1.Clinical value of miniprobe sonography in differential diagnosis of Crohn??s disease and ulcerative colitis

Objective To explore the value of miniprobe sonography (MPS) in differentiating Crohn??s disease (CD) and ulcerative colitis (UC). Methods 46 patients with active inflammatory bowel disease (IBD) were included into the patient group (16 patients with CD and 30 cases with UC). 20 healthy cases ( underwent physical examination in outpatient department) were recruited as the control group. All cases underwent MPS and high sensitive C-reactive protein (Hs-CRP) test.The MPS measurement index include thetotalwallthickness(TWT), mucosal thickness (MT), submucosal thickness (SMT), muscularispropria thickness (MPT), and the number of enlarged colic and paracolic lymph nodes. Results TheTWT, M, SM, MP(mm)and Hs-CRP(mg/L)in CD, UC and control group are 5.84 ± 1.42, 1.48 ± 0.23, 1.88 ± 0.28, 2.31 ± 0.85, 40.58 ± 19.33, 4.74 ± 1.01, 1.79 ± 0.35, 1.41 ± 0.25, 1.32 ± 0.34, 22.41 ± 15.25,2.86 ± 0.23, 0.97 ± 0.13, 1.06 ± 0.11, 0.64 ± 0.0 and 1.70 ± 0.65. TWT, MT, SMT, MPT and Hs-CRP in UC group has significant different with those in controp group(P<0.05). The mean value of TWT, MT, SMT, MPT and Hs-CRP in CD group are higher than those in UC group. M in UC group is higher than that in CE group. The difference is significant(P<0.05). 5 cases in CD and 2 cases with UC had enlarged colic or paracolic lymph nodes. Conclusions The MPS can distinctly observe the changesof different colonic tissue layers and the surrounding tissue structures in IBD patients,which helps for distinguishing active CD from UC.
2.Prognostic relevance of preoperative platelet-to-albumin ratio(PAR) in patients with upper urinary tract urothelial carcinoma

Objective:To assess the value of platelet-to-albumin ratio on overall survival(OS) and cancer specific survival(CSS) in patients with upper urinary tract urothelial carcinoma(UTUC).Methods:169 patients with UTUC in Qilu hospital of Shandong University from January 2006 to December 2013 were included in this study, retrospectively. 107(63.3%) were male and 62(36.7%) were female. Their age ranged from 36 to 87 years old, mean 65 years old. 73(43.2%)were less than 65 years old and 96(56.8%) were more than 65 years old. 38 patients had the history of smoking.15 patients had the history of bladder cancer. The single tumor was found in 159 cases (94.1%). 10 patients(5.9) owned more than one site of tumor. For the ECOG, 159 cases (94.1%) were 0 score and 10 cases(5.9%) were ≥1 score. The mean Platelet count was 217.0×10 9/L, ranging (68.0-772.0)×10 9/L. The average serum albumin was 42.0 g/L, ranging (27.3-52.7)g/L. The mean PAR was 5.40×10 9, ranging(0-17.31)×10 9. 147(87.0%) patients accepted radical nephroureterectomy and 22(13.0%) cases accepted segmental ureterectomy.155 cases(91.7%) used postoperative adjuvant chemoradiation. The patients were divided into two groups as low PAR or high according to the receiver operating curve (ROC)analysis of PAR. And the Kaplan-Meier analysis was used to assess the OS and CSS and test the equivalences of the curves by log-rank tests. Univariate and multivariate survival analyses were carried out using Cox proportional hazards regression model. Results:We set 6.67×10 9 was the optimal cutoff value of PAR. Therefore, the cohort was divided into 2 groups, 133(78.7%) patients had low PAR (<6.67×10 9) and 36 patients had high PAR (≥6.67×10 9). The 3-year OS and 5-year OS of high preoperative PAR group were 50.0% and 46.2%, respectively. And low preoperative PAR group were 77.6% and 66.7%, respectively. In univariate analysis, no difference was found in gender, age, tumor size, history of smoking and LVI. Meanwhile, tumor grade (G 1-2 or G 3), pathological T stage (pT a/T 1 or pT 2-4) and PAR (<6.67×10 9 or ≥6.67×10 9) were significantly associated with OS and CSS. Multivariate analysis with a Cox proportional hazards regression model were performed and showed that PAR was significantly associated with OS and CSS (OS: HR=1.850, 95% CI 1.095-3.127, P=0.022; CSS: HR=2.154, 95% CI 1. 242-3.736, P=0.006). Conclusions:Preoperative PAR is an independent prognostic factor in predicting patient with UTUC. And the platelet and serum albumin level are low cost and easy to obtain. PAR may be expected to become a biological factor to predict the prognostic factor in patients with UTUC.
3.A clinical trial to study the failure of treating initial Helicobacter pylori eradication with the doxycycline,amoxicillin and clavulanate potassium containing rescue quadruple regimen

Objectives To investigate the efficacy and safety of quadruple regimen containing doxycy-cline,amoxicillin and clavulanate potassium in the treatment of Helicobacter pylori(Hp)after the failed initial eradication. Methods A total of 136 patients who had a failed course of initial eradication therapy were randomly divided into four groups. The 46 patients in group A were given rabeprazole,colloidal bismuth tartrate,doxycy-cline,amoxicillin and clavulanate potassium for 14 days). The 45 cases in group B were treated with rabeprazole, colloidal bismuth tartrate,doxycycline,amoxicillin and clavulanate potassium for 10 days. The 45 cases in control group received rabeprazole,colloidal bismuth tartrate,levofloxacin,amoxicillin for 14 days. After 4 weeks of treat-ment,the 13C-urea breath test was performed. The Hp eradication rates,adverse reactions,treatment compliance and cost-effectiveness ratio of the three groups were evaluated. Results A total of 130 patients completed the study. The intention to treat(ITT)and Per-protocol(PP)analyses revealed that the eradication rates of the three groups were 71.74% and 76.74%(group A),71.11% and 72.72%(group B),51.11% and 53.49%(control group),respectively. The ITT and PP of group were significantly higher than those of control group(P < 0.05). But there was no significant difference between group B and control group(P > 0.05). The rates of adverse reac-tions in each group were not statistically significant(P > 0.05),and the patient compliance of three groups was more than 80%. The cost and cost-effectiveness ratio showed that group B < control group < group A. Conclu-sions The quadruple therapy containing doxycycline,amoxicillin and clavulanate potassium as a rescue regimen given for 14 days is safe,effective and with high patient compliance,which is of high clinical significance.
4.Construction and identification of non-replication recombinant vaccinia virus co-expressing human papillomavirus type 16 L1/L2/E6/E7 proteins.

OBJECTIVETo generate a human papillomavirus (HPV16) prophylactic and therapeutic vaccine candidate for cervical cancer.
METHODSHPV16 major capsid protein L1 gene/minor capsid protein L2 gene and HPV16 early E6/E7 genes were inserted into a vaccinia virus expression vector. A strain of non-recombinant vaccinia virus containing the sequences was obtained through a homologous recombination and identified.
RESULTSDNA hybridization confirmed that the HPV16L1/L2/E6/E7 genes were integrated into vaccinia virus DNA. Western Blot result showed that full-length L1/L2/E6/E7 proteins were co-expressed in CEF cells infected with the recombinant virus.
CONCLUSIONNTVJE6E7CKL1L2 could be taken as a candidate of prophylactic and therapeutic vaccine for HPV-associated tumors and their precancerous transformations.

Based on the"You Gu Wu Yun"theory in traditional Chinese medicine(TCM),this paper believes that"Gu"in"You Gu Wu Yun"is extended to"state"from the perspective of"TCM state".In order to avoid the adverse reactions of TCM,the macro,meso,and micro three views should be used together,and macro,meso,and micro parameters should be integrated.We should also carefully identify the physiological characteristics,pathological characteristics,constitution,syndrome,and disease of human body by combining qualitative and quantitative method,highlighting the relationship between the prescription and the"state".The correspondence between prescription and the"state"will reduce the risk of adverse reactions of TCM.In this paper,we hope to focus on the guiding role of the"You Gu Wu Yun"theory in TCM research,to give full play to the characteristics and advantages of TCM,and to dialectically treat the role of TCM.
