanks. Phenotypes of the spleen lymphocytes were examined by flow cytometry, Akt and PTEN expression in tumor cells was detected by Western blot and immunohistochemistry.
RESULTSTumor weights of DHEA treated group were less than those of the control (P less than 0.05), the inhibitory rate was 43%. The results of Western blot and immunohistochemistry showed that in DHEA tumor group,the expression of phosphorilated Akt protein was decreased, the expression of PTEN was enhanced, the percentage of CD3 positive cells and the ratio of CD4/CD8 were increased (P less than 0.05).
CONCLUSIONDHEA can inhibit tumor growth, possibly via the inhibition of the Akt signaling pathway as well as modulating the immune function.

10.Clinical features and prognosis of preterm infants with varying degrees of bronchopulmonary dysplasia.

OBJECTIVETo study the clinical features and prognosis of preterm infants with varying degrees of bronchopulmonary dysplasia (BPD).
METHODSThe clinical data of 144 preterm infants with a gestational age of <32 weeks who were admitted to the neonatal intensive care unit from March 2014 to March 2016 and were diagnosed with BPD were collected. According to the severity of BPD, these preterm infants were divided into mild group with 81 infants and moderate/severe group with 63 infants. The two groups were compared in terms of perinatal risk factors, treatment, comorbidities, complications, and prognosis of the respiratory system.
RESULTSCompared with the mild BPD group, the moderate/severe BPD group had a significantly higher gestational age and rate of small-for-gestational-age (SGA) infants (P<0.05), as well as a significantly higher rate of severe preeclampsia and a significantly lower rate of threatened preterm labor (P<0.05). Compared with the mild BPD group, the moderate/severe BPD group had a significantly higher percentage of infants who needed mechanical ventilation at 2 weeks after birth, longer duration of mechanical ventilation, total time of oxygen therapy, and length of hospital stay, and higher incidence rates of pneumonia and cholestasis (P<0.05), as well as a significantly lower application rate of caffeine citrate (P<0.05). The multivariate logistic regression analysis showed that SGA birth (OR=5.974, P<0.05), pneumonia (OR=2.590, P<0.05), and mechanical ventilation required at 2 weeks after birth (OR=4.632, P<0.05) were risk factors for increased severity of BPD. The pulmonary function test performed at the corrected gestational age of 40 weeks showed that compared with the mild BPD group, the moderate/severe BPD group had significantly lower ratio of time to peak tidal expiratory flow to total expiratory time, ratio of volume to peak tidal expiratory flow to total expiratory volume, and tidal expiratory flow at 25% remaining expiration (P<0.05). The infants were followed up to the corrected gestational age of 1 year, and the moderate/severe BPD group had significantly higher incidence rates of recurrent hospital admission for pneumonia and recurrent wheezing (P<0.05).
CONCLUSIONSSGA birth, pneumonia, and prolonged mechanical ventilation are associated with increased severity of BPD. Infants with moderate or severe BPD have poor pulmonary function and may experience recurrent infection and wheezing.
