1.A Case of Giant Ureteral Stone.

A case of giant ureteral stone on the left lower third is presented with a review of literature. The patient reported herein was 54 year-old woman with only mild flank and suprapubic discomfort but relatively silent process. Excretory urogram revealed a hen-egg sized lower ureteral stone with marked hydronephrosis on the left side The right one is of an appearance of normal urogram. She underwent ureteronephrectomy en the left side for poor function, urinary infection and marked hydronephrosis At operation, a large calculus was revealed in the left lower ureter, which was a grayish brown color. 4.2/6.1/7.0 cm in size and weighed 16.8Gm.

2.Horseshoe Kidney Associated with Tuberculosis.

A 30 years old male suffering from renal pain, hematuria and frequency of urination, was found to have horseshoe kidney with renal tuberculosis. The diagnosis was made through urinalysis, intra-venous pyelography and retrograde pyelography. This is a report of the good results gained by partial nephrectomy and antituberculous chemotherapy. It was compared with several literary reviews.

3.Pathologic Rupture of Flexor Pollicis Longus Tendon Secondaryto Kienbock's Disease: A Case Report.

A pathologic rupture of the flexor pollicis longus tendon secondary to Kienbock disease is extremely rare with only three cases being reported in the literature. We encountered a case of a pathological rupture of the flexor pollicis longus tendon secondary to longstanding Kienbock disease. The treatment included tendon ball insertion after excising the collapsed lunate and a flexor pollicis longus tendon reconstruction with autogenous palmaris longus tendons. Satisfactory results with a restoration of the active motion of the interphalangeal joint of the thumb, maintenance of the preoperative active range of motion of the wrist and markedly reduced pain was achieved after a one year follow up. We describe this case with a review of the relevant literature.

4.Diffuse Panbronchiolitis:Clinical Significance of High-resolution CT and Radioaerosol Scan Manifestations.

BACKGROUND: Diffuse panbronchiolitis(DPB) is a disease characterized clinically by chronic cough, expectoration and dyspnea; and histologically by chronic inflammation localized mainly in the region of the respiratory bronchiole. It is prevalent in Japanese, but is known to be rare in Americans. and Europians. Only few cases in Chinese, Italians, North Americans and Koreans have been reported. It is diagnosed by characteristic clinical, radiological and pathologic features. High-resolution CT(HRCT) is known to be valuable in the study of the disease process and response to therapy in DPB. To our knowledge, there has been no correlation of its appearance on HRCT with the severity of the disease process, and radioaerosol scan(RAS) of the lung has not previously been used for the diagnosis of DPB. METHOD: During recent two years we have found 12 cases of DPB in Kangnam St. Mary's Hospital, Catholic University Medical College. We analysed the clinical characteristics, compared HRCT classifications with clinical stages of DPB, and determined characteristic RAS manifestations of DPB. RESULTS: 1. The ages ranged from 31 to 83 years old(mean 54.5 years old), and male : female ratio was 4:8. 75%(9/12) of patients had paranasal sinusitis, and only one patient was a smoker. 2. The patients were assigned to one of three clinical stages of DPB on the basis of clinical findings, sputum bacterology and arterial blood gas analysis. Of 12 cases, 5 were in the first stage, 4 were in the second stage, and 3 were in the third stage. In most of the patients, pulmonary function tests showed marked obstructive and slight restrictive impairments. Sputum culture yielded P. aeruginosa in 3 cases of our 12 cases, K. pneumoniae in 2 cases, H. influenzae in 2 cases, and S. aureus in 2 cases. 3. Of 12 patients, none had stage I characteristics as classified on HRCT scans, 4 had stage II findings, 5 had stage III findings, and 3 had stage IV characteristics. 4. We peformed RAS in 7 of 12 patients with DPB. In 71.4% (5/7) of the patients, RAS showed mottled aerosol deposits characteristically in the transitional and intermediary airways with peripheral airspace defects, which contrasted sharply with central aerosol deposition of COPD. 5. There were significant correlations between HRCT stages and clinical stages(r=0.614, p<0.05), between HRCT types and PaO2(r=-0.614, p<0.05), and between HRCT types and ESR(r=0.618, p<0.01). CONCLUSION: The HRCT classifications correspond well to the clinical stage. Therfore in the examination of patients with DPB, HRCT is useful in the evaluation of both the location and severity of the lesions. Also, RAS apears to be a convenient, noninvasive and useful diagnostic method of DPB.

7.Hypersensitivity Reactions to Oxaliplatin.

Oxaliplatin is a third-generation platinum compound that is used as a single agent and in combination with fluorouracil (5-FU) to treat colorectal and gastric carcinoma. The patients treated with oxaliplatin may develop hypersensitivity and idiosyncratic reactions, although these complications are known to be rare. We report here on two patients who suffered with metastatic colorectal cancer and who underwent palliative combination chemotherapy with oxaliplatin; they then developed hypersensitivity reactions to oxaliplatin. The first case had an anaphylatic reaction immediately after the beginning of the 7(th) to 8(th) cycle infusion of oxaliplatin. The second case developed repeated febrile episodes from the 4th to 8th cycles of oxaliplatin infusion. With the increasing use of oxaliplatin in clinical practice, we are now encountering an increasing incidence of suspected hypersensitivity reactions. Physicians should keep their eyes wide open and carefully observe for the clinical manifestations of these hypersensitivity reactions.

8.Evaluation of Mumps Vaccine Effectiveness by outbreak investigation in one kindergarten in Ulsan city, 2006.

PURPOSE: Despite of the high MMR vaccine coverage, the mumps outbreaks is continued in most developed countries including South Korea. To evaluate the effectiveness of MMR vaccine, we carried out mumps outbreak investigation in one kindergarten. METHODS: In Dec. 2006, a mumps outbreak occurred in a kindergarten. Retrospective study was conducted among 212 respondents (205 children and 7 teachers) in 230 study populations (study participation rate: 92%). To define mumps cases, a questionnaire survey, telephone survey, and mumps antibody test with serum were conducted. MMR vaccination status was verified in 198 children using children's vaccination records, National Immunization Registration System and/or medical records of private clinics or hospitals. RESULTS: Over 90% children had received one dose of MMR vaccination. However, 2nd dose of MMR vaccination rate was 45.1%. Attack rates of mumps were 22.9%(22/96) for one dose MMR vaccinees and 3.3%(3/92) for two doses vaccinees. MMR vaccine effectiveness of two doses over one dose was 86%. Mumps attack rate in one dose MMR vaccinees increased by the duration after MMR vaccination. Among all mumps cases, asymptomatic mumps infection identified by IgM positive or IgG over 10,000mIU/ml was 58% (26/ 45). CONCLUSIONS: MMR one dose coverage rate was high(92.1%), but the second dose vaccination rate was very low (45.1%). Mumps attack rate was increased by the duration after the MMR vaccination, and the maximum effectiveness of one dose MMR vaccination was low (77.1%). Therefore, to prevent mumps outbreaks in the kindergarten, second MMR vaccination should be scheduled in 4 years old age, and the coverage rate should be increased over 90%. Further studies for the effectiveness of MMR two doses after 5 and more years are needed.

9.A Case of Idiopathic Bronchiolocentric Interstitial Pneumonia.

Idiopathic bronchiolocentric interstitial pneumonia is one of idiopathic interstitial pneumonia, which has a relatively aggressive course and poor prognosis. It is characterized by diffuse centrilobular nodules radiologically with mainly bronchiolocentric inflammation and fibrosis associated with patchy alveolitis lacking interstitial granuloma histologically. This disorder is a recently classified disease category, and to our knowledge, there is no case report in Korea. We present a case of idiopathic bronchiolocentric interstitial pneumonia. A 62-year-old man presented with exertional dyspnea with a 1 month duration. The radiological findings showed extensive centrilobular lesions at both lungs. The surgical lung biopsy specimen demonstrated a centrilobular inflammatory process with small airway fibrosis and inflammation partially radiating into the interstitium. Therefore, the patient was diagnosed with idiopathic bronchiolocentric interstitial pneumonia. He was treated with immunosuppressants including steroids and azathioprine. However, his symptoms did not improve and he expired 7 months later due to an acute exacerbation of the interstitial pneumonia and probable infectious pneumonia.

10.Morphological Study of Femoral Head and Acetabulum Using MRI in Legg-Calve-Perthes Disease.

PURPOSE: We used MRI to evaluate the morphological relationship of the femoral head and acetabulum with changes of hip position in LCP disease. MATERIALS AND METHODS: The MRIs of 35 patients (17 pillar B, 18 pillar C; mean age: 7.8 years old) were reviewed in neutral, abduction, abduction-internal rotation, abduction-internal rotation-flexion, and adduction positions. The measurements included epiphyseal extrusion index (EEI), head coverage (HC), and medial gap ratio (MGR). The congruence of the hip joint and adjacent soft tissue changes were also studied. RESULTS: Decreased EEI and increased HC in both pillar B and C from neutral to the abduction and abduction- internal rotation positions were statistically significant (p<0.05). However, in decreased MGR, statistical significance was seen only in pillar B. The increase of MGR in pillar C, from neutral to adduction, did not show statistical significance due to the tension of the soft tissues lying lateral to the hip joint. CONCLUSION: This study supports the basic concept of containment treatment in pillar B and suggests a rationale for valgus femoral osteotomy in cases of pillar C with hinge abduction.
