1.Analysis of Mycotic Superinfections and the Use of Antimicrobials in Senile Patients

OBJECTIVE:To study the relation between mycotic superinfections and use of antimicrobials in senile patients.METHODS:106 senile in-patients who had mycotic superinfections following administration of antimicrobials were retrospectively analysed.RESULTS:Most of the senile patients over 60 years of age had severe underlying diseases and hypoimmunity and needed staying hospital for long time.Antimicrobial-induced mycotic superinfection played an important role in leading to mycotic infection in hospital accounting for 84.6% of total mycotic infections in our hospital during the corresponding period.Pulmonary infection was the commonest one and urinary tract infection the next.CONCLUSION:Long-term use of broad-spectrum antimicrobials is the main contributing factor resulting in increase of mycotic superinfections of senile patients in hospital.
2.Research on the management and practice of equipment in the modern operation room

Objective:To explorehow to manage the equipments in the modern operation room in a scientific, efficient and safe way. Methods:This paper discusses key points of the equipment management in the modern operation room from two aspects of the management network and the system construction. Combined with the hospital practice, this paper elaborates the equipment management practice in the modern operation room from the aspects of personnel responsibilities, equipment trainings and assessment mechanisms. Results: Perfect equipment management network and system in the operation room make the management more standard, improving quality control of medical equipments and the level of safety supervision as well as enriching the equipment management connotation in the operation room. Conclusion:Strengthening the equipment management in the modern operation room is the premise of improving the medical level and ensuring the safe and smooth operation .The rational use of equipment resources can improve the economic benefits of the hospital and the patients are the biggest beneficiaries, which could bring the enormous social benefits.

7.Application of HL-K rigid gas permeable contact lens for keratoconus

Objective To observe the clinical efficacy of rigid gaspermeable contact lens (RGPCL) for correcting keratoconus and analyze the predictors for the base curve of RGPCL.Methods Data of 86 patients (143 eyes) with keratoconus fitted with RGPCL were retrospectively analyzed.All eyes were divided into the mild (n =18 eyes),moderate (n =73 eyes),advanced (n =34 eyes) and severe group (n =18 eyes).After evaluating the lens fitting,best corrected visual acuity of eyes wearing spectacle lenses and RGPCL among the 4 groups was statistically compared.And the correlation between the fitting guidance index and HL-K RGPCL base curve in each group was analyzed according to keratometry.Results All patients wearing RGPCL gained significantly better corrected vision in the mild (t =-3.336,P =0.004),moderate (t =-11.213,P <0.001),advanced (t =-13.959,P <0.001) and severe groups(t =-11.047,P <0.001) than those with spectacles.Equivalent spherical degree of spectacle lenses was lower than the diopter of HL-K RGPCL in the moderate (P =0.043) and severe group (P =0.006).The mean base curve of RGPCL was (50.52 ±5.64) diopter,and it was correlated with corneal curvature parameters.In mild group,the RGPCL base curve was correlated with the steep keratometry (P < 0.001).The average K and relative steep K of 0.2 mm (Rm-0.2) were highly correlated with RGPCL base curve in moderate group (r =0.798,P < 0.001;r =0.798,P < 0.001),in advanced group (r =0.745,P < 0.001;r =0.745,P < 0.001) and in severe group (r =0.616,P =0.007;r =0.617,P =0.006).Conclusion Patients with keratoconus wearing HL-K RGPCL can obtain ideal corrected vision.Determination of RGPCL base curve by adjusting the corresponding indicators is conducive to improving the long-term wearing safety of the lens and enhancing the efficiency of lens fitting.
8.Literature Analysis of Anaphylactic Shock Induced by Fosfomycin Injection:A Report from 1994 to 2004

OBJECTIVE:To provide into the regularity of anaphylactic shock induced by fosfomycin injection.METHODS:The reports of fosfomycin-induced anaphylactic shock from 1994 to 2004 were retrieved and analyzed.RESULTS:36 cases of anaphylactic shock were publicly reported in these 10 years,among which there were 3 cases of death.CONCLUSION:Fosfomycin may induce anaphylactic shock,which should arouse more attention in clinic.
9.A case of rhabdomyolysis with compartment syndrome in the right upper extremity

Acute compartment syndrome refers to a series of
symptoms of muscles and nerves in the compartment
of the fascia due to acute ischaemia and hypoxia. It is
more common in the volar and calf of the forearm.[1]
Due to long-term ischaemia of muscles and nerves in
the fascia, muscle necrosis and neurological dysfunction
can occur, causing serious irreversible damage to
the limbs. However, rhabdomyolysis is one of the
major complications in the absence of fractures or
other trauma. Rhabdomyolysis syndrome refers to the
destruction and disintegration of striated muscle caused
by various causes, by which intracellular components
such as creatine kinase and myoglobin enter the blood
circulation, causing systemic disorders in the body and
even acute renal failure and other clinical syndromes.[2]
This case introduces rhabdomyolysis induced by skeletal
muscle ischaemia and hypoxia caused by upper limb
compression after drinking, accompanied by upper limb
compartment syndrome.
