3.Evaluation of the effectiveness of exercise education for diabetic patients in the form of games

目的探讨以运动会形式进行糖尿病患者运动教育的效果。方法选择100例糖尿病患者,在饮食控制及降糖药或胰岛素治疗基础上举办一次运动会形式的运动教育,其中54人参加低、中、高强度项目比赛,监测其运动前后的血糖。对所有到场者进行现场教育及运动治疗知识问卷测试,了解患者运动治疗认知度的提高程度。结果运动教育前后平均答卷积分分别为4.23和8.26,两者有显著性差异(P<0.05)。结论通过运动会形式进行现场教育能提高患者对运动治疗的认知度,从而提高患者自觉进行餐后运动的意识。
4.The Development and Assessment of the Quality of Life Instrument for Hepatitis B Patients

Objective: To develop a quality of life instrument for hepatitis B patients(QOL-HBV) and examine it's reliability and validity.Methods:The item analysis, construct validity, content validity, criteria validity, reliability and discriminate were assessed by a survey on 134 HBV patients with QOL-HBV.Results:Item analysis showed that QOL-HBV had satisfactory item discrimination. Eight common factors were extracted by exploratory factor analysis and they can be reduced to three domains. The ?2/DF was 1.310, GFI was 0.972 and AGFI was 0.967 by the confirmatory factor analysis. The coefficients of Cronbach's ? of the instrument and all domains were above 0.80. In comparison with SF-36, there was good correlation among corresponding domains.Conclusion:QOL-HBV is a good scale in testing the quality of life, which has good reliability and validity.

6.Inverse "Y" incision technique of enlargement annulus for double valve replacement in small aortic root: seven cases report

Objective To report the surgical procedure and its results of enlargement annulus for double valve replacement in small aortic root. Methods There were 7 patients with small aortic root undergoing double valve replacement. Mean age of 42 years (from 34 to 52 years). The diameters of aortic annulus ranged from 17 to 20?mm. The aortotomy was extended through the commissure that separates the left and noncoronary cusps of the aortic valve. The aortic annulus was divided, and the incision was extended into anterior leaflet of the mitral valve as inverse "Y". The anterior leaflet of the mitral valve was excised 3~5?mm to its root. A prosthetic patch was attached to the aortic-mitral valve so the aortic annulus was enlarged. The prosthetic valve was implanted. Moreover, it was attached to the prosthetic patch, at the root of the anterior leaflet of the mitral valve. Results The diameters of aortic annulus after operation were enlarged to 4~7?mm. The prosthetic valve (21~23?mm) was implanted in aortic valve position and the prosthetic valve (25~27?mm) in mitral valve position. There were no operative and postoperative deaths. One patient was re-operated for bleeding. All patients were followed up from 1 to 26 months and their cardiac function was significantly improved. Conclusion This surgical procedure is safe and effective for replacement, for double valve replacement in small aortic root.
7.Evaluation of teaching reform based animation in medical statistics

Objective To study the effect of animation in the course of medical statistics.Methods By convenience cluster sampling,271 undergraduates in two classes were recruited from the specialty of medical image,medical anesthesia and medical laboratory.The experimental group (n =139) and control group (n =132) were set up randomly.The animation design is only used in the teaching process among the experimental group,while traditional teaching method without animation was used in the control group.All participants were surveyed by questionnaire for the effectiveness after one week of the curriculum closure.SPSS 15.0 software was used to do data entry and statistical analysis and two-sample t-test was used to compare the differences in text scores between two groups.Evaluation was made on the courseware of animation in experimental group.P < 0.05 signifies that the differences have statistical significances.Results The test scores in experimental group (79.62 ± 9.34)were significantly higher than those in control group (77.10 ± 9.18; P < 0.05).More than 85 % of the experimental students think animation can stimulate their enthusiasm in studying medical statistics; and help them better understand the key points and main difficulties of the statistical knowledge.Conclusion It suggests that animation can enable students to study better the course of medical statistics.
8.Role of TPMT Genetic Polymorphism in the Individualized Treatment of Thiopurine Drug

Thiopurine S-methyltransferase (TPMT) is an important and key cytoplasmic enzyme in the metabolism of thiopurine drugs,whose activity can directly determine the amount of thiopurine drugs metabolized to cytotoxic 6-thioguanine nucleotides and consequently influence clinical efficacy and adverse drug reactions of thiopurine drugs.In order to deepen knowledge and role of genetic polymorphism of tpmt in the individualized thiopurine drug treatment,this present review mainly covered the following three frequently concerned aspects,including i) whether or not to determine the activity of TPMT priot to treatment of thiopurine drugs;ii) to genotype or to phenotype;iii) how to choose genotype methods.
9.The difference of T lymphocyte subsets between mild type and severe type in patients infected with the 2009 pandemic influenza A (H1N1) virus and its clinical significance

Objective To study the changes of subgroups of peripheral blood T lymphocytes in the patients infected with the 2009 pandemic influenza A ( H1N1 ) virus of different severity type. Method A total of 66 patients infected by H1N1 evidenced by RT-PCR admitted from September 2009 to January 2010 were divided into three groups: mild type ( B group, n = 47 ), cured patients of severe and critical severe type ( C group, n = 14) and died patients ( D group, n =5), according to the severity and prognosis. A total of 20 healthy volunteers served as control group( A group). Peripheral blood lymphocyte count, CD3+,CD4+ and CD8+ T lymphocyte count were detected by flow cytometry at the different time points. Fever duration and H1N1 virus negative time were compared. Statistical analysis were performed by using SAS version 9.13 software and the data were processed with ANOVA and SNK test. Results Lymphocyte count, CD3+,CD4+ and CD8+ T lymphocyte count declined in the early period in all the groups, and there were significant differences compared with A group (P<0. 05), while rised with the clinical progression in group B and C,and those of C group were lower than B group ( P < 0.05 ), but those of D group were always low. Fever duration and H1N1 virus negative time were (4.4 ± 1.6) days vs. (4.4 ± 1. 4) days, ( 12.9 ± 3. 1 ) days vs.( 10.2 ± 2.6) days and ( 15.2 ± 7.3 ) days vs. ( 13.3 ± 2.9 ) days respectively, and there were significant differences among the three groups ( P < 0.05 ). Conclusions The cellular immune function was seriously damaged when patients were infected with H1N1. Further more, the changes of lymphocyte count, CD3+ , CD4+and CD8+ T lymphocyte count were tightly related with the degree of severity and prognosis. These findings can be used for clinical diagnosis and treatment.
10.Treatment of the advanced stage avascular necrosis of femoral head combined with severe femoral anteversion with total hip arthroplasty

Objective To explore the operative method and clinical effect of total hip arthroplasty with normal prosthesis in treatment of the advanced stage avascular necrosis of femoral head combined with severe femoral anteversion. Methods There were 15 patients ( 15 hips) including nine males and six females, at age range of 30-42 years (mean 37 years). The femoral anteversion was 40°-50° and Harris score of (59 ± 8) points. Total hip arthroplasty with normal prosthesis was performed to reduce the femoral anteversion for 20°-30° and increase the acetabulum anteversion for 10°-15°so as to recover a good involution relationship of the femoral head and the acetabulum and avoid anterior dislocation. The imaging examination and Harris scoring were performed regularly postoperatively. Results A follow-up for mean 2.9 years (2.5-3.7 years) in 15 patients showed that all patients obtained good range of joint motion and good stability of all the hip prostheses. Harris score was (88±6) points at 2 years post-opera-tively, which was significantly better than preoperation (P < 0.01 ). Conclusions Total hip arthro-plasty with normal prosthesis can obtain good involution of the femoral head and the acetabulum in patients with advanced stage avascular necrosis of femoral head combined with severe femoral anteversion by simul-taneously regulating implant angle of femoral prosthesis and acetabular cup, which helps avoid use of small or specially made femoral stem or subtrochanteric derotational osteotomy.
