1.Application of clinical health education path in patients with PCI of one-day ward

Objective To explore the influences of the path clinical health education on the self-management ability, health-promoting lifestyle, compliance, level of blood lipid in patients with PCI of one-day ward.Methods A total of 93 patients with PCI of one-day ward were selected as subjects and were randomly divided into observation group(n=47, the path of clinical health education) and control group(n=46, routine health education) by means of random number table. And then, we compared the self-care ability, health-promoting lifestyle, medication compliance, and level of blood lipid between two groups before and three months after intervention.Results After intervention, the proportion of low level, medium level, high level of self-care ability was 2.1%, 21.3%, 76.6% in observation group which was better than those(10.9%, 41.3%, 47.8%) in control group. The total score of health-promoting lifestyle in observation group [(157.40±17.86) points] was higher than that [(127.04±11.64) points] in control group. Moreover, the score of medication compliance in observation group [(27.98±3.89) points] was higher than that [(21.00±4.52) points] in control group. Besides, the level of total cholesterol, low density lipoprotein cholesterol, triglyceride in observation group was all lower than those in control. There were statistically significant differences in them between two groups( P<0. 05).Conclusions The path of clinical health education has an important role in improving health-promoting awareness and ability, self-management ability, compliance, level of blood lipid in patients with PCI.
2.Effect of 4-dimensional innovative model basing on JCI standards on health education to elderly patients with CHD

Objective To investigate implementing approach and clinical effect of 4-dimensional innovative model basing on JCI standards on health education to elderly patients with CHD.Methods Following JCI standards, a 4-dimensional model was innovated, regarding 4 aspects of health education for elderly patients with CHD:concept, platform, transmission system and technological application. Totals of 120 elderly patients with CHD were divided randomly into observation group and control group. For the patients in the observation group, health education was conducted according to the 4-dimentional innovative model, while in the control group, routine health education approach for CHD was used, which was to hand out health pamphlets explaining knowledge concerning diet, drug, exercise, follow-up visit, etc. After 5 days of intervention, CHD organism functional status and quality of life of the CHD patients in the two groups were evaluated and compared. Indexes, such as self-management skills, were compared 6 months after discharge. Results In SAQ scoring, there were significant differences between observation group and control group regarding limitation degree of motion, attacking condition of angina or stable condition of angina, scores of satisfaction on treatment and awareness of the disease ( P<0. 05 ) . Conclusions Conducting health education to elderly patients with CHD with the 4-dimensional innovative model basing on JCI standards plays a significant role in promoting guarantee of medical safety, prevention of harmful cardiac incidents, and enhancement of treatment compliance, patients′ self-management abilities and patients′satisfaction.
3.Application of SBAR communication mode in the patients with acute myocardial infarction

Objective To explore the application value of the situation-background-assessment-recommendation(SBAR) communication mode in the patients with acute myocardial infarction (AMI).Methods Ninety-four cases of AMI patients who were hospitalized in the department of cardiology in Wuxi No.2 People's Hospital of Nanjing Medical University were included and were randomly divided into the conventional group and the SBAR group. There were 47 cases in each group. The patients in the conventional group were given the routine care; the patient in the SBAR group was given the SBAR communication mode. The nursing satisfaction and the incidence of adverse events in the 2 groups were compared.Results The nursing satisfaction in the SBAR group was 95.74%, which was significantly higher compared to the conventional group (70.21%,P<0.05). The total incidence of adverse events in the SBAR group was 4.25%; in the conventional group the incidence was 25.53%. The difference between the 2 group was significant (χ2=6.42,P<0.05).Conclusions The SBAR communication model can effectively improve the quality of the nurses' disease observation and the nursing satisfaction and reduce the incidence of clinical adverse events, which was worth clinical promotion.
4.Relationship between psychological distress, intimate relationship and self-disclosure in spouses of breast cancer patients

Objective To understand the situation of psychological distress, self-disclosure and intimate relationship in spouses of patients with breast cancer and analysis their interrelationship. Methods Totally 240 spouses of breast cancer patients from 3 class Ⅲ grade A hospitals in Jiangsu Province were investigated with distress thermometer (DT), distress disclosure index(DDI), short marital adjustment and prediction test as well as general information questionnaire from July 2015 to July 2016. Single factor analysis was performed using rank test, and correlations analysis was performed by Spearman analysis. Results The average score of psychological distress was(5.2±4.7) points, self-disclosure (36.5±32.7) points, intimate relationship (118.2±36.5) points and marital disorder detection rate was 25.9%. The prevalence of psychological distress was 67.4% and there was a significant difference in DT score among the spouses of patients with different education level (Z= 7.519,P<0.05). Spouse psychological distress was negatively correlated with self-disclosure degree and the degree of intimacy(r=-0.673,-0.721;P<0.01). Intimate relationship took a partial effects as mediator between the psychological distress and the degree of self-disclosure. Conclusions The incidence of psychological distress is high in spouses of patients with breast cancer, and the severity is influenced by self-disclosure and intimate relationship. Intimacy takes a partial effects as mediator between the psychological distress and the degree of self-disclosure.
5.Application of concept map teaching method of group-discussing mode in standardized nursing training

Objective To explore the application effect of concept map teaching of group-discussing mode in standardized nursing training. Methods A total of 80 standardized training nurses in Wuxi No.2 People's Hospital from January 2016 to December 2016 were selected by purposive sampling and randomly divided into the intervention group and the control group, with 40 students in each. The control group was taught according to the conventional teaching method, and the intervention group was taught with the concept map teaching of group-discussing mode. The scores of the independent learning ability assessment scale and the scores of theoretical examination were compared before and after the intervention. The scores of the feedback evaluation scale of concept map teaching of group-discussing mode in the intervention group were observed. Results The score of each dimension and the total score of the independent learning ability assessment scale of the intervention group after intervention were higher than those in the control group, and the difference was statistically significant (P<0.05). The score of theoretical examination in the intervention group was higher than that in the control group with statistical difference (P<0.001). The nurses in the intervention group had a better feedback on the concept map teaching of group-discussing mode. Conclusions The concept map teaching of group-discussing mode obtains good application results in clinical nursing teaching and training, and it can improve the nurses' independent learning ability.
6.Application of evidence-based nursing in CHD patients with PCI in selected days

Objective To explore the application of evidence-based nursing in patients with coronary atherosclerosis heart disease (CHD)who received percutaneous coronary intervention (PCI)in selected days.Methods A total of 85 CHD patients undergoing PCI in selected days were randomly divided into observation group (43 cases)and control group (42 cases).Control group was treated with routine nursing while observation group with evidence-based nursing.Re-sults The compliance rates of observation group was 95.35%,evidently higher than the 78.57%in control group (P <0.05).Self-rating anxiety scale (SAS)and self-rating depression scale (SDS)scores decreased markedly in both groups after interventions than intervention before, which were apparently lower in observation group than in control group,and there was significant difference (P <0.05).One month after operation,observation group was obviously higher in seat-tle angina questionnaire (SAQ)than control group (P <0.01).Conclusion Evidence-based nursing can increase the therapeutic compliance,promote the negative emotions and improve the postoperative quality of life of CHD patients with PCI.
7.Three dimensional interesting electrocardiogram teaching to strengthen its comprehensive application ability for young nurses in CCU

Objective To explore the three-dimensional interesting electrocardiogram (3D-IECG) teaching to strengthen ECG comprehensive application ability for young nurses in CCU. Methods A total of 40 young nurses, who took part in rotary internship in 2013, had been chosen and randomly divided into experimental group and control group on average. The nurses of control group received traditional method of ECG training while the nurses of the experimental group underwent 3D-IECG teaching, and the satisfaction of teaching methods and ECG comprehensive ability were compared. Results The examination of comprehensive ability in the experimental group acquired basic ECG theory (83. 02 ± 10. 01) score, operation (92. 38 ± 4. 23) score, ECG documentation ability (89. 18 ± 9. 32) score, current ECG analysis (90. 01 ± 7. 03), nursing plan making (93. 23 ± 5. 61), personnel nursing round of arhythmia (91. 29 ± 8. 78) score higher those of control group (t=2. 13, 2. 41, 2. 03, 1. 95, 2. 32, 1. 83;P<0. 05). The satisfaction of ECG teaching method in young nurses were 85. 0% of experimental group and 50. 0% of control group with statistical significance between two groups (χ2 =4. 103,P<0. 05). Conclusions The application of 3D-IECG teaching can effectively improve young nurses ECG comprehensive application ability and satisfaction of teaching method, and is worthy of clinical application.
8.Reason analysis and nursing strategy of coronarospasm after percutaneous coronary intervention

Objective To explore the reason and nursing strategy of coronarospasm after percutaneous coronary intervention(PCI). Methods A total of 1 064 patients with coronary heart disease, who were given PCI from January 2012 to June 2013 in our department, were randomly divided into groups A, B, C and D, 266 cases for each. All patients were successfully punctured radial artery and given corresponding medicines through tubes in radial artery sheaths. Patients in the group A was given 200 μg nitroglycerin and 1 mg verapamil, patients of the group B with 200 μg nitroglycerin, patients with 1 mg verapamil in the group C, while patients in the group D served as control group and was given 5 - 10 ml normal saline. The incidences of patients with spasms in each group were compared. Results Radial artery spasm were 0. 75% in the group A, 1. 5% in the group B, 4. 4% in the group C, and 5. 64% in the control group, and the result of control group had statistical significance with that of group A and B ( χ2 = 6. 386 8,6. 604 3; P < 0. 05). A total of 32 cases with coronarospasm after PCI all had been rescued. Conclusions Application of medicines for preventing radial and coronary artery spasms and careful nursing can effectively decrease the rate of artery spasms.

10.Motivational interview combined with transtheoretical model in self-care of patients with coronary heart disease

Objective To explore the application effect of motivational interview combined with transtheoretical model in self-care of patients with coronary heart disease. Methods From August 2016 to April 2017, a total of 77 patients with coronary heart disease diagnosed in Wuxi Second Hospital Affiliated to Nanjing Medical University were selected by convenience sampling method, and randomly divided into control group (n=38) and observation group (n=39) by random number table. The two groups were given traditional health education and motivational interview combined with transtheoretical model health education. The self-efficacy, self-management ability, ejection fraction and regional wall motion abnormality (RWMA) were compared between the two groups before and after intervention. Results The scores of self-efficacy and self-management ability of patients with coronary heart disease in the observation group were (39.12±5.98) and (88.88±5.01) respectively, which were significantly higher than those in the control group (31.04±6.59) and (81.00±4.25). The differences were statistically significant (P<0.01). The ejection fraction of the observation group was higher than that of the control group, and the number of RWMA cases in the control group was higher than that in the observation group. The difference was statistically significant (P<0.05). Conclusions Using motivated interviews combined with cross-theoretical models in self-care of patients with coronary heart disease can effectively improve patients' self-efficacy and self-management ability,it is worthy of clinical promotion.
