Background and Purpose:Pancreatic cancer is malignant tumor of the digestive tract with poor prognosis and is not sensitive to conventional chemotherapy.This study investigates the safety and feasibility of regional arterial ligation followed by intra-arterial perfusion chemotherapy in 14 pigs divided into groups A,B and C.Methods:14 pigs were divided into 3 groups.Group A(n=5) underwent gastroduodenal artery ligation and catheterization through the right gastroepiploic artery.Group B(n=5) underwent splenic artery ligation and catheterization.Group C(n=4) underwent left periotic vein puncture.5-FU(20mg/kg) was perfused into these catheters in 60 min.Blood samples and pancreatic tissue were obtained for the purposes of the determination of drug concentration and pathological diagnosis.Results:At 80 min and 100 min the drug concentration in portal vein of group A and group B were higher than those of group C(P
2.Ultrasound elasticity imaging correction and biopsy diagnosis value in BI-RADS 4 breast mass

Objective:To compare the diagnostic value of strain ratio correction in ultrasound elasticity imaging and ultrasound-guided percutaneous biopsy for the diagnosis of breast imaging reporting and data system (BI-RADS) 4 breast mass. Methods:From January 2014 to June 2016 in the Affiliated Hospital of Chengde Medical College, 120 patients with breast tumor resection and preoperative BI-RADS4 of all the masses were included in this study. The diagnostic value of contrast ultrasound elastography strain ratio correction and biopsy in ultrasound-guided percutaneous biopsy was evaluated with pathology as the gold standard. Results:In the 120 cases of grade BI-RADS4 mass by ultrasound elastography after correction, 46 cases were graded BI-RADS4 without changing;59 cases were downgraded to BI-RADS3, and 15 cases were upgraded to BI-RADS5. The pathology, ultrasonic elastography strain ratio correction, biopsy diagnostic sensitivity, specificity, and accuracy were 90.7%, 81.8%, 85.5%, 88.8%, 98.5%, and 95.0%, respectively. A significant difference was found between ultrasonic elastography strain ratio correction and ultrasound-guided biopsy for the diagnosis of breast malignant mass (P<0.05), whereas no significant difference was found for the diagnosis of benign mass (P>0.05). Conclusion:Ultrasound-guided puncture biopsy is highly valuable in qualitative diagnosis of BI-RADS4 breast mass. After the pathological diagnosis, ultrasound elastography strain ratio has certain clinical value in BI-RADS4 guided biopsy and correction of mass classification.
3.Study on using CT to locating mark of sustentaculum tali and the parameter measurement

Objective To use CT and 3D reconstruction technique to locating mark of sustentaculum tali ,and measure the an‐gle & length of the specified point of the lateral wall of the calcaneus to sustentaculum tali for providing .To basis for the operation of calcaneus .Methods Thirty adult ankle wet specimens were chosen and scanned with CT ,and the dicom data were converted into the software to 3D‐reconstruction ,the observation and measurement to find new methods of locating mark of sustentaculum tali ;Forty adult calcaneus dried specimens were chosen and scanned with CT .The dicom data were converted into the software to 3D‐re‐construction ,using a workstation software measure the angle and length of the line which from the center of the sustentaculum tail to each point on the calcaneal lateral wall .Results First ,in the function of ankle joint position ,with the foot medial surface as “sag‐ittal reference plane” measurement ,the vertical line passing through the leading edge of medial malleols and the coronal line passing through the leading edge of sustentaculum tali intersect at a point .The vertical distance of the point to leading edge of medial malle‐ols was (34 .95 ± 2 .60) mm ,the length of sustentaculum tali was (21 .44 ± 1 .89) mm ,the height of sustentaculum tali was (10 .17 ± 1 .16) mm ,the anteversion of sustentaculum tali was (35 .31 ± 3 .73)° .Second ,in front of calcaneus ,upward angel of points G ,E were (28 .78 ± 3 .90 )° ,(29 .47 ± 2 .96 )° respectively .Frontward angle of points G ,E were ( - 19 .83 ± 4 .60 )° , ( - 11 .02 ± 4 .74)° respectively .Under the posterior articular surface ,upward angel of points C ,K ,I ,J were(2 .52 ± 2 .46)° ,(2 .92 ± 2 .28)° ,(14 .98 ± 2 .49)° ,(14 .38 ± 1 .90)° respectively .Frontward angle of points C ,K ,I ,J were(7 .32 ± 1 .66)° ,(19 .25 ± 1 .98)° , (10 .30 ± 2 .63)° ,(19 .33 ± 1 .97)° respectively .The length of screw was about 40 - 44 mm .The length of C point to the sustentacu‐lum tail was minimum ,and the length of G was maximum .Conclusion In the function of ankle joint position ,the sustentaculum tail can be located with the leading edge of medial malleols as a surface landmarks .The measurement of the angle and length of which from each point on the calcaneal lateral wall to sustentaculum tail by using CT .
4.The value of correction of ultrasound elastography strain rate ratio in BI-RADS grading of breast hyperplasia with fibroadenoma and breast cancer

Objective To investigate the value of correction of ultrasound elastography strain rate ratio in BI-RADS grading of breast hyperplasia with fibroadenoma and breast cancer.Methods The data of 120 cases of hyperplasia of mammary gland hyperplasia with fibroadenoma and 120 cases of breast cancer confirmed by operation or biopsy in this hospital from January 2012 to October 2016 were analyzed retrospectively.The boundary,shape,echogenicity,aspect ratio,calcification,blood flow distribution,resistance index,BI-RADS grade,ultrasonic elastography strain ratio were analyzed.The value of correction of ultrasound elastography strain rate in breast hyperplasia with fibroadenoma and breast cancer BI-RADS grading ratio were investigated.Results There were significant differences in boundary,rear echo,aspect ratio,calcification,ultrasonic elastography strain ratio between breast hyperplasia with fibroadenoma and breast cancer (P<0.05);There was no significant difference between the two groups about morphology and internal echo (P>0.05);Breast hyperplasia with fibroadenoma were mainly 0-Ⅰ in blood flow distribution,RI< 0.7,breast cancer showed Ⅱ-Ⅲ,RI≥0.7,the difference was statistically significant (P<0.05).Ultrasound elastography strain rate ratio improved the diagnose accordance rate of two groups after BI-RADS' classification correction.Conclusion Hyperplasia of mammary glands with fibroadenoma and breast cancer have overlapping ultrasound features.Ultrasound elastography strain rate ratio can improve the diagnose accordance rate of two groups after BI-RADS' classification correction,which has certain clinical value.
5.Bioinformatic analysis on related genes of lung adenocarcinoma

To indentify the candidate genes and signaling pathways in lung adenocarcinoma by analyzing gene profiles with bioinformatics. Methods: The expression profiles of GSE40791, GSE68571, GSE43458, and GSE18842 were down-loaded from the Gene Expression Omnibus (GEO) database. The four microarray datasets were integrated to obtain the differentially expressed genes related to lung adenocarcinoma. STRING database was used to construct the protein-protein interaction (PPI) network of differentially expressed genes, and to further explore the gene modules and the key genes. DAVID was used to perform the gene enrichment analysis of each gene module, and to explore the regulatory function of each gene module in adenocarcinoma cells, as well as the relationship between the key genes in the module and the prognosis of the patients. Results: Thirty-seven up-regulated genes and 120 down-regulated genes were obtained from the primary screen, and the protein-protein interaction(PPI) network was successfully constructed. According to MCODE algorithm, we constructed gene modules and calculated the core genes (KIF14, SEPP1, SPP1, RBP4) in the PPI network. Finally, four modules were proved to be involved in regulation of cell cycle, blood coagulation, cell adhesion and cell metabolism, and four key genes were proved to be differentially expressed between lung adenocarcinoma tissues and normal tissues (all P<0.05). Survival analysis showed that expressions of KIF14, SEPP1 and SPP1 had significant effect on the prognosis of lung adenocarcinoma (P<0.01 or P<0.05), while RBP4 exerted insignificant difference in the survival rate of lung adenocarcinoma patients (P>0.05). Conclusion: With bioinformatics, three differentially expressed genes between lung adenocarcinoma tissues and normal adjacent tissues were finally screened out and proved to be closely related to the prognosis of patients, which provided new thoughts in the diagnosis and prognosis prediction of lung adenocarcinoma and improved the study efficiency on the mechanism of lung adenocarcinoma.
6.Craniofacial therapy: advanced local therapies from nano-engineered titanium implants to treat craniofacial conditions.

Nano-engineering-based tissue regeneration and local therapeutic delivery strategies show significant potential to reduce the health and economic burden associated with craniofacial defects, including traumas and tumours. Critical to the success of such nano-engineered non-resorbable craniofacial implants include load-bearing functioning and survival in complex local trauma conditions. Further, race to invade between multiple cells and pathogens is an important criterion that dictates the fate of the implant. In this pioneering review, we compare the therapeutic efficacy of nano-engineered titanium-based craniofacial implants towards maximised local therapy addressing bone formation/resorption, soft-tissue integration, bacterial infection and cancers/tumours. We present the various strategies to engineer titanium-based craniofacial implants in the macro-, micro- and nano-scales, using topographical, chemical, electrochemical, biological and therapeutic modifications. A particular focus is electrochemically anodised titanium implants with controlled nanotopographies that enable tailored and enhanced bioactivity and local therapeutic release. Next, we review the clinical translation challenges associated with such implants. This review will inform the readers of the latest developments and challenges related to therapeutic nano-engineered craniofacial implants.

7.Force-induced Caspase-1-dependent pyroptosis regulates orthodontic tooth movement.

Pyroptosis, an inflammatory caspase-dependent programmed cell death, plays a vital role in maintaining tissue homeostasis and activating inflammatory responses. Orthodontic tooth movement (OTM) is an aseptic force-induced inflammatory bone remodeling process mediated by the activation of periodontal ligament (PDL) progenitor cells. However, whether and how force induces PDL progenitor cell pyroptosis, thereby influencing OTM and alveolar bone remodeling remains unknown. In this study, we found that mechanical force induced the expression of pyroptosis-related markers in rat OTM and alveolar bone remodeling process. Blocking or enhancing pyroptosis level could suppress or promote OTM and alveolar bone remodeling respectively. Using Caspase-1-/- mice, we further demonstrated that the functional role of the force-induced pyroptosis in PDL progenitor cells depended on Caspase-1. Moreover, mechanical force could also induce pyroptosis in human ex-vivo force-treated PDL progenitor cells and in compressive force-loaded PDL progenitor cells in vitro, which influenced osteoclastogenesis. Mechanistically, transient receptor potential subfamily V member 4 signaling was involved in force-induced Caspase-1-dependent pyroptosis in PDL progenitor cells. Overall, this study suggested a novel mechanism contributing to the modulation of osteoclastogenesis and alveolar bone remodeling under mechanical stimuli, indicating a promising approach to accelerate OTM by targeting Caspase-1.
