Objective:To explore the information technology to improve the efficiency of information sharing platform for obsolete and eliminated medical equipment.Methods: To design and develop a information sharing platform for obsolete medical equipment. This platform included platform construction, function introduction and application effect based on information characteristics of obsolete and elimination medical equipment, and the key point of information sharing. And to explore the next development direction for this platform.Results:This platform was based on internet technology and it can be used to share relevant information of obsolete and elimination medical equipment in a certain region. This platform included obsolete equipment inquiry, disposal information announcement and service obtaining and so on.Conclusion: The successful application of this platform standardize the management system to obsolete and dispose medical equipment. And it can unify the operation process and improve work efficiency. At the same time, it is a powerful management tool for relevant department and it can achieve preservation and increment of national asset in medical institutions.
2.Magnifying Endoscopy Combined with Narrow-band Imaging for Targeted Biopsy of Superficial Lesions in Esophagus

Background:Esophageal cancer is a commonly seen gastrointestinal malignancy. Early detection of superficial neoplastic lesions is critical for improving the prognosis. Therefore,it is of great importance to explore new endoscopic techniques for increasing the detection rate of early esophageal cancer. Aims:To assess the diagnostic accuracy of targeted biopsy guided by magnifying endoscopy combined with narrow-band imaging(ME-NBI)for suspected superficial lesions in esophagus. Methods:In a prospective cross-over designed trial,65 patients with suspected superficial lesions in esophagus detected by conventional gastroscopy were randomly assigned to group A and group B. Patients in group A received primary white light imaging(WLI)with Lugol’s staining followed by ME-NBI 4-6 weeks later,and patients in group B received primary ME-NBI followed by WLI with Lugol’s staining 4-6 weeks later. Random biopsy was performed in WLI with Lugol’s staining,while targeted biopsy was performed in ME-NBI based on Inoue’s intraepithelial papillary capillary loop (IPCL)classification. Results:A total of 58 patients completed the study and 68 lesions were eligible for statistical analysis. More biopsies were taken in WLI with Lugol’s staining than in ME-NBI(3. 7 vs. 2. 2 per lesion,P < 0. 05). The overall agreement of IPCL classification with definite pathological diagnosis was 89. 7% . The overall agreement of targeted biopsy by ME-NBI and random biopsy by WLI with Lugol’s staining was 85. 3% ;the specificity and positive predictive value of both ME-NBI and WLI with Lugol’s staining for neoplastic lesions were 100% ,but the sensitivity of ME-NBI was superior to that of WLI with Lugol’s staining(90. 0% vs. 70. 0% ,P < 0. 05). The agreement of ME-NBI-guided targeted biopsy with definite pathological diagnosis was slightly higher than that of random biopsy by WLI with Lugol’s staining(89. 7% vs. 86. 8% ,P > 0. 05). Conclusions:ME-NBI-guided targeted biopsy is superior to random biopsy by WLI with Lugol’s staining for detection of superficial neoplastic lesions in esophagus with higher sensitivity and less number of biopsy. It might benefit the follow-up endoscopic treatment.
3.Value of magnifying endoscopy with narrow-band imaging in diagnosis of 124 early gastric neoplastic lesions

Objective To investigate the value of intervening part (IP)ratio under magnifying endoscopy with narrow-band imaging (ME-NBI )in the diagnosis of early gastric neoplastic lesions. Methods From September 2012 to May 2015 ,a total of 124 patients with suspected superficial gastric neoplastic lesions under white light endoscope (WLI)were enrolled,87 male with mean age of (63.2 ± 7.9)years old and 37 female with mean age of (62.6±8.2)years old.All the enrolled patients received precision endoscopy examination,which were observed under WLI at first followed by ME-NBI.Vessel, surface classification and IP features of the lesions under WLI and ME-NBI were recorded,and targeted biopsies were performed.Patients received endoscopic submucosal dissection (ESD)or surgery according to lesions features,histopathological results.Agreement rate between WLI,VS classification with IP ratio and the histopathological results were analyzed.Chi-square test was performed for rate comparision Receiver operator characteristic (ROC)curve was drawn to compare the diagnostic accuracy in early gastriccancer (EGC)between WLI and ME-NBI.Results Among the 124 patients,a total of 118 patients completed precision endoscopy examination.A total of 162 lesions were detected,and 161 of which were analyzed.A total of 84 low grade intraepithelial neoplasia (LGIN),63 high grade intraepithelial neoplasia (HGIN), seven mucosal cancer and seven submucosal cacinoma were detected. The incidences of demarcation line (32.1 %(27/84)vs 96.1 %(74/77)),irregular microsurface pattern (45 .2%(38/84)vs 87.0%(67/77)),irregular-microvascular pattern (16.7%(14/84)vs 62.3%(48/77 )),increased gland tube density (48.8%(41/84)vs 85 .7%(66/77 ))and increased microvessel density (21 .4%(18/84)vs 80.5 %(62/77))of non-cacinoma lesions (LGIN)were significantly lower than those of cacinoma lesions (including HGIN, mucosa cancer and submucosa cacinoma ), and the differences were statistically significant (χ2 =67.6,29.1 ,33.5 ,22.9,53.7,all P <0.05).The sensitivities of WLI and ME-NBI in EGC diagnosis were 89.6% and 94.8%,respectively;the specificities were 61 .9% and 83.3%, respectively,area under curve (AUC)were 0.84 and 0.93,respectively.The diagnostic accuracy of ME-NBI in early gastric cancer was higher than that of WLI,and the difference was statistically significant (χ2 =49.0, P <0.01).The sensitivities of vessel plus surface (VS)classification and VS classification with IP ratio were 90.9% and 94.8%,respectively;and the specificities were 81 .0% and 83.3%,respectively;AUC were 0.89 and 0.93,respectively.The diagnostic sensitivity of vessel classification with IP ratio was higher than that of simple vessel classification,and the differnce was statistically significant (χ2 =41 .0, P <0.01).Conclusion Compared with WLI,the diagnostic accuracy of ME-NBI is higher in gastric superficial neoplasia lesions,and IP ratio is helpful in diagnosis of gastric superficial neoplasia lesions.
4.Comparison of two transmemembrane proteins as fusion partner for protein expression on the surface of cell.

The expression of a soluble protein on cell surface is often desirable for study of a functional protein, wide application of a protein or investigation of protein-protein interaction. The expression of a soluble protein on the surface of a cell is often achieved by genetically linking a protein to the extra-cellular fragment of a transmembrane partner. In this study, the myc epitope was linked with N terminal of transmembrane proteins either A2TM or deltaLNGFR amplified by overlapping PCR. The plasmids expressing fusion protein were transfected into 293FT cells and the expression of target proteins was evaluated by fluorescent microscope, flow cytometry and Western blotting. The results of flow cytometry revealed that both A2TM and deltaLNGFR were expressed on the cell surface, but A2TM could only be detected with high copy number. Western blotting showed that the expression level of deltaLNGFR was very high and protein was heavily glycosylated, by contrast the expression of A2TM was hardly detected. The results indicate that glycosylated deltaLNGFR is a good candidate partner for the expression of a soluble protein on the cell surface.

5.Practice and exploration of the construction of medical alliance for the transformation of scientific and technological achievements in prefecture-level cities

Objective:To discuss the construction mode of regional scientific and technological achievements transformation medical alliance in prefecture-level cities by analyzing and summarizing the work experience and reflections in the construction of medical alliances with medical alliances.Methods:Through investigation of the status quo, empirical study and summary analysis, explore the feasibility and operating mechanisms of the construction of regional scientific and technological achievements in prefecture-level cities.Results:Develop operational management plans for the construction of medical alliance for the transformation of scientific and technological achievements, establish a regional medical innovation exchange and sharing platform in prefecture-level cities, share knowledge training on the transformation of scientific and technological achievements, set up a patent application sharing platform, multi-dimensional assistance in medical technological innovation and achievement transformation, and establish a regional medical alliance to support the transformation of scientific and technological achievements, which can effectively improve the scientific and technological innovation capacity of regional medical alliance hospitals.Conclusions:Establishing a tailored needs-based training model, building a medical innovation exchange platform, and establishing a grid map for the transformation of scientific and technological achievements of the regional medical consortium are effective ways for prefecture-level cities to transform scientific and technological achievements into medical consortia.
6.Application of a traction metal clip with a fishhook-like device in wound sutures after endoscopic resection

Background/Aims:
Endoscopic wound suturing is an important factor that affects the ability to remove large and full-thickness lesions during endoscopic resection. We aimed to evaluate the effect of a traction metal clip with a fishhook-like device on wound sutures after endoscopic resection.
Methods:
From July 2020 to April 2021, patients who met the enrollment criteria were treated with a fishhook-like device during the operation to suture the postoperative wound (group A). Patients with similar conditions and similar size wounds who were treated with a “purse-string suture” to suture the wounds were retrospectively analyzed as the control group (group B). Difference in the suture rate, adverse events, time required for suturing, and number of metal clips were compared between the two groups.
Results:
The time required for suturing was 7.72±0.51 minutes in group A and 11.50±0.91 minutes in group B. This difference was statistically significant (F=13.071, p=0.001). The number of metal clamps used in group A averaged 8.1 pieces/case, and the number of metal clamps used in group B averaged 7.3 pieces/case. This difference was not statistically significant (F=0.971, p>0.05).
Conclusions
The traction metal clip with the fishhook-like device is ingeniously designed and easy to operate. It has a good suture effect on the wound after endoscopic submucosal dissection and effectively prevents postoperative adverse events.
7.Transplantation of adrenocortical cells in allorat using porous PHB as cell carrier.

This experiment was designed to investigate the feasibility of transplanatation of using porous PHB as cell carrier for the transplanatation of adrenocortical cells. Adrenocortical cells from rat adrenal gland were separated and cultured in vitro. The effect of PHB on the proliferation and secretory function of adrenocortical cells were evaluated by MTT and RIA methods. Then adrenocortical cells were seeded into porous PHB. After the cells were cultured in vitro for about seven days, they were implanted into the rats having undergone bilateral adrenalectomy. The changes of blood corticosterone and aldosterone and the local histological changes in these rats were observed. Adrenocortical cells were able to grow and survive on PHB. No effect on the proliferation and secretory function of adrenocortical cells were observed. Most bilateral adrenalectomized rats bearing the transplanted adrenocortical cells within PHB (study group) survived longer than did the adrenalectomized rats in control group. The blood corticosterone level and aldosterone level of study group were higher than those of control group. It was found that PHB has no effects on the survival, proliferation and secretory function of adrenocortical cells. Adrenocortical cells within PHB can survive a period of time and can secrete corticosterone and aldosterone which can meet the needs of the adrenalectomized rats. PHB can degrade slowly in vivo. It is feasible to perform transplantation of adrenocortical cells using porous PHB as cell carrier.

8.Correlation analysis of learning initiative and on-line teaching effectiveness in medical students—with Physiology teaching as an example

Objective:To evaluate the effects of learning initiative on teaching effectiveness.Methods:The research subjects were the 2nd year medical students of Tongji Medical College of Huazhong University of Science and Technology. The learning initiative factor was calculated from the data of the attendance registration of the on-line learning of Physiology, and the performance of the on-line test was used as an evaluation indicator of teaching effectiveness. SPSS software was used to perform correlation analysis between the learning initiative factor and teaching effectiveness. Results:We found that learning initiative could significantly affect the teaching effectiveness, with differences among different specialties. There was a positive correlation between learning initiative and teaching effectiveness in clinical and pediatric medicine, while no correlation was observed in preventive medicine and medical imaging.Conclusion:In conclusion, learning initiative can affect teaching effectiveness, and the intensity of this effect shows difference among different specialties.
9.Interpretation of the guideline or consensus on screening strategy and detection methods for prevention of perinatal group B streptococcal infection in European and American countries

Group B streptococcus ( GBS) is the most common cause of neonatal invasive infections in western countries , leading to severe pneumonia , sepsis and meningitis with high mortality and morbidity . Neonatal GBS infections are mainly transmitted from mother to baby .Since the end of 1990 s, prevention strategies of antenatal GBS screening and intrapartum antibiotic prophylaxis ( IAP) have been implemented by European and American area , decreasing the incidence of neonatal GBS disease significantly .The harm of GBS to neonates has been recognized and valued in recent years in China , but the authorized prevention measures are still lacking.Efficacy of IAP-based prevention depends on the accuracy of GBS screening results during pregnancy .Here, the GBS prevention guideline or consensus recommended by European and American CDC, including the screening strategies , detection methods, sample collection, storage and transportation were interpreted .
10.Clinical analysis and serotypes distribution of neonatal purulent meningitis caused by group B streptococcus

Objective To investigate the clinical characteristics,antibiotic susceptibilities and serotypes of Group B Streptococcus(GBS)isolated from neonatal meningitis to provide references for the prevention and treatment of neonatal GBS meningitis. Methods From June 2013 to June 2016,we surveyed the GBS strains iso-lated from purulent meningitis of < 90 days infants from Guangzhou Women and Children′s Medical Center. The GBS isolates were identified and the minimum inhibitory concentration of the antibiotics was determined by Vitek 2 Compact automatic bacterial identification system.GBS serotyping was performed using Strep-B-Latex?rapid latex agglutination test kit. Results A total of 46 cases of neonatal GBS meningitis,15 cases of early-onset infection and 31 cases of late-onset infection were diagnosed. 78.3% of GBS meningitis with varying degrees of complica-tions.Among 41 survivors with 3~24 months follow-up,50% of the early-onset and 44.8% of the late-onset GBS meningitis with varying degrees of neurological sequelae.Four capsular types were identified among the 46 isolates, serotype-Ⅲwas the most prevalent(73.9%),followed by Ib(19.6%),V(4.3%)and Ia(2.2%).All the isolates were susceptible to penicillins,cephalosporins,linezolid and vancomycin. Conclusion The highly pathogenic serotype-Ⅲ was the predominant serotype among neonatal GBS meningitis in Guangzhou,Therefore,it is neces-sary to strengthen the epidemiological surveillance of GBS invasive infection and the effective implementation of pre-ventive measures.
