1.One-stage total hip arthroplastyversusfemoral head arthroplasty for elderly femoral neck fractures:a meta-analysis of hip function and complications

BACKGROUND:There are objections to the efficacy and safety of total hip arthroplasty and artificial femoral head arthroplasty in the treatment of femoral neck fracture.
OBJECTIVE:To assess the efficacy and safety of one-stage total hip arthroplasty and femoral head arthroplasty for > 60-year-old patients with femoral neck fractures.
METHODS:According to the search strategy of Cochrane colaboration network, we searched PubMed (1966 to December 2014), EMbase (1974 to December 2014), Cochrane Library (Issue 3, 2011), China Biology Medicine database(1978 to December 2014), China National Knowledge Infrastructure (1994 to December 2014), VIP database (1989 to December 2014), and Wanfang Database (1979 to December 2014). Twenty-one articles on total hip arthroplasty and hemiarthroplasty for elderly femoral neck fractures were included. Two reviewers independently evaluated the quality of the included studies and extracted the data. In case of disagreement, settlement was made by negotiation. Meta-analysis was performed by RevMan 5.0 software in the included studies.
RESULTS AND CONCLUSION:(1) Literature analysis: five randomized controled studies, three quasi-randomized controled studies, and thirteen retrospective cohort studies were included, containing 2 250 patients. (2) Meta-analysis: No significant differencein rate of dislocation, deep infection rate and mortality rate in 1 year after replacement was detected between total hip arthroplasty and hemiarthroplasty for elderly femoral neck fractures (dislocation rate:RR=1.38, 95%CI: 0.81-2.34; deep infection rate RR=1.12, 95%CI: 0.60-2.11; mortality rateRR=0.90, 95%CI: 0.69-1.18). Reoperation rate was higher in the hemiarthroplasty group than in the total hip arthroplasty group (RR=0.46, 95%CI: 0.32-0.66). Harris score on the affected side between1 and 4 years was significantly higher in the total hip arthroplasty group than in the hemiarthroplasty group (MD=5.64, 95%CI: 2.82-8.46). (3) Results suggested that if physical conditions permit, compared with hemiarthroplasty group, femoral neck fractures patients aged > 60 years old in the total hip arthroplasty group had better hip function, but no significant difference was found in dislocation, deep infection and mortality between both groups.
2.Pathogens diversity and in vitro drug sensitivity test in fungal keratitis

Objective To analyze the distribution, diversity, and the sensitivity of common pathogen species to antifungal drugs in fungal keratitis during 2000 - 2006 in Shandong province. Methods Samples from corneal serapings and corneal buttons from keratoplasties were used for fungal culture and identification. The distribution and diversity of the pathogens in different years were analyzed. The sensitivity of some common species to antifungal drugs were tested. Results A total of 898 patients were diagnosed as fungal keratitis, in which 770 (85.7%) were positive in fungal culture. Pathogens in 748 cases caused by fungal infection were identified, of which 547 cases were caused by Fusarium (71.0%). The prevelence of Fusarium from 2000 to 2006 was 75.5%, 72.8%, 71.5%, 75.2%, 76%, 68. 8% and 56.4% respectively. Eighty-four cases were caused by Aspergillus( 10.9% ) , which accounted for 15.1% , 15.2% , 13.1%, 10. 2%, 10. 4% , 8.0% and 6. 9% of the cases from 2000 to 2006, respectively. Seventy-four eases were caused by Alternaria (9. 6% ), the percentage of which during 2000 -2006 was 1. 9%, 3.3%, 3.7%, 6. 6%, 8. 8%, 12. 0 and 29. 7% respectively. In genus Fusarium, 10 species were found and the most common species were Fusarium Solani ( 33. 8% ), Fusarium oxysporum ( 28.2% ) and Fusarium moniliforme( 27.4% ). Six species were identified in genus Aspergillus, with Aspergillus flavus (53.6%) and Aspergillus fumigatus (39.3% ) being the most common species. The geometric mean MICs of amphotericin B, terbinafin, itraconazole, 5-flucytosine and fluconazole against the common species were 0. 647, 0. 714, 1. 624, 15. 108 and 27. 070 μg/ml, respectively. The mean MICs of the above five antifungal drugs against Fusarium were 0. 899, 0. 893, 3.077, 37. 124 and 21. 687 μg/ml, respectively, and for Aspergillus, the mean MICs of these agents were 0.794, 0.591, 0.416, 23.973 and 7.127 μg/ml, respectively; for Alternaria,the MICs were 0. 409,0. 479,0. 433,11. 655 and 7. 104 μg/ml ,respectively. Conclusions Genus Fusarium, Aspergillus and Alternaria were the predominant pathogens in fungal keratitis in Shandong Province. During 2000 -2006, Fusarium was the most common one, followed by Aspergillus. The percentage of Aspergillus decreased annually. The third most common pathogen, Alternaria increased during the period. The geometric mean MICs of amphotericin B and terbinafin against Fusarium were lower than those of itraconazole, 5-flucytosine and fluconazole. The MICs of these five anti-fungal drugs against Aspergillus and Alternatia were lower than those for Fusarium.
3.Acetabular liner wear of cross-linked versus conventional polyethylene for total hip arthroplasty:a meta-analysis

BACKGROUND: Aseptic loosening occurs after long-term total hip replacement, which directly affects the service life and prospective efficacy of artificial joints. The particles produced by artificial joint wear lead to the surrounding bone dissolved, further cause loosening, among which, polyethylene particles because of acetabular liner wear stand out. OBJECTIVE: To systematically assess the acetabular liner wear, loosening and osteolysis caused by cross-linked polyethylene or conventional polyethylene after total hip arthroplasty. METHODS: A computer-based research of Medline, EMbase, CBM, CNKI, CqVip, WanFang databases before December 2015 and Cochrane (3rd issue, 2011) was performed in accordance with the retrieval strategy made by Cochrane collaboration. A manual retrieval of related bone journals and conference papers was conducted. Eleven randomized controlled trials about the wear caused by cross-linked polyethylene or conventional polyethylene after total hip arthroplasty were enrolled based on inclusion criteria, followed by a Meta-analysis using RevMan 5.0 software. RESULTS AND CONCLUSION: (1) Eleven randomized controlled trials involving 952 patients were included. (2) Meta-analysis showed that the acetabular liner wear rate of cross-linked polyethylene was significantly lower than that of conventional polyethylene at 5 years postoperatively [MD=-0.07, CI(-0.09, -0.05), I2=93%, P < 0.00001]; the large heterogeneity was decreased [MD=-0.06, 95%CI (-0.07, -0.04), I2=39%, P < 0.00001] after three research removed through sensitivity analysis. (3) The osteolysis rate in the cross-linked polyethylene group was significantly lower than that in the conventional polyethylene group [RR=0.39, 95%CI (0.27, 0.57), I2=0%, P < 0.00001]. (4) These results suggest that the cross-linked polyethylene liners exhibit reduced radiological wear and osteolysis, but the mean follow-up of 5 years (1.8 to 8.0) cannot meet the long-term requirements. 4.Diagnostic value of 18 F-FDG PET/CT for hepatic epithelioid hemangioendothelioma

Objective To explore the imaging characteristics and diagnostic value of 18 F-FDG PET/CT in patients with hepatic epithelioid hemangioendothelioma ( HEH) . Methods From January 2014 to December 2015, 5 HEH patients (2 males, 3 females; average age (49.2±13.7) years) confirmed by pathology and underwent 18 F-FDG PET/CT imaging were enrolled in this retrospective study. The character-istics of PET/CT images were analyzed. Two-sample t test and Pearson linear correlation analysis were used for data analysis. Results A total of 26 lesions were detected in 4 patients with multiple lesions, of which 88.5%( 23/26) distributed at the liver edge, and 42. 3%( 11/26) had hepatic capsule retraction sign. SUVmax of the 26 lesions was 5.1±2.6 and it was not correlated with tumor size (r=0.054, P>0.05). Lesion FDG uptake in patients with distant metastases was higher than that in patients without distant metastases (6.08±1.80 vs 3.84±1.14;t=4.15, P<0.05). Two patients underwent the delayed imaging, and FDG up-take in 11/14 lesions was higher compared with that of early imaging, and 3/14 lesions were newly detected by the delayed imaging. One patient had diffuse low-density lesions with diameters between 6.1 and 35.5 mm and SUVmax between 2.6 and 8.1. Conclusion Liver lesions of HEH patients show some features on 18F-FDG PET/CT images which can find distant metastases, and the delayed imaging can improve the detection rate of lesions.
5.Packaging of Rift Valley fever virus pseudoviruses and establishment of a neutralization assay method

Rift Valley fever (RVF) is an acute, febrile zoonotic disease that is caused by the RVF virus (RVFV). RVF is mainly prevalent on the Arabian Peninsula, the African continent, and several islands in the Indian Ocean near southeast Africa. RVFV has been classified by the World Organisation for Animal Health (OIE) as a category A pathogen. To avoid biological safety concerns associated with use of the pathogen in RVFV neutralization assays, the present study investigated and established an RVFV pseudovirus-based neutralization assay. This study used the human immunodeficiency virus (HIV) lentiviral packaging system and RVFV structural proteins to successfully construct RVFV pseudoviruses. Electron microscopy observation and western blotting indicated that the size, structure, and shape of the packaged pseudoviruses were notably similar to those of HIV lentiviral vectors. Infection inhibition assay results showed that an antibody against RVFV inhibited the infective ability of the RVFV pseudoviruses, and an antibody neutralization assay for RVFV detection was then established. This study has successfully established a neutralization assay based on RVFV pseudoviruses and demonstrated that this method can be used to effectively evaluate antibody neutralization.

6.Famine exposure in early life and type 2 diabetes in adulthood: findings from prospective studies in China

BACKGROUND/OBJECTIVES:
This study examined the relationship between famine exposure in early life and the risk of type 2 diabetes in adulthood during the 1959–1961 Chinese Famine.
SUBJECTS/METHODS:
A total of 3,418 individuals aged 35–74 years free of diabetes from two studies in 2006 and 2009 were followed up prospectively in 2009 and 2012, respectively. Famine exposure was classified as unexposed (individuals born in 1962–1978), fetal exposed (individuals born in 1959–1961), child exposed (individuals born in 1949–1958), and adolescent/adult exposed (born in 1931–1948). A logistic regression model was used to assess the relationship between famine exposure and diabetes after adjustment for potential covariates.
RESULTS:
During a three-year follow-up, the age-adjusted incidence rates of type 2 diabetes were 5.7%, 14.5%, 12.7%, and 17.8% in unexposed, fetal-exposed, child-exposed, and adolescent/adult-exposed groups, respectively (P < 0.01). Relative to the unexposed group, the relative risks (95% confidence interval) for diabetes were 2.15 (1.29–3.60), 1.53 (0.93– 2.51), and 1.65 (0.75–3.63) in the fetal-exposed, child-exposed, and adolescent/adult-exposed groups, after controlling for potential covariates. The interactions between famine exposure and obesity, education level, and family history of diabetes were not observed, except for the urbanization type. Individuals living in rural areas with fetal and childhood famine exposure were at a higher risk of type 2 diabetes, with relative risks of 8.79 (1.82–42.54) and 2.33 (1.17–4.65), respectively.
CONCLUSIONS
These findings indicate that famine exposure in early life is an independent predictor of type 2 diabetes, particularly in women. Early identification and intervention may help prevent diabetes in later life.
7.Early Efficacies of Drainage in Arthroscopic Reconstruction with Anterior Cruciate Ligament Reconstruction

Objective To investigate the impact of not using drainage on clinical outcomes after arthroscopic autologous semitendinosus tendon reconstruction with anterior cruciate ligament reconstruction.Methods From March 2022 to June 2023,59 patients undergoing arthroscopic autologous semitendinosus tendon reconstruction with anterior cruciate ligament reconstruction in Lincang People's Hospital were collected and randomly divided into the non-drainage group consisting of 29 cases(observation group)and the 24-hour drainage group consisting of 30 cases(control group).The pain levels of the two groups of patients were recorded on the 1st,3rd,7th,14th,and 30th day after the surgery by using a visual analog scale.Additionally,the knee joint range of motion,length of hospital stay,and occurrence of postoperative complications were monitored.The circumference of the thigh was measured before and after the surgery,and the difference was calculated.Results Repeated measures analysis of variance revealed that there were statistically significant within-subject differences in pain ratings,thigh circumference,and knee joint range of motion(P<0.05),but no statistically significant between-subject differences(P>0.05).Independent samples t-tests showed that on the first day after the surgery,the observation group had lower pain ratings(P<0.001),and higher thigh circumference and knee joint range of motion compared to the control group(P<0.05).There were no statistically significant differences in pain ratings,knee joint range of motion,and thigh circumference between the two groups at the remaining follow-up times(P>0.05);The observation group had a shorter hospital stay than the control group(P<0.001);Both groups of patients had no complications such as lower limb nerve damage,deep vein thrombosis,knee joint infection,or hematomas requiring puncture and aspiration.Conclusion In the early postoperative period following arthroscopic autologous hamstring tendon reconstruction of the anterior cruciate ligament,the omission of drainage does not affect the clinical outcomes in terms of pain,knee joint mobility,and thigh circumference.Moreover,omitting drainage reduces the level of pain experienced by patients on the first day after the surgery,improves the knee joint mobility,and decreases the length of hospital stay.Therefore,in arthroscopic autologous hamstring tendon reconstruction of the anterior cruciate ligament,it is not recommended to routinely use drainage for preventive purposes.
8.Preparation and application of a colloidal gold strip to detect the rabies antibody.

To develop a specific, rapid, and convenient immunochromatography assay (ICA) to detect the rabies antibody in clinical sample from immuned dogs by rabies vaccine. Colloidal gold particles labeled with purified rabies virus (CVS11) were used as the detector reagent. The staphylococcal protein A (SPA) and pured rabbit anti-rabies virus IgG were blotted on the test and control regions of nitrocellulose membrane. Then the strip was assembled with sample pad, absorbing pad, and dorsal shield. The assay samples (261 dog's serum) were collected from Wildlife Rabies Disease Diagnostic Laboratories of Ministry of Agriculture in China, Institute of Military Veterinary, Academy of Military Medical Sciences and other six provinces, including rabies virus positive and negative serum. The performance of the strip was compared to fluorescent antibody virus neutralization test. The neutralizing antibody titer could be detected above 0.5 IU. The strip did not change of performance when stored at room temperature for 12 months. It may offer reference of neutralizing antibody titer level after dogs immuned rabies vaccine and determin whether the dogs need to be immuned again.

On account of the problem that traditional C-arm X-ray apparatus cannot provide precise route guidance for minimally invasive surgeries, we designed and developed a laser positioning and navigation system based on C-arm X-ray apparatus, which can achieve precise positioning function and reduce the exposure of doctors and patients to radiation in minimally invasive surgeries under the linear guidance of a laser beam. Furthermore, this system can enhance the refinement level of surgical operation in minimally invasive surgeries.

Objective:To evaluate the effect of definitive radiotherapy with different doses on overall survival (OS) and identify the prognostic factors of patients with non-metastatic esophageal squamous cell carcinoma (ESCC).Methods:Clinical data of 2 344 ESCC patients treated with definitive radiotherapy (RT) alone or chemoradiotherapy from 2002 to 2016 in 10 hospitals were collected and analyzed retrospectively. After the propensity score matching (PSM)(1 to 2 ratio), all patients were divided into the low-dose group (equivalent dose in 2 Gy fractions, EQD 2Gy<60 Gy; n=303) and high-dose group (EQD 2Gy≥60 Gy; n=606) based on the dose of radiation. Survival analysis was conducted by Kaplan- Meier method. Multivariate prognostic analysis was performed by Cox′s regression model. Results:The median follow-up time was 59.6 months. After the PSM, the 1-, 3- and 5-year overall survival (OS) rate was 66.5%, 34.7%, 27.2% in the low-dose group, 72.9%, 41.7% and 34.7% in the high-dose group, respectively ( P=0.018). The 1-, 3-and 5-year progression-free survival rate was 52.2%, 27.2%, 23.1% in the low-dose group, 58.3%, 38.1% and 33.9% in the high-dose group, respectively ( P=0.001). The outcomes of univariate analysis indicated that cervical/upper esophagus location, early (stage Ⅱ) AJCC clinical stage, node negative status, tumor length ≤5 cm, receiving intensity-modulated radiation therapy (IMRT), receiving concurrent chemotherapy and EQD 2Gy≥60 Gy were closely associated with better OS (all P<0.05). Multivariable analysis demonstrated that tumor location, regional lymph node metastasis, concurrent chemotherapy and EQD 2Gy were the independent prognostic factors for OS (all P<0.05). Conclusion:Three-dimensional conformal or IMRT with EQD 2Gy≥60 Gy yields favorable survival outcomes for patients with locally advanced ESCC.
