1.Effect of Cross-legged Sitting Posture on Joint Range of Motion: Correlation with Musculoskeletal Symptoms and Facial Asymmetry

Purpose:
This study sought to study the effects of cross-legged sitting posture on joint motion. It also examined the correlation between the changes in the joint range of motion, musculoskeletal symptoms, and facial asymmetry.
Methods:
The Acumar Digital Inclinometer (Lafayette Instrument Company, USA) was used to measure the range of motion (ROM). We measured the flexion and extension of the cervical, thoracic, and lumbar spine using a dual inclinometer, and measured the ROM of the shoulder and hip joint with a single inclinometer. The Likert scale questionnaire was used to investigate musculoskeletal symptoms and facial asymmetry.
Results:
The data analysis was performed using the Jamovi version 1.6.23 statistical software. After confirming the normality of the ROM with descriptive statistics, it was compared with the normal ROM through a one-sample t-test. Correlation matrix analysis was performed to confirm the association between facial asymmetry and musculoskeletal symptoms. The result of the one-sample t-test showed a significant increase in the thoracic spine extension and right and left hip external rotation (p < 0.001***), while most other joints were restricted. As per the frequency analysis, facial asymmetry was found to be 81.70%.
Conclusion
The independent variable, namely cross-legged sitting posture led to an increase in ROM. The study also suggests that facial asymmetry and musculoskeletal symptoms could occur. Therefore, to prevent the increase and limitation of ROM and to prevent the occurrence of facial asymmetry and musculoskeletal symptoms, it is suggested that the usual cross-legged sitting posture should be avoided.
2.A Case of Continuous Positive Airway Pressure Therapy in a Patient with Central Sleep Apnea and Heart Failure.

Central sleep apnea (CSA) is a highly prevalent comorbidity in patients with heart failure and may present in 25 to 40 percent of heart failure patients. Continuous positive airway pressure (CPAP) is the primary therapeutic option and effective in treatment of obstructive sleep apnea (OSA). In heart failure patients with CSA, several trials of CPAP showed a number of positive effects in heart failure treatment. A 58-year-old male visited the hospital because of dyspnea and he was diagnosed as heart failure with ischemic heart disease. He underwent coronary angiography and received percutaneous coronary intervention due to stenosis at the middle of left anterior descending coronary artery. However, dyspnea was not completely improved after treatment with percutaneous coronary intervention. The patient also experienced snoring and sleep apnea which worsened with symptom of dyspnea in the recent year. We suspected CSA and the patient underwent polysomnography to confirm whether sleep apnea was present. During the polysomnography, CSA with Cheyne-Stokes respiration (CSR) was observed and apnea-hypopnea index was 45.9/hr. The patient was treated with CPAP. After CPAP treatment, hypoxemia and CSA were resolved and dyspnea was improved with reducing NYHA class. We report a case successfully treated with clinical improvement by presuming CSA in a patient with heart failure.

3.Effectiveness and Learning Experience of Dental Communication Course Training in Dental Hygiene Schools.

The purpose of this study was to develop a dental communication course in dental hygiene schools that included theory, practice, and skill for effective dental communication. Thirty-six senior dental hygiene students in a dental hygiene school took a dental communication course and responded to a questionnaire. The instrument used in the study was a modified form of Kim's communication skill self-assessment sheet. The self-assessment questionnaire about communication competencies was administered before and after the class, and the difference between scores at each time point was analyzed using the Wilcoxon signed rank sum test. Among seven elements of dental communication competencies, the competency of “information gathering” was significantly improved (p=0.008). Students' scores on perception of the importance (p=0.019) and necessity (0.016) of a communication course significantly increased after the course. Competencies in communication are essential requirement for dental hygienists. This study showed the possibility of communication skill training. An objective evaluation tool regarding students' communication competencies should be developed. Further studies with larger samples size are needed to develop a standardized comprehensive communication course in dental hygiene schools.

4.Clinical Values of HRCT for Diagnosis of Incus Dislocations.

BACKGROUND: The quick and accurate radiologic evaluation is essential to minimize the sequelae of temporal bone trauma. High resolution computed tomography(HRCT) is regarded as a method of choice in the evaluation of the head trauma including temporal bone fractures. Although the diagnosis of ossicular dislocation with HRCT has been described, we could find only one report on the usefulness of coronal sections in the diagnosis of lateral displacement of the incus. OBJECTIVES: The purpose of this study is to evaluate the usefulness of a recently reported radiologic sign, named the "Y" sign, for the diagnosis of lateral dislocation of the incus on the coronal HRCT sections of the temporal bones. MATERIALS AND METHODS: We retrospectively reviewed the axial and coronal HRCT sections of 13 cases who the laterally dislocated incus was confirmed operatively. RESULTS: In the axial HRCT sections, abnormal ice cream cone was shown in only 3 cases, but normal ice cream cone was shown in 10 cases. In the coronal HRCT sections, the Y-sign was shown in 11 cases-open Y-sign: 3 cases, closed Y-sign: 8 cases, in contrast, only 2 cases did not shown Y-sign. CONCLUSION: The Y-sign in the coronal sections is more correlated with the dislocated incus than with abnormal ice cream cone configuration in the axial sections. So, the Y-sign seems to be very useful in the diagnosis of the dislocated incus.

5.Treatment of Gap Nonunion of the Tibia by the Ilizarov Method

Twenty four patients with gap nonunions of the tibia were treated with the Ilizarov intercalary bone transport method. The purpose of this retrospective study is to evaluate the results and complications in gap nonunions of the tibia treated using the Ilizarov method. There were 22 males and two females with an average age of the 35.8 years(range, 23-63 years). The mean follow up period was 30.2 months(range, 17-70 months). Ten of the 24 patients had no external shortening with bone gap, 13 external shortening with gap and one hemicircumferential defect. On average, the size of bone loss measured 8.2 cm(range, 3-18 cm). Twenty-one of 24 patients were treated by whole segment internal transport(fifteen were proximal fragment transport, five were distal fragment transport, one was proximal and distal fragment transport), one patient by anterior hemicircumferential corticotomy and partial bone fragment internal transport and tow patients by fibula transfer. All patients healed with solid bony union, although twelve patients required bone grafts, eleven at the docking site and one at the distraction site. On average, the healing index was 49 days/cm(range, 18-82 days/cm). There was on relationship between the percentage transport(size of the distraction gap/the transporting fragment length x 100) and healing index. Numerous complications were encountered, most commonly delayed union and pin site infection, Conclusively, the application of Ilizarov techniques to gap nonunions of the tibia was very effective, but required correct technique and careful follow-up examination.

6.Formula Comparison for Intraocular Lens Power Calculation Using IOL Master and Ultrasound for the ZCB00 IOL.

PURPOSE: To evaluation the accuracy of the IOL power calculation formulae measured by IOL Master(R) and applanation ultrasonography for the Tecnis ZCB00 IOL. METHODS: We performed a retrospective study of 170 eyes in 121 patients who underwent cataract surgery in our hospital with AMO Tecnis ZCB00 IOL.s. The SRK/T formula was used to predict the patient's implanted IOL power. Differences in the predicted refractive errors between IOL Master(R) and ultrasonography were analyzed and factors attributed to the differences were also analyzed. Three months after cataract surgery, mean numeric error and mean absolute error were analyzed. RESULTS: SRK/II and SRK/T formulas calculated using ultrasonography showed differences compared to the same formulas calculated using IOL Master(R), in which hyperopic shift was also demonstrated. No definite factor was attributed to the differences between the 2 methods. Although the 3 formulas of IOL Master(R) showed no significant difference in refractive errors, the SRK/T formula calculated using IOL Master(R) showed the least mean absolute and numeric errors. CONCLUSIONS: IOL Master(R) is considered more suitable when determining proper AMO Tecnis ZCB00 IOL power in cataract surgery. The hyperopic shift should be considered when calculating the IOL power using only ultrasonography.

7.Plate Fixation for Fractures of the Coronoid Process of the Ulna.

PURPOSE: The purpose of this study was to evaluate the results of eight cases of coronoid process fractures that were fixed with a plate. MATERIALS AND METHODS: Eight coronoid process fractures were treated by plating and these cases were reviewed retrospectively. Six patients were men and two were women. The average age was 41 years (range: 22-79) at the time of injury. According to Regan's classification, there were five type 2 and three type 3. According to O'Driscoll's classification, there were five anteromedial type and three base type. Open reduction and internal fixation with a plate were performed through a medial approach by splitting of the two heads of the flexor carpi ulnaris. The patients were follow-up for a mean of 15.8 months (range: 6-25). We evaluated the clinical outcomes with using the Mayo Elbow Performance Score. RESULTS: The average active motion of the elbow joint was 120 degrees. The average Mayo Elbow Performance Score was 86.9. There were 5 excellent results, 1 good result and 2 fair results. SUMMARY: Plating through a medial approach of the elbow provided stable fixation and satisfactory union for treating displaced coronoid process fractures with the unstable elbow.

8.Effects of Ethanol on the Activities and Inducibility of Trichloroethylene Metabolic Enzyme System in Rat Liver.

This study was performed to find out the influences of ethanol on the metabolism of trichloroethylene(TRI) in rats. TRI in corn oil at the dosage of 150, 300, 600 mg/kg was injected peritoneally once a day for two days to two groups. In one group ethanol(4 g/kg) was taken orally 30 minutes before TRI injection, and the other group ethanol was not. The results of experiments are as follows: 1. The contents of cytochrome P-450 and b5 had inverse relationship with in-jected TRI amounts in both groups. 2. The activity of NADPH P-450 reductase was decreased slowly in TRI injected group related with TRI amount, but decreased drastically in the group pretreated with ethanol. 3. The activity of NADH b5 reductase had relationship with injected TRI amount, but the statistical significance was found only in the groups of 300 and 600 mg/kg of TRI injected without relevance to ethanol when compared with the group that was not injected. 4. The activity of ADH was more decreased and ALDH activity was more increased in groups that TRI injected and ethanol was pretreated with ethanol groups than in group without any treatment. These results suggest that ethanol may inhibit epoxide formulation, the first step or TRI metabolism, and change from TCE-OH to TCA also.

9.Utilization of Transferrin-Bound Iron by Medically Important Staphylococcal Species.

Staphylococcus aureus is able to utilize efficiently transferrin-bound iron as an iron source, whereas other staphylococci are not. The reason for this difference remains unclear. We compared the activity of siderophore-mediated iron-uptake systems among S. aureus, S. epidermidis, and S. saprophyticus. S. aureus was more susceptible to streptonigrin than the other two staphylococci. S. aureus was able to utilize efficiently transferrin-bound iron in proportion to the level of iron-saturation and produced siderophores in an inverse relation to iron-saturation. In contrast to S. aureus, S. epidermidis and S. saprophyticus were able to utilize only holotransferrin (HT; about 80% iron- saturated) and produced siderophores only in media containing HT. Moreover, they utilized HT less efficiently than S. aureus, though they produced greater amount of siderophores than S. aureus in media containing HT. The ability of the equivalent siderophores per se to capture iron from HT was not significantly different among the three species. Nevertheless, the siderophores from S. aureus stimulated the growth of the staphylococci to a greater degree than did the siderophores from S. epidermidis and S. saprophyticus. The siderophores from S. epidermidis and S. saprophyticus also stimulated the growth of S. aureus to a greater degree than those of the original bacteria which produced them. This indicates that S. aureus possesses a greater ability to produce more-efficient siderophores responding to very low iron-availability, as well as a greater ability to utilize iron-siderophore complexes, than the other two staphylococci. This explains in part the higher virulence of S. aureus compared to other staphylococci.

10.Anesthetic management for the endoscopic sinus surgery of a patient with coexisting severe cervical spine ankylosing spondylitis and unruptured cerebral aneurysm: A case report.

A 61-year-old man was admitted to the emergency room complaining of a severe left exophthalmos caused by frontal and ethmoid sinus mucoceles that were visualized on a brain computerized tomogram. In addition, he had coexisting ankylosing spondylitis with a 20 year duration that resulted in total fixation of the cervical spine and progressive thoracic kyphosis. An unruptured anterior communicating artery aneurysm was found incidentally on the cerebral angiogram. We report that the anesthetic management for endoscopic sinus surgery of a frontal sinus mucocele in a patient with coexisting severe cervical spine ankylosing spondylitis and an unruptured cerebral aneurysm requires a detailed preoperative assessment of the airway, cardiac, pulmonary, and neurologic system. This case highlights the need for careful measures to avoid rupturing the cerebral aneurysm by the increased blood pressure induced by endotracheal intubation and the infiltration of an epinephrine-containing local anesthetic.
