Objective:To compare the sealing ability and marginal adaptation of three obturation techniques when used in root canal obsturation of c-shaped canals of mandibular second molars. Methods:Twenty-one extracted mandibular second molars with C-shaped canals were randomly divided into three groups. After instrumentation, the canals were filled using three different techniques: ObturaⅡ(group A), cold lateral condensation (group B) and Thermafil (group C). Then, gaps, obturated lateral canals and reticular apical triangles on the X-ray were counted. After dyeing for 7 days in the ink, dyeing lines were measured to reflect the microleakage. The percentages of gutta-percha, sealer and gaps on the root canal surface were determined by analyzing the images of 3 sections per tooth. Results:The reticular apical triangles were most frequently observed in group C(P

Objective:To investigate the relationship between the expression of inhibin subunit Beta A (INHBA) and the clinicopathological data and its effect on proliferation, invasion and metastasis of gastric cancer cells and its possible mechanisms. Methods:Using Gene Expression Profiling Interactive Analysis (GEPIA) public database to verify the expression of INHBA mRNA in gastric cancer and adjacent tissues and its relationship with pathological stage and prognosis; the relationship between INHBA and clinical prognosis was analyzed using the Kaplan-Meier Plotter online database. Immunohistochemistry was used to confirm the correlation between protein expression level of INHBA in gastric cancer and adjacent tissues and clinical pathological staging. In vitro, the cell proliferation was detected by tetrazolium salt (MTT) method; the cell migration ability was detected by scratch test, and cell invasion and metastasis ability was detected by transwell chamber assay. The expression of INHBA and epithelial-mesenchymal transition (EMT) related proteins was detected by Western blot. Results:The GEPIA database and Kaplan-Meier Plotter online database analysis showed that INHBA mRNA was highly expressed in various cancer tissues and significantly higher in gastric cancer tissues than normal tissues ( P<0.05). The high expression of INHBA mRNA was associated with clinical stage and poor prognosis of gastric cancer ( P<0.05). The results of immunohistochemistry showed that the staining score of INHBA in gastric cancer tissues was significantly higher than that in adjacent tissues ( P<0.05), and its expression level was correlated with clinical tumor node metastasis (TNM) stage ( P<0.05). The results of MTT, scratch test and transwell chamber showed that INHBA overexpression could promote the proliferation, migration, invasion and metastasis of gastric cancer cells, while interference with INHBA expression could inhibit the proliferation, migration, invasion and metastasis of gastric cancer cells; Western blot results showed that the expression of CDH1 was down regulated and the expression of CDH2 was up-regulated after INHBA overexpression. The expression of CDH1 was up-regulated and the expression of CDH2 was down-regulated after INHBA overexpression was inhibited. Conclusions:INHBA is highly expressed in gastric cancer tissues compared with adjacent tissues. The expression level of INHBA is related to tumor progression and poor prognosis. INHBA can promote the proliferation, migration and invasion of gastric cancer MGC-803 cell line and its mechanism may be related to INHBA promoting cell EMT.

Objective:
To examine the association between serum vitamin D level and thyroid function indicators among elderly patients with type 2 diabetes mellitus (T2DM), so as to provide the evidence for the prevention and treatment of thyroid function abnormality among elderly patients with T2DM.
Methods:
Inpatients aged 60 years and older and admitted to the department of endocrinology of Zhejiang Hospital were selected as the study subjects. Gender, age, course of disease and other basic information were collected through questionnaire surveys. The serum 25-hydroxyvitamin D[25-(OH) D], thyroid stimulating hormone (TSH), free triiodothyronine (FT3), free thyroxine (FT4), total triiodothyronine (TT3), and total thyroxine (TT4) were measured. The correlation between serum vitamin D level and thyroid function indicators in elderly patients with T2DM was evaluated by a multiple linear regression model.
Results:
A total of 402 elderly patients with T2DM were surveyed, including 210 males (52.24%) and 192 females (47.76%), and had a median age of 70.00 (interquartile range, 12.00) years and a median course of disease of 14.00 (interquartile range, 14.00) years. There were 162 patients with insufficiency of vitamin D (40.30%) and 182 patients with deficiency (45.27%). The levels of TSH and glycated hemoglobin in the vitamin D deficiency group were (2.34±1.66) μIU/mL and (8.83±2.14) %, respectively, which were higher than those in the normal group [(1.74±1.10) μIU/mL and (8.11±1.75) %; P<0.05]. The levels of FT3 and FT3/FT4 in the vitamin D deficiency group were (2.86±0.48) μIU/mL and 2.85±0.71, respectively, which were lower than those in the vitamin D insufficiency group [(3.09±0.47) pg/mL and 3.14±0.81, P<0.05]. Multiple linear regression analysis showed a negative correlation between 25- (OH) D and TSH (β'=-0.159, P=0.001).
Conclusion
The vitamin D deficiency may be associated with the increase of TSH level among the elderly patients with T2DM.

Objective To explore the levels of urine iodine in mothers and urine iodine and blood thyroid-stimulating hormone (TSH) in their neonates, and the correlations. Methods A total of 695 pairs of mothers and their neonates were included. Urine sample of both mothers and newborns were collected and the levels of urine iodine were detected. Heel blood sample of newborns were collected and the levels of TSH were detected. Results The median urine iodine level in 695 mothers was 212 . 9μg/L, among whom 163 case ( 34 . 39%) were iodine deifcient (149μg/L), 143 cases ( 20 . 58%) adequate (≥500μg/L), 163 case ( 23 . 45%) more than adequate ( 250?~?499μg/L) and 150 case excessive (≥500μg/L). The median urine iodine level in neonates was 345 . 7μg/L and was positively correlated with mothers’ (rs?=?0 . 576 , P0 . 001 ). The median TSH level in neonates were 3.24±1.75 mIU/L, among whom 88 cases (12.66%) were higher than 5 mIU/L. The TSH level and the proportion of neonates with TSH?>?5 mIU/L in neonates whose mother were iodine deifcient or excessive were higher than those in neonates whose mother were iodine adequate or more than adequate, and the difference was statistically signiifcant (P0 . 05 ). Conclusions The investigation found that the level of iodine in mothers was adequate, but there was still a high proportion of iodine deifciency or excess. The levels of urinary iodine in mothers were closely related to those in newborns. There is a high risk for mothers with iodine deifciency or excess to give birth to infants with higher level of TSH.

Objective To evaluate the pattern of energy metabolism and nutrients intake in patients with chronic viral hepatitis and posthepatitic cirrhosis to effectively direct their nutrition therapy. Methods Resting energy expenditure (REE) was measured with open-circuit indirect Jorimetry in 60 patients with chronic viral hepatitis and 60 patients with posthepatitic cirrhosis.Their normal basal energy expenditure (BEE) was predicted by Harris-Benedict equation and energy intake (EI) was determined by diet recall. Correlation between REE and indicators for nutrition assessment was analyzed. Results REE was (77? 21) kJ?kg~(-1)?d~(-1) in 60 patients with pusthepatitic cirrhosis, significantly lower than BEE[(95?16) kJ? kg~(-1)?d~(-1)(P0.05,and their EI was (127?34) kJ?kg~(-1)?d~(-1),1.41?0.43 times as REE,in which PROI was (1.02?0.29) g?kg~(-1)?d~(-1),1.31?0.61 times as PROE (0.87?0.34) g?kg~(-1)?d~(-1),also indicating a negative nitrogen balance (-2.02?4.07).REE,EI and intake of three nutrients, serum level of albumin and prealbumin (PA) and body weight significantly decreased in patients with posthepatitic cirrhosis,as compared to those in patients with chronic viral hepatitis (P
9.Mechanism of IL-38 inhibits LPS/TLR4 induced inflammation in patients with rheumatoid arthritis
Chinese Journal of Immunology 2017;33(11):1647-1651
Objective:To investigate the proteins association of IL-38 and TLR4 in rheumatoid arthritis (RA) and its mechanism in RA pathogenesis.Methods:Forty-one rheumatoid arthritis patients(observation group) and forty-five patients with post-traumatic synovial membrane resection (control group) in our hospital from Jan 2013 to Feb 2016 were selected as study subjects.Peripheral blood mononuclear cells (PBMCs),synovial tissues and serum from patients with RA and controls were collected. The expression of IL-38 and TLR4 in PBMCs and synovial tissues were detected by real-time polymerase chain reaction (realtime-PCR).The protiens of IL-38 and TLR4 in synovial tissues and fluid from RA patients and controls were detected by enzyme-linked immunosorbent assay (ELISA) or Western blot assay,respectively.RAW264.7 cells were stimulated by lipopolysaccharide (LPS) and/or IL-38.The production of inflammatory cytokines including IL-6,IL-8 as well as TNF-α were detected by real-time-PCR and ELISA,respectively. The activation of nuclear factor-κB(NF-κB) signaling were determined by nuclear transfer reagent kit for NF-κB and Western blot. Results: Compared with control group, the expression of IL-38 in PBMCs,synovial tissue, serum and synovial fluid of patients with RA increased significantly,meanwhile,TLR4 was significantly increased in PBMCs and synovium of RA patients.Moreover,IL-38 was negatively associated with TLR4 in RA,suggested by Pearson′s correlation analysis. When RAW264.7 cells were stimulated by LPS with or without IL-38 in vitro,IL-38 could suppress LPS-mediated expression of TLR4 and the production of IL-6,IL-8 and TNF-α in RAW264.7 cells.IL-38 can inhibit the activation of NF-κB signaling pathway,so we hypothesized that IL-38 may inhibit the expression of inflammatory factor which induced by LPS/TLR4 signaling via inhibiting the activation of NF-κB signaling pathway. Conclusion: IL-38 can attenuates rheumatoid arthritis through inhibiting LPS/TLR4 induced inflammation in rheumatoid arthritis, and the mechanism may be through inhibiting the activation of NF-κB signaling pathway.

OBJECTIVETo compare the efficacy difference in the treatment of shoulder pain in post-stroke shoulder-hand syndrome among floating acupuncture, oral administration of western medicine and local fumigation of Chinese herbs.
METHODSNinety cases of post-stroke shoulder-hand syndrome (stage I) were randomized into a floating acupuncture group, a western medicine group and a local Chinese herbs fumigation group, 30 cases in each one. In the floating acupuncture group, two obvious tender points were detected on the shoulder and the site 80-100 mm inferior to each tender point was taken as the inserting point and stimulated with floating needling technique. In the western medicine group, mobic 7.5 mg was prescribed for oral administration. In the local Chinese herbs fumigation group, the formula for activating blood circulation and relaxing tendon was used for local fumigation. All the patients in three groups received rehabilitation training. The floating acupuncture, oral administration of western medicine, local Chinese herbs fumigation and rehabilitation training were given once a day respectively in corresponding group and the cases were observed for 1 month. The visual analogue scale (VAS) and Takagishi shoulder joint function assessment were adopted to evaluate the dynamic change of the patients with shoulder pain before and after treatment in three groups. The modified Barthel index was used to evaluate the dynamic change of daily life activity of the patients in three groups.
RESULTSWith floating acupuncture, shoulder pain was relieved and the daily life activity was improved in the patients with post-stroke shoulder-hand syndrome, which was superior to the oral administration of western medicine and local Chinese herbs fumigation (P < 0.01). With local Chinese herbs fumigation, the improvement of shoulder pain was superior to the oral administration of western medicine. The difference in the improvement of daily life activity was not significant statistically between the local Chinese herbs fumigation and oral administration of western medicine, the efficacy was similar between these two therapies (P > 0.05).
CONCLUSIONThe floating acupuncture relieves shoulder pain of the patients with post-stroke shoulder-hand syndrome promptly and effectively, and the effects on shoulder pain and the improvements of daily life activity are superior to that of the oral administration of western medicine and local Chinese herbs fumigation.
