Journal of Interventional Radiology 2009;18(9):702-704
Objective To investigate the technical points and the clinical effectiveness of transcatheter arterial chemoembolization (TACE) combined with CT-guided ~(125)I radioactive particle implantation for the treatment of liver cancer. Methods Twenty-seven patients with hepatic cancer, proved by color Doppler ultrasonography, CT and AFP, were enrolled in this study. All the patients received embolization therapy with lipiodol not long before. Of the 27 patients, preoperative CT scanning was performed in 16. Based on the CT findings, the therapeutic protocol was formulated to determine the amount and site of ~(125)I radioactive particle to be implanted. When drawing the outline of target area, the targeted sedimentation extent which was delineated on CT scan should be exceeded the area with deposits of lipiodol by 0.5-1.0 cm. The average energy of ~(125)I radioactive particle was 27-35 keV. Results Of 27 patients, complete remission was seen in 2, partial remission in 16, unchanged condition in 6 and exacerbation of the condition in 3, with a total efficiency of 66.7%. The patients were followed up for 6 months. One patient died of distant metastasis and the remaining ones survived so far. Conclusion Transcatheter arterial chemoembolization combined with CT-guided ~(125)I radioactive particle implantation is a safe and effective treatment for liver cancer.
2.Dose issue of novel oral anticoagulants to prevent stroke in patients with atrial fibrillation
International Journal of Cerebrovascular Diseases 2016;24(4):343-348
Oral anticoagulants are important for preventing stroke in patients with atrial fibrillation.Compared with the traditional oral anticoagulant warfarin, the novel oral anticoagulants (NOACs) have the characteristics of high efficiency, safety, and no need to monitor coagulation function. However, current clinical reports have showed that the dose is usually low when NOACs were used for preventing stroke in patients with atrial fibrillation. Its main reason may be associated with the risk avoidance and the difference in doctor and patient preferences. The analysis from the aspects of safety and effectiveness, the risk of ischemic stroke or systemic embolism is lower when the dose of NOACs is relatively high, and the risk of hemorrhage is lower when the dose of NOACs is relatively lower.Given the differences between the incidences of different events, the disability rate and the mortality rate, the patient with atrial fibrillation are more suitable for using high-dose drug. When choosing a specific dose, taking into account the specificity, the appropriate dose, intensity, and dosing regimen should be given according to the guideline recommendations, appropriate reference to renal function and patient preferences, and individual differences in order to obtain the best clinical efficacy..

Objective To investigate the indications to use a temporary pacemaker for bifascicular block in perioperative period. Methods The treatment for 40 patients with heart bifascicular block and a special case were retrospectively studied. All cases, according to their disease history, clinical symptoms, physical ability and atrio-ventricular conduction ability, were divided into two groups:P group(prophylactic insertion of a temporary pacemaker) and N group(without inserting a pacemaker).During operation,we observed whether atrio-ventricular block occurs, the performance of pacemaker and atropine's therapeutic efficacy to bradycardia. Results Anesthesia was postponed in one case because of acute left bundle branch block(LBBB) and the patient died with severe myocardium damage soon after failed resuscitation. All other cases went through anesthesia and operation smoothly.Bradycardia and hypotension could be relieved somewhat by medication in operation, and no complete atrio-ventricular block developed. Conclusion There is no need inserting a temporary pacemaker for chronic bifascicular block if asymptomatic and no atrio-ventricular block.Transesophageal atrial pacing is an easy way to evaluate in quantification atrio-ventricular conduction ability.

Objective To explore artifactual responses of auditory steady-state reponses (ASSRs) to bone-conduction stimuli. Methods The present study involved a group of subjects, aged between 5 to 39 months, with bilateral profound sensorineural hearing losse.Recorded were the reponses to multiple bone-conducted 0.5,1,2,4 kHz tone pips repeated at the rates of 77~101 Hz.All the subjects were sedated by Hydrate Chloride during the testing. Results Artifactual ASSRs were seen at 115 and 100.5 dB (re:1?N) (The above levels are equivalent to around 60 dB HL) at 1 and 4 kHz.No artifactual responses were recorded at 0.5 and 2 kHz stimuli at the maximum output level. Conclusion Bone-conducted hearing loss greater than moderate can not be tested reliablely due to artifactual responses and lower maximum output level than that of air conduction.
7.Abnormal activation of Notch1 and T-cell acute lymphoblastic leukemia
Journal of Clinical Pediatrics 2015;33(5):483-485
Abnormal activation of Notch1 plays pivotal roles in the molecular pathogenesis of human T-cell acutelymphoblastic leukemia (T-ALL). Activating Notch1 mutations present in over 60% of the T-ALL patients. However, so far, there is no therapy with little side effects that specifically targets the abnormally activated Notch1 pathway-induced T-ALL. Thepresent study briefly reviewed the effects of abnormal activation of Notch1 in the pathogenesis of T-ALL, as well as the currentapproaches targeting Notch1 and its limitations, thus providing some guidance for the research and development of clinicaltherapies targeting T-ALL.

OBJECTIVETo explore the effects of chronic hypoxia on left and right ventricular function and the expression of cardiac transient receptor potential canonical (TRPC) channels in rats.
METHODSForty eight SD male rats were randomly divided into control group (CON) and chronic hypoxic pulmonary hypertension model group (CH) (n = 24). In CH group, rats were exposed in chronic hypoxia environment (10% +/- 0.2% O2) to induce myocardial hypertrophy. After 3 weeks, mean systemic arterial pressure (mSAP), right ventricular systolic pressure (RVSP), left ventricular systolic pressure (LVSP), left or right ventricular pressure maximum rate of rise (LV/RV + dp/dt(max)), left or right ventricular pressure maximum rate of descent (LV/RV-dp/dt(max)), right ventricular hypertrophy index (RVMI) and left ventricular hypertrophy index (LVMI) were measured. Left and right ventricular myocardium tissue sections were stained by HE and observed under light microscope. Real-time polymerase chain reaction (real-time-PCR) and Western blot were performed to detect the expression of TRPC subfamily.
RESULTSRVSP, RVMI, RV + dp/dt(max) and RV-dp/dt(max) were markedly elevated in CH group (P < 0.01) in comparison to CON group. LVMI was markedly reduced in CH group in comparison to CON group (P < 0.01). LVSP, LV + dp/dt(max) and LV- dp/dt(max) had no significant changes in CH group in comparison to CON group. Right ventricular myocardial cells of CH group became thick, the nuclei stained deeply, the shape of nuclei became not regularity. Left ventricular myocardial fibers did not change significantly. There was significant difference in the levels of mRNA and protein of TRPC1 between CON and CH groups.
CONCLUSIONFor three weeks exposed to chronic hypoxia induced right ventricular hypertrophy specifically, raised the mRNA and protein expression of TRPC1 on right ventricular myocardial cells . TRPC1 might be involved in the development of cardiac hypertrophy.
