1.Different surgical approaches of endoscopic thyroidectomy for patients with separation of flap size,complications and the corresponding damage

Objective To explore the effect of trans-axillary single-port and bilateral breast of ipsilateral axillary approach in endoscopic thyroidectomy on area of skin flap ,complications and the corresponding impact damage .Methods A total of 72 patients who were underwent either BBIAA or TASPET in our hospital from February 2013 to April 2015,were divided into BBIAA group and TASPET group ,each group had 36 cases.The area of skin flap ,complications and the corresponding impact damage between two surgical methods were compared .Results The average separation of the flap area in TASPET group was significantly higher than that in BBIAA group (P<0.01).The VAS score of BBIAA group was (0.24 ±0.39) point,which was lower than (1.85 ±1.37)point of TASPET group,the difference was significant(P<0. 01 ) .The patients of BBIAA group had no complications ,while there were 10 patients suffered complications in TASPET group ,and the differ-ence of postoperative complications was significant (P<0.01).Conclusion Bilateral breast and ipsilateral axillary approach have the advan-tage of less operative time ,smaller separation of the flap area ,lower complication incidence and less damage on parathyroid gland and nerve , which had an ideal safety and cosmetic result .
2.Effects of hypoxia-inducible factor-1α on the expression of heat shock proteins 90, Caspase 3 and Caspase 9 after cerebral ischemia-reperfusion

Hypoxia-inducible factor-1α (HIF-1α)is a transcription factor that acts as a master regulator of gene expression induced by low oxygen conditions.HIF-1α is the most sensitive indicator of hypoxia and can be activated by systemic hypoxia,cerebral hemisphere ischemia and cerebral focal ischemia.HIF-1 expression can be induced by hypoxic-ischemic damage after cerebral infarction and exert its effects on hypoxia-inducible downstream genes,thus protecting hypoxic tissues and cells under hypoxic conditions via regulating the expression of its downstream target genes.This article reviews the expression of HIF-1α and its downstream target genes in acute cerebral ischemia,in order to provide a theoretical basis for gene therapy.
3.Meta-analysis of entecavir in lamivudine-refractory patients with chronic hepatitis B

Objective To evaluate the efficacy of entecavir in lamivudine-refractory patients with chronic hepatitis B.Methods Literature from Medline,Embase and CBM between January 2001 to December 2008 were reviewed.Acccording to the including and excwding criteria,four randomized controlled trials (RCT) were enrolled.The results of the trials were reviewed and analysed in software Revman 4.2.Results Patients in the entecavir group enjoyed significantly higher negative conversion ratios of HBV-DNA and ALT,and positive seroconversion of HBeAg:RR were 13.90(95% CI 6.39~30.24,P<0.00001),2.49(95% CI 1.40~4.45,P=0.002),and 3.53 (95% CI 2.85~4.38,P<0.000 01) respectively.There was a trend for the improvement of negative conversion ratio of HBeAg (RR=2.27,95% CI 1.00~5.15,P=0.05),but the incidence of side-effects of the drugs has no difference (RR=1.05,95% CI 0.97~1.14,P=0.21).Conclusions Compared with lamivudine or placebo,entecavir can significantly decrease HBV-DNA,improve liver function and positive seroconversion ratio of HBeAg without raising the incidence of drug side-effects.
4.Diagnosis and treatment of primary hyperparathyroidism:a report of 8 cases

Objective: To study the diagnosis and treatment of primary hyperparathyroidism.Methods: The clinical data of 8 primary hyperparathyroidism patients(1992 2002) in our hospital were summarized and the references were reviewed.Results: It was found that primary hyperparathyroidism was liable to be misdiagnosed.Information on blood calcium,parathyroid hormone(PTH) and imaging may be helpful for diagnosis. Surgical resection(4 cases), anhydrous alcohol injection (2 cases), electron radiation (1 case) and internal medicine (1 case) were applied. Conclusion:Information on blood calcium,PTH and early imaging is helpful for diagnosis.Operation is the first choice of treatment, and for patients who can not tolerate surgery, anhydrous alcohol injection is an effective and safe treatment.
5.Significance of hypoxia induced alveolar epithelial cell apoptosis and hypoxia inducible factor-1alpha expression

Objective To investigate the effects of cobalt chloride(COCl2)induced rat alveolar epithelial cell apoptosis and the significance of related apoptosis protein hypoxia inducible factor-lalpha(HIF-1 a)expression. Methods Alveolar epithelial cells were cultured in vitro and divided into four groups with 6 samples in each.Hypoxia 4 h group:added 500μmol/L COCl2 for 4 hours inducing alveolar epithelial cell hypoxia;hypoxia 24 h group:co-cultured with COCl2 for 24 hours;hypoxia 48 h group:co-cultured with CoCl2 for 48 hours;control group:no CoCl2 added.The apoptosis rates were detected by fluorescent microscope and flow cytometry.The apoptosis morphological changes of the cell were observed by transmission electron microscopy.The protein expression of HIF-la was detected bv Western Blot. Results (1)Fluorescent microscope results showed that apoptosis rate in hypoxia 4 h、24 h、48 h group[(5.83±0.76)%,(15.00±3.28)%and(51.50±3.00)%],were higher than control group[(1.50±0.50)%](P<0.05).(2)Flow cytometry analysis reached the same results as with fluorescent microscope.(3)Typical morphological changes of apoptosis were observed in hypoxia 24 h group.(4)HIF-1α expression in hypoxia 4 h、24 h and 48 h groups[0.69±0.035,1.02±0.044 and 0.71±0.046],were higher than control group(0.21±0.026)(P<0.01).(5)Positive correlations were found between relative amount of HIF-1αprotein and apoptosis rate by fluorescent microscope and flow cytometry analysis(r=0.484,P=0.016;r=0.713,P=0.009).Conclusions Hypoxia could induce apoptosis of alveolar epithelial cells,which may participate in the pathogenesis of hypoxia-induced lung injury.
7.Effects of the aqueous extract of dendrobium nobile lindl on NF-κB and IL-6 expression of renal tissue in diabetic rat model

Objective To observe the effects of dendrobium nobile Lindl(DNL) on Nuclear factor(NF)-κB and interleukin(IL)-6 expression of renal tissue in diabetic rat model .Methods The diabetes mellitus rat model was induced by streptozotocin .All rats were divided into four groups at random :the normal control group(group N) ,diabetic control group(group diabetes mellitus) ,drug group one(group DNL1) ,drug group two(group DNL2) ,and administrated for 12 weeks .The levels of the fasting plasma glucose , urea nitrogen ,serum creatinine ,urine creatinine ,24 hours urinary protein were determined .NF-κB and IL-6 protein expression levels were measured by Western-Blot .IL-6 mRNA expression levels were measured by Real-time fluorescence quantitative PCR . Results Compared with group N ,the blood sugar ,urea nitrogen ,24 hours urinary protein of group diabetes mellitus were signifi-cantly higher(P<0 .01) ,creatinine clearance was significantly reduced (P<0 .01) ,the NF-κB、IL-6 protein expression levels and IL-6 mRNA level increased significantly (all P<0 .05) .Compared with group diabetes mellitus ,the blood sugar ,urea nitrogen ,24 hours urinary protein of group DNL1 and group DNL2 were significantly decreased(P<0 .05) ,creatinine clearance increased significantly (P<0 .05) ,the NF-κB、IL-6 protein expression levels and IL-6 mRNA level decreased significantly(all P<0 .05) .Conclusion The DNL could protect the kidney of diabetic rats ,reduce the levels of NF-κB 、IL-6 protein and IL-6 mRNA expression in renal tissue of diabetic rats .
8.Microanatomy and hemodynamic numerical simulation of the cerebral bridging veins entering superior sagittal sinus

Objective To simulate the hemodynamic feature in cerebral bridging veins (BVs), in order to provide a morphologic basis for the pathogenesis explanation and imaging diagnosis of cerebral venous thrombosis (CVT). MethodsTotally 6 human cadavers (12 sides) were examined in this study. Each head of the cadavers was injected with blue-coloured latex via the superior sagittal sinus (SSS) and internal jugular veins. The diamter and the angle of BVs entering SSS were measured. Based on the data of cadavers and computational fluid dynamics software pack, the hemodynamic models were established. The wall shear stress (WSS) was carefully studied and compared between different models. Results The total of 137 BVs formed two clusters along the SSS: anterior group and posterior group. Compared with anterior group BVs, the diameter of posterior group BVs was large, and the angle was smaller. In 137 models,when the diameter of a BV was more than 1.2mm, and the angle was between 65 and 105 degree, the local WSS decreased in the downstream wall of SSS. When the diameter of a BV was more than 1.2mm, and the angle was less than 65 degree, the local WSS decreased in the downstream wall of SSS and the upstream wall of BVs. The minimum WSS in BVs was 63% of the minimum WSS in SSS. Compared with the anterior group BVs, the minimum WSS in the wall of posterior group BVs was samller, and the distance from the minimum WSS to the dural entrance was longer. Conclusion CVT occurs easily when the diamter of a BV is more than 1.2mm and the angle is less than 65 degree. The embolus forms early in the upstream wall of BVs entering the posterior part of SSS.
9.Wnt signaling pathway and osteosarcoma

The Wnt pathway plays a crucial role in skeletal development and is indispensable for determination of OS cell lines. In recent years, experimental evidences on Wnt signaling pathway in OS cell lines and OS animal models, and that of Wnt signaling pathway as a diagnosis and prognosis marker of OS suggested that Wnt signaling pathway plays an important role in the progression, invasion and metastasis of OS. In addition, the strategy and safety evaluation to target Wnt to treat OS are underway. Exploring the significant role of Wnt signaling pathway in OS may aid in personalizing therapeutics to increase patient survival.
10.Barton fractures treated by π-shaped locking compression plates

Objective To discuss the effects, indications and methods of π-shaped locking com-pression plates plus open reduction for Barton fractures. Methods From January 2006 to January 2007, 21 cases of Barton fractures were treated by open reduction from dorsal incision and fixation by π-shaped locking compression plates. Of them, 14 also had bone grafting and 6 had protection by a plaster brace. Results Follow-ups for 6 to 18 (average, 10) months showed all the patients got bone union. The differences between preoperative and postoperative measurements in palmar inclination, ulnar deviation and relative length of distal radius were statistically significant (P < 0.05). At the last follow-up, the affected wrist showed no significant difference from the opposite side in dorsal extension, palmar flexion and ulnar deviation (P > 0.05) except in ulnar deviation (P < 0.05). According to the improved Gartland and Werkey grading system, the wrist function was evaluated as excellent in 15 eases, good in 4, and fair in 2. The good to excellent rate was 90. 5%. Conclusion Open reduction from dorsal incision and fixation by π-shaped locking compression plate is an effective treatment for Barton fractures.
