1.Relation of idiopathic thrombocytopenic purpura with Helicobacter pylori

Objective Clinical research was made to observe the relationship between idiopathic thrombocytopenic purpura(ITP)and Helicobacter pylori(Hp)and to evaluate the effect of Hp eradication in the treatment of refractory ITP.Methods 13C urea breath test and Hp antibody of serum were made in 48 previously untreated patients with ITP from Department of Hematology of the First Affiliated Hospital of China Medical University during November 2004 to May 2005 years and normal controls consisted of 52 heathy peoples whose gastroscopy was normal.Omeprazole,clarithromycin and amoxicillin were used for Hp eradication to refractory ITP with Hp infection.13C urea breath test,Hp antibody of serum and the level platelet and PAIgG was detected after therapy 4~8 weeks.Results Of 48 cases with ITP,33(68.18%)had Hp infection and 24 of 52(46.12%)normal population had Hp infection,the infection rate of Hp in ITP patients being statistically higher than the control group(P
2.The looped ankle injury caused by cord strangulation

Objective To investigate the characteristics of looped ankle injury caused by cord strangulation, seek proper treatment to decrease amputation rate, and restore limb function as much as possible. Methods Among the nine patients with looped strangulation ankle injury, 7 had both their anterior and posterior tibia arteries involved, while the other 2 patients had only their posterior tibial artery involved. There were 4 patinets suffered from combined open tibia and fibula fractures, 2 patients from close tibia and fibula fractures; and another 2 patients from external malleolus fracture. The interval from injury to treatment varied from 6 to 27 hours(average 12 hours). All patients were treated with surgical procedures as soon as possible, blood vesseles and nerve injuries were detected and anastomosis had been done if necessary. All fractures were fixed externally. Results The limbs of all 9 patients were salvaged successfully. Most function of the injured limbs were restored. Only one external malleolus fracture malunited and was treated successfully by subsequent osteotomy. Conclusion Emergent intervention is of great importance to looped ankle injury caused by cord strangulation. Once blood vessel or nerve injuries are suspected, surgical detection or anstomosis if neccessary, should be done as soon as possible. All the associated fractures should better be fixed externally, and in order to avoid "tourniquet effect"the injured skin should be incised as early as well. With proper limb salvage procedure, good or excellent result may be achieved even in the patient with a delayed diagnosis.
3.Intravenous urography CT virtual cystoscopy in the diagnosis of bladder tumor

Objective To evaluate CT virtual cystoscopy (CTVC) in the diagnosis of bladder tumor. Methods 22 cases of bladder cancer were examined by spiral CTVC with intravenous injection of 60% urografin or omnipaque.The images obtained were compared with that observed on coventional cystoscopy and operation. Results Based on CTVC images and transverse section images of CT,tumors were classified as 3 catogories:(1)superficial papillary tumor.(2)tubercular mass papillary tumor.(3)infiltrated tumor.Of the 22 cases examined with CTVC,18 were papillomas,11 tubercalar mass papillomas and 1 infiltrated tumor.The smallest was 3.0 mm in diameter.CTVC findings have been similar to those observed on conventional cystoscopy and operation. Conclusions CTVC,a new,noninvasive,safe and useful technique,can correctly demonstrate the details of surface of bladder tumor when the latter was greater than 3.0 mm. It could show the normal or abnormal mucosal folds and the relationship of the tumor to the ureteral orifices and the bladder neck.CTVC my be helpful in the choosing of management,in the clinical staging and diagnosis of bladder tumor.
4.Electrocardiogram analysis of 339 patients with abnormal blood potassium

Objective To research the electrocardiogram (ECG) manifestations of common electrolyte disturbance and to discuss the accuracy of ECG diagnosis. Methods One hundred and sixty-nine hospitalized patients with hyperkalemia and 170 patients with hypokalemia were selected and the results of ECG were analyzed. Results In mild hyperkalemia patients (serum potassium concentration 5.5- 6.5 mmol/L), the ECG abnormal rate was 91.4% (64/70). In mild hypokalemia patients (serum potassium concentration 3.0-3.5 mmol/L), the ECG abnormal rate was 56.9% (41/72). There was significant difference (P < 0.05). The greater the deviation of serum potassium concentration, the more obvious the characteristic changes of ECG. Conclusions In addition to the blood biochemical test, ECG is also a kind of check tool that can help to diagnose abnormal blood potassium.

Objective To assess the dlagnostic value of a triple stimulation technique(TST)for benign monomelic amyotrophy(BMA). Methods A total of 15 BMA patients and 15 healthy control subiects were exam-ined with the TST.The latent periods,amplitudes,areas and durations of the negative waves of the compound muscle action potential(CMAP)and motor evoked potential(MEP)induced by the stimulation were recorded,and the de-creasing ratio of amplitudes and areas in the two groups were analyzed. Results There were significant differences in amplitude and area ratios between the BMA patients and the healthy control subjects.Both ratios were significantly lower in the BMA patients. Conclusion TST has some value in diagnosis of BMA.
8.The value of contact heat evoked potentials for the early diagnosis of diabetic peripheral polyneuropathy

Objective To study the value of contact heat evoked potentials (CHEPs) for the early diagnosis of diabetic peripheral polyneuropathy. Methods Seventy-five diabetic patients were examined by conventional nerve conduction velocity studies and then divided into 2 groups: diabetics with normal nerve conduction and diabetics with abnormal nerve conduction. Thirty-three normal subjects were used as controls. Toronto Clinical Neuropathy Sco-ring System was used to evaluate the patients. CHEPs were recorded using different stimulation intensities at different temperatures. Results The peak latencies of 45℃ , 50℃, 53℃ in diabetics with normal nerve conduction group were longer than those in normal control group, with a significant difference between the 2 groups(P<0.05). The peak latencies of diabetics were positively related to Toronto scores. Conclusion CHEPs could detect the impair-ment of diabetic peripheral nerve and reveal the impairment earlier than conventional nerve conduction velocity exami-nation, The prolongation of peak latency in diabetics group was significantly and positively related to clinical condi-tion.
10.Hepassocin (HPS)-a new serum biomarker of liver injury in mice

Objective Acute liver failure is one of the significant causes of death clinically.It is important to explore new serum markers of liver injury for early diagnosis and prognosis prediction of severe liver disease.Hepassocin ( HPS) is a liver-specific mitogenic growth factor.Our study is intended to investigate the correlation between HPS serum levels and the degree of liver injury.Methods Firstly, a mouse model of acute liver injury was constructed via intraperitoneally injection with different doses of CCl4 .Then the survival rate, alanine aminotransferase ( ALT) , aspartate aminotransferase ( AST) and the pathological changes in the liver were detected.Meanwhile, ELISA assay was performed to detect the serum level of HPS.In addition, the mRNA level and protein level of HPS were detected by real-time PCR and Western blotting, respectively.Results The mortality rate was increased and the liver damage was aggravated with the increase of the CCl4 dose.Besides, the ALT and AST levels were also increased in a dose-dependent manner.Additionally, the mRNA and protein levels of HPS were significantly up-regulated and closely related to the degree of the liver injury in the model. Conclusion HPS can be used as a new marker of liver injury in mice.
