1.SCANNING ELECTRON MICROSCOPIC OBSERVATION ON ENDOTHELIAL LIKE CELLS OF THE MARGINAL EAR VEIN INTIMA OF RABBIT

Objective To observe the novel endothelial like cells of the marginal ear vein intima of rabbit by scanning electron microscope.Methods The marginal ear veins were obtained from male Japanese big ear white rabbits,and prepared for scanning electron microscope(SEM).The novel endothelial like cells of the marginal ear vein intima were observed.Results Between endothelial cells and basement membrane of the marginal ear vein intima of rabbit,some oval-shaped endothelial like cells were found.This kind of cells was measured,the length was(8.32?1.04)?m,width was(5.79?0.68)?m,and length/width is 1.45?0.22.The cells were connected each other by some filament structure,and the average length between two adjacent cells was(6.41?2.45)?m.Conclusion There are novel endothelial like cells in the marginal ear vein intima of rabbit.The filament structure,connected between endothelial like cells,might be a new type of cell junction.
2.Feasibility of tissue engineered small vessel scaffolds applying to blood vessel transplantation

BACKGROUND: The glycoprotein which ends of a-galactosyi residues (a-Gal) is the major heterogenic antigen for hyperacute rejection. OBJECTIVE: To observe the distribution characteristic of the small tissue engineered vessel scaffold, and a-Gal in endothelial cells and smooth muscle cells of Wistar rats and Japanese white rabbits, in addition, to discuss the feasibility of applying acellular tissue vessel scaffold to heterogeneous blood vessel transplantation. DESIGN, TIME AND SETTING: The contrast observation was conducted at the Department of Human Anatomy of China Medical University between March 2003 and December 2004. MATERIALS: Totally 30 caudal arteries of Wistar rats were collected. Fifteen of which were prepared for small vessel scaffold (small vessel scaffold group), the remained 15 served as caudal artery group. Additionally, 15 central arteries were obtained from each ear of Japanese white rabbits (central artery group). METHODS: Totally 16 mg/L Bandeiraea Simplicifolia I Isolectin B4 (BSI-B4) was added for DAB staining with Affinity histochemistry method. Then MetaMorth/C5050/BX41 microscopic image analysis system was used to detect the positive reaction product of a-Gal. MAIN OUTCOME MEASURES: Color changes of vascular wall were observed under light microscope; the optical density of the positive reaction product of a-Gal was measured. RESULTS: The expression of a-Gal was mainly located in the cell membrane, as well as cell nucleus of endothelial cells in the central artery group. The expression of a-Gal of endothelial cell was strong positive in the caudal artery group, which was weak or negative expressed in the small vessel scaffold group. The optical density of a-Gal expression was lowest in the tunica intima of small vessel scaffold group, which was less in the central artery group than the caudal artery group (P < 0.001). The a-Gal expression in the tunica media of small vessel scaffold group was less than the central artery and the caudal artery groups (P < 0.001). CONCLUSION: In the caudal artery of Wistar rat, the expression of a-Gal is higher than that in the central artery of Japanese white rabbit. Therefore, the heterogeneous tissue engineered vessel material from acellular caudal artery of Wistar rats can be used in blood vessel transplantation.
3.Liver transplantation:an early clinicopathological study

Objective: To study and explore the early clinicopathological characters and complications by liver biopsy after liver transplantation. Methods:Fifteen liver biopsies from 13 patients were performed in one month after liver transplantation. The liver biopsy samples were preserved in 10% buffered formalin, embedded in paraffin and stained with H-E. All the samples were assessed by light microscopy. Results: The patients included 11 males and 2 females, median 48 years old. Chronic hepatitis B with cirrhosis was the major indication for liver transplantation. Fifteen liver biopsies from 13 patients were assessed. Of the 8 specimens presented hepatocellular ballooning and cholestasis around central vein, one with acute cellular rejection, one with severe ischaemic necrosis. One of them showed that hepatocellular ballooning and cholestasis progressed to necrosis. One biopsy failed. Conclusion:At least 5 portal tracts in liver transplantation biopsy were necessary for analysis. Hepatocellular ballooning and cholestasis which caused from hepatocytic ischemia were common in early post-transplantation. The acute rejection and other causes could result in liver graft dysfunction.
4.Construction and application of clinical microbiology laboratory data management expert system

Objective To introduce the construction and application of clinical microbiology laboratory data management expert system.Methods Firstly, the process management was introduced to clinical microbiology laboratory. Then the characteristics of data on each node of work process were analyzed, and SQL Server data table was created as the knowledge base of the expert system.Finally, VB6.0 was used to compile the knowledge acquisition module, reasoning desktop module and input/output interface procedures to finally construct the expert system.Rates of defect report, errors in bacterial identification and drug sensitivity test, delay in culture results reporting and average delayed days were compared before and after the application of the expert system.Results The expert system could be used for data management in process nodes like sample reception, information collection and input, bacteria culture medium selection, bacterial identification and drug sensitive test, interpretation of drug sensitivity results, comprehensive evaluation in bacterial identification and drug sensitivity results, report of negative result, report of positive result, blood culture, Mycoplasma culture, time limit of detection, and nosocomial infection indicators.No defect report was found after the application of expert system; rate of errors in selection of drug sensitivity test medium was reduced from 0.81% ( 31/3 836 ) in 2012 to 0.02%(1/5 433) in 2014;rate of delay in culture results reporting was reduced from 1.78% (320/17 983) to 1.18%(232/19 692), and the average delayed days was also reduced (3.8 d vs.3.2 d).Conclusion Clinical microbiology laboratory data management expert system can improve work efficiency and reduce errors, which can enhance the overall management of laboratory and the quality of clinical service.
5.The blood concentration of hydroxychloroquine and its safety in patients with systemic lupus erythematosus

Objective To study the blood concentration of hydroxychloroquine in patients with systemic lupus erythematosus (SLE) treated with different doses, and analyze the relationship between blood concentration of hydroxychloroquine and disease activity, and evaluate its safety.Methods Forty SLE patients were randomly divided into 2 groups, each group contained 20 cases.The patients in group A were treated with hydroxychloroquine (0.4 g, qd), while patients in group B were treated with hydroxychloroquine (0.2 g, qd).The treatment lasted more than six months in every patient.The blood concentrations of hydroxychloro-quine were detected by high performance liquid chromatography.The clinical and laboratory indices were collected.The systemic lupus erythematosus disease activity index (SLEDAI) score was recorded.The doses and varieties of combined hormone, immunosuppressant were recorded.The correlation of blood concentration of hydroxychloroquine and disease activity was analyzed.The significance was determined by Student's t test and Pearson correlation analysis.Results In SLE patients, the average blood concentration of hydro-xychloroquine was (402±190) ng/ml in group A and (150±60) ng/ml in group B (t=7.471, P<0.01).The disease activities of patients in the two groups showed no significant difference (t=-0.172, P>0.05).The platelet counts of patients in group A were significantly higher than those in group B[(188±88)×109/L vs (158 ±87) ×109/L] (t=4.375, P<0.05).However, the other laboratory parameters showed no significant difference between the two groups (P>0.05).Conclusion The results of this study indicate that the blood concentrations of hydroxy-chloroquine are significantly different in different dosages.The high dose of hydroxy-chloroquine is related to high platelet number in lupus patients.These findings suggest that hydroxychloroquine is safe and effective for SLE patients.
6.The impact of image-guided radiation therapy on treatment of cervical cancer

Objective:To evaluate the effect of radical image-guided radiotherapy (IGRT) on the target dose in cervical cancer and investigate the appropriate application mode.Methods:Twenty patients with cervical cancer treated with helical tomotherapy (HT) in Seventh Medical Center of PLA General Hospital from 2012 to 2016 were selected. A megavoltage CT (MVCT) scan was performed before each treatment. The obtained MVCT images were used for dose parameter in the adaptive module of HT to obtain the actual dose (Plan 1) and the non-image-guided dose parameter was simulated (Plan 2). Each single dose distribution and the corresponding fused CT images were transferred to the software Mimvista 6.5 to obtain the total radiation dose parameter by dose superposition.Results:The motion of CTV, uterus and GTV in Plan 2 was significantly larger than that of Plan 1(all P<0.05), and the largest changes were seen in the ventrodorsal and uterine direction. The V 45Gy, V 50Gy, D 98% and D mean of CTV and uterus and V 50Gy of GTV in Plan 2 were significantly decreased compared with those in Plan 1(all P<0.05). The left-right motion of Plan 1 was negatively correlated with D 2% and D 98% of CTV and uterus (both P<0.05). The head-foot motion was negatively associated with V 45Gy and V 50Gy of GTV (both P<0.05). The ventrodorsal motion was negatively correlated with D 98% of uterus ( P<0.05). The left-right motion of Plan 2 was negatively correlated with D 2% of CTV and V 50Gy of uterus (both P<0.05). The head-foot motion was negatively associated with D 98% of CTV, and D 98%, D mean, V 45Gy and V 50Gy of uterus (all P<0.05). The ventrodorsal motion was negatively correlated with D 98% of CTV, D 98%, D mean, V 45Gy and V 50Gy of uterus, and D mean and V 45Gy of GTV (all P<0.05). Conclusions:In intensity-modulated radiotherapy for cervical cancer, the uterine body displacement is large and the low CTV area is mainly located in the uterine body. IGRT can significantly reduce the dosimetric deviation induced by organ movement.
7.Comparison of the role that poly hydroxy acrylic acid plays in the detection ofHER-2 gene in breast cancer by hematoxylin and eosin staining and fluorescencein situ hybridization with that of 4% neutral buffered formaldehyde

Background and purpose: Adequate tissue ifxation, transparent dewaxing is an important step of hematoxylin eosin (HE) staining and lfuorescence in situ hybridization (FISH) in detection of breast cancerHER-2 gene. The purpose of this study was to make a comparison between poly hydroxyl acrylic acid which is an environmen-tally friendly ifxation liquid and 4% neutral buffered formaldehyde in tissue ifxation for HE staining and FISH to detect theHER-2 gene in the breast cancer tissue sections. The study aimed to evaluate the feasibility of replacing 4% buffered formaldehyde, a traditional ifxation liquid, with the poly hydroxyl acrylic acid, an environmentally friendly ifxation lfuid.Methods:This project was performed on tissue samples collected from 69 cases of breast cancer, 41 cases of breast ifbroadenoma, 40 cases of uterine leiomyoma, 25 cases of cervical tissue, 25 cases of placenta obtained from the outpatient and inpatient departments of Zhongshan Boai Hospital from Mar. 2011 to Jan. 2015, from each of which two samples were drawn and two blocks of each specimen were divided into two groups randomly. Then one group was ifxed with 4% neutral buffered formaldehyde and made into 200 sections by HE while the other group was ifxed with poly hydroxyl acrylic acid and made into another 200 sections. The slice level of the two groups was determined by the staining condition of the sections, and SPSS 19.0 was employed to compare the excellent and good rate of HE staining. Additional 69 sections were produced with two groups of breast invasive ductal carcinoma tissues, and SPSS 19.0 was used to detect the ampliifcation ofHER-2 gene by FISH.Results:First, the number of best-quality slices stained with HE ifxed separately by poly hydroxyl acrylic acid and 4% neutral buffered formaldehyde was 155 and 166, respectively. The number of excellent pieces was 41 and 33, respectively, while the number of mediocre pieces was 3 and 1 with bad pieces being 1 and 0, respectively. The excellent and good rates of HE staining were 98% and 99.5%, respectively. There was no significant difference between the two groups (χ2=1.33,P>0.05).Second, the positive rates of the tis-sue slices by FISH ifxed separately by poly hydroxyl acrylic acid and 4% neutral buffered were 26.09% and 23.19%, respectively. There was no signiifcant difference between the two groups (χ2=0.50,P>0.05).Conclusion:The results obtained with HE staining and FISH using poly hydroxyl acrylic acid as a ifxation liquid are not signiifcantly different from those using 10% neutral buffered formaldehyde. Therefore, poly hydroxyl acrylic acid meets the requirements of environmental protection, and thus has the potential to be promoted and widely used.
8.Comparison between poly hydroxy acrylic acid and Van-clear replacing the tradi-tional reagents to detect the cervical hTERC genes by adopting FISH technique

Objective:To observe the difference of the human telomeres RNA component (hTERC) genes’amplification in the cervical tissue by applying the environment-friendly fixative poly hydroxy acrylic acid and the transparent dewaxing solution Van-clear separately or jointly to replace the traditional fixative 4% (volume fraction)neutral buffered formalin and the conventional transparent dewaxing solu-tion xylene in the use of fluorescence in situ hybridization (FISH)for detection.Methods:In the study, 255 cases of cervical tissue specimens submitted by the Department of Gynecology in Zhongshan Boai Ho-sipital were collected from Mar.2013 to Apr.2015.Four samples were taken from the same lesion site. All the cases were divided into 4 groups and named group A,B,C,and D.Group A used 4% neutral buffered formalin fixed and xylene dewaxing to make slices.Group B used poly hydroxy acrylic fixed and xylene dewaxing to make slices.Group C used 4% neutral buffered formalin fixed and Van-clear trans-parent to make slices.Group D used poly hydroxy acrylic fixed and Van-clear transparent dewaxing to make slices.The amplification of hTERC genes in the four groups of cervical specimens was also detected by FISH technique.Results:When the hTERC genes were detected by FISH method under the fluore-scence microscope,it was obvious that the tissue profile and the background of group A,B,C and D were all clear.The probe was fixed in the accurate position so that the bright red or green fluorescence signals were easily found in these four groups.Compared with the positive rate of group A,there was no statistical significance in that of group B,C and D (P>0.05).At the same time,the coincidence rate of the FISH results was high,which showed that the new environment-friendly reagent had no significant difference in the detection of cervical hTERC genes by FISH technique.Conclusion:It is possible for the environment-friendly reagent poly hydroxy acrylic acid and Van-clear to replace 4%neutral buffered for-malin and xylene separately or jointly to detect the cervical hTERC genes by adopting FISH technique.
9.The effect of enteral immune nutrition on occurrence of acute kidney injury in patients with sepsis

Objective To evaluate the effect of enteral immune nutrition on the occurrence of acute kidney injury(AKI) in sepsis patients. Methods A retrospective study was conducted, 80 patients with sepsis admitted and treated in the intensive care unit (ICU) of Quzhou People's Hospital from January 2015 to December 2016 were enrolled, and according to different treatment programs, they were divided into an immune nutrition group and a standard nutrition group, each group 40 cases. The two groups received an equal amount of calories and nitrogen in enteral nutrition (EN). After treatment, the changes of serum total protein (TP), albumin (Alb), prealbumin (PA), hemoglobin (Hb), C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), interleukin (IL-6, IL-10), infection site, length of stay in ICU, mortality in ICU, AKI incidence, AKI staging, etc in the two groups were observed. Results ① After treatment, the nutritional indicators TP, Alb, PA, Hb levels were significantly increased, the serum inflammatory factors CRP, TNF-α, IL-6 were obviously decreased and IL-10 was markedly increased in the two groups compared with those before treatment (all P < 0.05), the changes in the immune nutrition group were more obvious than those in the standard nutrition group [TP (g/L): 60.84±5.90 vs. 58.32±6.11, Alb (g/L): 33.95±3.83 vs. 31.79±3.44, PA (g/L): 0.24±0.04 vs. 0.21±0.03, Hb (g/L): 117.47±16.15 vs. 112.50±15.71, CRP (mg/L): 53.04±23.76 vs. 82.33±37.09, TNF-α (ng/L): 20.29±17.74 vs. 29.63±18.43, IL-6 (ng/L): 50.74±28.55 vs. 80.32±31.67, IL-10 (ng/L): 41.09±24.65 vs. 40.86±24.73]; ② The length of stay in ICU (days: 10.54±4.33 vs. 14.80±5.19), ICU mortality [15.00% (6/40) vs. 32.50% (13/40)] and the incidence of AKI [22.50% (9/40) vs. 47.50% (19/40)] of immune nutrition group were significantly lower than those of standard nutrition group (all P < 0.05); There were 9 cases of AKI mainly at stage I andⅡ (8 cases) in the immune nutrition group, while in the standard nutrition group there were 19 cases with AKI mainly at stageⅢ (11 cases). Conclusion The enteral immune nutrition can effectively reduce the incidence of AKI in sepsis patients, and the mechanism may be related to the immune nutritional preparation can inhibit the expressions of pro-inflammatory factors such as TNF-α, IL-6, etc.
10.Significance of changes of difference in percutaneous-arterial blood partial pressure of carbon dioxide in liquid resuscitation of patients with septic shock

Objective To approach the significance of changes of percutaneous-arterial blood carbon dioxide partial pressure difference [P(tc-a)CO2] in liquid resuscitation of patients with septic shock. Methods One hundred and sixty-eight patients with septic shock admitted and treated in the Department of Intensive Care Unit (ICU) of Quzhou People's Hospital from January 2015 to January 2018 were enrolled, and after early goal-directed therapy (EGDT) for 6 hours, according to central venous oxygen saturation (ScvO2) and lactate clearance (LC), they were divided into ScvO2 and LC achievement group (ScvO2 ≥ 0.7 and LC≥10%), ScvO2 achievement group (ScvO2 ≥ 0.7 and LC < 10%), LC achievement group (ScvO2 < 0.7 and LC≥10%), and un-achievement group (ScvO2 < 0.7 and LC < 10%). The mechanical ventilation time, ICU hospitalization time, 28-day mortality, P(tc-a)CO2 etc. were compared among the four groups; the receiver operating characteristic curve (ROC) was used to evaluate the predictive value of P(tc-a)CO2 for 28-day prognosis in patients with septic shock. Results The trends of mechanical ventilation time, ICU hospitalization time, and 28-day mortality were all ScvO2 and LC achievement group < LC achievement group < ScvO2 achievement group < un-achievement group [the mechanical ventilation times (days) were respectively 6.12±2.59, 8.43±3.24, 11.78±4.12, 13.03±4.75, ICU hospitalization times (days) were 10.31±2.32, 13.85±3.56, 16.41±3.83, 18.52±4.05, and 28-day mortality rates were 28.85% (15/52), 40.91% (18/44), 51.28% (20/39), 69.70% (23/33)] and the differences among the four groups were statistically significant (all P < 0.05). After 6 hours of EGDT, the heart rate (HR), lactate (Lac), and P(tc-a)CO2 were lower than those before fluid resuscitation, but the mean arterial pressure (MAP), central venous pressure (CVP), and ScvO2 were higher than those before fluid resuscitation among four groups. Except CVP, the differences of other indicators compared among the ScvO2 and LC achievement group, ScvO2 achievement group, LC achievement group and un-achievement group were statistically significant (all P < 0.05). After 6 hours of EGDT, HR, Lac, P(tc-a)CO2 in ScvO2 and LC achievement group, ScvO2 achievement group and LC achievement group were significantly lower than those in the un-achievement group [HR (bpm): 89.05±29.43, 98.82±30.21, 94.33±28.64 vs. 112.85±32.74, Lac (mmol/L): 2.97±1.95, 3.87±2.32, 2.69±1.52 vs. 4.17±2.44, P(tc-a)CO2 (mmHg, 1 mmHg = 0133 kPa): 7.18±4.61, 12.61±5.34, 9.71±4.11 vs. 16.56±10.19], MAP and ScvO2 were significantly higher than those of the un-achievement group [MAP (mmHg): 88.05±21.67, 77.33±18.56, 83.11±19.71 vs. 70.32±18.79, ScvO2: 0.76±0.14, 0.75±0.16, 0.67±0.14 vs. 0.63±0.18, all P < 0.05]. The P(tc-a)CO2 of 28 days survivors were significantly lower than that of the deaths among four groups (mmHg: 5.78±2.27 vs. 14.14±3.65, 7.07±2.81 vs. 15.06±4.11, 6.35±2.09 vs. 14.94±4.06, 7.93±3.81 vs. 18.34±4.63, all P < 0.05). When P(tc-a)CO2 > 7.24 mmHg predicted 28-day mortality in ScvO2 and LC achievement group, the sensitivity was 89.29%, specificity was 91.45%, and the area under ROC curve (AUC) was 0.86; when P(tc-a)CO2 > 9.46 mmHg predicted 28-day mortality in LC achievement group, the sensitivity was 88.72%, specificity was 85.83% and AUC was 0.91; when P(tc-a)CO2 >12.05 mmHg predicted 28-day mortality in ScvO2 achievement group, the sensitivity was 82.79%, specificity was 86.90% and AUC was 0.79; when P(tc-a)CO2 > 16.22 mmHg predicted 28-day mortality in un-achievement group, the sensitivity was 73.35%, specificity was 80.68% and AUC was 0.68. Conclusion P(tc-a)CO2 can be used as an indicator to evaluate fluid resuscitation effect and prognosis in patients with septic shock.
