2.Clinical application of vaccum sealing drainage in plasma cell mastitis

Objective To study the clinical value of vaccum sealing drainage in plasma cell mastitis.Methods 59 patients with plasma cell mastitis admitted in our hospital were treated with vaccum sealing drainage (the experimental group) from Jan.2011 to Apr.2014.59 plasma cell mastitis patients were treated with traditional vaseline gauze drainage (the control group).Clinical data like hospitalization length,healing time,healing grade,scar size,dressing change frequency,one year recurrence rate and breast deformity were retrospectively analyzed.Results Patients treated with vaccum sealing drainage had shorter hospitalization length and healing time,lower one year recurrence rate and less breast deformity compared to patients treated with traditional Vaseline gauze drainage (P<0.01),while there was no significant difference in terms of healing grade (P>0.05).Conclusions Vaccum sealing drainage can significantly shorten the hospitalization time and incision healing time,reduce dressing change frequency and one year recurrence rate in patients with plasma cell mastitis.Because of the smaller scar caused by vaccum sealing drainage,the breast deformity is also less.It is of clinical value for plasma cell mastitis.
3.Clinical value of ultrasound and X-ray guided percutaneous nephrolithotomy (PCNL) for treatment of kidney calculi:a meta-analysis

Objective To compare the clinical value of ultrasound and X-ray guided percutaneous nephrolithotomy ( PCNL) for treat-ment of kidney calculi. Methods We searched relevant clinical controlled articles in PubMed,EMBASE/Cochrane library and CNKI data-base. The outcome indicators included stone clearance rate, complication rate,puncture time,blood loss,and hospital days. All data were ana-lyzed by Revman 5. 0 software. Results There were 9 articles involved in 1693 patients. The results suggested that there was no significant difference in stone clearance rate between ultrasound group and X-ray group[OR=1. 11,95%CI(0. 83,1. 49),P=0. 47]. However,the complication [OR=0. 43,95%CI (0. 27,0. 69),P=0. 0004],time to puncture[WMD= -4. 54,95%CI( -0. 45,-8. 62),P=0. 03], blood loss[WMD= -22. 77,95%CI( -36. 57,-8. 97),P=0. 001],and hospital days[WMD= -0. 16,95%CI( -0. 28,-0. 03),P=0. 02] were better in ultrasound group than those in X-ray group. Conclusion Ultrasound and X-ray guided PCNL had an equal stone clear-ance rate,but it seemed that ultrasound was better than X-ray in time to puncture and hospital days after operation. However,the results of this meta-analysis should be treated carefully because of limitation of data and heterogeneity.
4.Determination of Capsaicin in Different Kinds of Dried Capsicums by RP-HPLC

Objective To determine the content of capsaicin in different kinds of dried capsicums.Methods A RP-HPLC method was established.The chromatographic column was DiamonsilTM C18(250 mm ? 4.6 mm,5 ? m) and the column temperature was 30 ℃.The mobile phase consisted of methnol ∶ 0.1 % phosphoric acid aqueous solution(70 ∶ 30) with a flow rate of 1.0 mL? min-1.The detection wavelength was 281 nm.Results A good linearity of capsaicin was in the rang of 0.100 6~ 4.024 0 ? g.The average recovery was 101.18 % and RSD was 1.81 %(n=6).Conclusion The method can be used to determine the content of capsaicin in different kinds of dried capsicums.
5.Laparoscopic surgical treatment for schistosomiasis liver fibrosis portal hy-pertension combined with calculous cholecystitis

Objective To evaluate the curative effect of laparoscopic cholecystectomy(LC)in the treatment of schistosomia-sis liver fibrosis portal hypertension combined with calculous cholecystitis. Methods The clinical data of 196 cases of schistoso-miasis liver fibrosis portal hypertension combined with calculous cholecystitis(Child A 160 cases,Child B 36 cases)treated with LC were collected and analyzed from June 2006 to June 2013. Results Among the 196 cases,there were 154 cases of schistoso-miasis liver fibrosis portal hypertension combined with chronic calculous cholecystitis,and 42 cases of schistosomiasis liver fibro-sis portal hypertension combined with acute calculous cholecystitis. Totally 189 patients underwent LC successfully,but 7 were transited to the general operation because of LC failure,including 3 cases of adhesion around gallbladder and ambiguous dissec-tion of gallbladder triangle,and 4 cases of intraoperative bleeding and the bleeding was difficult to stop under the laparoscopy. All the 196 patients were cured. Conclusion LC is effective and safe in the treatment of schistosomiasis liver fibrosis portal hyperten-sion combined with calculous cholecystitis.
6.Effect of Local Insulin Injection on Crushed Injury of Facial Nerve in Rats

Objective To investigate the effect of local insulin injection on neurological function after crushed injury of facial nerve in rats. Methods Facial nerve was crushing injured in 30 Sprague-Dawley rats, from which 20 selected rats were divided into treatment group (n=10) and control group (n=10). The treatment group was injected with insulin 250 ng/kg at innervated muscle, and the control group was injected with normal saline. They were assessed with neurologic deficit score everyday within 7 days after injection, and the regeneration of nerve fiber was observed with retrograde fluorogold labeling. Results Compared with that in the control group, the neurological deficit score was improved more 5~7 days after injection in the treatment group, and the count of fluorogold labeling cells increased more in the treatment group. Conclusion Local injection of insulin can promote the recovery of neurological function after crushed injury of facial nerve.
7.Skin prick test using two different allergen prick reagents, a comparative study in Shanghai.

OBJECTIVE:
To compare the results of skin prick test with allergens of Allergopharma company or ALK company in Shanghai area.
METHOD:
The study included 9 233 patients diagnosed as allergic rhinitis. The patients were divided in to two groups (Allergopharma group and Alutard-SQ group) SPT was given from June, 2005 to Feb, 2013. The results of the positive rate using two reagents were analyzed and compared.
RESULT:
There were no significant differences between the two groups in age and gender (P > 0.05). The total positive rate in Allergopharma group was 62.90%, and 59.68% in Alutard-SQ group. The positive rate in Allergopharma group was significantly higher than that in Alutard-SQ group (P < 0.01). Dermatophagoides farinae (Der f) and Dermatophagoides pteronyssinus (Der p) were identified as the mostly responsible allergens. The positive rate of Der f in Allergopharma group and Alutard-SQ group was 55.23% and 45.57% respectively. The same rate were 54.83% and 47.89% of Der p. There were significant differences of the positive rate of two allergens in both groups (P < 0.01).
CONCLUSION
The total positive rate was different using different allergens for SPT among the patients diagnosed as allergic rhinitis of the same area. Dust mites were identified as main allergens. The positive rate was different when using different reagents, which needed more attention to make the decision of specific immune therapy.

8.Comparison of the efficacy of laryngeal mask of Supreme and laryngeal mask of i-gel for the airway man-agement in patients undergoing elective lumbar vertebral surgery in prone position

Objective To compare the efficacy of laryngeal mask of Supreme and i-gel for the airway management in patients undergoing elective lumbar vertebral surgery in prone position. Methods A total of 264 patients,ASA physical status Ⅰ or Ⅱ,scheduled for elective lumbar verte-bral surgery in prone position under general anesthesia were divided into two groups (n =132)using a random number table:laryngeal mask of Supreme group (group S)and laryngeal mask of i-gel group (group I).The laryngeal mask of Supreme or i-gel were respectively placed in group S and group I im-mediately after anesthesia induction according to manufacturers’specification.Volume-controlled ven-tilation was used to observe the ventilation quality.The number of laryngeal mask insertion,laryngeal mask insertion time and the ventilation quality in supine and prone position were recorded,as well as the airway seal pressure.The position of laryngeal mask was evaluated by fibero ptic examination in both supine and prone positions.The blood on the laryngeal mask was noted.Finally,the occurrence of laryngeal mask related complications was observed.Results One patient in group I was excluded due to the failure of laryngeal mask insertion after three attempts.No significant difference in type of laryngeal mask,insertion time,adjusted cases,peak airway pressure,and fiber optic examination in both supine and prone positions between two groups.Compared with group S,group I required signif-icantly more attempts of insertion (P < 0.01 ).Nevertherless,ventilation quality in prone position was improved and airway seal pressure in supine and prone position increased (P <0.01 or P <0.05) in group I.No significant difference was found in the occurrence of laryngeal mask related complica-tions.Conclusion In the airway management of patient undergoing elective lumbar vertebral surgery in prone position,laryngeal mask of i-gel is more difficult to be inserted but can maintain higher air-way seal pressure compared with laryngeal mask of Supreme.
9.Discussions of Multi-Detector Computed Tomography (MDCT) dose assessment based on information in DICOM images

Objective To establish an accuracy test method for MDCT dose assessment based on information in DICOM images.Methods The type of MDCT studied in this paper was widely used in clinical practice.A software package developed by java language was used to automatically read doserelated information from DICOM files of MDCT.The CTDIvol and DLP of each pectoral or abdominal scan was calculated based on these information and the basic scan parameters such as collimation,mAs and pitch.The calculated values were compared with the displayed values.Results For pectoral scans,the difference between the calculated and displayed values was between-2%-8% for CTDIvol,and-2%-5% for DLP.For abdominal scan,the difference between the calculated and displayed values was 0-2% for CTDIvol,and-2%-3% for DLP.Conclusions This method is useful for MDCT dose assessment and is worth disseminating its application for general use.
