1.Comparison of the effect of oxcarbazepine and carbamazepine in the treatment of primary trigeminal neuralgia

Objective To compare the effect of oxcarbazepine and carbamazepine in the treatment of primary trigeminal neuralgia(PTN),to find a therapy with reliable curative effect and less adverse reaction.Methods 44 patients with PTN received oxcarbazepine (treatment group) treatment,and 34 PTN patients received carbamazepine (control group) treatment.The clinical effect of two drugs in the treatment of PTN was compared.Visual analogue scale(VAS) was used to assess the pain of patients before treatment,1 week after treatment,2 months after the treatment.Results After 1 week and 2 months treatment,VAS score of the two groups were (52.09 ± 7.14)points,(45.10 ± 5.53) points,(28.47 ± 3.22) points,(16.79 ± 2.50) points,which were significantly lower than that before treatment [(80.67 ± 10.32) points,(81.94 ± 12.09) points],the differences were statistically significant (t =0.341,0.589,0.374,0.675,all P < 0.05).After 1 week and 2 months treatment,the VAS score in treatment group was significantly lower than that in the control group (t =0.310,0.532,P < 0.05).The incidence rate of adverse reaction in the control group was 17.6%,the incidence rate of adverse reaction in the treatment group was 9.1%,which had significant difference(x2 =16.521,P < 0.05).The adverse reactions of two groups including rash,nausea and vomiting,drowsiness.Liver function examination showed that oxcarbazepine had less liver injury than carbamazepine.Conclusion Oxcarbasepine is a new drug for the treatment of PTN,its curative effect and safety is better than carbamazepine,it has high value in clinical application.

Objective To analyze the risk factors for shortterm recurrence of cerebral infarction. Methods The age, gender, blood pressure, blood sugar, serum triglyceride, serum cholesterol, blood coagulation, atrial fibrillation, intaking of tobacco and alcohols, and body mass index (BMI) were detected in the patients with initial cerebral infarction. During the follow-up of 2 years, they were observed on the factors that might be associated with the recurrence of cerebral infarction. 3.Empirical Study on the Inhibiting Effect of Shenqi Decoction on Myocardial Cell Apoptosis of Model Mice Induced by Reperfusion

OBJECTIVE:To probe into the mechanism of Shenqi decoction on inhibiting apoptpsis of myocardial cell in?duced by ischemic reperfusion.METHODS:The mice were randomly divided into Shenqi decoction group(high,middle and low dosage groups),isosorbide dinitrate control group,blank control group and model group.The Shenqi decoction group and isosorbide dinitrate group were respectively administered with drugs by lavage for5days while the blank control group and model group were respectively administered with normal sodium by lavage for5days.Except the blank control group the is?chemic reperfusion model of each group was made,then the activity of superoxide dismutase(SOD),the level of malondi?aldehyde(MDA)of cardiac muscle tissues were determined;the apoptotic index of myocardial cells and the expression of apoptosis-correlated protein Bcl-2and Bax were analyzed.RESULTS:Compared with the model group,the SOD activity and Bcl-2express index have been increased,but MDA level and bax express index have been decreased in Shenqi decoction group.CONCLUSION:Shenqi decoction does have a good protection effect on myocardial ischemia and myocardial cell injury induced by postischemia reperfusion.
4.Effect of nursing intervention on preoperative anxiety of patients with gynecological carcinoma

Objective To study the effect of nursing intervention on preoperative anxiety of patients with gynecological carcinoma.Methods 60 patients with gynecological carcinoma in our hospital who were to receive operations were randomly distributed into the control group and the intervention group with 30 in each group.The control group was given routine nursing,while the intervention group received nursing intervention based upon routine nursing.Indexes such as preoperative anxiety,sleeping quality and satisfaction degree to nursing care were evaluated within them.Results Through targeted nursing intervention preoperative anxiety of patients in the intervention group alleviated,their confidence enhanced and satisfaction degree to nursing care improved compared to those of the control group.Conclusions Targeted nursing intervention can reduce preoperative anxiety of patients with gynecological carcinoma and increase satisfaction degree to nursing care.

6.Influence of insulation measures on blood coagulation function during laparotomy in the aged patients

Objective To explore the effects of insulation measures on blood coagulation during laparotomy in the aged patients. Methods Sixty patients were randomly divided into the experimental group and the control group with 30 cases in each group from September 2007 to August 2008. The experimental group received a series of insulation measures based on general perioperative nursing care, which was given to the control group. Patients' temperature, platelet count (PLT), prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TV),fibrinogen (Fbg) were measured and these preand postoperative values together with blood loss during operation were compared between two groups. Results No significant change was seen in perioperative body temperature in the experimental group, the incidence rate of hypothermia was lower, the change in coagulation index was better than that of the control group. Conclusions These results suggested that mild hypothermia could influence aged patients' blood coagulation function and increase blood loss during laparotomy. Active insulation measures can maintain the stability of patients" temperature.
7.Application of a population pharmacokinetic model for vancomycin in patients with severe neurosurgical disease

Objective This study was designed to validate the utility of a population pharmacokinetic model established for vancomycin in patients with severe neurosurgical disease . Methods The clinical data including patient gender , age , body weight ,serum creatinine and albumin were collected retrospectively from patients in Nanjing Drum Tower Hospital to calculate the steady trough concentration of vancomycin using the previously established pharmacokinetic model .The predicted value was compared with the actual value .Results During the period from March 2013 to March 2014 ,53 blood samples with serum trough concentration of vancomycin were collected from 42 patients .The average trough concentration of vancomycin was 10 .9 mg/L (range from 1 .6 to 49 .1 mg/L) .The predicted trough level of vancomycin based on the population pharmacokinetic model was significantly correlated to the actual value(r=0 .857 ,P<0 .001) .The mean absolute percentage error was 0 .407 9 . The confidence interval was 9 .36‐14 .07 for the predicted values ,and 8 .92‐14 .32 for the actual values .Conclusions The pharmacokinetic model is valid and useful for planning intravenous dose of vancomycin in patients with severe neurosurgical disease .Large error (about 30% ) was observed in estimation of body weight due to coma .Reduced renal function following contrast agent and/or diuretic drug has an impact on the predicted results . The accuracy of prediction can be increased to nearly 70% after adjusting the covariates .
8. Current status of malaria rapid diagnostic testing strips application in primary-level healthcare providers in Jiangsu Province

Objective:
To investigate the current status of malaria rapid diagnostic test (RDT) strips application and malaria laboratory technicians' evaluation about them at primary healthcare provider level in Jiangsu Province.
Methods:
From November to December 2016, 878 medical institutions and 118 CDCs of city, county and township/community level in Jiangsu Province were selected as study samples using stratified random sampling method. Self-designed questionnaire was distributed to investigate the institution's malaria work task, RDT strips application and evaluation status in 2015. We also investigated the socio-demographic information and collected the RDT strips evaluation score from the malaria laboratory technicians selected from the institutions investigated (one technician from each institution). Rank sum test was performed to compare the RDT strips evaluation scores between medical institutions and CDCs, and among different medical institutions and CDCs.
Results:
In 2015, 405 cases of malaria were reported, 362 200 person-time of malaria blood testing task was conducted, and 100 000 RDT strips were procured and provided for healthcare providers in Jiangsu province for free.
