1.Paclitaxel plus carboplatin or cisplatin for patients with advanccd head and ncck cancer

Purpose:To study the efficacy and toxicity of paclitaxel in the treatment of patients with advanced head and neck cancer. Methods:51 patients with stage Ⅲ-Ⅳadvanced head and neck cancer were enrolled, including l9 patients who had no prior chemotherapy, 32 patients who had recurrent head and neck cancer after surgery and/or radiation therapy, and then had become resistant after about two cycles of chemotherapy with fluorouracil plus cisplatin. Three to six cycles of chemotherapy with paclitaxel plus carboplatin or cisplatin were administered: paclitaxel 75-l00mg/m2, qd. d 1?d 8?d 15 plus carboplatin 300-350 mg/m2, qd. d 2 (or cisplatin 20 mg/m2,qd. d 1-5 ), q 4 wk.Results:The total response rate in 51 patents was 19 (37.2%), with 4 (7.8%) complete response, 15 (29.4%) partial respond, l8(35.3%) stable disease and 14 (27.4%) progression. Major toxicities were neutropenia, GI tract reactions and alopecia. One patient had allergic shock and patient died at the beginning of the second cycle of chemotherapy. Conclusions:Paclitaxel is efficient in treating the advanced head and neck cancer. Except for rare cases of allergy, major toxic effects can be tore rated by the patients.

Phase angle (PA), derived by bioelectrical impedance analysis, has been used to evaluate nutritional status.It has a unique advantage in diagnosing early malnutrition related to diseases by the detection of human body cell membrane integrity and fluid composition change, and being associated with bad disease prognosis in part of clinical research.Because of its non-invasive, convenient, accurate and affordable technique, it has a wide prospect of clinical application.However, lack of uniform PA reference value somehow limited its application.We summarize the latest clinical research and application situation of PA, and explore the characteristics and the influence factors of PA in different disease group, in order to provide theoretical basis for the Chinese PA reference range.

Objective To study the vascular endothelium growth factor(VEGF) antisense oligonucleotide(ASON) Cinobufotalin in enhancing apoptosis induced by human chronic myeloid leukemia K562 cell line.Methods The synthesis of VEGF ASON was transfected into the K562 cells;Cinobufotalin of four concentrations(1,3,5 and 7mg/L) acted on the K562 cell line for 24h,48h and 72h.Western blot was used to detect VEGF protein expression,while in situ apoptosis(TUNEL) and flow cytometry method(FCM method) were employed to detect the apoptosis.Results The different doses of Cinobufotalin all inhibited K562 cell line in time-and dose-dependent manners.K562 cell line transfected by VEGF ASON had a more pronounced induction of apoptosis.Conclusion The in vitro Cinobufotalin at a certain range of concentration can induce apoptosis in K562 cell line,and VEGF ASON enhances Cinobufotalin's effect in inducing apoptosis of K562 cells.
5.TAD regimen with melphalan to treat 18 cases of multiple myeloma

Objective To observe the clinical results of combination therapy for multiple myeloma by arsenic trioxide,melphalan,vitamin C and dexamethasone.Methods 18 cases with multiple myeloma,12 males and 6 females were given the treatment including arsenic trioxide,melphalan,vitamin C and dexamethasone combination therapy;the dosage of arsenic trioxide 10 mg,d1~5,d8~12;vitamin C 1.0 g,d1~5,d8~12;dexamethasone 20 mg,d1~4,d8~11;melphalan 2 mg,tid,d1~12.Complete response(CR),partial response(PR),minimal response(MR),no ehange(NC)were observed.Results The CR werel6.7%(3/18),PR were 77.7%(14/t8),MR were 5.6%(1/18).Conclusion Combinational arsenic trioxide,melphalan,vitamin C and dexamethasone is an effective therapy with less side effects,which can be considered for treating the initial or relapsed or refractory MM.
6.Tissue culture of Vinca minor and determination of vincamine.

OBJECTIVETo study tissue culture of Vinca minor and determine the content of vincamine.
METHODLeaf blades, stalks, root segment of V. minor were used as explants to study the effect of 2, 4-D,6-BA,NAA on its callus induction and vincamine contents in the orthogonal design experiment. In the peak period of callus formation, vincamine content in callus of V. minor and sterile plants was determined by HPLC. The experimental data was statistically analyzed.
RESULTThe content of 6-BA and NAA had no significant effect on its callus induction. But the content of 2, 4-D had significant effect on its callus induction. Within 20,40,60 d, the content of vincamine in sterile plant was (0.015 +/- 0.003)%, (0.097 +/- 0.001)% , (0.113 +/- 0.06)%, respectively. In the peak period of callus formation, vincamine content in callus of leaf blades, stalks, root segment was (0.024 +/- 0.0025)%, (0.016 +/- 0.0015)%, (0.010 +/- 0.0015)%, respectively. To 30 days of subculture, vincamine content in callus of leaf blades, stalks, root segment was (0.041 +/- 0.002)%, (0.019 +/- 0001)%, (0.016 +/- 0.002)%, respectively.
CONCLUSIONThe optimal hormone combination for callus initiation was MS +2, 4-D 1.0 mg x L(-1) +6-BA 0.5 mg x L(-1) + NAA 0.5 mg x L(-1). In different growth periods, vincamine content in sterile plants is significantly different. From different explants in callus vincamine content is different, in which leaves callus is significantly higher than that of stems, roots produced callus organization.

7.Gene expression profile changes in NB4 cells induced by realgar.

OBJECTIVESTo compare the gene expression profiles of acute promyelocytic leukemia cell line NB(4) before and after 12 hours of realgar treatment using cDNA microarray.
METHODSTwo cDNA probes were prepared through reverse transcription from mRNA of both untreated and realgar treated NB(4) cells. The probes were labeled with Cy3 and Cy5 fluorescence dyes individually, hybridized with cDNA microarray representing 1003 different human genes, and scanned for fluorescent intensity. The genes were screened through the analysis of the difference in two gene expression profiles.
RESULTSThe analysis of gene expression profiles indicates that 9 genes were up-regulated and 37 genes were down-regulated. Among the 9 up-regulated genes, 2 genes were involved in a proteasome degradation pathway. Some genes related to protein synthesis, signal transduction and cell receptors were down-regulated.
CONCLUSIONPSMC2 and PSMD1 genes may play an important role in the apoptosis and partial differentiation of NB(4) cells.

8.Clinical observation of 21 cases of unrelated donor hematopoietic stem cell transplantation for leukemia

non-myeloablative BuCy+fludarabine conditioning regimen,and another one was treated with TBI+VP-1 6 +CTX+CCNU conditioning regimen.Only one case received short-term MTX,cyclosporin A and ATG regimen for prevention of graft-versus-host disease (GVHD).The GVHD prevention regimens of the other patients were based on short-term MTX,cyclosporin A,ATG and mycophenolate mofetil regimen.The hematopoietic reconstitution, complications and prognosis were observed.Results One patient died of intracranial hemorrhage,and hematopoi-etic reconstitution was achieved in the other 20 patients.The median time for hematopoietic reconstitution shortened by one day in large-dose group compared with that in low-dose group.Adverse reactions included high fever, shivering,gastrointestinal tract adverse reaction,liver injury,oral mucositis and other rare side effects.GVHD occurred more frequently in patients with HLA mismatched transplantation.Nine patients with aGVHD and 9 patients with cGVHD recovered after effective treatment.Within 100 days after transplantation,18 patients had bacterial or fungal infection,mainly upper respiratory tract infection;7 patients had cytomegalovirus infection;2 had EB viremia,and one had urinary BK virus infection.Only one patient died of VOD.Hemorrhagic cystitis occurred in 5 patients and improved after treatment.The median survival time was 24 months (ranging from 136 days to 9 years).One-year and 3-year overall survival rates were 85.2% and 63.9%,the disease free survival rates were 81% and 23.8%,recurrence free survival rates were 71.4% and 14.3%,respectively.Conclusion URD-HSCT was an effective method to treat leukemia.Conditioning regimen of BuCy and modified BuCy2 were safe and effective,the adverse reactions were reversible and well tolerated.Hematopoietic reconstitution time shortened in large-dose MNC and CD34 + cell number groups compared with that in low-dose group.The occurrence rate of GVHD with HLA mismatched transplantation was more than that of HLA matched transplantation.Low-dose heparin,prostaglandin E1 and Danshen injection can effectively prevent VOD.
9.Effect of optimized green channel under plan-do-check-action on shortening door-to-needle time and improving early neurological function in patients with intravenous thrombolysis

Objective:To investigate the effect of optimized construction of green channel under plan-do-check-action (PDCA) on shortening the door-to-needle time (DNT) and improving early neurological improvement in patients with acute ischemic stroke (AIS) treated with intravenous thrombolysis.Methods:The clinical data of 120 patients with AIS treated with intravenous thrombolysis from June 2018 to September 2019 in Liangxiang Teaching Hospital of Capital Medical University were retrospectively analyzed. Among them, 60 patients were admitted to hospital before optimized as non-optimized group, and 60 patients were admitted to hospital after optimized as optimized group. The time from visit to CT examination completion, time from CT examination completion to informed consent, time from informed consent to intravenous thrombolysis, DNT, DNT ≤ 60 min ratio, National Institutes of Health stroke scale (NIHSS) score 2 h after intravenous thrombolysis and at discharge from hospital, early neurological improvement (ENI) rate, hospital stays, adverse outcomes and mortality were compared between 2 groups.Results:The time from CT examination completion to informed consent and DNT in optimized group were significantly shorter than those in non-optimized group: 29 (14, 36) min vs. 37 (21, 54) min, 55 (45, 67) min vs. 76 (54, 93) min, the DNT ≤ 60 min ratio was significantly higher than that in non-optimized group: 68.3% (41/60) vs. 35.0% (21/60), the ENI rate was significantly lower than that in non-optimized group: 28.3% (17/60) vs. 46.7% (28/60), and there were statistical differences ( P<0.05 or<0.01). There were no statistical differences in the time from visit to CT examination completion, time from informed consent to intravenous thrombolysis, NIHSS score 2 h after intravenous thrombolysis, hospital stays, NIHSS score at discharge from hospital, adverse outcomes rate and mortality between 2 groups ( P>0.05). Conclusions:The optimized construction of green channel under PDCA can effectively shorten the DNT of intravenous thrombolysis in patients with AIS, and may be significantly important for improving the long-term prognosis of patients.
10.Correlation between dynamic high-density lipoprotein trajectories and clinical outcomes in critically ill children

Objective:To characterize the longitudinal and dynamic high-density lipoprotein (HDL) trajectories in critically ill children and explore their correlation with clinical outcomes.Methods:Retrospective cohort study.All critically ill children admitted to the Pediatric Intensive Care Unit (PICU) of West China Hospital, Sichuan University from January 1, 2015 to October 1, 2020 were included in this retrospective study.Group-based trajectory modeling (GBTM) was applied to characterize the HDL trajectories in days 0-6 post-PICU admission and develop HDL trajectory groups.The in-hospital mortality rate was reported as frequency (%) and then compared by the Chi-square test or Fisher′s exact test between HDL trajectory groups.The length of stay (LOS) in the PICU was described by M( Q1, Q3), and its difference between HDL trajectory groups was evaluated by the Kruskal Wallis test.Logistic regression and multiple linear regression were used to determine the correlation between HDL trajectories and clinical outcomes.The primary outcome was in-hospital mortality rate, and the secondary outcome was LOS in the PICU. Results:A total of 4 384 critically ill children were ultimately enrolled in the study, and 6 HDL trajectory groups were developed based on GBTM analyses: group 1 (758 cases), the lowest HDL group; group 2 (1 413 cases), the low HDL group; group 3 (74 cases), the low-to-high HDL group; group 4 (621 cases), the medium HDL group; group 5 (1 371 cases), the high HDL group; and group 6 (147 cases), the highest HDL group.Logistic regression analysis showed that compared with critically ill children in group 1, those belonging to groups 2, 3, 4, 5, and 6 were at lower risks of in-hospital mortality with odds ratio ( OR): 0.475, 95%confidence interval ( CI): 0.352-0.641, P<0.001; OR: 0.093, 95% CI: 0.013-0.679, P=0.019; OR: 0.322, 95% CI: 0.208-0.479, P<0.001; OR: 0.263, 95% CI: 0.185-0.374, P<0.001, and OR: 0.142, 95% CI: 0.044-0.454, P=0.001, respectively.Multiple linear regression analysis revealed that compared with critically ill children in group 1, those belonging to groups 4, 5, and 6 had the trend of shorter LOS in PICU, and the β value and 95% CI were β: -4.332, 95% CI: -5.238- -3.426, P<0.001; β: -3.053, 95% CI: -3.809--2.297, P<0.001; β: -6.281, 95% CI: -7.842--4.721, P<0.001, respectively. Conclusions:The dynamic HDL trajectories during 0-6 days after PICU admission are associated with in-hospital mortality rate of critically ill children.The HDL trajectory at a persistently low level is associated with higher mortality, while the HDL trajectory at a persistently high level or with the trend from a low level rising to a high level shows a lower risk of mortality.It is suggested that the HDL trajectory model may become an indicator to predict the condition and prognosis of critically ill children.
