Gastrointestinal stromal tumors(GISTs) are rarely one of gastrointestinal carcinomas that originate from mesenchymal tissue.GISTs are characterized by expression of CD117 receptor in cells and have variable biological phenotypes ranging from benign to highly malignant.The most common complaints of patients with GISTs are abdominal pain,peptic ulcer,bleeding or/and obstruction in digestive tract.GISTs have an extraordinarily high rate of recurrence after surgical resection and are highly resistant to radiation and standard chemotherapy.The imatinib mesylate(Gleevec) is a selective inhibitor of the tyrosine kinase receptor.The outcome for the patients with GISTs treated by Gleevec was promising in terms of immediate response and survival fraction.Recent research shows that a new targeting therapeutic agent,SU11248,can prolong the survival time of the patients with GISTs who were resistant to Gleevec therapy.

Objective To explore the expression of myelin transcription factor 1 (MyT1) and its significance in the brain of epileptic rats induced by lithium chloride-pilocarpine. Methods Models of epilepsy of SD rats were established by intraperitoneal injection with lithium chloride and pilocarpine. 4.Animal study on the feasibility of regional gastroduodenal artery or splenic artery ligation followed by regional intraarterial perfusion of pancreas with 5-FU

Background and Purpose:Pancreatic cancer is malignant tumor of the digestive tract with poor prognosis and is not sensitive to conventional chemotherapy.This study investigates the safety and feasibility of regional arterial ligation followed by intra-arterial perfusion chemotherapy in 14 pigs divided into groups A,B and C.Methods:14 pigs were divided into 3 groups.Group A(n=5) underwent gastroduodenal artery ligation and catheterization through the right gastroepiploic artery.Group B(n=5) underwent splenic artery ligation and catheterization.Group C(n=4) underwent left periotic vein puncture.5-FU(20mg/kg) was perfused into these catheters in 60 min.Blood samples and pancreatic tissue were obtained for the purposes of the determination of drug concentration and pathological diagnosis.Results:At 80 min and 100 min the drug concentration in portal vein of group A and group B were higher than those of group C(P
5.The early diagnostic value and prognostic significance of serum presepsin (sCD14-ST)in patients with sepsis

Objective To investigate the early diagnostic value and prognostic significance of serum presepsin in patients with sepsis.Methods A prospective observational study of 80 patients from ICU was carried out between January 2014 and June 2015.According to sepsis criteria,the cases were divided into sepsis group (n =50)and noninfectious systemic inflammation syndrome group (SIRS group,n =30), and another 25 healthy volunteers were collected as control group.Meanwhile,sepsis group was further divided into survival and death subgroups according to 28-day mortality.The dynamic changes of serum presepsin,procalcitonin (PCT),soluble myeloid cells expressing triggering receptor-1 (sTREM-1 ),C-reactive protein (CRP),interleukin 6 (IL-6)were monitored on the 1st,3rd and 7th day after admission. Results (1)The serum presepsin level of sepsis group was higher than SIRS group and control group on the 1st day,and the difference was statistically significant [912.0 (563.5,1514.8)pg/mL vs.462.0 (334.0,556.0)pg/mL vs.120.0 (70.2,320.1)pg/mL,P <0.05];(2)The area under the ROC curve (AUC)of presepsin for the sepsis diagnosis was greater than PCT,sTREM-1,CRP,IL-6.As cut-off point set at 672.5 pg/mL,presepsin measurements showed a sensitivity of 80%,specificity of 96.7%, combination of presepsin,PCT and sTREM-1 could offer better diagnostic confirmation;(3)The level of serum presepsin,PCT and sTREM-1 in death group were higher than those in survival group.The levels of serum presepsin,PCT,sTREM-1 and IL-6 in survival group gradually decreased to normal level following the therapy,but the levels of above markers in death group didn’t decrease significantly;(4)The AUC of presepsin in prognosis of sepsis prediction was greater than that of PCT,sTREM-1,and combination of the above three markers could offer better prognostic value.Conclusions Serum presepsin provided superior diagnostic and prognostic accuracy to PCT and sTREM-1,and the combination of presepsin,PCT and sTREM-1 can offer the reliable guide for sepsis diagnosis and death risk prediction.
6.Delta-shaped anastomosis in total laparoscopic distal gastrectomy

Objective To explore the feasibility of laparoscopically intracorporeal delta-shaped anastomosis in distal gastrectomy.Methods 82 consecutive patients with distal gastric cancer received laparoscopic distal gastrectomy, including intracorporeal delta-shaped anastomosis in 39 cases, and handassisted Billroth-Ⅰ anastomosis in 43 cases.The operative time, blood loss, anastomosis time were compared between the two groups.Results There were no differences between the two groups with regard to the operative time [(215 ± 24) min vs.(207 ± 13) min, t =0.406, P > 0.05] and blood loss [(56 ±21)ml vs.(65 ± 26)ml, t =1.441, P > 0.05], although the intracorporeal Delta-shaped anastomosis was more time-consuming than the extracorporeal Billroth-Ⅰ anastomosis [(17 ± 8) min vs.(12 ± 3) min, t =2.529, P < 0.05].Conclusions Demanding expertise and some skills, totally laparoscopic distal gastrectomy with intracorporeal delta-shaped anastomosis is safe and feasible.
7.Expression and relationship of MMP-2 and COX-2 in bladder transitional cell carcinoma

Objective To evaluate the level of MMP-2 and COX-2 Protein in bladder transitional cell carcinoma tissue and explore their relationships. Methods A total of 42 patients with bladder transitional cell carcinoma, including Ta-T1 (n=18), T2-T4 (n=24), G1(n=12), G2 (n=19), G3 (n=11), metastasis (n=26) and without metastasis (n=16), were enrolled in the study. Eight normal bladder tissues were selected as control group. Western blotting was performed todetect the mRNA level of MMP-2 and COX-2. Results The relative COX-2 protein level of Ta-T1 (0.729±0.458), T2-T4 (1.248±0.425), G1 (0.61±0.486), G2 (1.055±0.406), G3 (1.422±0.341) were all higher than that of the control group significantly (0.31±0.149, t = 3.56, 4.13; F = 5.98, P <0.05). The relative MMP-2 protein level of Ta-T1 (0.844±0.345), T2-T4 (1.458±0.463), G1 (0.971 ±0.370), G2(1.445±0.378), G3 (1.755±0.387) were all higher than that of the control group (0.460±0.213, t = 3.91, 4.83;F = 6.35, P <0.05). The COX-2 and MMP-2 protein level in tumor tissues with and without metastasis were 1.246±0.426 vs 0.668±0.421, 1.430±0.461 vs 0.814±0.341, t = 5.89, 6.27, P <0.01, respectively. The level of COX-2 protein was positively correlated with MMP-2 positively (r =0.48, P <0.01). Conclusion MMP-2 and COX-2 protein are highly expressed in bladder transitional cell carcinoma tissue and their expression is positively correlated with the malignant degree. MMP-2 and COX-2 might play a synergetic role in the carcinogenesis of bladder transitional cell carcinoma.

9.Treatment of septic shock from upper urinary obstruction

Objective To investigate the treatment of septic shock(SS) from upper urinary obstruction(UUO). Methods Continuous veno-venous haemofiltration(CVVH) combined with surgical method was applied to 42 SS patients from UUO. Their general conditions, liver and kidney functions, APACHE Ⅱ, therapeutic intervention scorisystem(TISS), multiple organ dysfunction syndrome (MODS), complication rate and main outcomes were analysed comparing with traditional therapies groups(n=30). Results Compared with those traditional therapies, the APACHEⅡ , MODS and TISS score decreased (P<0.05). Thirty-seven out of 42 patients survived and the survival rate was 88.0% during ICU. Conclusion CVVH combined with surgical methods may effectively decrease the incidence of complications and mortality of SS from UUO, and the mechanism may be related to the remove of mediators of inflammation.
10.Establishment of an allogenetic skin transplant model in mice for evaluating immunosuppressive drugs

Aim To establish an allogenetic mouse skin trans-plant model,in order to provide a research model for immunosup-pressive drugs. Methods Skins from the ears of C57BL/6 mice were transplanted to the back of BALB/c mice and skin isografts ( BALB/c mice to BALB/c mice) were used as control. Cyclos-porin A( CsA) was used as a model compound to test the imm-nosuppresive effect on allogenetic graft rejection. Following the transplation and CsA treatment, the graft rejection score and graft skin survival rate were quantified. Four and nine days after transplantation,serum IL-4,IL-12 and IFN-γ levels were meas-ured using ELISA kits. Twelve days after transplantation, mice were sacrificed. The weight of spleen and thymus was obtained, and CD4 + and CD8 + population of spleenic T cells were ana-lyzed using flow cytometer. Histological features were assessed by hematoxylin-eosin( HE) staining of formalin-fixed, paraffin-em-bedded graft skins. Results After transplantion, the graft rejec-tion score increased and graft skin survival rate decreased gradu-allly. Serum IL-12 and IFN-γ levels of allograft mice increased markedly. Compared with those of isograft mice, mice with skin allograft displayed a significant increase in the percentage of the CD8 + T cell subpopulation. Remarkable inflammation, such as edema, inflammatory cell infiltration were observed in allograft mice. Compared with saline treated mice, CsA significantly re-duced the graft rejection score and improved survival rate of skin grafts. And also, CsA treated mice had smaller spleen and thy-mus. Mice that received high doses of CsA had significantly less CD8 + T cells than those treated with saline. Moreover, allograft skins in mice that received CsA had less inflammation. Conclu-sions Allogenetic mouse skin transplantation exhibits acute graft rejection. CsA can inhibit the rejection in a dose dependent manner.
