1.Study on Angiotensin Converting Enzyme Gene Polymorphism in Children with Congenital Heart Disease

Objective To investigate the relationship between the insertion/deletion(I/D) polymorphism of the angiotensin convcer-(ting) enzyme(ACE) gene and congenital heart disease(CHD) and to quest the predisposing gene of congenital heart disease by analyzing the polymorphism of the ACE gene in patients with CHD.Methods Sixty-eight patients with CHD were served as observing group,sixty-four patients without CHD as control group.We drew DNA of leukocyte from blood by the method of saturation phenol and chloroform and we amplified the ACE gene using the DNA polymerase chain reaction technique.The amplification products were employed electrohoresis in 2% sepharose.the polymorphism of the angiotensin converting enzyme gene was analyzed under ultraviolet lump.Results In the obser-(ving) group,the genotype distribution frequency of the ACE was II in 12(17.65%),DD in 32(47.06%) and ID in 24(35.29%),and in the control group,it was II in 20(31.25%),DD in 10(15.63%) and ID in 34(53.12%).Conclusions The study result clarifies that CHD has close correlation to the polymorphism of the angiotensin converting enzyme gene.The relative risk of DD genotype is higher than that of the others,and DD genotype possibly is the predisposing genotype of CHD D allele perhaps is the susceptibility gene of CHD.
2.Expression and clinical significance of hypoxia-inducible factor-1? (HIF-1?) in squamous cell carcinoma of buccal mucosa

Objective:To study the expression and clinical significance of hypoxia-inducible factor-1? (HIF-1?) in squamous cell carcinoma of buccal mucosa.Methods:45 cases of paraffin embedded samples of squamous cell carcinoma of buccal mucosa were examined with HIF-1? antibody by immunohistochemistrical method. The relationship between HIF-1? expression and clinico-pathological factors such as sex, age, duration of tumour, T classification, differentiation, lymph node metastasis in the neck, recurrence, clinic classification, tumour thickness, the number of mitosis and the degree of lymphocyte invasion was analyzed.Results:The positive expression of HIF-1? was found in 31 out of the 45 cases(69%). HIF-1? positive reaction was predominately located in the center of tumour nest. While analyzing clinico-pathological factors, only T classification (between T1+T2 and T3+T4, P=0.008), lymph node metastasis of the neck (P=0.037) and clinic classification (between stage I+II and III+IV, P=0.011) showed a significant correlation with HIF-1? expression. HIF-1? possitive expression is higher in T3+T4(d=4.87 mm) than that in T1+T2(3.81 mm) (P=0.046). Logistic analysis showed that the longer the patients lived after operation, the less the HIF-1? positive expression.Conclusions:HIF-1? positive expression in squamous cell carcinoma of the buccal mucosa correlates to T classification, lymph node metastasis of neck, clinical classification and survival
3.Comparison and experience of two vacuum assisted biopsy systems in benign breast lesions: EnCor and Mammotome

Objective To investigate application value of two vacuum assisted biopsy systems in benign breast lesions and compare the advantages and disadvantages of EnCor and Mammotome system.Methods Vacuum assisted excision was performed on 346 benign breast lesions in 227 patients (EnCor with 103 cases, Mammotome with 124 cases) from November 2012 to May 2013, including 162 cases using EnCor system and 184 cases using Mammotome system.Results Duration of operation in EnCor group was (14.18± 4.21) minutes,in Mammotome group was (18.77 ± 7.06) minutes, and the difference was significant (P < 0.001) .Weight of tissues in Mammotome group was (6.87± 3.43) g, in EnCor group was (8.32± 1.18) g, and the difference was significant (P < 0.001).No significant differences were found in postoperative hematoma (P =0.58), postoperative pain (P =0.35), incidence of residual tumor (P =0.15).Conclusion Compared Mammotome group, EnCor group had significantly shorter duration.But using Mammotome system, the specimens can be observed directly.Mammotome is more suitable for women of childbearing age, it can preserve lactation function.There is no significant difference in complication and residual tumor of two vacuum assisted biopsy system in benign breast lesions.
4.Arthroscopic treatment for tuberculous synovitis of the knee: A report of 10 cases

Objective To observe the therapeutic effect of arthroscopic synovectomy for tuberculous synovitis of the knee. Methods Ten patients with tuberculous synovitis received arthroscopic biopsy and synovectomy. Postoperatively, isoniazid was injected into the articular cavity, and systemic antituberculosis drugs were administrated. Results These patients received continuous passive motion (CPM) from the second postoperative day. In 9 patients, the flexion ranges of the knee were improved from 90??5? before the surgery to 120??14? after the surgery, and the extension limit of the knee, from 20??3? before the surgery to 5??1? after the surgery, with statistical significant differences ( t =6.9 and 6.3, respectively; P
5.Maternal and fetal outcomes in pregnant women with abnormal glucose metabolism

Objective To understand the incidence of abnormal glucose metabolism during pregnancy and the maternal and neonatal outcomes after standard management.Methods A retrospective study of maternal and neonatal outcomes was conducted in 1490 pregnant women who were diagnosed and treated for abnormal glucose metabolism and delivered in the Department of Obstetrics and Gynecology of First Hospital of Peking University from Jan 1995 to Dec 2004 by reviewing the medical records.The selected cases consisted of 79 women with diabetes mellitus(DM group),777 with gestational diabetes mellitus(GDM group),including 355 cases of A1,316 with A2 and 106 cases unclassified,and 634 with gestational impaired glucose test(GIGT group).Maternal and fetal outcomes were analysed in comparison with the controls of 19 013 pregnant women with normal glucose metabolism who delivered during the same period.Results(1)The total incidence of gestational abnormal glucose metabolism was 7.3% and increased gradually from 1995 to 2004.The first stage,from Jan 1995 to Dec 1999,saw a slow increase in the incidence [4.3%(376/8739)];the second stage,from Jan 2000 to Dec 2001,showed a fast increasing trend.The average incidence was 10.8%(445/4133).The incidence in the third stage kept stable at 8.9% (678/7640)from Jan 2002 to Dec 2004.(2)The incidence of macrosomia,preeclampsia and preterm birth were 12.1%(180/1490),9.5%(141/1490)and 9.4%(140/1490),which were significantly higher than those women with normal glucose metabolism(P0.05).(3)The perinatal mortality rate(PMR)of abnormal glucose metabolism group was 1.19%(18/1513)which was significantly higher in the DM group (4.93%)than GDM(1.14%)and GIGT groups(0.78%,P

8.Effect evaluation on employee assistance program (EAP) for medical workers

Objective To evaluate the mental health of medical workers, and develop an EAP suitable for heahhcare sector. Methods A 1-year EAP was implemented in a hospital in Beijing, in which SF-36 and JSS scales were used to study the health-related quality of life and job satisfaction of these workers. Results Work pressure is found to be the main source of pressure for medical workers, accounting for 63.7%. Scores of the group receiving EAP service are found to be significantly higher (P<0.01) in all dimensions (excepting work nature and supervisor dimension) of both SF-36 and JSS. Conclusions Medical workers are exposed to high pressure of work, having strong need for EAP service as result. EAP is an effective means to maintain mental health and improve quality of life and job performance, if applied widely.
9.Application of confocal laser scanning microscopy in the diagnosis of superficial cutaneous fungal infections

Objective To estimate the performance of confocal laser scanning microscopy (CLSM) in the diagnosis of superficial cutaneous fungal infections. Methods This study recruited 59 patients with clinically suspected superficial cutaneous fungal infections.Three typical lesions were selected in each patient for CLSM and microscopic examination.Results CLSM revealed hyphae in stratum corneum in 56% (14/25) of tinea manus or pedis and 79.17% (19/24) of tinea cruris lesions,7 out of 8 tinea manus or pedis and 94.12%( 16/17 ) of tinea cruris early lesions (< 3 weeks ),and 41% (7/17) of tinea manus or pedis and 3 out of 7 tinea croris old lesions (> 3 weeks).All the CLSM-positive specimens were positive for microscopic examination,and among the CLSM-negative specimens,fungal elements were observed by microscopic examination in 8 out of 11 tinea manus or pedis specimens,4 out of 5 tinea cruris specimens,1 tinea manus or pedis and 1 tinea cruris specimen of early lesions,and 7 out of 10 tinea manus or pedis and 3 out of 4 tinea cruris specimens of old lesions.No hypha was found by CLSM in any of the 10 tinea versicolor specimens,while microscopic examination revealed fungal elements in 8 of them.Neither CLSM nor microscopy revealed fungal elements in lesions from 5 patients with tinea manus or pedis and 5 patients with tinea cruris after treatment with topical bifonazole cream for 2 weeks.Conclusions CLSM shows a good consistency with light microscopy in the examination of early lesions of tinea marnus and pedis as well as tinea croris,and may serve as a valuable tool for clinical diagnosis.
10.Maternal and perinatal prognosis of pregnancy with chronic hypertension and analysis of associated factors
