1.The effectiveness of the cytokines and underglycosylation IgA_1 in mesangial deposition with IgA nephropathy

Objective To investigate the effects of TNF-?,IL-12,IL-15 and underglycosylation IgA_1 in mesangial deposition in the clinicopathological study of IgA nephropathy(IgAN).Methods Seventy-four patients with IgAN were enrolled in the study,with 10 patients randomly selected after remission,10 MCD and 10 healthy volunteers as control groups.The levels of TNF-? in serum and urine were examined by radioimmunoassay,and the levels of IL-12 and IL-15 were examined by ELISA.Immunohistochemistry was used to detect the expression of TNF-? in renal tissue,and the directed immunofluorescence was used to detect the underglycosylation of IgA_1(UGIgA_1)in mesangial deposition.Results The levels of TNF-?,IL-12 and IL-15 in serum of patients with IgAN were higher than those of MCD group,healthy control group and remission group.Compared with the negative group,33 patients with UGIgA_1 positive in renal tissues had lower levels of TNF-? in serum,longer course and less class of renal tissue injury.The positive areas of TNF-? in renal tissue and levels of TNF-? in serum were positively correlated with urinary protein(r=0.249,0.702,P=0.000).There was negative correlation between the levels of TNF-? in serum and Ccr(r=-0.231,P=0.048).The levels of IL-15 was positively correlated with the course of disease(r=0.266,P=0.022) and negatively with Scr(r=-0.320,P=0.005).Conclusions The TNF-?,IL-12 and IL-15 are all involved in the onset of proteinuria in IgAN patients,and IL-15 acts as a factor against renal injury.The patients with UGIgA_1 positive in mesangial have longer course and less pathological changes damaged than those of the UGIgA_1 negative patients.The TNF-? may serve as an important factor in the aggravation of IgAN.
2.The water-soluble TF3 component from Eupolyphaga sinensis Walker promotes tibial fracture healing in rats by promoting osteoblast proliferation and angiogenesis

Objective:
To determine the active components of Eupolyphaga sinensis Walker (Tu Bie Chong) and explore the mechanisms underlying its fracture-healing ability.
Methods:
A modified Einhorn method was used to develop a rat tibial fracture model. Progression of bone healing was assessed using radiological methods. Safranin O/fast green and CD31 immunohistochemical staining were performed to evaluate the growth of bone cells and angiogenesis at the fracture site. Methylthiazoletetrazolium blue and wound healing assays were used to analyze cell viability and migration. The Transwell assay was used to explore the invasion capacity of the cells. Tubule formation assays were used to assess the angiogenesis capacity of human vascular endothelial cells (HUVECs). qRT-PCR was used to evaluate the changes in gene transcription levels.
Results:
Tu Bie Chong fraction 3 (TF3) significantly shortened the fracture healing time in model rats. X-ray results showed that on day 14, fracture healing in the TF3 treatment group was significantly better than that in the control group (P = .0086). Tissue staining showed that cartilage growth and the number of H-shaped blood vessels at the fracture site of the TF3 treatment group were better than those of the control group. In vitro, TF3 significantly promoted the proliferation and wound healing of MC3T3-E1s and HUVECs (all P < .01). Transwell assays showed that TF3 promoted the migration of HUVECs, but inhibited the migration of MC3T3-E1 cells. Tubule formation experiments confirmed that TF3 markedly promoted the ability of vascular endothelial cells to form microtubules. Gene expression analysis revealed that TF3 significantly promoted the expression of VEGFA, SPOCD1, NGF, and NGFR in HUVECs. In MC3T3-E1 cells, the transcript levels of RUNX2 and COL2A1 were significantly elevated following TF3 treatment.
Conclusion
TF3 promotes fracture healing by promoting bone regeneration associated with the RUNX2 pathway and angiogenesis associated with the VEGFA pathway.
3.Effect of cluster nursing on bladder function recovery of cervical cancer patients with radiotherapy

Objective To discuss the clinical application of cluster nursing in bladder function recovery of cervical cancer patients with radiotherapy.Methods Ninety-four patients with cervical cancer receiving radiotherapy in our hospital from September 2016 to August 2017 were selected as observation group,and 70 patients from January to August 2016 were included in the control group.The control group treated by routine nursing care,while the observation group received cluster nursing.The incidence rate of urinary retention,the success rate of one-time extubation,bladder function recovery and pelvic floor muscle strength before discharge,and quality of life scores before and after radiotherapy were compared between the two groups.Results The incidence of urinary retention in the observation group was lower than that of the control group (4.3% vs.14.3%,P <0.05).The success rate of one-time extubation was higher than that in the control group (94.7% vs.85.7%,P <0.05).The bladder recovery rate was higher than that in the control group (94.7% vs.83.4%,P < 0.05).The pelvic floor muscle strength was obviously superior than that in the control group (P < 0.05).Quality of life (QOL) score of two groups after treatment were higher than treatment before (P < 0.05),and the observation group was higher than that in the control group (P <0.05).Conclusion Cluster nursing can effectively prevent the occurrence of postoperative urinary retention,improve bladder function recovery,and improve quality of life in patients with cervical cancer receiving radiotherapy,so it is worthy of clinical promotion.

5.Clinical research on the application of family-based symptoms self-management program in myocardial infarction patients

Objective To explore the effects of family-based symptoms self-management program on quality of life,psychology,physiologic function and other rehabilitation indexes in myocardial infarction(MI) patients. Methods A total of 146 myocardial infarction patients from Nanjing Gaochun People′s Hospital, the Branch of Nanjing Drum Tower Hospital were selected by convenience sampling from January 2013 to January 2016. They were divided into control group(n=72,routine health education provided by cardiovascular specialist nurse,three times a week,12 weeks)and experimental group(n=74,family-based symptoms self-management program,12 weeks)with the random number table. And then,the quality of life,psychology, physiologic function and other rehabilitation indexes were compared between two groups. Results The scores of four dimensions on physical activity,diet,concerns over medication and medical side effects of the myocardial infarction dimensional assessment scale(MIDAS)in experimental group were lower than those in control group with significant differences(t=53.31,60.90,71.89,24.69;P<0.05). The scores of two dimensions on anxiety and depression of the hospital anxiety and depression scale(HADS)in experimental group were significantly lower than those in control group(t=36.76,30.05;P < 0.05). The total cholesterol,triglyceride,blood pressure,body mass index and preprandial blood glucose in experimental group were lower than those in control group with significant differences(t=31.86,29.34,36.28,35.34,33.55,31.86;P < 0.05). Compared with control group,the family-based symptoms self-management program could effectively reduce the readmission rate of myocardial infarction patients,the incidence of complications,quit smoking rate and increase the exercise time weekly with significant differences(χ2/t=5.13,4.00,4.78,38.90;P<0.05). Conclusions The family-based symptoms self-management program for 12 weeks can improve the quality of life of MI patients, relieve anxiety and depression and debase the levels of MI related physiological indexes and other rehabilitation indexes.
