2.Screening Analysis of Neonatal Congenital Hypothyroidism and Phenylketonuria in Lianyungang Area

Objective To explore the incidence and distribution features of neonatal phenylketonuria(PKU) and congenital hypothyroidism(CH) in Lianyungang area.Methods The heel blood samples dried on filter paper was collected from newborns after 72 hours.As the screening indicators,PKU was detected through serum phenylalanine concentration(Phe) detection by Guthrie′s bacterial inhibition assay and CH was detected through serum thyroid stimulating hormone(TSH) levels detection by time-resolved fluorometry(TRFIA) or enzyme linked immunosorbent assay(ELISA).Results Sixty-four children with CH were diagnosed and the rate of CH was 1∶2423,41 patients were examined by thyroid gland nuclein kinescope picture,among which there were 23 patients whose thyroid glands were unusal.These patients lived in 45 towns(streets),which had no difference between men and women,city and country side.A couple of twins were screened for CH;15 cases of PKU was detected with an incidence of 1∶10 339.These patients′ parents were neither consanguineous marriage nor dominance hereditary family history.Their physical and mental development were similar to the same age children after interventive treatment.Conclusions The distribution of CH and PKU is diffuse in Lianyungang.Neonatal screening is the only valid means to find patients with CH and PKU.
3.Effect of trimetazidine combined with atorvastatin on cardiac function andlevel of hs-CRP, IL-6, and Fib in patients with coronary heart disease

Objective To investigate the effect of trimetazidine combined with atorvastatin on cardiac function and the level of hs-CRP,IL-6,and Fib in patients with coronary heart disease.Methods Patients (120 cases) with coronary heart disease in the first people's Hospital of Xianyang City from January 2014 to January 2016 were selected and randomly divided into two groups.The observation group was treated with trimetazidine combined with atorvastatin,the control group was treated with trimetazidine,the curative effect,left ventricular ejection fraction,left ventricular diastolic end systolic diameter,left ventricular posterior wall thickness and serum hs-CRP,IL-6,and Fib levels were compared.Results The effective rate of observation group was 91.67% (55/60),significantly higher than the control group 76.67% (46/60) (P < 0.05);The left ventricular ejection fraction,left ventricular end-diastolic diameter,and left ventricular posterior wall thickness of two groups were significantly improved (P < 0.05),and the improvement of the observation group was significantly better than that of control group (P < 0.05);The serum levels of hs-CRP,IL-6,and Fib were significantly lower (P < 0.05),and the observation group was more obvious (P < 0.05);The incidence of adverse reactions of two groups had no significant difference.Conclusion Trimetazidine combined with atorvastatin have high curative effect on coronary heart disease,and can improve cardiac function,significantly reduce the levels of hs-CRP,IL-6,and Fib,worth clinical promotion.
4.Imaging of primary chondrosarcomas:radiologic-pathologic correlation

Objective To study imaging features of primary chondrosarcomas and the correlation with pathologic basis.Methods The imaging materials of primary chondrosarcoma in 21 cases were retrospectively analyzed and correlated with pathological results. Results In 21 cases,the pathologic classifications were as following:15 cases of conventional intramedullary,1 case mesenchymal,2 cases myxoid,1 case dedifferentiated and 2 cases with no subtypes.For 6 cases of conventional intramedullary in gradeⅠand 8 cases in gradeⅡ,bone destruction in varying degrees accompanied with soft tissue mass(n=1 1),regional thickening cortical bone (n=2),“ring-arc”like matrix mineralization(n=6),amorphous (n=3)were showed on X-ray study.CT were performed,in 6 cases in gradeⅠ,6 cases in gradeⅡ and 1 case in grade Ⅲ in which there were intraosseous calcification (n=12),regional thickening cortical bone (n=2)and periosteal new bone formation (n=1).Lesions appeared expansive or osteolytic bone destruction accompanied with soft tissue mass on CT and MRI srudy MRI were performed in 5 cases in gradeⅠ and lesions were heterogeneous high signal intensity on T2 WI with spot like and “ring-arc”like low signal intensity.Conclusion Pathologic classifications and grading of primary chondrosarcoma may be presumed with imaging features.Pathologic characteristic manifestions on MR are superior to that on CT and X-ray.
5.Effect of cardiac troponin T on prognosis in the elderly patients with chronic heart failure

Objective To study the effect of cardiac troponin T(cTnT)on the prognosis of chronic heart failure in the elderly patients.Methods Serum level of cTnT,and serum activities of creatine kinase (CK)and creatine kinase-MB(CK-MB)were measured in 60 elderly patients of chronic heart failure with varied classes of cardiac function.Cardiac events,including malignant cardiac arrhythemia,repeated or aggravated heart failure or cardiac death,in patients with increased cTnT during hospitalization were compared to those without it.Results Serum level averaged(89.1?13.6)mmol/L for CK and(7.9? 1.4)mmol/L for CK-MB in 20 patients with Class Ⅱ cardiac function by New York Heart Association (NYHA)classification and without increased cTnT,(88.2?13.4)mmol/L for CK and(7.5?1.8) mmol/L for CK-MB in 19 patients with Class Ⅲ cardiac function and four of them with increased cTnT,and (93.3?14.2)mmol/L for CK and(8.1?1.6)mmol/L for CK-MB in 21 patients with Class Ⅳ cardiac function and 14 of them with increased cTnT.Malignant cardiac arrhythemia occurred in six,repeated heart failure in eight,and death in three of 18 patients with increased cTnT,and one,two and one in 22 patients without increased cTnT,respectively.Incidence of cardiac events was higher in patients with increased cTnT than those without it(P
6.Primary Application of 3D DCE MRA in the Liver Pretransplantation

Objective To evaluate the clinical efficacy of 3D DCE MRA(three dimensional dynamic contrast-enhanced MR angiography)in patients of preoperation of liver transplantion.Methods 8 cases of potential liver transplant recipients suffering from severe liver disease underwent MRI and 3D DCE MRA, accessed the images synthetically. All of them had DUS examination, 4 cases received liver transplantation successfully.Results Satisfactory angiography images were obtained in all cases, the grade Ⅱ~Ⅲ branches of the hepatic artery, the grade Ⅱ~Ⅴ branches of the portal vein and gradeⅡ branches of the hepatic vein could clearly be visualized. Gastric-oesophageal varices were found in 3 cases of cirrhosis, compression and displacment of hepatic artery and portal vein were shown in one case of polycystic liver.Conclusion 3D DCE MRA is an efficiency, noninvasive technique, it offers great help in evaluating pre-operative vasculature of liver transplantation.
7.CE-MRA of the Arteries of the Pelvis and legs with Automatic Table Movement

Objective To evaluate the clinical efficacy of MRA in the arteries of the pelvis and lower extremity with automatic table movement (MobiTrak).Methods 12 cases suspected of pelvis and lower extremity artery diseases underwent dynamic 3D contrast enhanced MRA and automatic table movement at the same time.Three cases underwent artery angiography,four cases were detected by operation.Results All diseased arteries were well demonstrated.Among them,lower extremity artery occlusion 8,failing vascular grafts 2,artery aneurysms 2.Conclusion Automatic table movement is of value in assessing pelvis and lower extremity artery diseases accurately.And it is a reliable and potential new technique.

Objective To estimate the application value of dual-source CT on preoperational evaluating new type of three branches aortic arch covered stent graft implantation on DeBakey type I aortic dissection.Methods 20 cases with DeBakey type I aortic dissec-tion prepared to performe three branches aortic arch covered stent graft implantation were reviewed,and their CT findings were ana-lyzed in order to evaluate the guiding value for operation.Results The location and size of entries were determined and the diameter of aortic arch and three branchesas well as the intervals of three branches were also measured by dual-source CT.Furthermore, blood-supply for main vessel and complications related with covered stent were further analyzed prior to operation.The surgical pro-tocols were designed according to preoperational CT findings.Among all cases,13 cases were fit for new type of three branches aor-tic arch covered stent graft implantation.While the rest cases (N=7)were adopted other operations due to non-fitted indications. Conclusion Dual-source CT is of significance for preoperational evaluating new type of three branches aortic arch covered stent graft implantation on DeBakey type I aortic dissection.

Berberine, an isoquinoline alkaloid isolated from some Chinese medicinal herbs such as Coptidis rhizoma, has been used for the treatment of diarrhea and other gastrointestinal infections as an antibacterial drug in Chinese medicine. In recent years, it was reported to have beneficial effects on the metabolism disorders states of diabetes. The mechanisms involve many aspects of the diabetes, including regulating the blood cholesterol and triglyceride, lowering blood glucose, ameliorating the insulin resistant state and influencing the function of the pancreatic beta cell.
10.Diagnostic value of non-invasive cardiac output parameters in premature infants with patent ductus ;arteriosus

Objective To investigate the diagnostic value of non-invasive cardiac output parameters:cardiac index (CI) and minute distance (MD), in premature infants with patent ductus arteriosus (PDA) and determine the cut-off value. Methods Clinical data of 98 premature infants admitted to the neonatal intensive care unit from January 2015 to June 2015 were collected. These premature infants were divided into the treated PDA group (n=30),the untreated PDA group (n=28) and the normal premature group (n=40) based on the results of echocardiogram in the first three days after birth and the use of drugs. Non-invasive cardiac output parameters were measured in the first three days after birth. The data were analyzed by t test, analysis of variance and SNK-q test. The diagnostic value of CI and MD for PDA was analyzed by the receiver operating characteristic curve. Results By preliminary analysis of the ROC curve,CI and MD were the most representative parameters for the diagnosis of PDA which need to be treated clinically, we thus chose CI and MD in this study. The aortic and pulmonary arterial CI and MD in the treated PDA group were significantly higher than in the untreated PDA group and the normal premature group (all P<0.05). There were no significant differences between the untreated PDA group and the normal premature group in the aortic and pulmonary arterial CI and MD (all P>0.05). The cut-off value of the aortic CI and MD was 2.95 L/(min·m2) and 21.50 m/min, respectively, while that of the pulmonary arterial CI and MD was 4.55 L/(min·m2) and 26.50 m/min, respectively. The sensitivity and specificity of the combined aortic CI and MD for the treated PDA group were 0.90 and 0.82, and those of combined pulmonary arterial CI and MD were 0.87 and 0.82;and those of combined aortic and pulmonary arterial CI and MD were 0.80 and 0.88, respectively. Conclusions The non-invasive cardiac output parameters CI and MD have good diagnostic value for the PDA needing clinical treatment, and the combined use of the two parameters can improve specificity, and help formulate the early treatment strategy for premature infants with PDA. When aortic CI was ≥ 2.95 L/(min·m2) and MD was ≥ 21.50 m/min, a preliminary diagnosis of the PDA needing clinical treatment, can be made, and simultaneously when the pulmonary arterial CI was ≥4.55 L/(min·m2) and MD was≥26.50 m/min, the arterial duct should be closed timely.
