2.The associated investigation between overexpressed MUC-1mRNA and the pathogenesis of lung adenocarcinoma

Objective To explore the preliminary significance of overexpressed MUC-1 mRNA in the pathogenesis of lung adenocarcinoma. Method Total RNAs of 23 paired fresh lung samples and lung adenocarcinoma samples were extracted by routine Trizol way. After total RNAs were transcribed into cDNA, the expression of MUC-1 gene was detected in these samples by real-time PCR. Finally, the correlation of MUC-1 expression with the pathogenesis of lung adenocarcinoma was analyzed. Result Compared with normal lung tissues, the expression of MUC-1 in 20( 87% ) lung adenocarcinoma samples was markedly increased. Compared with early and mid-stage lung adenocarcinoma samples,. the overexpressed MUC-1 in late stage lung adenocarcinoma samples was increased by 2.2 folds. In addition, compared with primary lung adenocarcinoma samples without lymph node metastasis, MUC-1 mRNA was increased in primary lung adenocarcinoma samples with lymph node and distant metastasis ( 2.5 folds). Conclusion Overexpressed MUC-1 may participate in the pathogenesis of lung adenocarcinoma.

4.multivariate Logistic regression analysis of risk factors for ventilator- associated pneumonia in comprehensive ICU and nursing countermeasures

Objective To explore the morbidity, risk factors and nursing of ventilator-associated pneumonia (VAP) in comprehensive ICU. Methods It was a retrospective survey. 98 mechanical ventilation (> 48 h) patients from Jan to Dec, 2009 in comprehensive ICU were reviewed using questionnaires to collect the clinical data. They were divided into the VAP and the non-VAP group. Several statistically significant risk factors were screened out with univarite analysis, then independent risk factors were determined with multiple Logistic regression. Results The morbidity of VAP was 35.7%. Univariate analysis showed that the level of APACHE Ⅱ score, duration of mechanical ventilation, whether primary lung disease, whether acid-suppressing agents, whether the semi-supine, whether accidental tube removal, oral care approach, whether attracted to subglottic were statistically significant risk factors of VAP. While multivariate Logistic regression analysis showed that the duration of mechanical ventilation, whether the semi- supine,whether attracted to subglottic were the major risk factors of VAP. Conclusions The occurrence of VAP is related with multiple factors. Application of comprehensive prevention strategies in accordance with these risk factors of VAP can reduce the morbidity of VAP effectively.
5.OPG/RANKL gene expression of bone marrow stromal cell under stimulation of PTHrP

Objective To study the OPG/RANKL gene expression of bone marrow stromal cell under stimulation of PTHrP.Method Primary bone marrow cells were cultured under the stimulation of 45 ng/ml PTHrP. Number of osteoclasts was counted after 6-day's culture.90 ng/ml PTHrP was used to stimulate adherent bone marrow stromal cells for 3 or 6 days.OPG and RANKL gene expression level were determined by real-time PCR. Result Large number of osteoclasts were formed in primary bone marrow cell culture after 6-day stimulation of PTHrP.RANKL gene expression level was up-regulated by PTHrP,while OPG gene expression level were down-regulated.Conclusion PTHrP stimulates osteoclast formation via up-regulating RANKL level and down-regulating OPG level.
6.Design and application of ventilator-associated pneumonia prevention checklist in ICU

Objective To design and apply ventilator-associated pneumonia prevention checklist in ICU.Methods The ventilator-associated pneumonia prevention checklist was designed by referring to guidelines and related literature.The checklist was applied to ventilator-dependent patients.The head of nursing group evaluated prevention status of ventilator-associated pneumonia performed by nurses every day.Results After application of the checklist,the average indwelling time of tracheal catheter and the incidence of ventilator-associated pneumonia decreased among ICU patients.Conclusion Using the ventilator-associated pneumonia prevention checklist in quality management in the prevention of ventilator-associated pneumonia can shorten the average indwelling time of tracheal catheter,and reduce the incidence of ventilator-associated pneumonia,and the checklist can be promoted and applied.
7.Association of blood lipid with distant metastases and clinical response in patients with hormone receptor negative breast cancer

Objective To investigate the relationship between dyslipidemia and distant metastases in pa-tients with hormone receptor( HR) negative breast cancer,and to explore the impact of blood lipid change during chemotherapy on clinical response.Methods We collected the complete clinicopathological data and fasting blood lipid in 154 patients with HR negative breast cancer.The secondary blood lipid level were measured on day14 after two cycles of chemotherapy in patients with distant metastases.Single factor chi -square test was choosen to analyze the relationship between clinicopathological characteristics,including baseline lipid level,and distant metastases,then independent risk factor for distant metastases were evaluated by logistic regression model. Data about altered blood lipid and clinical response analysis were performed using paired t-test pre-or post-chemotherapy.Results Our results from single chi-square test showed that distant metastasis from breast canc-er was significantly associated with tumor size,local lymph node metastases,grade,higher total cholesterol( TC) , higher triglycerides(TG)and higher low-density lipoprotein(LDL)(P<0.05).For distant metastases,inde-pendent risk factors were tumor size(OR=1.563),local lymph node metastases(OR=1.983),higher TC(OR=1.502),higher TG(OR=1.877).Decreased trend of TC,TG and LDL and increased trend of HDL were showed in disease control( PR+SD) group,and TG had significantly tend in objective response( PR) group.Conclusion Hyperlipidmia is significantly associated with distant metastases in patients with HR negative breast cancer,andblood lipid levels decrease with effective anti -tumor chemotherapy.Dynamic monitoring of blood lipid levelscould be a candidate surrogate biomarker for outcome prediction and distant metastases in patients with HR negativebreast cancer.
