Objective The severe hypospadias is present with complicated deformity,and the successful rate of treatment is low with high complication rate.Here is to present a kind of useful and reliable treatment for it.Methods From 2004 to 2005,the scrotal septal island skin flap combined with the scrotal split thickness skin graft to form the neourethra in a stage was used to treat the hypospadias without underdeveloped scrotum.Results A total of 15 patients including proximal penile,penioscrotum and scrotal hypospadias who were followed up for about 1 year healed well without the occurrence of urethral stricture and urethral fistula,the penile curvature was corrected and the urine line was direct,and the endoscopic examination showed the inner wall of the urethra was smooth and intact.Conclusions The application of the scrotum septum flap combined with the scrotal splitted thickness skin graft to form the neourethra in one stage is reliable to treat the severe hypospadias with the simple procedure.The combined technique has obtained both the advantages of the skin graft and the skin flap to form the neourethra,and it could be used to correct the curvature of the penile.Thus the satisfactory result of the appearance and the function could be achieved.The technique is an excellent procedure.

Objective To discuss the value of essential hypertension arteriosclerosis as a predictor of early renal damage.Methods 90 cases of essential hypertension with early renal damage were selected and statistical analysis to result of AASI,Scr,ACR and eGFR with SPSS17.0、Excel statistical analysis software were carried out.Group A was defined as AASI≥0.55 and group B was defined as AASI＜0.55.Compared the two groups with relevant analysis,and then built multivariate linear regression equation.Results ① The group A showed higher age(P＜0.01),24 h DBP(P＜0.05),ACR(P＜0.01),ACR(+) rate(P＜0.01)and lower eGFR (P＜ 0.01),compared with the group B.②AASI showed a positive correlation with age (r＝ 0.483,P＝ 0.000),24 h SBP (r＝ 0.350,P ＝0.001),ACR (r＝0.474,P＝ 0.000),and a negative correlation with eGFR (r＝-0.383,P＝ 0.000).Two multiple linear regressions showed that AASI was associate with eGFR (B ＝-0.001,P＝ 0.038) and ACR(B ＝ 0.025,P ＝ 0.000) independent of other factors.Conclusion ①Arteriosclerosis is closely related with age and blood pressure mutation rate day and night.②Arteriosclerosis has a closely relationship with essential hypertension early renal damage,and can predict early renal damage in hypertension patients.

Objective To establish a mathematical model for describing and predicting the effectiveness of physical therapy on lumbar disc hernation(LDH).Methods Daily comprehensive physical therapy interventions including lumbar traction,mobilization and physical agents were administered to 31 LDH patients for 10 sessions.A visual analogue scale was used to measure the pain intensity before each session.The logistic curve model was used in the regression analysis.The first 2 to 9 pain measurements were involved in different predictive models,and the predictive results were compared among different models.Results In regression analysis,the global R2 was 0.719.In predicting analysis,the more measurements involved in the predictive model,the more effective the prediction.The predictive power was low when the model included less than 4 measurements.the prediction power was high when more than 5 measurements were used in the model.Conclusion The logistic model can describe and predict the effectiveness of multiple physical therapy on LDH patients.

Objective To investigate the analysis of Symbicort in stable stage of chronic obstructive pulmonary disease treatment and intervention. Methods 80 patients with chronic obstructive pulmonary disease in stable period from June 2014-2016 year in May in our hospital were randomly divided into control group and experimental group, 40 cases in each group. The control group of patients with the use of simple Symbicort treatment and routine nursing care to patients, patients in the experimental group using Symbicort combined with Bailing Capsule for treatment, and the implementation of targeted nursing intervention measures, compare the effect of two groups of patients and nursing satisfaction. Results the clinical treatment of patients in the experimental group the total efficiency 95.0%, nursing satisfaction was 97.5%, the control of clinical patients in the treatment group the total effective rate was 80.0%, nursing satisfaction was 87.5%, the experimental group was significantly higher than the control group, the difference was statistically significant (P<0.05). Objective To estimate the efficacy of super-early minimal invasive evacuation of hematoma via keyhole for treating hypertensive basal ganglionic hemorrhage. Methods 36 patients with hypertensive basal ganglionic hemorrhage(9 cases in grade Ⅱ, 17 in grade Ⅲ and 10 in grade Ⅳ) were treated by minimal invasive evacuation of hematoma via keyhole at super-early stage, and their efficacy was statistically compared with 35 patients with hypertensive cerebral hemorrhage underwent conservative therapy or lateral ventriculopuncture. Results In the 36 patients underwent minimal invasive evacuation of hematoma via keyhole, 22 cases had a good prognosis(61.1%), 9 had a poor prognosis(25.0%), and 5 died(13.9%). Among the 35 patients underwent conservative therapy or lateral ventriculopuncture, 5 cases had a good prognosis(14.3%), 22 had a poor prognosis(62.9%), and 8 died(22.9%). In order to investigate the dynamic changes in nitric oxide synthase (NOS) activity in organs during traumatic shock and to evaluate the effects of inhibitor of NOS and L Arginine (L Arg) on traumatic shock, rat model of traumatic shock was established, and the constitutive NOS and inducible NOS in the heart, small intestine, liver, lung , spleen were measured 0 5h, 3h, 5h after traumatic shock. Followed by intravenous injection with aminoguanidine (AG ), N G nitro L arginine methyl ester (L NAME), or L Arg, NOS were again measured, and the survival time and 24h survival rate were recorded. It was found that in normal rats cNOS was expressed in all organs, but only a weak iNOS expression in lung and liver. Half an hour after shock, cNOS was elevated in almost all organs in various degrees but there was no change in iNOS. 3h after shock, an increase in iNOS activity and a decrease in cNOS were observed. The iNOS was synthesized in large quantity 5h after shock. AG and L Arg markedly prolonged the survival time but L NAME did not in shock rats. AG inhibited the activity of iNOS but promoted the synthesis of cNOS, and both NOS were inhibited in L NAME group, but not in L Arg group. The results that iNOS was synthesized in large quantity only at later period of traumatic shock. To treat shock, the inhibitors of NOS and L Arg should be given according to the degree of shock, the dosage of drugs, and the time of administration
