2.Simultaneous correction procedures for nipple hypertrophy and micromastia

Objective The female nipple hypertrophy and mammary hypotrophy can be corrected by simultaneous operation to get more beautiful mammary profile. This study is to explore the new methods for mammary augmentation and nipple reduction. Methods The modified Regnault method was used to correct nipple hypertrophy. The procedures included removing a peripheral portion of the nipple and collapsing the distal portion of it. Other surgical technique was also applied for reduction of nipple height and diameter without affecting the central lactiferous ducts. Mammary augmentation was performed by putting a prosthesis in pectoralis major muscle through an endoscopy on transaxillary incision. Results The procedures were undertaken in 9 cases since 1998, in which 8 cases had been followed up for 3 to 12 months. The results of mammary augmentation and reduction of nipple were satisfactory. The appearance was fine with inconspicous scars, and there were no complications related to surgery. Conclusion The procedures for simultaneous plasty of the nipple hypertrophy and mammary hypotrophy are safe, simple and effective. [
3.Correction of secondary nasal alar deformities using onlay implantation in patients with unilateral cleft lip

Objective To attempt to achieve a more satisfactory results in correcting the unilateral cleft lip nasal deformities caused by deficiency of the maxilla and collapse of the alar cartilage, using onlay implantation and stable fixation for implants. Methods Since 1996, the authors had implanted rib cartilage (12 cases), rib bone and cartilage (4 cases), silicon rubber (8 cases), skull bone and ePTFE (one case each) in total 26 patients with moderate to severe nasal deformities of unilateral cleft lip to elevate the retro-position alar base in hypoplastic maxillar area and to augment the buckled lateral alar cartilage in effected side. All implants were fixed by absorbable sutures on alar cartilage, and titanic screw and/or plate on maxilla under alar base. Results All wounds recovered without any complication. During 3 to 18 months follow-up postoperatively in 19 cases, the nasal appearences were significantly improved. The implants still provided support for the depressed alar base and the collapsed alar cartilage without obvious displacement, absorption and rejection. Conclusion The onlay implantation and stable fixation using suture and rigid fixation of titanic screw and/or plate for implants is reasonably good in nasal deformity correction of unilateral cleft lip advocated. The definitive effects and well-maintenance of the implant dimension and position have been documented.

Objective To explore the relationship between the associated expression of mucin MUC1、MUC2、Matrix Metalloproteinase(MMP-3)and clinicopathological parameters in colorectal adenoma and colorectal carcinoma.Methods The expression of MUC1、MUC2 and MMP-3 were detected in 20 colorectal adenomas and 60 colorectal carcinomas by immunohistochemical method.Results The positive expression rates of MUC1 、MUC2 and MMP-3 in 20 colorectal adenomas and 60 colorectal carcinomas were 10% 、46.7%(P=0.013);100%、58.3%(P=0.000);10%、41.7%(P=0.031)respectively.The expression of mucin MUC1 was negative correlated to the degree of differentiation(P=0.006),but positively correlated to the depth of invasion(P=0.004),lymph node metastasis(P=0.010),Dukes staging(P=0.033)and negafively correlated with proghosis.Mucin MUC2 expression was unrelated to differentiation(P=0.143),but negative correlated to the depth of invasion(P=0.016),lymph node metastasis(P=0.002).Dukes staging(P=0.005)and posifively correlated with survival period.The positive expression of MMP-3 was positively correlated to lymph node metastasis(P=0.002),but negatively correlated to prognosis.Associated detection of MUC1 、MUC2 and MMP-3 showed that MUC1 was negatively correlated to MUC2,MUC2+MUC1-gave the best prognosis,while MUC1+MUC2-gave the worst prognosis.Conclusions The up-regulation of MUC1、MMP-3 expression or down-regulation of MUC2 expression may be involved in carcinogenesis,progression,invasion and metastasis.And it is very important to predict the prognosis in colorectal carcinomas.
6.Clinical therapeutic effect of tigecycline combined with cefoperazone/sulbactam on treatment of multiple drug resistant strains and pandrug-resistant acinetobacter baumannii infection

Objective To explore the efficacy of tigecycline combined with cefoperazone/sulbactam in the treatment of infections due to multiple drug resistant strains and pandrug-resistant acinetobacter baumannii, so as to guide the reasonable clinical medication.Methods A total of 16 cases of ventilator associated pneumonia caused by multiple drug resistant strains and pandrug-resistant acinetobacter baumannii treated in the First Hospital of Shijiazhuang from November 2012 to November 2014 were retrospectively analyzed, and the severity of the infection, clinical efficacy and mortality were observed.Results The multiple drug resistant strains and pandrugresistant acinetobacter baumannii were frequently detected in the 16 patients.Fifteen cases had been used other antibiotics before tigecycline, such as imipenem, cefoperazone/Shubatan, minocycline etc.The severity of underlying disease was assessed with the acute physiology and chronic health score(APACHE Ⅱ sore) within 24 h of admission, on the first day of tigecycline (TGC) therapy and after 7 days of TGC therapy.It showed that the average APACHE Ⅱ score were (25±6.0), (24.2±6.4) and (17.8±6.6) within 24 hours of admission(P<0.01), on the first day of TGC therapy and after 7 days of TGC therapy.Thirty days after application of the TGC, the bacterial eradication rate was 56.25% (9/16).The effective rate was 87.5% (14/16).The failure rate was 12.5% (2/16).Conclusion The effect of the tigecycline combined with cefoperazone/sulbactam on the clearance of the multiple drug resistant strains and pandrug-resistant acinetobacter baumanniiis is satisfied.
7.Molecular characterization of mutations associated with resistance among 72 multidrug-resistant strains of Mycobacterium tuberculosis by whole genome sequencing

Abstract: Objective To understand the characteristics of mutations associated with resistance among 72 multidrug-resistant tuberculosis (MDR-TB) strains using whole genome sequencing (WGS) and to evaluate the performance of WGS for predicting MDR-TB drug resistance. Methods The clinical strains isolated from patients who visited the outpatient department of Tianjin Center for Tuberculosis Control from January to September in 2020 were collected. Identification tests using p-nitrobenzoic acid (PNB) medium were performed. Drug susceptibility tests (proportion method) on L-J medium were performed. After excluding duplicate strains, 72 MDR-TB strains were selected for WGS. Data were analyzed by using online databases and the phenotypic drug susceptibility test results were compared with resistance profiles predicted by WGS. Results All of 72 MDR-TB strains belonged to linage 2, and there was no significant difference in rate of pre-extensive drug-resistant tuberculosis (pre-XDR-TB) between modern type and ancestral type (χ2=0.287, P=0.592). A total of 81 mutation types were found from resistance-related genes for 12 anti-tuberculosis drugs, and the common mutation types in different drug-resistant strains were: streptomycin (SM): rpsL Lys43Arg; isoniazid (INH): katG Ser315Thr; rifampicin (RIF): rpoB Ser450Leu; ethambutol (EMB): embB Met306Val; ofloxacin (OFX), levofloxacin (LFX), moxifloxacin (MFX): gyrA Asp94Gly; kanamycin (KAM), capreomycin (CAP), amikacin (AMK): rrs 1401a>g; para-aminosalicylic acid (PAS): folC Ile43Thr. Nine mutation types were found in 9 prothionamide (PTO)-resistant strains, one type for each strain. The sensitivity and specificity of WGS for predicting resistance to different drugs were SM: 98.15% and 88.89%, INH: 90.28% and -, RIF: 98.62% and -, EMB: 79.49% and75.76%, OFX: 97.30% and 85.71%, KAM: 85.71% and 98.46%, PAS: 27.27% and 95.08%, PTO: 81.82% and 60.66%, CAP: 60.00% and 98.51%, LFX: 97.22% and 83.33%, MFX: 97.30% and 85.71%, AMK:100.00% and 100.00%, respectively. Conclusion WGS is a rapid and promising method which has high consistency with the phenotypic drug sensitivity test. Therefore, it has good application prospects in predicting drug resistance in MDR-TB.
8.Health risk assessment of dietary lead exposure among children in Jiading District

Objective:
To investigate the dietary lead exposure level among children aged 3 to 4 years in Jiading District, Shanghai Municipality, and assess the health risk caused by lead exposure, so as to provide the basis for the management of children's food safety.
Methods:
Based on the 2023 Dietary and Health Status Surveillance Project in Jiading District, children aged 3 to 4 years from 12 streets (townships) in Jiading District were selected as the subjects using a multi-stage stratified random sampling method. The consumption frequency and daily intake of seven subcategories of four major food groups (beans, tofu), cereals (wheat, rice), meat (animal viscera), and aquatic products (mantis shrimps, bivalves) consumed by children over the past three months were collected using a food frequency questionnaire. Food samples were collected according to the food safety risk monitoring plan in Jiading District, and the lead content was detected. The health risks of dietary lead exposure among 3-4-year-old children were quantitatively assessed using the single-factor pollution index method, the Nemerow comprehensive pollution index method, and the margin of safety (MOS) method.
Results:
A total of 143 3-4-year-old children were surveyed, including 69 boys (48.25%) and 74 girls (51.75%). A total of 317 food samples were tested, and lead was detected in all seven subcategories of food samples, with an overall detection rate of 77.29%. The detection rates of lead in bivalves and mantis shrimps of aquatic products were relatively high (98.75% and 100.00%, respectively). The mean lead content of various foods ranged from 0.003 4 to 0.090 7, with the highest level found in bivalves. The lead content of all food samples did not exceed the standard. The single-factor pollution index of seven subcategories ranged from 0.017 2 to 0.148 0, and the Nemerow comprehensive pollution index ranged from 0.116 5 to 0.424 4, both of which were less than 0.7. The MOS (mean) ranged from 0.000 3 to 0.003 9, with an overall MOS (mean) of 0.012 2. The MOS (P95) ranged from 0.005 7 to 0.055 9, with an overall MOS (P95) of 0.112 4, both of which were less than 1.
Conclusions
The lead pollution level in the diet among children aged 3 to 4 years in Jiading District is safe and clean, with an acceptable impact on food safety. However, the detection rate of lead is relatively high, and the main source of dietary lead exposure is aquatic products.
9.Influence of BMP-7 on chondrocyte secretion and expression of Col-Ⅱ, AGG and Sox9 mRNA in porous tantalum-chondrocyte composites in vitro

Objective:To study the influence of bone morphogenetic protein-7 ( BMP-7 ) on chondro-cyte secretion and expression of type Ⅱ collagen ( Col-Ⅱ) , aggrecan ( AGG ) and SRY-related high mobility group-box gene 9 ( Sox9 ) mRNA in porous tantalum-chondrocyte composites.Methods: The articular chondrocytes were isolated from 3-week-old New Zealand immature rabbits and identified.The 2nd generation of chondrocytes with 1 ×106/mL inoculate concentration was seeded in porous tantalum and divided into 4 groups, and control group ( tantalum/chondrocyte) , 50μg/L BMP-7 group (50μg/L BMP-7/tantalum/chondrocyte) , 100 μg/L BMP-7 group ( 100 μg/L BMP-7/tantalum/chondrocyte ) , and 200 μg/L BMP-7 group ( 200 μg/L BMP-7/tantalum/chondrocyte ) .The proliferation of chondro-cytes was measured by CCK-8 assay.The chondrocyte growth and morphology were observed by scanning electron microscopy ( SEM) .The synthesis of glycosaminoglycan ( GAG) in chondrocytes was tested by dimethyl methylene blue ( DMMB) colorimetric quantification method.Col-Ⅱ, AGG and Sox9 mRNA in chondrocytes were detected by real-time PCR.Results: The chondrocytes were spindle-shaped in 24 hours of primary cell culture and most cells became polygonal shaped in 4 days.The chondrocytes were affirmed by alcian blue, safranin O and Col-Ⅱimmunocytochemistry staining.The result of CCK-8 assay showed that the level of cell proliferation in 100 μg/L BMP-7 groups were higher than those in the other groups ( P<0 .05 ) .The chondrocytes implanted into porous tantalum scaffolds with BMP-7 had better functions, by which cytoplasmic processes developed and extended to the surface and inner of porous tan-talum by SEM observation.DMMB quantitative determination of GAG showed that GAG amount of chon-drocytes in 100 μg/L BMP-7 groups was significantly higher than those in the other groups ( P<0 .05 ) . The expressions of Col-Ⅱ, AGG and Sox9 mRNA in chondrocytes were up-regulated in the experimental groups, compared with the control group and the best effect appeared when concentration of BMP-7 was 200μg/L.(P<0.05).Conclusion:BMP-7/tantalum/chondrocytes composites enhanced in vitro chon-drocyte proliferation and extracellular matrix greatly, and can promote chondrogenic gene expression.
10.Effect of cervical curvature and intervertebral height on the surgical outcome of posterior decompression

Objective To analyze the effect of cervical.curvature and intervertebral height on the surgical outcome of posterior decompression in patients with multi-level cervical spondylotic myelopathy.Method A total of 50 patients with multi-segment cervical spondylotic myelopathy (≥ 3 segments),treated with posterior decompression from June 2006 to January 2009 were retrospectively analyzed.Twenty six patients underwent cervical omni-posterior decompression and lateral mass screw internal fixation (group A); 24patients underwent expansive open-door laminoplasty (group B).The effects of changing of cervical curvature and intervertebral height on JOA score and VAS score were analyzed.Results All patients were followed up for 8 months to 4 years (average,24 ±5.5 months).There were statistical differences of cervical curvature angles between preoperative and 3 days postoperatively in group A,while no statistical differences in group B.There were no statistical differences of cervical curvature angles between 3 days,6 months and 3 years postoperatively in group A,while there were differences in group B After further analysis of the loss of cervical curvature and intervertebral height,the result showed that JOA score and VAS score in the low-loss group were better than those in the high-loss group.Conclusion Lateral mass screw internal fixation after cervical omni-posterior decompression had many advantages such as reducing the change of cervical curvature,the loss of intervertebral height and incidence of cervical axial symptoms.
