Objective:To explore the constitution characteristics in patients with vascular dementia(VaD),and to analysis the constitutional risk factor in VaD.Methods:This was a case-case control study.66 VaD patients and 51 stroke patients were selected by MMSE and Similarities test in WAIS-R、Blessed Behavior Scale.The diagnosis of VaD based on the DSM-Ⅳ and the NINDS-AIREN criteria、Hachinski Ischemia Scale、Cornell Scale of Depression in Dementia and the Clinical Dementia Rating.The stroke group was of MMSE≥28.The structured questionnaire of constitution was used to investigate and Wang Qi's method was used to constitution diagnose.Results: Yin Deficiency was the most common type in the VaD group and the stroke group(31.8% vs 45.1%),and the second was Phlegm-Damp(30.3% vs 35.3%).The frequencies of Yang Deficiency(15.2% vs 2%,P

3.Medical equipment management and risk control based on risk analysis

Objective To analyze the risks for medical equipment and realize risk control.Methods The medical risk management plan was proposed based on the classification of medical equipment risks. Results Hierarchical management of medical equipment was realized based on the quantitative evaluation of risk grade. 6.Effects of continuous infusion of moderate dose tranexamic acid and ulinastatin on fibrinolysis during orthotopic liver transplantation

Objective To investigate the effects of continuous infusion of moderate dose tranexamic acid and ulinastatin on fibrinolysis during orthotopic liver transplantation (OLT).Methods Thirty ASA Ⅲ or Ⅳ patients aged 34-63 yr with a body mass index of 17-37 kg/m2 and end-stage liver disease score 6-34,undergoing OLT,were randomly assigned to one of 3 groups ( n =10 each):group control (group C) ; group tranexamic acid (group T) and group ulinastatin (group U).The patients received continuous infusion of normal saline at 10 ml/h in group C or ulinastatin at 100 000 U/h in group U immediately after tracheal intubation until 120 min after portal vein was declamped,while in group T the patients received a loading dose of tranexamic acid 1 g followed by continuous infusion at 10 mg· kg-1 ·h- 1.Prothrombin time (PT),activated partial thromboplastin time (APTT),international normalized ratio ( INR),fibrinogen (Fg),D-dimers (D-D) and fibrin degradation product (FDP) were measured before induction of anesthesia (baseline),at 120 min after skin incision,30 min after clamping of portal vein (anhepatic phase),30 and 120 min after declamping (neohepatic phase) and at the end of operation.The amount of blood loss and transfusion were recorded.The patients were followed up after operation for hepatic artery and portal vein thrombosis in groups U and T.Results There were no significant differences in PT,APTT,Fg,INR and amount of blood loss and transfusion among the 3 groups.The plasma D-D concentration and percentage of patients with plasma FDP > 20μg/ml were significantly lower in group T than in group C.There were no significant differences in plasma D-D concentration and percentage of patients with plasma FDP > 20 μg/ml between groups U and C.No hepatic artery and portal vein thrombosis was detected after operation in groups T and U.Conclusion Continuous infusion of moderate dose tranexamic acid can inhibit fibrinolysis during OLT,but can not reduce the amount of blood loss.Continuous infusion of ulinastatin ( 100 000 U/h) has no significant effect on fibrinolysis during OLT.
7.Hematopoietic stem cell transplantation for the treatment of multiple myeloma

Autologous stem cell transplantation(ASCT) significantly increased the complete response rate(CR) and long-term survival in the patients younger than 65 years with multiple myeloma(MM). ASCT in the early stage of disease is currently considered the standard therapy in younger patients with newly diagnosed MM. High-dose melphalan with 200 mg/m2 is still considered the best conditioning. Double autotransplant is recommended for the patients without having VGPR after the first ASCT. Novel pretransplant induction regimens and conditioning with novel drugs such as bortezomib could improve the effect of ASCT. The best curative approach in multiple myeloma is allogeneic transplantation (allo-HSCT) with much higher CR. The long-term survival of patients receiving conventional/myeloablative conditioning (MAC) allo-HSCT is not better than that of the ASCT because of higher transplant-related mortility (TRM). Allo-HSCT with MAC has almost universally been replaced by the dose-reduced intensity conditioning allogeneic transplantation (Allo-RIC),which has lower TRM. More patients and long-term follow-up are needed to test the efficacy of single ASCT followed by Allo-RIC in patients with multiple myeloma.
8.Study on TCM Syndromes of Parkinson's Disease

Objective To investigate the distribution of TCM Syndromes of Parkinson's disease(PD).Methods The 125 PD patients were graded according to Hoehn-Yahr(H&Y) criteria for assessment.The assigned TCM physician investigated TCM syndromes of these patients with the structured questionnaire which was based on literature and experience of experts.Results Of 125 cases,96 ones(76.8%) presented a single syndrome,and 29 ones(23.2%) presented complex syndrome.The positive rate of main syndrome was in an order of deficiency of both the Spleen and Kidney(22.4%),yin deficiency of the Liver and Kidney(18.4%),yang deficiency of the Spleen and Kidney(16%),qi deficiency of both qi and yin(15.2%),phlegm-heat stirring up the wind(12.8%),qi deficiency with blood stasis(4.8%),deficiency of both yin and yang(4%),damp abundance due to the Spleen deficiency(3.2%),endoretention of damp heat(1.6%),stagnation of the Liver qi with deficiency of the Spleen and Liver fire flaring(0.8%).There existed a significant difference in the distribution of TCM syndromes in H&Y 1.0~1.5 and H&Y 2.0~3.0(P
9.Effects of treadmill training after cerebral ischemia on expression in the brain of axonal guidance factor Netrin-4 and its receptor protein

Objective To observe the effects of treadmill training on the expression of axonal guidance factor Netrin-4 and its receptor deleted in colorectal cancer (DCC) protein in the brains of rats with cerebral ischemia.Also to explore how training promotes the recovery of neurological function after cerebral ischemia.Methods Sixty-three adult, male Sprague-Dawley rats were randomly divided into a sham group (n =9) , a model group (n =27) and an exercise group (n =27).A modified version of Longa's method was used to establish an animal model of cerebral ischemia by occluding the right middle cerebral artery (MCAO) in the rats of the model and exercise groups while the rats of the sham group were subjected to the same surgical procedure except that no thread was inserted.The rats of the exercise group were given treadmill training beginning 24 h after the modeling, while those of the other two groups were left on the treadmill without training.The rats' neurological functioning was tested by assigning modified neurological severity scores (mNSS) on the 3rd, 7th and 14th day after modeling.Then the ischemic brain tissue was dissected to detect the expression of Netrin-4 and DCC protein using western blotting and immunofluorescent staining and analysis.Results On the 3rd, 7th and 14th day after modeling, the model and exercise groups had significant differences in their average mNSS scores when compared to the sham group.The average mNSS scores of the exercise group on the 7th and the 14th day were significantly lower than those of the model group at the same time point.The average expression level of Netrin-4 and DCC protein in the exercise group was significantly higher in than the model group on the 7th and 14 th days.Immunofluorescent staining showed that Netrin-4 was mainly observed in the blood vessels and astrocytes in the ischemic brain area.DCC protein was found mainly in neural processes and astrocytes in the same area.Conclusions Treadmill training can improve the recovery of neurological function after cerebral ischemia.Its mechanisms may be partly attributed to up-regulating the expression of Netrin-4 and DCC protein in the ischemic brain area, which strengthens the regeneration and reconstruction of nerves and blood vessels.
10.Differentiation of transplanted fetal liver stem cell antigen-positive cells into neural cells in mice

Objective To study whether stem cell antigen-positive(Sca-1 + )cells from fetal liver can differentiate into neural cells. Methods Sca-1+ cells from male fetal liver were isolated with a magnetic cell sorting kit and transplanted into lethally irradiated female mice (2?103 cells/mouse). The donor cells and their characteristics in recipient brains were identified and detected by FISH and immunohistochemistry double-staining analysis at 2, 4, 6 months after transplantation. Results There existed many male cells in brains of female recipients, including white and gray matters, for brain, midbrain, the ependyma of the ventricular system, and the choroid plexus of the lateral ventri cle. Immunochemistry revealed that the Y chromosome-positive cells expressed many neural specific markers, including Neu N, TuJ-1 , NF-M, GFAP and so on. Statistic analysis showed that ratio of Y chromosome positive cells in brain was (4. 5 ? 0. 5) %, ratio of both Y chromosome and NeuN posi live cells (1. 2 ? 0. 3) %. ratio of both Y chromosome and GFAP positive cells (1. 0 ? 0. 2) %. There was no difference between the ratios of Y chromosome-positive cells in brains 2,4, and 6 month after transplantation. Conclusion Sca-1 + cells from fetal liver, the most of which are regarded as hemato-poietic stem cells, can differentiate into neural cells and astrocytes in the brains of adult mice and survive more than 6 months.
