1.The cross-sectional study on diabetic peripheral neuropathy among the middle-aged and old-aged population with type 2 diabetes mellitus in a community

Objective To study the prevalence and the related risk factors of diabetic peripheral neuropathy(DPN) after good blood glucose well controlled among in middle-aged and old-aged population with type 2 diabetes mellitus in a community of Beijing.Methods DPN was screened by Michigan neuropathy screening instrument (MNSI),and common blood biochemical parameters were tested,among over 45 years of age registered population patients with type 2 diabetes mellitus in Shuangjing community of Chaoyang District of Beijing.Results The blood glucose control rate was 90.36% (253/280),and the prevalence of DPN was 25.36% (71/280) with male of 24.47 % (23/94) and female of 25.81% (48/186).Univariate analysis showed that both age and HbA1c in DPN group were higher than that of non-DPN group,and the differences were statistically significant(P =0.000,0.008).The level of plasma low density lipoprotein cholesterol(LDL-C) of the DPN group was lower than that of the non-DPN group,and the difference was statistically significant (P=0.017).The hypertension prevalence rate of the DPN group was 87.32%(62/71),higher than that of the nonDPN group(75.6% (158/209)),and the difference was statistically significant (P =0.037).The multivariate logistic regression analysis showed that the estimate of parameter for age was 0.143 (P =0.0001),OR was 1.153,95%CI was from 1.029 to 1.219,for HbA1c was 0.529(P=0.03),OR was 1.698,95%CI was from 1.053 to 2.738,and for LDL-C was-O.919 (P =0.018),OR was 0.399,95% CI was from 0.186 to 0.854.Conclusion The prevalence of DPN is still above 25% among middle-aged and old-aged in a community,even though the well controlled rate of blood glucose control rate is above 90%.It is possible that old age and HbA1c are related to DPN,apart from blood glucose.
2.The effect of intrauterine device application on post-menopausal reproductive health

To explore the potential effects of intrauterine device ( IUD) usage in reproductive ages on women′s postmenopausal reproductive health.Women in Shanghai Minhang district were enrolled and categorized into group Ⅰ( IUD) , groupⅡ( tubal ligation ) and control group ( external ejaculation ).Their data were collected and analyzed retrospectively.A total of 2 744 women during postmenopausal period were enrolled (group Ⅰ=2 253, group Ⅱ=202, control =289).Pearl index of each group was 1.08 (groupⅠ) , 0.00 ( group Ⅱ) and 15.22 ( control ) , respectively.Women with IUD usage in reproductive ages were less likely to develop reproductive benign diseases after menopause (χ2 =67.51, P=0.00, OR=0.44, 95%CI:0.32-0.59).Logistic regression indicated pregnancy history (OR=3.85), child-bearing history ( OR =0.52 ) , IUD usage ( OR =0.48 ) , tubal ligation ( OR =1.74 ) , longer duration using contraceptive methods ( OR =0.99 ) , contraception failure ( OR =1.74 ) and family planning surgical procedures(OR=1.73) were risk factors for reproductive benign diseases after menopause.IUD utility in reproductive ages may reduce the risks of benign diseases in reproductive system for postmenopausal women .
3.The value of 18F-FDG PET/CT in diagnosing giant cell arteritis presenting as fever of unknown origin

Objective To evaluate the clinical diagnostic contribution of 18 F-FDG PET/CT in giant cell arteritis with initial presentation as fever of unknown origin (FUO).Methods Eight cases with initial presentation as FUO diagnosed with the contribution of PET/CT were retrospectively studied in Peking Union Medical College Hospital.The radiologic manifestations of PET/CT were analyzed.Results Eight patients (4 men and 4 women) with average (63 ± 7) years (range from 55-75 years) were included in our study based on the criteria.Non-specific clinical symptoms were common in these patients,including fatigue,night sweat and weight loss.They all suffered from anemia of chronic disease.Not surprisingly,the inflammatory parameters were elevated significantly in all patients.The medical history,physical examination and routine lab and radiologic examinations couldn't reveal the causes of fever.PET/CT was performed in all of them,which demonstrated intense 18F-FDG uptake in the area of aorta and its major branches with maximal standard uptake value (SUVmax) 2.1-4.6 (3.6 ± 0.9).Immunosuppressive agents were effective to control the inflammation activity.The SUV decreased significantly after treatment in the follow-up PET/CT.Conclusions PET/CT has demonstrated high yield of diagnostic contribution in giant cell arteritis with initial presentation as fever of unknown origin.As to elderly FUO patients presenting with prominent inflammatory reaction,PET/CT may provide potential value to differentiate diagnosis from maligancies.
4.Application of virtual simulation technology in basic nursing teaching

Objective To explore the effects of application of virtual simulation technology in basic nursing practice teaching.Methods Convenience sampling method was used to select 204 nursing students of grade 2013 in Fenyang College of Shanxi Medical University as the research subjects.Virtual reality software and simulator were used to build realistic experiment environment.Tasks of virtual simulation training were primary,intermediate and advanced.Questionnaire survey was conducted after training to investigate teaching effects,and theoretical and operating test scores of grade 2013 were compared with those of grade 2012.Results In the primary,intermediate and advanced tasks,there were 87.3%,77% and 60.8% of the students having scores of 91~100;theoretical and operating test scores of grade 2013 were better than those of grade 2012(P<0.01);98% of the students considered virtual simulation enhanced proficiency of operation,93.1% of the students considered virtual simulation facilitated learning,91.1% of the students liked virtual simulation learning,and 90.1% of the students considered virtual simulation stimulated interests in learning.Conclusion The application of virtual simulation experiment teaching in basic nursing practice teaching can promote the innovation of teaching mode and improve the teaching quality and teaching effect.
5.Using the PDCA cycle to improve the rehabilitation of postoperative patients with anal fistula

Objective To evaluate the PDCA cycle on the rehabilitation of postoperative patients with anal fistula, aiming at improving the recovery of patients and preventing recurrence. Methods 144 postoperative patients of anal fistula were randomly divided into the observation group and the control group with 72 cases in each group. The observation group received intervention based upon the PDCA cy-cle, and the control group was given the traditional oral propaganda. The recovery time, recurrence rate, mastering of education content and self-control ability were compared in the two groups using t test and χ2 test. Results The mastering level of education content and the self-management skills in the observation group were superior to those of the control group. After 3 to 24 months of follow-up, the observation group showed shorter recovery time and less recurrence rate than that of the control group. Conclusions Nurs-ing intervention based upon PDCA cycle can strengthen the patients' mastering of related knowledge, pro-mote the recovery, reduce recurrence rate and improve the patients' quality of life.
6.A clinical and laboratory investigation of myelodysplastic/myeloproliferative neoplasms

ObjectiveTo investigate distingwished clinical and experimental characteristics of the four main subtypes in myelodysplastic/myeloproliferative neoplasms (MDS/MPNs).MethodsMDS/MPNs 53 cases from Provincial Hospital Affiliated to Shandong University,including 24 cases CMML,13 cases aCML,12 cases JMML,4 cases MDS/MPN-U,were analyzed regarding to 2001 WHO classification.Morphology (M) of peripheral and bone marrow blood cells were observed under microscope.FCM was used in immunological(Ⅰ) analyse on blasts and myelomonocytes in peripheral blood and bone marrow.G-banding technique was used in cytogenetic (C)examination.PCR was used in molecular genetic (M) mutation detection.Numeric data,such as mean Hb,WBC,PLT et al,among several groups,were compared using Single-factor analysisof variance.Student-Newman-Keulstestwasuseincomparingmeansof two groups.Proportions,such as percentage of clinical features,immunological and cytogenetic abnormal cases among different groups,were compared using Chi-square test or Fisher exact test.Results( 1 ) In the course of MDS/MPNs,there were 46 cases (86.8% ) had paleness and fatigue 33 cases (62.5% ) had palpable spleen.JMML had most fever and enlargement of lymph node (75.0%,75.0% ),statistically distinguished from CMML ( 12.5%,12.5% ) (x2 =14.89,17.98,P < 0.05 ).(2) The hemoglobin was ( 83.1 ± 24.6 ) g/L.WBC counts were ( 19.8 ± 8.1 ) × 109/L.PLT counts were ( 158.7 ± 108.2) x 109/L.Immature neutrophils and blasts were found in peripheral blood.(3)JMML and CMML had most monocytes absolute counts among the subtypes (4.25 ±0.76) (3.62 ±0.76).(4) Almost 100% JMML had monocytes abnormalities.(5)For 15 cases were detected immunological characteres by FCM,13 cases showed abnormalities.(6)For 29 cases of MDS/MPNs had been analyzed chromosome karyotypes and 12 out of them (41.4%) were abnormal,Ph chromosomes and those AML-defining translocations were all negative,+ 8 and 7-involved- karyotypes were more frequent.(7)23 cases were detected molecular genetic features,in which were all negative.BCR/ABL1 and JAK2 V617F mutation were all negative in the 13 cases of aCML.JAK2 V617F mutation was positive in 1 case of MDS/MPN-U.ConclusionsMost MDS/MPNs had paleness and fatigue,light to mild anemia,cytosis,monocytes low grade of blast and immature neutrophils in peripheral blood with dysplasia in bone marrow.JMML seems has more severe clinical features and more distinguishing laboratory characters.Immunological abnormalities and abnormalkaryotypes are found frequently in MDS/MPNs with no statistical differences among the four subtypes.There is no specific molecular abnormals in MDS/MPNs.( Chin J Lab Med,2012,35:832-837)
7.A clinical analysis of 25 cases of prosthetic valve endocarditis

Objective To report the clinical characteristics of prosthetic valve endocarditis (PVE).Methods All 25 cases of definite PVE (Duke criteria) diagnosed at our hospital between January 1992 to December 2008 were retrospectively analyzed. Among them, 7 cases were pathologically confirmed and the others were clinically confirmed with either 2 major criteria or 1 major and ≥3 minor criteria. Their clinical characteristics, underlying heart diseases, previous heart operations, presenting manifestations, causative microbes, echocardiographic findings and prognosis, were studied. Results (1) Although most cases underwent valve transplantations for underlying heart diseases of rheumatic heart diseases and congenital heart diseases, 10 patients were complicated with infectious endocarditis (IE) prior to the operations, 4 of them were PVE. (2) Eleven of them developed PVE within 2 months postoperatively. Fever (100%),major vessel embolism (48%), and anemia (36%) were the most frequently manifestations. Fourteen cases (56%) had positive culture results with 15 causative pathogens, including 5 coagulase-negative Staphylococcus (CNS, 3 were methicillin-resistant coagulase-negative Staphylococcus, MRSCoN), 4 fungi, 2Enterococcus faecalis, 2 Burkholderia cepacia, 1 Stenotrophomonasmal-tophilia, and 1 Streptococcus.(3)Prosthetic valve vegetations, periannular leakage, regurgitation, were the main echocardiographic findings.Transesophageal echocardiography (TEE) revealed 13 PVE who had no positive findings on previous transthoracic echocardiography (TTE). (4) Eighteen PVE (72%) developed peri-annular complications (12 leakage, 3 dehiscence, 2 abscesses, 1 fistula), major vessel embolism, congestive heart failure (16%) were frequently observed, 9 of the 17 patients died in hospital, in spite of intensive managements.Conclusions PVE has a high mortality and is a severe complication for patients who underwent heart surgery. Its causative pathogen spectrum is quite different from that of native valve endocarditis. TTE is not sensitive for some PVE cases.
8.Association of nutrition related knowledge and psychosocial factors on screen related sedentary of primary school students aged 10-12 in Beijing

Objective:
To explore the screen related sedentary behavior among senior primary school students in Beijing and to analyze the influence of psychosocial determinants and nutrition related knowledge on the behavior.
Methods:
In January 2020, a total of 1 316 students in grade 4-6 from two primary schools in Beijing were selected. Information on video viewing (watching TV or playing video games), self efficacy, habit strength, nutrition related knowledge, outcome expectation and the capacity to persist toward goal attainment were collected through questionnaires. Disordered multi classification Logistic regression and random forest algorithm were used to analyze the influencing factors.
Results:
The frequency of screen related sedentary was 5.0 (3.0, 10.5) times/week, and the duration was 37.5(9.6, 97.5) min/d in senior elementary school children. The results of disordered multi classification Logistic regression showed that the capacity to persist toward goal attainment, nutrition related knowledge, habit strength, self efficacy and gender positively correlated with the frequency of screen related sedentary ( OR =1.6, 1.7, 4.9, 4.2, 1.5 ), while the nutrition related knowledge, habit strength, self efficacy, outcome expectations, grade and gender positively correlated with screen time ( OR =1.7, 5.6, 5.7, 1.6, 1.6, 1.7)( P <0.05). Random forest regression tree model showed that the top four influencing factors on screen related sedentary frequency were self efficacy, the capacity to persist toward goal attainment, habit strength and nutrition related knowledge and the top four influencing factors on screen time were self efficacy, outcome expectation, nutrition related knowledge, habit strength.
Conclusion
Screen related sedentary behavior is prevalent among senior primary school students in Beijing. Health education should be strengthened regarding influencing factors of screen related sedentary behavior.
9.Association between psychosocial determinants with primary school students snacking behavior

Objective:
The association of self efficacy, habit strength, goal intention and outcome expectation with the consumption of snack in senior primary school students were investigated, providing evidence for the application of psychosocial determinants based on Social Cognitive Theory in children s nutrition education.
Methods:
A total of 1 353 students, 10-12 years old, participated in this study. Participants completed questionnaires for assessing self efficacy, habit strength, goal intention, outcome expectation and snack consumption in January 2020.
Results:
Average times of snack consumption among primary school students were 5.0 (3.0, 8.5) times per week, and the snack intake were 4.0(0.9, 22.9)g/d. The outcome expectation score was -1.0(-2.0, -1.0 ) points, while the score of children s snack related selfefficacy was (4.3±0.9) points, habit strength score 1.0(1.0, 3.0) points, and the goal intention (4.2±1.2) points. Students with higher snack intake showed lower scores in outcome expectation, self efficacy and goal intention( t=-9.0, 6.8, 5.2, P <0.01). Logistic regression showed negative association between outcome expectation and self efficacy factors with snacking behavior( OR =0.7, 0.8, respectively), as well as positive association between habit strength and snacking behaviors( OR=1.3, P <0.05).
Conclusion
The evidence indicated that self efficacy, habit strength, goal intention and outcome expectation are associated with child s snack consumption, and can be used as theoretical foundation for child snack consumption education.
10.Development and progress in the application of smart health technologies for older adults with mild cognitive impairment

Under the backdrop of smart health management technology development, this article reviews research advances in smart monitoring, assessment and intervention technologies for older people with mild cognitive impairment, including the types, typical applications and results of monitoring, assessment and intervention technologies.In addition, from the perspective of community-dwelling older adults' cognitive health management, a model for innovative management of community-dwelling older adults' cognitive function taking advantage of smart health management technologies is proposed, aiming to enhance the acceptance of smart health technologies among older people with cognitive impairment and to provide policy advice on developing friendly communities for older people with cognitive impairment.
