OBJECTIVE To comprehensively analyze the risk factors for infection and colonization of community-acquired MRSA.METHODS The results of 12 studies were analyzed by meta-analysis and the OR value of every factor was calculated.RESULTS All eight risk factors were evaluated:prior hospitalization(OR=2.46,CI1.25-4.85),antibiotics exposure recently(OR=2.77,CI1.34-5.74),contact with healthcare system and medical workers frequently(OR=6.48,CI2.38-17.63),surgery or invasive procedure(OR=2.53,CI1.90-3.36),age(OR=-1.99,CI-9.21-5.23),gender(OR=1.04,CI0.71-1.51),intravenous drug use(OR=1.49,CI0.34-6.54),and underlying diseases(OR=1.12,CI0.55-2.28).CONCLUSIONS Prior hospitalization,antibiotics exposure recently,contact with healthcare system and medical workers frequently and surgery or invasive procedure are risk factors of community-acquired MRSA.The effects of age,gender,intravenous drug use and underlying diseases need further investigation.

Objective To investigate the expression of tissue transglutaminase(tTG) and vascular endothelial growth factor(VEGF) in epithelial ovarian carcinoma and the relationship of invasion and metastance in ovarian carcinoma.Methods The expression of tTG and VEGF was detected in 55 malignant and 21 benign ovarian neoplasms by immunohistochemical SP method.The clinical significance of tTG and VEGF and their relationship were analyzed.Results The expression rates of tTG and VEGF in malignant epithelial ovarian tumors were significantly higher than in benign epithelial ovarian tumors(tTG:92.7％ vs.38.1％,x2 =32.620,P ＜0.01 ;VEGF:90.9％ vs.47.6％,x2 =19.570,P ＜0.01).In ovarian cancer,the expression of tTG in tumor cells and stromal cells was significantly related to clinical phases and the presence of ascites.The expression of VEGF in tumor cells had a significant revelance to clinical phases,pathological grades and ascites.The expression of tTG in ovarian cancer cells was positively correlated with VEGF(r =0.8856,x2 =43.14,P ＜0.01).Conclusion Tissue transglutaminase and VEGF were detected not only in tumor cells,but also instromal cells.High expression of tTG and VEGF may play an importaant role in the pathogenesis of ovarian cancer and their synergistic effect may act as an important factor in promoting the invasion and metastasis of this cancer.

ObjectiveTo investigate the expression of EMMPRIN and MMP-9 in epithelial ovarian carcinoma and the relationship of invasion and metastasis in ovarian carcinoma.MethodsThe expression of EMMPRIN and MMP-9 was detected in 55 malignant and 21 benign ovarian neoplasms by immunohistochemical Sp method.ResultsThe expression rate of EMMPRIN and MMP-9 in malignant, benign epithelial ovarian tumors was 89.1％, 14.3％ and 96.4％, 23.8％, respectively.There were significance different between malignant tumors and benign tumors.In ovarian cancer, the expression of EMMPRIN in tumor cells had a significant relevance to clinical phases and pathology types, while the expression of EMMPRIN in stromal ceils was associated with clinical phases, pathology types and pathological grades.The expression of MMP-9 in tumor cells had a significant relevance to clinical phases and pathology types while the expression of MMP-9 in stromal cells was associated with clinical phases, pathology types and ascites.ConclusionsEMMPRIN and MMP-9 were detected not only in tumor cells, but also in stoma cells.High expression of EMMPRIN and MMP-9 may play an important role in the pathogenesis of ovarian cancer, and their synergistic effect may act as an important factor in promoting the invasion and metastasis of ovarian cancer.

Objective To acknowledge the spouse comorbid conditions of diabetes patients by comparing the average blood sugar level of the spouse of the diabetes patients with normal Guangzhou residents.Methods To sift and analyze the spouse test results of 152 hospitalized diabetes patients from January 2012 to January 2013,by testing the blood sugar level of the experimental subjects,comparing the different blood sugar level results between when they were with empty stomach and 2 hours after oral glucose tolerance test (OGTT).Results There were 32 cases with diabetes,9 cases with impaired glucose tolerance (IGT) and 11 cases with impaired fasting glucose (IFG) in the above 152 experimental subjects.The morbidity rates were 21.05％,5.92％ and 7.24％.All the above results were higher than the Guangzhou residents survey level,there were statistical differences between them.Conclusions The high morbidity rates of diabetes and impaired glucose regulation in diabetes patients spouse are obvious.The works for diabetes sifting and analyzing together with the health education instruction need to be strengthened.

Objective To evaluate the long-term result of jejunum-interpositioned choledochoduodenostomy with a antireflux valve after resection of choledochocele in 178 patients.Methods Postoperative patients were followed up by barium series of the upper digestive tract at different interval. To observe whether reflux remains and the severity of the reflux.Results142 out of the 178 cases (798%) were followed up, among them 21 cases (48%) without reflux, 26 cases (83%) with mild reflux, 51 cases (359%) with moderate reflux,and 28 severe cases(9%).16 cases(11.3%) had to undergo reoperation.Conclusion The technique has the effect of anti-reflux temporarily.Long-term follow-up found an increased morbidity of biliary infection, obstruction, perforation and stone formation.Severe postoperative complications necessitates a reoperation in some cases.

Objective To summarize the clinical features and therapy of sarcoidosis in the South of China.Methods A retrospective analysis was perfomed in 31 patients with sarcoidosis in past 22 years.Results The most frequent age at onset of sarcoidosis was 30～50 years old.Female patients were more than that in male patients. X-ray test of chest was the major way to diagnose the disease at early stage.The typing of lymphocytes in broncho-alveolar lavage fluid(BALF) has valuable to diagnose,but confirmed diagnosis depended on biopsy of histopathology.Conclusions Glucocorticoid is effective drug treatment for the most the patients who are at stage Ⅰ and Ⅱ. Every patient should be treated individually .If necessary,we should add immunosuppressors.

Objective To study the operative methods and clinical outcome of laparoscopic ileal colpopoiesis with long vascular pedicle.Methods We reviewed 31 cases which had received ileal colpopoiesis,including 25 cases of congenital absence of vagina and 6 cases of male-to-female transsex.All cases were operated on by laparoscopic(8 cases) and laparoscopy assisted(23 cases) ileal colpopoiesis.The postoperative results were analyzed through physical examination and interview regarding the patient′s functional status and satisfaction during sexual intercourse.Results Ileal colpopoiesis were preformed successfully on all 31 cases.Follow-ups of 3-13 months were done for all of them.Patients used vaginal mould for at least 8 months.All artificial vaginas had excellent cosmetic results,including adequate vaginal length,good lubrication,and appearance and physical functions similar to female natural vagina,and sexual function was reportedly satisfactory.One of the patients had an operative complication of an intestinal obstruction,which was cured by a second operation and other treatments.Conclusions The laparoscopic and laparoscopy assisted ileal vaginoplasty is a feasible,safe and ideal method of current vaginoplasty due to its minor damage,excellent cosmetic results and similarity to normal vaginal features.However,vaginal mould should be used for a long period of time(at least 8 months) after an operation,to prevent vaginal orifice shrinkage.
