1.Coronary endothelial injury and jeopardy score of coronary artery lesion by angiography in angina patients

Objective To investigate the level of coronary endothelial injury and dysfunction, the jeopardy score of coronary artery lesion by angiography, and their correlations in patients with stable angina (SA) or unstable angina (UA). Methods The 20 patients with UA, 17 patients with SA and 18 patients excluding coronary heart disease (control) were studied. Blood samples were collected during percutaneous coronary intervention (PCI). To investigate the coronary endothelial injury and dysfunction, we measured the concentrations of nitric oxide (NO)? endothelin (ET) and the number of circulating endothelial cells (CEC) in coronary sinus blood. The jeopardy score for coronary artery lesion was evaluated according to the method by Califf. Results In angina patients, especially in UA patients, NO concentration in coronary sinus blood was lower, while ET concentration and CEC number were markedly higher than those in control group. The jeopardy scores showed no difference between UA and SA. The coronary endothelial injury and dysfunction showed a positive correlation with jeopardy scores. Conclusion The coronary endothelial injury and dysfunction is not only the initiation factor, but may play a key role in the progression of atherosclerosis and coronary heart disease. Coronary angiography combined with detection of coronary endothelial injury and dysfunction may better describe the lesion of coronary artery, and evaluate the prognosis of patients with coronary heart disease.

Objective To summarize our experience with combined cardiac pacing and beta-blocker therapy in patients with long-QT syndrome. Methods Nine patients with long-QT syndrome were treated with Propranolol. Its effectiveness was analyzed during matched periods before and after starting beta-blocker therapy. If sinus bradycardia, sinus arrest and atrioventricular occurred during the Propranolol treatment, DDD pacemaker were implanted. The ongoing health reform is bringing reforms of public hospitals under spotlight in China, attracting growing attention onto its progress and roadblocks. Probing into the "three steps" of public hospital system reform in Wuxi, this article analyzed such a reform in the city. It described the measures and initial outcomes gained in such fields as the hospital trusteeship reform, the separation of hospital regulation from its administration, and the hospital board system. Citing the case of Wuxi No. 2 People's hospital as an example, the authors introduced the innovative efforts made in hospital internal mechanism, management innovation, medical service model, hospital connotation construction, and talents cultivation.
5.Pattern recognition of the fingerprint of Polygonum orientale in Guizhou Province

AIM: To study pattern recognition of the fingerprint of Polygonum orientale from Guizhou Province. METHODS: 20 batches of samples,Polygonum orientale,come from different producing areas and harvest time were carried out to gain fingerprints by HPLC-DAD,as compared with retention time and ultraviolet spectra of standard substances.The pattern recognition of the characteristic fingerprint of Polygonum orientale,known as cluster analysis and principal component analysis,formed. RESULTS: The fingerprint of 20 batches of Polygonum orientale showed 12 characteristic peaks,in which 9 common peaks were confirmed.the samples were grouped into 3 types of harvest time. CONCLUSION: The quality of Polygonum orientale in Guizhou Province is stable,the pattern recognition of the fingerprint provides the experimental basis for manufacture and quality control of Polygonum orientale.

Aim To investigate changes of eosinophil(EOS) and mast cells(MC) in airway of young asthmatic rats and to evaluate of achyranthes bidentata polysaccharides(ABPS) on it.Methods Fifty Sprague-Dawley(SD) rats were randomly divided into five groups,ten rats per group:asthmatic group(A),control group(C),pretreatment groups with ABPS which was done according to three different schedules:consecutively 3 days at sensitization(T_1),at challenge(T_2) or both of the two periods(T_3).Asthmatic rats were induced by intraperitioneal sensitization and challenged with nebulized ovalbumin(OVA).Lungs were embeded in paraffine and sliced,Hematoxylin and eosin(HE) staind tobuidine blue restained and TUNEL method after animals were executed,then the quantity and degranulation phenomenon of EOS and MC and the apoptosis of EOS in airway were observed.Results ① The number of inflammatory cells and EOS in the airway was significantly increased in asthmatic group but reduced in group T_1 and T_3(P
7.Titanium wiring for fixation of distal clavicle fractures and acromioclavicular joint dislocation in 37 cases

AIM: To observe the curative effect of titanium wiring in treatment of the fractures of Neer Ⅱ type distal clavicle and dislocation of Tossy Ⅲ type acromioclavicular joint. METHODS: From March 2004 to December 2006, 37 cases with Neer Ⅱ type distal clavicle fractures or Tossy Ⅲ type acromioclavicular joint dislocation were treated with titanium wiring in Changzheng Hospital, Second Military Medical University of Chinese PLA. Titanium wiring (produced by Sofamor-Danek Company in USA) had good histocompatibility, intensity of tension and flexibility. The broken coracoclavicular ligament was repaired directly by titanium wiring in all cases. The outcomes were evaluated according to Herscovici's criteria. RESULTS: All the cases were followed up for 6 to 24 months. All the dislocated acromioclavicular joints returned to normal 2 weeks after operation. Excellent and good rate of shoulder function was 100% according to Herscovici standard. X-ray imaging showed all clavicle fractures united, acromioclavicular joint had no redislocation or titanium wiring breaking. The incisions of all cases had no obvious swelling and exudation. CONCLUSION: When Neer Ⅱ type distal clavicular fracture and Tossy Ⅲ type acromioclavicular joint dislocation are treated with titanium wiring, the acromioclavicular joint can exercise earlier and recover quickly.
8.The expression and clinical significance of cyclase-associated protein 2 in hepatocellular carcinoma

Objective To explore the cyclase-associated protein 2 (CAP2) expression in hepatocellular carcinoma (HCC).Method The expression of CAP2 was determined by reverse transcription PCR,Western blot,and immunohistochemistry in normal liver,cirrhotic liver,and HCC.Results According to reverse transcription PCR and Western Blot,the level of CAP2 was up-regulated in HCC and was further up-regulated in progressed HCC than in early HCC.CAP2 expression was almost absent in normal and cirrhotic liver.The same results were seen in immunohistochemical examination.Also,CAP2 showed stronger expression in medium and poor-differentiated HCC than in well-differentiated HCC.Conclusion CAP2 is an up-regulated gene in HCC and may relate to the stages of hepatocarcinogenesis.Therefore,CAP2 could be used as a valuable molecular marker for HCC diagnosis in the future.
9.Clinical analysis of lateralorbital keyhole approach for microsurgical treatment of ruptured tiny aneurysms

Objective To investigate the clinical application and surgical techniques of lateral-orbital keyhole approach to clip ruptured tiny aneurysms.Methods A retrospective analysis was performed on the clinical data of 56 patients with ruptured tiny aneurysms, including examination methods, imaging features, diagnosis and treatment process, and operation effect, which underwent lateral-orbital keyhole approach from January 2013 to April 2014.Results Among 56 patients with a total of 61 aneurysms(56 tiny aneurysms) was examined by CT angiography (CTA) and/or digital stubtraction angiography (DSA), all achieved successful clipping with a rupture rate of 16%(13 cases).Forty-two cases got a good recovery,8 cases owned the self-care ability, and 6 cases could carry out daily lives with help.There was no rebleeding and mortality, according to a 2-28 months' follow-up of 49 patients.Conclusion As a surgical approach for Willi'S cycle aneurysm,the lateralorbital approach is simple,rapid,minimally-invasive way.It is available in the clipping of ruptured tiny anterior communicating aneurysms.The option of operative time, perfect microdissection and exquisite intraoperative microinvasive operative skills, will be the key point of clipping aneurysms, preventing and controlling of intraoperative bleeding.
10.Annexin A2 protein expression in liver fibrosis and hepatocellular carcinoma

Objective To investigate the clinicopathological significance of the expression of annexin A2 (ANXA2) in liver fibrosis and hepatocellular carcinoma (HCC).Methods The expression level of ANXA2 in normal liver,liver cirrhosis and HCC were examined by Western blot.The correlation between ANXA2 expression and clinicopathological parameters in liver fibrosis and HCC were analyzed by immunohistochemistry.Results Compared with the normal liver tissue,ANXA2 protein expression level increased significantly in HCC and liver cirrhosis,with the highest expression in HCC (P =0.000).There was significantly positive relationship between ANXA2 protein expression and stages for liver fibrosis (P < 0.01).The expression of ANXA2 protein in HCC was closely associated with HBV infection,differentiation degree and the recurrence (P < 0.05).In some cases,ANXA2-positive cancer cells were often dispersed in the periphery of cancer nodules and were adjacent to stromal cells.Conclusion Overexpression of ANXA2 may be involved in liver fibrosis and play a role in the development of HCC,indicating ANXA2 may serve as a diagnostic biomarker for liver fibrosis and tumor differentiation in HCC.
