1.Characteristics and treatment strategy of fresh high-energy fracture dislocation of tarsometatarsal joints

[Objective]To investigate the characteristics and treatment strategy for fresh high energy fracture dislocation of tarsometatarsal joints in a retrospective study.[Method]From September 2002 to April 2008,43 patients with fresh high energy fracture dislocation of tarsometatarsal joints were treated with cannulated screw combined with Kirschner wire fixation or Kirschner wire fixation alone.There were 35 males and 8 females,with a mean age of 37.5 years(range,18 to 55 years).The injury was caused by a road accident in 15 patients,by a heavy crash in 21 patients,and by a fall from a height in 7 patients.According to the Myerson damage typing,there were 9 cases of type A,2 cases of type of B1,25 cases of type B2,4 cases of type CI and 3 cases of type C2.Eight were open injuries(6 of Gustilo-Anderson type I and 2 of type Ⅱ)and 35 closed injuries.Internal fixation were undertaken at an average of 4.6 days after injury(range,2.5 hours to 21 days).At 4-6 weeks all the patients began functional exercises after removal of the plaster casts.At 16-18 weeks all the patients began to walk with partial weigh loading after removal of the internal fixators.[Result]All the 43 patients were followed up for 26 months on average(range,13 to 38 months).No infection,breakage,loosening of internal fixator,nonunion or longitudinal arch collapse was found.According to ZHOU'S assay standard.excellent result was achieved in 26 cases,good in 14 cases,and fair in 3 cases,with excellent to good results of 95.3%.Six patients had post-traumatic arthritis,which was complicated with injured articular surface.[Conclusion]Fresh high-energy fracture dislocation of tarsometatarsal joints can be diagnozed correctly.Early diagnosis and correct classification,proper operation timing,anatomical reduction,rational internal and external fixation and timely functional training are essential for patients to achieve good clinical result.
2.Arthroscopic suture fixation with cannulated screw and washer for bony avulsion of the posterior cruciate ligament

[Objective]To investigate the feasibility and short-term effects of arthroscopic suture fixation with cannulated screw and washer for bony avulsion of the posterior cruciate ligament.[Method]Arthroscopic suture fixation with cannulated screw and washer was carried out in 30 patients with bony avulsion of the posterior cruciate ligament.The posterior cruciate ligament and avulsion bone fragment were held together with two Ethibond 5 polyester threads,which were first wrapped around the posterior cruciate ligament from the anterior to the posterior,and then tied behind the posterior cruciate ligament and over the bone fragment.The threads were pulled out through tibia tunnels,tighted and fixed on cannulated screw and washer under the tibia tunnels 2 cm.[Result]The operation time were 60-90 min.A follow-up was made in 10 patients for 6-12 months.All fractures were united without displacement at 6 weeks after operation.No postoperative complications occurred.The Lysholm scores were 83-95(89.6?4.03).[Conclusion]Arthroscopic suture fixation with cannulated screw and washer for bony avulsion of the posterior cruciate ligament technique is simple to perform and gives a satisfactory short-term results.
3.Blood lead level and related factors in children from 2 kindergartens in Chongqing

Objective To investigate the blood lead level and its related factors in children in Chongqing and to provide evidence for the prevention and treatment of lead contamination. Methods In June 2001, the blood lead levels in 173 children(aged 2~7 2 years old) selected from 2 kindergartens representing the children in urban district and suburb were detected. At the same time, the related factors were investigated by using questionnaires. Results The highest, the lowest and the mean blood lead levels were 270 ?g/L, 18 ?g/L and 77 73 ?g/L, respectively. The blood lead level of 23 70% children investigated surpassed 100 ?g/L(the international diagnostic criterion of lead poisoning in children). Significant regional difference in blood lead level was found. The mean blood lead levels in children in Yuzhong District and Dadukou District were 111 23 ?g/L and 57 57 ?g/L, respectively. The percentages of children in the two districts with blood lead levels higher than 100 ?g/L were 52 31% and 6 48%, respectively. Blood lead level was closely correlated with the dwelling condition, the dwelling location where factories were nearby or the number of factories was large, health habits, the frequency of eating canned food and popcorn, and culture levels of the mothers. Conclusion Blood lead level of children in the urban distict is significantly higher than that in the suburb, to which more attention should be paid.
4.ADR Case Reports from 2006~2007 in Our Hospital:A Retrospective Analysis

OBJECTIVE: To investigate the characteristics and regularity of adverse drug reactions(ADR) occurred in our hospital for reference of clinical rational drug use.METHODS: 1 183 ADR cases reported in our hospital between 2006 and 2007 were analyzed statistically using Excel in respect of patients' sex,age,routes of administration,variety of drugs and clinical manifestations etc.RESULTS: Of the 1 183 ADR cases,268(22.65%) were submitted by clinical departments,462(39.05%) by the department of pharmacy,and 441(37.28%) by central injection room.ADR incidence rate was slightly lower in males than in females.Patients over the age of 31 years showed a higher incidence of ADR.901 cases(76.16%) were induced by intravenous route.662 were induced by anti-infectives,followed by the traditional Chinese medicine(163 cases) and cardiovascular system drugs(149 cases).The most common manifestation of ADR was lesion of digestive system(301 cases),followed by lesions of skin and its appendages(236 cases) and damage of central nervous system(224 cases).CONCLUSION: Great importance should be attached to ADR monitoring as well as the analysis and evaluation of ADR so as to ensure safe and effective medication.
5.Effects of Fluvastatin Combined with Aspirin on the Related Indicators of Patients with Diabetic Lower Ex-tremity Arteriosclerosis Occlusive Desease

OBJECTIVE:To investigate the effects of fluvastatin combined with aspirin on the related indicators of patients with diabetic lower extremity arteriosclerosis occlusive desease(LEAOD). METHODS:80 patients with diabetic LEAOD were ran-domly divided into control group(40 cases)and observation group(40 cases). Based on convertional treatment,control group was given Rosuvastatin calcium tablet 5 mg+Aspirin enteric-coated tablet 100 mg every evening;observation group was given Fluvas-tatin sodium capsule 40 mg+Aspirin enteric-coated tablet 100 mg. They were treated for 3 months. Fibrinogen,plasma D-dimer, blood viscosity, cholesterol (TC), triglyceride (TG), glycosylated hemoglobin (HbAlc), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), carotid intima-media thickness, compliance situation of TC, TG, HbAlc,LDL-C and HDL-C,cases of carotid intima thickening,plaque formation,stenosis and occlusion before and after treat-ment were observed,and the incidence of adverse reactions was recorded. RESULTS:After treatment,fibrinogen,plasma D-di-mer,blood viscosity,TC,TG,HbAlc,LDL-C and carotid intima-media thickness in 2 groups were significantly lower than before treatment,and observation group was significantly lower than control group;HDL-C in 2 groups was significantly higher than be-fore,and observation group was significantly higher than control group,with statistical significance (P<0.05). Compliance rates of TC,TG,LDL-C and HDL-C in observation group were significantly higher than control group,proportion of carotid intima thickening and carotid plaque formation were significantly less than control group,with statistical significance (P<0.05),there were no significant differences in the compliance rate of HbA1c,proportion of carotid stenosis and occlusion and the incidence of adverse reactions in 2 groups(P>0.05). CONCLUSIONS:Fluvastatin combined with aspirin can significantly improve the pa-tients 'blood viscosity,blood lipid,vascular endothelial function in the treatment of diabetic LEAOD,it can reduce the thrombotic disease risk,and does not increase the incidence of adverse reactions.
6.Factors influencing long-term effects of interventional therapy on diabetic lower extremity vascular disease

Objective To investigate the influencing factors involved in the long-term effect after interventional treatment of diabetic lower extremity vascular disease.Methods The data of 358 diabetic patients with lower extremity vascular disease were collected retrospectively.The ankle-brachial index (ABI) values at the 6th, 12th and 24th month after interventional treatment were documented and the factors may influence ABI value werc analyzed.Results Single factor analysis showed that after 24 months treatment, the proportion of ABI<0.5 (severe ischemia) in the patients combined with obesity, smoking, older age were 15.0% (31/206), 18.4% (25/136) , 12.1%(19/157) ,7.5%(15/200) ,while non obesity,younger age,no smoking,no hypertension were 6.6% (10/152), 14.8% (33/222), 6.5% (13/201), 1.9% (3/158), and the differences were significant (P <0.05).Logistic regression analysis showed that old age (OR =1.341,95% CI: 1.281-1.496, P =0.045), smoking(OR=2.301,95% CI: 1.903-4.844,P=0.028) and obesity(OR=1.299,95% CI: 1.270-1.588,P =0.038) were the independent factors influence the long-term therapeutic effects on diabetic lower extremity vascular disease.Conclusion Obesity, smoking, old age are the main factors influence the long-term therapeutic effects on diabetic lower extremity vascular disease.
7.The analysis of Chinese official pharmacy talent training and education model in Sui and Tang dynasties

Objective By finding out the Chinese official pharmacy talent training and education model in Sui and Tang dynasties,we can used it in the cultivation of contemporary talents in pharmacy.Methods pharmacy education in CNKI,Sui and Tang history books,Chinese herbal medicine and legal literature were collected and categorized in training organizations,education and evaluation three aspects.Results In the Sui and Tang dynasties,pharmacy institutions included imperial pharmacies,drug possession Bureau and the Department of imperial medicine disciplines.Meanwhile,there were relevant regulations in education,teaching and quality assessment.According to the traditional theory,practice and correlation characteristics,focusing on the connection of clinical pharmacy practice courses and lessons,the government trained a large number of professional talents in pharmacy.Conclusion In the Sui and Tang dynasties,Chinese material medical attaches great importance to the comprehensive quality of talents cultivation,which is worthy for modern reference.
8.Umbilical cord blood mesenchymal stem cell transplantation for treatment of a child with spinal muscular atrophy

BACKGROUND: Many animal and clinical studies have reported that the safe and effective usage of umbilical cord blood-derived mesenchymal stem cells (UCB-MSCs) transplantation for treatment of neurological genetic diseases.OBJECTIVE: To investigate the feasibility and effect of UCB-MSCs transplantation in the treatment of spinal muscular atrophy (SMA).METHODS: A child admitted at January 2010 had been confirmed as having SMA, and drug and rehabilitation therapies were invalid. Then, the child received UCB-MSCs transplantation via the first intravenous infusion and three times of subarachnoid injection, once a week, (4-6)×107 cells once and four times as a course. Neurological physical examination, biochemical test, muscle enzymes detection, FIM scoring and electromyography (EMG) examination were conducted. RESULTS AND CONCLUSION: Compared with prior to transplantation, the level of muscle enzymes decreased, FIM scores were increased from 68 to 93 points, EMG results showed that the motor units with re-contraction in each 10.0 ms were increased that the motor function was improved, the lower extremity muscle strength elevated, and the self-care ability was improved in the SMA child at 6 months after transplantation. During the 10-month follow-up, the child had no adverse effects. It is indicated that UCB-MSCs transplantation is effective to treat SMA, and the neurological function has a remarkable restoration.
9.Comparison on model establishment methods and index in common of Yang deficiency mice induced by hydrocortisone

Objective To compare and analyze the index variation of Yang deficiency mice induced by hydrocortisone in different way of administration and dose. Methods The mice model of Yang deficiency were induced by low dose and high dose hydrocortisone(12.5 mg/kg, 25 mg/kg) in two ways of intramuscular and intraperitoneal injection. The symptoms, body weight, the average intake, the index of stress and the coefficient of sex organs and immune organs of animals were observed. Results In the way of intraperitoneal injection, the weight(24.52±3.29)g, body temperature(35.24±0.32)℃ of high dose of model group were significantly lower than that of control group(31.10±6.11)g,(37.02±0.64)℃. The average intake of low dose group(4.30 ± 0.29)g/(per?d) was lower than control group(7.38±0.53)g/(per?d), and the coefficient of preputial glands(0.10±0.02), penis(0.15±0.03), thymus(0.12±0.03)were lower than that of control group. In the way of intramuscular injection, the average intake of high dose group(5.92±2.01)g/(per?d) was lower than control group(8.60±1.33)g/(per?d). The body temperature(34.90±0.22)℃ and the time of swimming in low temperature (34.00±22.41)s of low dose model group were lower than that of control group(36.43±0.91)℃, (67.17±21.93)s, and the coefficient of thymus of two model groups(0.10±0.02),(0.11±0.06)were lower than that of control group. Conclusion Various dose and model establishment methods of Yang deficiency mice have different the time of symptom appearing, the degree of symptom and sensitive index.
10.Infection-associated Factors of Peritoneal Cavity Infection by Enterococcus

OBJECTIVE To study the role of Enterococcus in peritoneal cavity infection and the infection -associated factors, by which the prophylactic measures are put forward. METHODS The role of Enterococcus in peritoneal cavity infection and the infection-associated factors were discussed with the analysis of the investigated 33 cases with the peritoneal cavity infection by Enterococcus. RESULTS The peritoneal cavity infection was mainly caused by Enterococcus faecalis, for which penicillin and ampicillin were the first choice of empirical therapy. For regime of the combination use of antibiotics might be feasible for the infection caused by the speeics with unshowed high-level resistance to aminoglycosides. Many related risk factors on peritoneal infection caused by Enterococcus were included such as the colonization of the drug resistant strain of Enterococcus promoted by cephalosporins Ⅲ and the abuse of large amount of antibiotics as a selective pressure on the resistant strain. CONCLUSIONS Much attention should be paid on the role of Enterococcus in peritoneal infection clinically. The effective therapy is associated with the eradication of the pathogen. and the better control measures against infection.
