2.Study on the Quality Standard for Gardenia jasminoides Commercially Available

OBJECTIVE:To establish the quality standard for Gardenia jasminoides. METHODS:The moisture,total ash and extract were determined. HPLC was used for contents determination of geniposide,rutin,crocinⅠand crocinⅡ:the column was Waters Xbridge-C18 with mobile phase of acetonitrile-0.2% phosphoric acid(gradient elution,gardenia and rutin),methanol-water (45∶55,V/V,crocinⅠ,crocinⅡ)at a flow rate of 1.0 ml/min;detection wavelength was 256 nm for gardenia and rutin,440 nm for crocinⅠ,crocinⅡ;column temperature was 30℃;injection volume was 10μl for gardenia and rutin,5μl for crocinⅠ,cro-cinⅡ. RESULTS:The moisture total ash and ethanol-soluble extract of G. jasminoides were 5.8%-8.4%,3.7%-5.9% and 29.5%-37.9%,respectively. The linear range was 162.08-1 620.84 μg/ml for geniposide(r=0.999 9),2.07-20.72 μg/ml for rutin (r=0.999 9),8.04-80.41 μg/ml for crocinⅠ(r=0.999 9)and 1.05-10.53 μg/ml for crocinⅡ(r=0.999 9);RSDs of precision,sta-bility and reproducibility test were lower than 2%;recoveries were 99.33%-101.43%(RSD=1.09%,n=6),97.97%-101.83%(RSD=1.39%,n=6),97.97%-101.30%(RSD=1.36%,n=6) and 98.65%-103.04%(RSD=1.84%,n=6). CONCLUSIONS:The established standard can be used for the quality control of G. jasminoides commercially available.
3.Effects of treadmill training after cerebral ischemia on expression in the brain of axonal guidance factor Netrin-4 and its receptor protein

Objective To observe the effects of treadmill training on the expression of axonal guidance factor Netrin-4 and its receptor deleted in colorectal cancer (DCC) protein in the brains of rats with cerebral ischemia.Also to explore how training promotes the recovery of neurological function after cerebral ischemia.Methods Sixty-three adult, male Sprague-Dawley rats were randomly divided into a sham group (n =9) , a model group (n =27) and an exercise group (n =27).A modified version of Longa's method was used to establish an animal model of cerebral ischemia by occluding the right middle cerebral artery (MCAO) in the rats of the model and exercise groups while the rats of the sham group were subjected to the same surgical procedure except that no thread was inserted.The rats of the exercise group were given treadmill training beginning 24 h after the modeling, while those of the other two groups were left on the treadmill without training.The rats' neurological functioning was tested by assigning modified neurological severity scores (mNSS) on the 3rd, 7th and 14th day after modeling.Then the ischemic brain tissue was dissected to detect the expression of Netrin-4 and DCC protein using western blotting and immunofluorescent staining and analysis.Results On the 3rd, 7th and 14th day after modeling, the model and exercise groups had significant differences in their average mNSS scores when compared to the sham group.The average mNSS scores of the exercise group on the 7th and the 14th day were significantly lower than those of the model group at the same time point.The average expression level of Netrin-4 and DCC protein in the exercise group was significantly higher in than the model group on the 7th and 14 th days.Immunofluorescent staining showed that Netrin-4 was mainly observed in the blood vessels and astrocytes in the ischemic brain area.DCC protein was found mainly in neural processes and astrocytes in the same area.Conclusions Treadmill training can improve the recovery of neurological function after cerebral ischemia.Its mechanisms may be partly attributed to up-regulating the expression of Netrin-4 and DCC protein in the ischemic brain area, which strengthens the regeneration and reconstruction of nerves and blood vessels.
4.Influence of pivot distance on the performance of a mechanic heart valve.

Based on the analysis of the influence of the valve pivot distance on the performance of mechanical heart valve (MHV), such as the valve opening and closing features, flow field characteristics and the valve assembly properties, value constraints of the valve pivot distance were established, and the reasonable valve was obtained by means of the finite element method. It can be shown that the central flow characteristics of the valve could be enhanced with the increasing of the ratio of pivot distance to valve inner diameter, but the plastic deformation of the ring could be liable to occur in the MHV assembly process. It is proved that the valve of specifications can be designed in similar ratio of pivot distance to valve inner diameter according to the result of the valve performance experiment.

5.Effect of mild hypothermia on cerebral oxygen metabolism in a rat model of chronic cerebral hypoperfusion and reperfusion

Objective To investigate the effect of mild hypothermia on cerebral oxygen metabolism in a rat model of chronic cerebral hypoperfusion and reperfusion. Methods Twelve healthy SD rats weighing 170-210 g were randomly divided into normal body temperature group (group NT, n = 6) and mild hypothermia group (group MH, n = 6). Arterio-venous fistula was established by end-to-end anastomosis between the right common carotid artery and right external jugular vein according to Yassari. Mild hypothermia was induced in group MH before reperfusion and maintained for 3 h. The rectal temperature was reduced to 32 ℃ while in group NT rectal temperature was maintained at about 37 ℃ . Cerebral blood perfusion (CBP) was assessed by using a lasser Doppler perfusion image system before reperfusion (T_1), immediately after reperfusion (T_2) and 24 h after reperfusion (T_3). Venous and arterial blood samples were collected from superior sagittal sinus and femoral artery respectively for blood gas analysis at T_(1-3) . Cerebral arterial venous O_2 saturation difference (Sa-vO_2), cerebral O_2 extraction rate (CERO_2) and cerebral arterial-venous lactic acid concentration difference ( Da-vL) were calculated. Results In NT group left CBP was significantly decreased at T_2 as compared with the baseline value at T_1 , while at T_3 bilateral CBP was decreased. In MH group bilateral CBP was significantly decreased at T_2 but returned to baseline level at T_3. CERO_2 was significantly decreased at T_2 as compared with the baseline value at T_1 in MH group. Da-vL was significantly increased at T_3 in NT group. Compared with group NT, bilateral CBP was significantly decreased, CERO_ and Da-vL were significantly decreased at T_2 ,while no significant change was found in Sa-vO_2 in group MH. Conclusion Mild hypothermia is beneficial for the balance of cerebral oxygen metabolism in the rats with chronic cerebral hypoperfusion and reperfusion.
6.Capability to Absorb High Concentration Benzene and Formaldehyde of Four Indoor Potted Plants

Objective To study the capability of absorbing benzene and formaldehyde of four indoor potted plants.Methods To place four indoor potted plants,Sansevieria trifasciata‘Laurentii',Syngonium podophyllum‘Gold Allusion',Scindarpsus aureus,Chlorophytum comosum‘Vittatum' in a sealed chamber filled with benzene at the initial concentration of 1.11-2.32 mg/m~3 and formaldehyde at the initial concentration of 17.0-34.7 mg/m~3.The concentration change of benzene was determined every two hours and for formaldehyde every six hours.Results As for absorbing benzene,four plants ranked as follows,Syngonium podophyllum‘Gold A llusion',Sansevieria trifasciata‘Laurentii',Scindapsus aureus,Chlorophytum comosum‘V ittatum'.As for absorbing formaldehyde,four plants ranked as follows,Chlorophytum comosum‘Vittatum',Sansevieria trifasciata‘Laurentii', Scindapsus aureus,Syngonium podophyllum‘Gold Allusion'.Conclusion The investigated plants can present the selective absorbing ability to the two gases,for example,Chlorophytum comosum‘Vittatum' can absorb formaldehyde obviously,but can't absorb benzene.The character of plant absorbing deleterious gas is that the absorbing capability will gradually reduce,but it can be resumed and repetitively used.
7.Correlation between Drug Quality Control Standard and Drug Quality Evaluation

OBJECTIVE:To emphasis the importance to unceasingly upgrading of drug quality standard for drug quality and to arouse the attention of the related enterprises, medical institutions and governmental department to this matter. METHODS: The correlation between quality standard and drug quality was expounded systematically by citing examples and reviewing the pertinent literature, and the current status of drug quality standard in China was analyzed briefly. RESULT & CONCLUSION: Great importance should be attached to the evaluation of drug quality standard to improve drug quality standard level step by step and to ensure efficacy and safety of drugs.
8.Perinatal outcomes of hypoglycemic women during 75 g oral glucose tolerance test

Objective To analyze the perinatal outcomes of those women experienced hypoglycemia during 75 g oral glucose tolerance test(OGTT).Methods A retrospective study was conducted in 774.pregnant women who underwent OGTT and delivered in our institution from January 2005 to January 2006.The distribution of glucose levels of the fasting,1 h,2 h and 3 h of the OGTT was analyzed.Plasma glucose level below the 5th percentile and above the 1h percentile was defined as hypoglycemia and those below the 1th percentile as exceeding hypoglycemia.Perinatal outcomes including birth weight,birth height,obstetric complications,the incidence of small for gestational age (SGA)were compared between women with hypoglycemia or exceeding hypoglycemia. Results The birth weight in exceeding hypoglycemia group of fasting glucose was significantly lower than that of the others [(2817±764)g vs(3439±487)g,t=3.344,P<0.01]and the incidence of SGA in this group was also higher(3/7 vs 3.4%,x2=22.78,P<0.01);the proportion of babies with birth weight<3000 g in the same group was higher than the others(5/7 vs 14.73%,P<0.01).The newhorns height was significantly shorter in the hypoglycemia group at 2 h than the normal glycemic group [(49.03±1.68)cm vs(49.48±1.65)cm,P<0.05].Conclusions The plasma glucose 1evel is associated with the newborns'birth weight and height.Women with hypoglycemia during OGTT are more likely to have babies with lower birth weight and height.
9.Etiological research and medication rationality analysis of acute pneumonia in 0~1 years old

Objective To analyze the etiological results and clinical characteristics in infants with acute pneumonia at the age of 0 ~ 1 years and to evaluate the rationality of the use of antibiotics.Methods Thirty-four infants hospitalized during Dec 2010 to Nov 2012 were selected.The throat swabs were sent to the Beijing Disease Prevention and Control Center for rapid nucleic acid test for influenza; the sputum was sent to the bacteriological laboratory of the hospital for the bacterial culture.Children drug utilization index (cDUI)was used to assess the rationality of the medication amount of antibiotic drugs.For defined daily dose statistic (DDDs),the larger the value of DDDs,the more frequent the clinical use of the drug.Results In the confirmed cases of acute pneumonia,21 cases were of viral infection,9cases were of bacterial infection and 4 cases were of mixed infection.The probability of bacterial pneumonia (4/9) was higher than that of viral pneumonia(5/21) in the case with underlying disease (P <0.001) ;the virus detection rate(12/21) of children with contact history with respiratory tract infection patients was higher than bacteria detection rate(4/9)(P =0.01).In case of no risk of cross infection,the infants were more susceptible to bacterial infection (P =0.006).The sputum culture of 25 cases of positive virus detection showed 13 cases of colonization of conditional pathogenic bacteria and 4 cases of pathogenic bacteria.The ratio of combination use of antibiotic drugs in case of bacterial infection (4/9) was higher than that in mixed infection (1/4) (P =0.001).The durations of the use of antibiotic drugs for every group showed no statistical difference (P =0.29).This research involved 7 antibiotic drugs.The medication frequencies and durations of ceftriaxone (DDDs =1813.45,total medication duration =187 days) and cefuroxirne (DDDs =1513.73,total medication duration =162 days) were higher than those of the other 5 antibiotic drugs.The cDUI of all drugs ≤ 1.0.The decreasing order of drugs by cDUI was amoxicillin and clavulanate potassium (cDUI =1.0),cefuroxime (cDUI=0.89),ceftriaxone (cDUI =0.87),azithromycin oral agent and cefepime (cDUI =0.80),midecamycin oral agent (cDUI =0.75) and penicillin injections (cDUI =0.50).Conclusion Virus is the main pathogen of the acute pneumonia in children in this research.Virus provides preliminary conditions for bacterial colonization.The overuse of ceftriaxone aggravates the pathogenic risks of conditional pathogenic virus.Some children are treated by antibiotic drugs for a long time or by the combination use of drugs,which are irrational mediation.
10.Percutaneous nephrolithotomy combined with endoscopic balloon dilation in the treatment of upper ureterostenosis with recurrent renal calculi

Objective To analyze the technique and clinical effect of percutaneous nephrolithotomy combined with endoscopic balloon dilation in the treatment of upper ureterostenosis with recurrent renal calculi.Methods From February 2009 to May 2012,20 ureteral stenosis patients with the history of ureteral open surgery,postoperative residual or recurrent kidney renal calculi were treated.Fifteen cases were with hydronephrosis of 2-4 cm,4 cases were with hydronephrosis of 5-6 cm and 1 case was with hydronephrosis > 6 cm.Subsequent calculi size < 1 cm was found in 16 cases,1-3 cm in 3 cases,> 3 cm in 1 case.All patients were treated with percutaneous nephrolithotomy ultrasonic lithotripsy combined with balloon dilatation.The calculi clearance rate,hydronephrosis changes,complications,intraverous urography (IVP) and CT urography (CTU) situation before and after surgery were analyzed.Results All the 20 cases were completed surgery successfully.There was 1 case with renal hemorrhage 3 d after surgery and controlled with DSA hemostasis.There was 1 case accepted adjusting double-J tube by ureteroscopy.Seventeen patients'calculi were completed removed.Two cases with residual calyceal calculi size < 5 mm was not further treated.There was 1 case treated with nephrectomy because of renal calculi with infection.The patients were followed up for 6-39 (12.5 ± 1.9) months.Hydronephrosis improved significant changes [2.1-4.0 (2.2 ±0.9) cm vs.2.5-7.5 (3.9 ± 1.2) cm,P < 0.05].All patients achieved significant improvement in IVP and CTU comparing of preoperative and postoperative data.Conclusion The use of percutaneous nephrolithotomy combined with endoscopic balloon dilation is a safe and effective treatment option in the treatment of kidney calculi with ureteral stenosis.
