In recent years,a considerable number of epidemiological investigations and animal studies have confirmed that metabolic diseases, such as obesity, type 2 diabetes mellitus and metabolic syndrome, have adverse effects on brain functions, inducing mood disorders and cognition impairment. Brain dysfunctions induced by obesity and related complications are associated with numerous central abnormalities, involving brain shrinkage and neurotrophic function impairment, brain insulin resistance, brain oxidative stress, and brain leptin resistance,as well as dysfunctioned dopamine motivation and the reward system. Moreover, these brain dysfunctions are mediated by several peripheral factors, such as triglycerides/free fatty acids, proinflammatory cytokines, and corticosterone/glucocorticoid. On the other hand, metabolic disturbances correlated with emotional-cognitive disorders are evident, but the mechanisms remain obscure. Because of the drawbacks of animal models, the majority of researches focus on the impact of mental stress on the metabolism of lipid and glucose. The interrela?tionship between metabolic diseases and brain functions has become one of the hot spots for research. In this review,we mainly discussed the potential mechanisms underlying mood disorders and cognition impairment induced by obesity and related complications.

Objective To explore the diagnostic value of Talaromyces marneffei (T.marneffei)-specific mannose glycoprotein Mp1p antigen for T.marneffei infection in acquired immune deficiency syndrome (AIDS) patients. Methods All cases were recruited in this study from January 2012 to June 2015 in Guangzhou No.8 People′s Hospital, including 184 AIDS patients with T.marneffei infection confirmatively diagnosed by culture, and 205 controls including 176 AIDS patients without T.marneffei infection and 29 health controls.Double antibody sandwich enzyme linked immunosorbent assay and fluoroimmunoassay combined with double-antibody sandwich were both utilized to detect serum Mp1p antigen levels, and their sensitivity and specificity for diagnosing T.marneffei infection in patients with AIDS were analyzed.x2 test and t test were used for statistical analysis. Results The ratio of males to females and age of the study group were both comparable to those of the control group (x2=0.019, P=0.889;t=1.810,P=0.07, respecitvley). The sensitivities of double antibody sandwich enzyme linked immunosorbent assay and fluoroimmunoassay combined with double-antibody sandwich were 82.07%(151/184) and 83.15%(153/184), respectively (x2=0.076, P=0.783).The specificities were 93.17%(191/205) and 92.68%(190/205), respectively (x2=0.037, P=0.847).The accuracy values were 87.92%(342/389) and 88.17%(343/389), respectively (x2=0.012, P=0.912).The false positive rates were 6.83%(14/205) and 7.32%(15/205), respectively. The false negative rates were 17.93%(33/184) and 16.85%(31/184), respectively (x2=0.049, P=0.829).The positive predictive values were 91.52%(151/165) and 91.07%(153/168), respectively (x2=0.021, P=0.886).The negative predictive values were 85.27%(191/224) and 85.97%(190/221), respectively (x2=0.045, P=0.832).The Kappa values were 0.83 and 0.80, respectively. Conclusion Detection of serum Mp1p antigen of T.marneffei possesses high specificity and sensitivity, which may be utilized for rapid and early diagnosis of T.marneffei infection in patients with AIDS.

Objective:To investigate the routine ultrasonographic findings of fibrinous breast disease lesions , and to provide the basis for the differential diagnosis of fibrocystic breast disease lesions and breast cancer. Methods:The two-dimensional ultrasonography , color doppler flow and elastography of 166 lesions in 144 patients with fibrocystic breast disease confirmed by pathology in Peking University Shenzhen Hospital from January 2015 to February 2021 were retrospectively analyzed. Results:The ultrasonographic characteristics of fibrocystic breast disease lesions were as follows: 74.1%(123/166) lesions were hypoechoic, 57.2%(95/166)lesions were irregular, 49.4%(82/166) lesions were incomplete, 90.4%(150/166) lesions were parallel, 25.9%(43/166) lesions showed small cysts, 24.1%(40/166) lesions showed coarse calcification or fine calcification, 65.1%(108/166) lesions showed no change in posterior echo, 90.4%(150/166) blood flow was poor. The elastography score of 98.2%(162/166) lesions was 2-3 points. Conclusions:The ultrasonographic features of fibrocystic breast tumor are hypoechoic, irregular shape, incomplete edge, with fine or coarse calcification, which are easy to be misdiagnosed as breast malignant tumor. However, the former is characterized by parallel growth, small cyst in the interior, no change in posterior echo, lack of blood supply and soft texture, which provide important basis for clinical differential diagnosis.

Objective To explore the clinicopathological characters of T2 gastric cancer and the value of MSCT in the preop‐erative TNM staging of T2 gastric cancer .Methods A total of 93 patients with T2 gastric cancer were included in our study and un‐derwent preoperative MSCT staging ,who were confirmed by pathologic results .Then the results were compared with those of path‐ologic TNM staging .Also the clinicopathological features of the T2 gastric cancer were analyzed .Results There were no statistical‐ly significant differences in the clinicopathological characters among T2a and T2b patients (P>0 .05) .Comparing with pathologic TNM stage ,the T staging accuracy of MSCT was 91 .40% (85/93) ,the N staging accuracies of CT was 66 .67% (62/93) ,in which , 68 .18% (30/44) ,65 .00% (26/40) ,60 .00% (3/5) and 75 .00% (3/4) were for pN0 ,pN1 ,pN2 and pN3 .And the TNM staging ac‐curacies of CT was 67 .74% (63/93) ,in which ,68 .18% (30/44) ,64 .10% (25/39) ,60 .00% (3/5) and 100% (5/5) were for stageⅠ ,Ⅱ ,Ⅲ and Ⅳ .Conclusion There are no significant different on clinicopathology features among T2a and T2b patients .MSCT can clearly determine the preoperative TNM staging of T2 gastric cancer .
