1.A study on characteristics of cognitive function of bipolar disorder

Objective To explore the patterns of cognitive performance across different clinical states of bipolar disorder(BPD).Methods 8 neuropsychological tests (17 items) which examine attention,verbal learning and memory,visual memory and executive function were given to 30 depressed BPD ,30 manic BPD ,30 remitted BPD and 34 normal controls.The difference of cognitive functions between the four groups were compared using multivariate analysis of variance (MANOVA).Results Acute patients with BPD showed dysfunction in almost all neuropsychological tests compared with controls [ the score of WCST categories of manic BPD,depressed BPD,remired BPD and normal controls was( 1.58 ±0.90),( 1.89± 1.56),(2.44 ± 1.42),(4.90 ± 1.73 ) respective-ly.The score of immediately logical memory was (10.68±3.92),(10.00±3.80) ,(12.11±3.72),(15.80±4.21) respectively] (P<0.05,P<0.01).The remitted patients showed deficits in attention,verbal learning and memory,executive function tasks in relation to controls (P<0.05,P<0.01).Conclusion There were general cognitive deficits in acute patients with BPD,and some of these deficits may persists in remission state.
2.Expression of pituitary tumor transforming gene and c-myc genes in non-Hodgkin lymphoma patients

Objective To explore the expression of pituitary tumor transforming gene (PTTG) and c-myc gene in patients with non-Hodgkin lymphoma (NHL),its relationship with NHL and its clinical significance.Methods PTTG mRNA and c-myc mRNA levels in bone marrow mononuclear cell (BMMNC) isolated from 38 NHL patients and 10 chronic lymphadenitis patients were quantified by reverse transcriptionpolymerase chain reaction (RT-PCR).Results mRNA expression of PTTG and c-myc gene in BMMNC was significantly higher in NHL patients than those in normal controls (PTTG:0.567 7±0.270 7 vs 0.071 2± 0.020 1,t =4.706,P < 0.05; c-myc:0.352 6±0.185 4 vs 0.107 3±0.043 5,t =3.303,P =0.002).The expression of PTTG and c-myc gene in peripheral T cell lymphoma and diffuse large B cell lymphoma showed no significant difference (PTTG:0.556 8±0.211 3 vs 0.602 8±0.244 6,t =0.640,P =0.527; c-myc:0.350 1± 0.177 6 vs 0.361 0±0.190 2,t =0.302,P =0.765).Both expression of PTTG and c-myc were positively related to NHL clinical stage and IPI.Expression of PTTG mRNA was positively related to the expression of c-myc mRNA.Conclusion There was overexpression of PTTG and c-myc in NHL,which indicates that PTTG might be involved in tumorigenesis of NHL through direct or indirect activation of c-myc oncogene.
3.Protective effect and mechanism of IPQDS on acute myocardial infarction in rats.

OBJECTIVETo observe the protective effect of Injection of Panax quinquefolium diolsaponins (IPQDS) and its mechanism on acute myocardial infarction in rats.
METHODThe acute myocardial infarct model was prepared by left anterior descending coronary occulusion for 24 hours in open chest anesthetized rats. The myocardial infarct size (MIS) was calculated. The activities of serum creatine hosphokinase (CK), lactate dehydrogenase (LDH), aspartate aminotransferase (AST), superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px), and the contents of malondialdehyde (MDA) and nitric oxide (NO) were determined. Blood was collected to observe low shearing specific viscosity, middle shearing specific viscosity and high shearing specific viscosity of whole blood and plasma viscosity. At the same time, the platelet aggregation rate and platelet adherence rate were also determined.
RESULTIn rats treated by IPQDS (in a dosage of 12.5, 25 and 50 mg x kg(-1) d, i.p. 3d ), the MIS was significantly reduced. The activities of serum CK, LDH and AST, and the content of serum MDA were declined. The activities of SOD and GSH-Px, and the content of NO were increased markedly. In addition, low shearing specific viscosity, middle shearing specific viscosity and high shearing specific viscosity of whole blood and plasma viscosity as well as platelet aggregation rate were also declined significantly. But platelet adherence rate had no significant change.
CONCLUSIONIPQDS has a protective effect on acute myocardial ischemia, which may be related to increasing activity of antioxidase in body, scavenging the damage of peroxidation from oxygen free radicals, decreasing the viscosity of blood and plasma and preventting thrombosis etc.

4.Effect of nursing intervention on alleviating the cancer pain and improving the life quality of advanced malignant tumor patients in the department of hepatobiliary surgery

Objective To explore the effect of nursing intervention on alleviating the cancer pain and improving the life quality of advanced malignant tumor patients in the department of hepatobiliary surgery . Methods One hundred and twenty patients with advanced malignant tumor in the department of hepatobiliary surgery were chosen and divided into the control group and the intervention group according to the admission time, each with 60 cases.The control group received the routine nursing , and the intervention group received the cancer pain nursing intervention on the basis of the routine nursing .The degree of cancer pain , the level of self-control disorder for the cancer pain and life quality was compared between two groups .Results The average scores of the degree of cancer pain were respectively (1.63 ±0.53), (3.14 ±0.87) in the intervention group and the control group, and the scores of most serious degree of cancer pain were respectively (3.64 ±1.39), (4.89 ±1.78) in two groups, and the differences were statistically significant (t =-2.453, -2.135, respectively;P<0.05).The scores of the level of self -control disorder for the cancer pain were respectively (1.08 ±0.32),(2.84 ±0.47) in the intervention group and the control group , and the scores of life quality were respectively (3.56 ±1.03),(2.34 ±0.81) in two groups, and the differences were statistically significant (t =-2.585,2.101, respectively;P <0.05).The average hospital time was (14.57 ±5.13) d in the intervention group , and was ( 22 .14 ±6 .44 ) d in the control group , and the difference was statistically significant (t=-7.974,P<0.01).Conclusions The nursing intervention can effectively relieve cancer pain in the advanced malignant tumor patients in the department of hepatobiliary surgery , and improve their levels of self-control for the cancer pain and life quality , and shorten the hospital time , and is worthy of further clinical promotion and application .
5.Correlation between stress, self efficacy and quality of life in patients with gallstone

Objective To explore the effects of stress and self-efficacy on the quality of life in patients with gallstones and the effect relationship of self-efficacy in the quality of life and stress, and provide guidance for the health intervention of patients with gallstones. Methods Totally 362 patients with gallstones after operative that already returned to the community for 12 months were investigated, and their stress condition, self-efficacy and quality status of life were measured by the life events scale ( LES ) , self-efficacy scale and gastrointestinal quality of life index (GIQLI). The effect relationship of self-efficacy in the quality of life and stress was analyzed.Results The self-efficacy played partial mediating effect between stress and quality of life. When the intermediary variables-self-efficacy sense was participated in, the regression coefficient between stress and quality of life was significantly reduced (β was reduced from -0. 695 to -0. 548 ) . Self efficacy played regulating effect between stress and quality of life, which means that self efficacy sense × stress partial regression coefficient in the regression which regard the quality of life as the dependent variable can reach to a significant level (β=-0.086, P<0.05) , and introduced interaction terms after new interpretation (△R2 ) also can reach to a significant level (△R2 = 0. 017, P<0. 05 ) . Conclusions Self efficacy played the mediating effect and regulating effect in the relationship between stress and quality of life. Nursing staff can reduce the negative effects of stress events in patients′quality of life through the interventions in patients′self-efficacy.
6.Effects of follow-up management based on social goals model on social relation quality in hepatocellular carcinoma patients after TACE

Objective To inverstigate the effects of follow-up management based on social goals model on social relation quality in hepatocellular carcinoma patients after transcatheter arterial chemoebolization (TACE).Methods By convenience sampling method, a total of 113 patients after TACE were divided into the control group ( n=56) and the intervention group ( n=57) according to the treatment time. In the control group, routine follow-up based on case-management was adopted, while in the intervention group, the follow-up based on social goals model was adopted. The intervention effect in two groups were tested by social relation quality scale.Results After intervention, the total score of social relation quality, score of family adaptability and score of family commitment were (53.60±8.86),(20.34±3.28) and (18.07±2.96), and they were all significantly higher than the scores in the control group [(49.25±9.75),(18.52±3.55),(16.22±3.21)](P<0.05). Conclusions The follow-up management based on social goals model can effectively improve the level of family adaptability and family commitment. The intervention could be one of the effective ways to implement social relational quality fin patients after TACE.
7.Optimization of porcine embryo vitrification.

The purpose was to optimize the vitrification for porcine embryos cryopreservation. Blastocyst/Morula (5-6th day-embryos) were collected from superovulated Bama mini-pigs (sows/gilts). We compared different cryopreservation methods, cryopreservation tools, thining of zona pellucida (ZP) and recipient breeds on the efficiency of porcine embryo cryopreservation. The results showed that: in embryo survival rate and blastocyst cell number, there were no significant differences between cryopreservation method I [embryos were vitrified by two step method with open pulled straw (OPS) and glass micropipette (GMP) in solution 1 (TCM199 + 20% FBS + 10% EG + 10% DMSO) for 3 min, and solution 2 (TCM199 + 20% FBS + 20% EG + 20% DMSO + 0.4 mol/L SUC) for 1 min, stored in liquid nitrogen] and method II[Blastocysts were cultured for 25 min in NCSU23 + 7.5 microg/mL cytochalasin B, centrifuged at approximately 13 000 xg for 12-13 min, and recovered back into pNCSU23. They were then equilibrated for 5 min in 2 mol/L ethylene glycol in pNCSU23, washed quickly in the vitrification medium, 8 mol/L ethylene glycol, 7% polyvinylpyrrolidone (PVP) in pNCSU23, loaded into OPS/GMP, and plunged into liquid nitrogen]. GMP vitrification method was more suitable and efficient than OPS method (P < 0.05) in embryo survival rate (83.8% vs 77.6%) and blastocyst cell number (53.1 vs 47.5) after thawing. Thining of ZP did not increase the survival rate, but significantly improved blastocyst cell number in the survival blastcysts (60.1 and 46, P < 0.01). Local pig breeds (Fengjing sows) were more suitable as recipients for embryo transfer of vitrified/warmed blastcysts, which can improve pregnant rate and embryo efficiency.

8.Analysis of influencing factors of kinesiophobia in patients after cardiac valve surgery under cardiopulmonary bypass

Objective:To know the current situation of kinesiophobia in patients after cardiac surgery under cardiopulmonary bypass, and to clarify its influencing factors, so as to provide reference for developing intervention strategies to improve kinesiophobia level.Methods:This was a cross-sectional study. From February 2022 to September 2022, the patients after cardiac valve surgery under cardiopulmonary bypass in the Second Affiliated Hospital Zhejiang University School of Medicine were investigated by convenience sampling methods. The survey was conducted using the General Information Questionnaire, The Tampa Scale for Kinesiophobia Heart, Exercise Self-Efficacy Scale, Pain Catastrophizing Scale, and Adaptation, Partnership, Growth, Affection and Resolve(APGAR) as research tools, and the influencing factors were analyzed using univariate and binary Logistic regression analysis.Results:A total of 219 patients were included, of which 97 patients (44.3%) had kinesiophobia. The results of binary Logistic regression analysis showed that monthly family income level, first time out of bed after operation, fear of falling, the family APGAR, and pain catastrophizing were significant influencing factors of kinesiophobia in patients after heart valve surgery under cardiopulmonary bypass (all P<0.05). Conclusions:The prevalence of kinesiophobia is high among patients after heart valve surgery under cardiopulmonary bypass. Clinicians should pay attention to patients with low monthly family income level, late first time out of bed after surgery, and fear of falling, as well as strengthen communication with patients and families, focus on the management of acute postoperative pain. In order to reduce or avoid the occurrence of kinesiophobia and enable patients to benefit from early ambulation.
9.'s flying acupuncture for acute cerebral infarction hemiplegia: a randomized controlled trial.

OBJECTIVETo compare the efficacy difference between's flying acupuncture combined with conventional treatment and conventional treatment alone on acute cerebral infarction hemiplegia.
METHODSA total of 120 patients were randomly divided into an observation group and a control group, 60 cases in each one. The control group was treated with conventional treatment, including anti-platelet aggregation, lipid-lowering, formula of traditional Chinese medicine which could promote circulation and remove stasis, neurotrophic medication and symptomatic treatment; mannitol was used for cerebral infarction with large area or increased intracranial pressure. Based on the conventional treatment applied in the control group, the observation group was treated with flying acupuncture at the affected Jianyu (LI 15), Quchi (LI 11), Shousanli (LI 10), Waiguan (TE 5), Hegu (LI 4), Huantiao (GB 30), Biguan (ST 31), Futu (ST 32), Zusanli (ST 36), etc. The treatment was given once a day, six days per week, for totally 2 weeks. The simplified Fugl-Meyer score, National Institute of Health Stroke Scale (NIHSS) and ADL-Bathel index (BI) score were evaluated before and after treatment in the two groups.
RESULTSAfter the treatment, the simplified Fugl-Meyer and BI were significantly increased in both groups (all<0.05), which was significantly higher in the observation group (both<0.05); after the treatment, the NIHSS was significantly lowered in both groups (both<0.05), which was significantly lower in the observation group (<0.05).
CONCLUSION 's flying acupuncture combined with conventional treatment were effective for acute cerebral infarction hemiplegia, which have better efficacy than conventional treatment on improving motor function, neurological deficit and daily living ability, and the pain is mild.
